Molecular Guided Therapy Provides Sustained Clinical Response in Refractory Choroid Plexus Carcinoma.

Choroid plexus carcinomas (CPCs) are rare, aggressive pediatric brain tumors with no established curative therapy for relapsed disease, and poor survival rates. TP53 Mutation or dysfunction correlates with poor or no survival outcome in CPCs. Here, we report the case of a 4 month-old female who presented with disseminated CPC. After initial response to tumor resection and adjuvant-chemotherapy, the tumor recurred and metastasized with no response to aggressive relapse therapy suggesting genetic predisposition. This patient was then enrolled to a Molecular Guided Therapy Clinical Trial. Genomic

Impact of environmental tobacco smoke exposure on anaesthetic and surgical outcomes in children: a systematic review and meta-analysis.

BACKGROUND: Tobacco smoke exposure in adults is linked to adverse anaesthetic and surgical outcomes. Environmental tobacco smoke (ETS) exposure, including passive smoking, causes a number of known harms in children, but there is no established evidence review on its impact on intraoperative and postoperative outcomes. OBJECTIVES: To undertake a systematic review of the impact of ETS on the paediatric surgical pathway and to establish if there is evidence of anaesthetic, intraoperative and postoperative harm. ELIGIBILITY CRITERIA PARTICIPANTS: Children aged 0-18 years undergoing anaesthetic o

Intermittent Hypoxia Impairs Trophoblast Cell Viability by Triggering the Endoplasmic Reticulum Stress Pathway.

Intermittent hypoxia (IH) is a prominent characteristic of many clinical complications such as obstructive sleep apnea syndrome (OSAS). OSAS is related to a higher incidence of adverse pregnancy outcomes, and IH has been suggested as the preliminary physiological etiology. However, further studies remain to be performed on the underlying cellular and molecular pathogenic mechanisms of OSAS-related IH on adverse pregnancy outcomes. Here, we used a trophoblast cell line (HTR8/SVneo), primary extravillous trophoblast cells (EVTs), and a normal-term placenta villi explant culture model in vitro in

Prediction of the allergic mechanism of haptens via a reaction-substructure-compound-target-pathway network system.

Skin sensitization, frequently leading to allergic contact dermatitis (ACD), is authenticated to be a significant endpoint in the field of drug discovery and cosmetics. The initiation of ACD, also known as the skin sensitization mechanism, has been documented as an adverse outcome pathway (AOP), which can be studied experimentally and computationally. In this study, we collected 154 haptens and applied systems toxicology methods to develop a reaction-substructure-compound- target-pathway network system. For the collected haptens, their key substructures were identified and associated with thei

The Fontan Pathway: Change in Dimension and Catheter-Based Intervention over Time.

An unobstructed Fontan pathway is essential for optimal hemodynamics. We hypothesize that more extracardiac conduit (ECC) Fontan pathways develop obstruction compared to lateral tunnel (LT) Fontans and that the dilation typically observed in LTs results in similar mid-term clinical outcomes. A single-center, retrospective study was done including all Fontan cardiac catheterizations from 2006 to 2019. Angiography and medical records were reviewed to define Fontan pathway dimensions, interventions, and clinical outcomes. 232 patients underwent cardiac catheterization, where 60% were ECCs and 30%

Toxicological Mechanism of Individual Susceptibility to Formaldehyde-Induced Respiratory Effects.

Understanding the mechanisms of individual susceptibility to exposure to environmental pollutants has been a challenge in health risk assessment. Here, an integrated approach combining a CRISPR screen in human cells and epidemiological analysis was developed to identify the individual susceptibility to the adverse health effects of air pollutants by taking formaldehyde (FA) and the associated chronic obstructive pulmonary disease (COPD) as a case study. Among the primary hits of CRISPR screening of FA in human A549 cells, HTR4 was the only gene genetically associated with COPD susceptibility i

Preeclampsia link to gestational hypoxia.

Complications of pregnancy remain key drivers of morbidity and mortality, affecting the health of both the mother and her offspring in the short and long term. There is lack of detailed understanding of the pathways involved in the pathology and pathogenesis of compromised pregnancy, as well as a shortfall of effective prognostic, diagnostic and treatment options. In many complications of pregnancy, such as in preeclampsia, there is an increase in uteroplacental vascular resistance. However, the cause and effect relationship between placental dysfunction and adverse outcomes in the mother and

Effects of early life adversity on acute metabolic outcomes during neonatal development.

Early life adversity has lasting effects on neuroendocrine function and increases risk for psychopathology and metabolic disease. Stress and metabolism are closely intertwined, with critical periods for early life stress overlapping those for metabolic programming. Early life adversity models in rodents have the potential to affect development through multiple pathways, including nutrition, parental behavior, and direct physical stress. In the current study, we investigate how a limited bedding and nesting material (LBN) postnatal environment affects maternal behavior, milk composition, and ac

Wnt5a is associated with right ventricular dysfunction and adverse outcome in dilated cardiomyopathy.

The Wingless (Wnt) pathway has been implicated in the pathogenesis of dilated cardiomyopathy (DCM). To explore the role of Wnt modulators Wnt5a and sFRP3 in DCM patients we analyzed the expression of Wnt5a and sFRP3 in plasma and myocardium of DCM patients and evaluated their effects on NFAT luciferase activity in neonatal mouse cardiomyocytes. Elevated circulating Wnt5a (n = 102) was associated with increased pulmonary artery pressures, decreased right ventricular function and adverse outcome, with a stronger association in more severely affected patients. A higher Wnt5a/sFRP3 ratio (n = 25)

The safety of general anaesthesia in paediatric patients undergoing the application of Biobrane® for small scalds.

BACKGROUND: Each year more than 5000 children present to English and Welsh hospitals for the management of scalds; 60% of these are small scalds of less than 10% body surface area. There are no agreed UK care pathways for this injury. One method of management is to use a biosynthetic wound dressing after thorough wound cleaning. In children, this usually utilises general anaesthesia. This study investigates the incidence of adverse events during anaesthesia for the application of biosynthetic dressings in children with small-area scalds. METHODS: The medical records of 500 consecutive admissi

Biopesticide spinosad induces transcriptional alterations in genes associated with energy production in honey bees (Apis mellifera) at sublethal concentrations.

Bees experience substantial colony losses, which are often associated with pesticides. Besides synthetic insecticides biological compounds such as spinosad are used in agriculture and organic farming against insect pests. However, potential adverse effect at sublethal concentrations to pollinators are poorly known. Here we aim to determine potential adverse outcome pathways of spinosad and to identify molecular effects by investigating transcriptional alterations in the brain of honey bees. We experimentally exposed bees to three sublethal concentrations of 0.05, 0.5 and 5 ng spinosad/bee, and

Using qualitative data to build causal pathways on perceived associations between poor menstrual health and hygiene and harms among schoolgirls in western Kenya.

BACKGROUND: Poor menstrual health and hygiene (MHH) is a global public health concern, particularly in low- and middle-income countries. It can result in adverse health and educational impacts. Narratives from baseline focus group discussions (FGDs) conducted during a pilot intervention study in rural western Kenya were explored to reveal perceived associations. METHODS: Transcripts of 19 FGDs involving 87 post-menarche schoolgirls, 47 parents, and 46 teachers were analysed thematically to identify perceived causal links between menstrual issues and health and educational outcomes. Evaluation of bone safety and antiseizure medications: a multi-source pharmacovigilance and Mendelian randomization analysis.

The association between antiseizure medications (ASMs) and bone disorders remains inadequately investigated despite growing concern about drug-induced bone adverse events (AEs). We analyzed data from the FDA Adverse Event Reporting System (FAERS) database (Q1 2004-Q2 2025), identifying 7688 reports of bone health-related AEs listing ASMs as primary suspect drugs. Disproportionality analysis was performed using the reporting odds ratio (ROR) for signal detection. Subsequently, network pharmacology was applied to explore underlying biological pathways. Finally, two-sample Mendelian randomization

Impact of Intratympanic Gentamicin on Cochlear Implant Outcomes.

INTRODUCTION: Intratympanic gentamicin (ITG) instillation is a therapeutic option in advanced Menière's disease (MD) to control vertigo. ITG aims to induce vestibular hypofunction by selective ablation of vestibular hair cells. However, it may also adversely affect cochlear hair cells and the neural auditory pathway. Preservation of the neural auditory pathway is critical, as today cochlear implantation (CI) is the gold standard for auditory rehabilitation in patients with MD. This study assesses the impact of prior ITG treatment on CI outcomes in patients with MD. METHODS: This retrospective

Correlation of Sex With Adverse Outcomes Following Total Hip Arthroplasty: A Matched Cohort Analysis.

INTRODUCTION: Total hip arthroplasty (THA) is a commonly performed orthopaedic procedure, yet the correlation of patient sex with outcomes has been inadequately explored. Previous studies have been limited by population/database constraints. Thus, 90-day adverse event and 5-year outcome differences were assessed in a large national database. METHODS: Patients undergoing THA from the 2010 to 2022 M165 Ortho PearlDiver Mariner Database were identified. Male and female patients were matched 1:1 based on age and Elixhauser Comorbidity Index. Ninety-day adverse events were assessed and compared wi

Trajectories of risk and resilience: The role of empathy and perceived social support in the context of early adversity.

BACKGROUND: Evidence overwhelmingly suggests that adverse childhood experiences (ACEs) is a risk factor for poor mental health outcomes. However, the specific mechanisms via which ACEs confer an increased risk of psychopathology are less well understood. OBJECTIVE: The study modelled the effect of empathy and perceived social support (PSS) on mental health outcomes in a mixed clinical and non-clinical population, within the context of exposure to ACEs. PARTICIPANTS AND SETTING: A total of 575 participants (comprising a treatment-receiving and community-based sample), aged 18 to 65 completed

Noninvasive Testing for Metabolic Dysfunction-Associated Steatohepatitis (MASH) and Fibrosis.

Patients with metabolic dysfunction-associated steatohepatitis (MASH), particularly those with clinically significant fibrosis (≥F2), are at highest risk for adverse liver outcomes. This narrative review summarizes blood-based scores, imaging tools, and combined indices used to detect and risk stratify patients with MASH and fibrosis. We focus on cutoffs and dynamic trajectories that best capture patients at highest risk for adverse liver events and outline how these noninvasive tests can be integrated into simple stepwise pathways to guide referral and treatment and reduce reliance on liver b
