The prevalence of amblyopia in 7-year-old schoolchildren in Iran.

PURPOSE: To determine the prevalence of amblyopia in schoolchildren aged 7 years in Iran, its relation with refractive errors, and its determinants. METHODS: In this cross-sectional study, cluster sampling was done from elementary school students in 7 cities in Iran. In all schools, an optometrist conducted all tests, including measurement of uncorrected and corrected visual acuity, cycloplegic refraction, and cover test. In this study, amblyopia was defined as best corrected visual acuity 20/30 or less or a 2-line interocular optotype acuity difference with no pathology. RESULTS: Of the 415

Anisometropic amblyopia in a case of type 2 Waardenburg syndrome.

This study presents a case of an 8-year-old boy with iris heterochromia and anisometropic amblyopia who was diagnosed with Waardenburg syndrome (WS) type 2. An ophthalmic examination revealed iris heterochromia and anisometropic amblyopia in our patient. In the systemic examination, a white forelock and vitiligo on the arms and body were observed and neurosensory hearing loss was revealed, for which the patient used hearing aids. Identification and typing of patients with WS is crucial to address neurosensory hearing loss, glaucoma and fundus changes. While it might be challenging to communica

Sparing of coarse stereopsis in stereodeficient children with a history of amblyopia.

Stereoscopic depth perception may be obtained from small retinal disparities that can be fused for single vision (fine stereopsis), but reliable depth information is also obtained from larger disparities that produce double vision (coarse stereopsis). Here we assess the possibility that the early development of coarse stereopsis makes it resilient to the factors that cause amblyopia by comparing performance in children with a history of strabismic, anisometropic, or aniso-strabismic amblyopia and age-matched controls (5-12 years). The task was to indicate whether a cartoon character was nearer

The association of refractive error, strabismus, and amblyopia with congenital ptosis.

PURPOSE: To determine the frequency of amblyopia and its associations among children with congenital ptosis. METHODS: A retrospective chart review of 92 active patients with congenital ptosis in the practices of 2 of the authors. The main outcome measures were relative eyelid ptosis severity, presence and type of strabismus, refractive error, and amblyopia. RESULTS: Amblyopia was identified in 22 of 92 patients (23.9%), in almost every case occurring in the context of coexisting anisometropia or strabismus and affecting the eye with the more severe ptosis (P = 0.0001). Amblyopia was more lik

The effect of recent amblyopia research on current practice in the UK.

BACKGROUND/AIMS: Several studies have recently provided insights into how amblyopia may be most effectively managed. Despite the new evidence, a US study reported that a recent randomised controlled trial had made little influence on clinical practice. The aims of this research are to assess current practice of amblyopia management in the UK and to determine the comparability with the evidence-based recommendations. METHODS: A questionnaire was constructed to assess current amblyopia management practice, particularly in relation to areas investigated by recent research and emailed to every he

Brain plasticity in the adult: modulation of function in amblyopia with rTMS.

Amblyopia is a cortically based visual disorder caused by disruption of vision during a critical early developmental period. It is often thought to be a largely intractable problem in adult patients because of a lack of neuronal plasticity after this critical period [1]; however, recent advances have suggested that plasticity is still present in the adult amblyopic visual cortex [2-6]. Here, we present data showing that repetitive transcranial magnetic stimulation (rTMS) of the visual cortex can temporarily improve contrast sensitivity in the amblyopic visual cortex. The results indicate conti

The Amblyopia and Strabismus Questionnaire: English translation, validation, and subscales.

PURPOSE: To establish the English-language version of the Amblyopia and Strabismus Questionnaire (ASQE). DESIGN: Prospective cohort study. METHODS: A structured translation process was followed to generate the ASQE, a 26-item instrument (originally in Dutch) containing five subscales for fear of losing the better eye, distance estimation, visual disorientation, double vision, and social contact and appearance. The ASQE was administered to 150 adults in a private practice setting. All had strabismus with or without amblyopia and visual acuity of 20/50 or better in at least one eye. Subjects a

Relating binocular and monocular vision in strabismic and anisometropic amblyopia.

OBJECTIVES: To examine deficits in monocular and binocular vision in adults with amblyopia and to test the following 2 hypotheses: (1) Regardless of clinical subtype, the degree of impairment in binocular integration predicts the pattern of monocular acuity deficits. (2) Subjects who lack binocular integration exhibit the most severe interocular suppression. METHODS: Seven subjects with anisometropia, 6 subjects with strabismus, and 7 control subjects were tested. Monocular tests included Snellen acuity, grating acuity, Vernier acuity, and contrast sensitivity. Binocular tests included Titmus

Non-concordance in amblyopia treatment: the effective use of 'smileys'.

PURPOSE: The success of any specific occlusion regimen is limited by the level of concordance, which is difficult to measure accurately in patching treatment. The aim of this study was to investigate prospectively the concordance rate of a group of children with strabismic and/or anisometropic amblyopia using a schematic diary based on completing 'smiley' images, filled in by the child under parental supervision, and to assess the effect of correlates such as initial visual acuity, age, and total and daily occlusion time on concordance. METHODS: We recruited 51 amblyopic children aged 13 mont

[Some aspects of the current controversy about the treatment of amblyopia].

Occlusion has been used for centuries for treating amblyopia but in recent years an intensive debate about the treatment has arisen. The aim of this article is to summarize some of the new aspects of this discussion: on the one hand the effect of occlusion treatment during childhood is being questioned and on the other hand experimental approaches are being tested which are aimed at treating amblyopia in adults. Evidence from own experimental work is presented showing the influence of occlusion on the spatial localization of visual stimuli. A continuation of research on amblyopia and the effec

The history of research into the disorders of reading and accommodation in strabismic amblyopia.

During the past 150 years, research on amblyopia and nystagmus has developed in five different waves (W1-W5). Amblyopia was initially regarded to be enigmatic and believed to be related to eccentric fixation (W1, after 1850); later, it was ascribed to disorders in refraction or accommodation (W2, around 1900). After 1900 (W3), phylogenetic explanations appeared, and after 1945 (W4) concepts based on successful orthoptic practice came to the fore. Since 1965 (W5), research on amblyopia has been guided by modern insights into neuroanatomy and neurophysiology. In contrast to amblyopia, nystagmus

Congenital ptosis and amblyopia: a retrospective study of 130 cases.

PURPOSE: To provide supplementary data on the association between congenital ptosis and amblyopia. METHODS: In a retrospective chart study of 130 patients whose congenital ptosis was surgically corrected between 1987 and 1999, 27 (20.8%) had strabismus and 30 (23%) had amblyopia. In 9 patients (6.3%), it was not attributable to any cause except ptosis. RESULTS: There was a high incidence (6.9%) of amblyopia in patients with congenital ptosis. CONCLUSION: We recommend early examination and follow-up of patients with congenital ptosis to diagnose and treat any subsequent amblyopia. Surgery ma

Effect of ongoing treatment of amblyopia on surgical outcome in esotropia.

PURPOSE: Classic teaching recommends completion of amblyopia therapy prior to surgical correction of esotropia. Recent reports, however, suggest that incomplete treatment does not adversely affect surgical outcome. This study assesses the effect of incompletely treated amblyopia on the success rate of bimedial rectus recession in infantile and acquired esotropia. METHODS: All patients (n = 102) with esotropia undergoing bimedial rectus recession in 1994 who met inclusion criteria were reviewed. Subjects were classified as having infantile; acquired, partially accommodative; or acquired, nonac

Posterior polymorphous dystrophy associated with astigmatism and amblyopia in children.

BACKGROUND: Posterior polymorphous dystrophy has been described in the scientific literature as having more than a casual association with keratoconus in adults. We studied four consecutive children with posterior polymorphous dystrophy who had coexisting astigmatism. In our patients, the posterior changes appeared to be associated with the corneal astigmatism, as evidenced by videokeratography. METHODS: Four consecutive cases of posterior polymorphous dystrophy in children were found over a 3-year period by one of the authors (PD). They presented with varying degrees of amblyopia in the eyes

Amblyopia and suppression in binocular cortical neurones of strabismic cat.

The neural mechanism of strabismic amblyopia was investigated by recording the spatio-temporal properties of striate cortical neurones of cats raised with a surgically induced esotropia. Multiple pseudo-random flashed or phase-alternating stimuli were used, and through cross-correlation, the Wiener kernels were extracted. This analysis has shown that in neurones which retain binocularity to adulthood, two different neural mechanisms of adaptation to ocular misalignment exist. Some of these neurones become amblyopic, such that the non-deviating eye dominates the response for high spatial freque

A study of amblyopia in 18-19 year old males.

During a vision screening of 6,556 National Service pre-enlistees aged 18-19 years at the Medical Classification Centre of Central Manpower Base, 48 subjects were found to have visual acuity of 6/12 or less in one or both eyes in the absence of ocular pathology. The prevalence of amblyopia in this population is 0.73%. Amblyopia was due to anisometropia in 24 cases (50%), strabismus in 9 cases (18.7%), high astigmatism (meridional) in 7 cases (14.5%) and other causes or a combination of factors in 8 cases (16.7%). Strabismic amblyopia is most commonly associated with esotropia. There is a posit

Effects of enucleation of the fixating eye on strabismic amblyopia in monkeys.

The effects of enucleation of the fixating eye on the visual function of the deviating eye were studied in two rhesus monkeys with strabismic amblyopia. An esotropia was surgically induced when the monkeys were approximately 1 mo of age, and the fixating eyes were then enucleated at age 3 yr 11 mo. Four measures of visual function (photopic increment-threshold spectral sensitivity, scotopic spectral sensitivity, spatial modulation sensitivity, and temporal modulation sensitivity) were determined for both eyes prior to enucleation and for the deviating eyes over an 11-month period following the

[Differential diagnosis of amblyopia and retrobulbar neuritis with electroophthalmologic methods].

The electroretinogram (ERG), the brightness visual evoked cortical potential (H-EP) and the pattern reversal visual evoked cortical potential (M-EP) were recorded from 7 patients with amblyopia and 5 patients with neuritis retrobulbaris. The ERGs and the H-EPs of the amblyopic eyes were normal whereas the amplitude of the M-EP was significant diminished. The parameters of the ERG recorded from the patients with neuritis retrobulbaris showed normal values, however the H-EPs were significantly decreased and the peaktimes were delayed. Also a decrease in the amplitudes of the M-EPs was noticed. T
