Septic Arthritis of the Knee with Toxoplasma gondii in a Patient with Rheumatoid Arthritis.

Toxoplasmosis is a parasitic infection that may affect several viscera. The locomotor system is rarely involved, and no case has been published of a septic arthritis in which Toxoplasma gondii was identified in the cultures of the joint fluid and of the synovial specimen. We describe a patient with septic arthritis of the knee in which Toxoplasma gondii was found by culture in a patient with seropositive rheumatoid arthritis treated with corticosteroids and gold salts who developed multi-visceral toxoplasmosis.

Knee arthritis revealing acute leukemia in a patient with rheumatoid arthritis.

We report one case of leukemic synovitis in a patient with a 40-year history of rheumatoid arthritis. The synovial fluid sample and synovial biopsy specimen showed myeloblastic cells. So, leukemic synovitis can be suspected in a patient with inflammatory rheumatism and adequate diagnostic procedures should be carried out.

Antibodies to DNA in patients with rheumatoid arthritis and juvenile rheumatoid arthritis.

Antibodies to double-stranded DNA (DSDNA) were found in 18 patients with RA, in 5 patients with JRA, and in 5 patients with undiagnosed connective tissue disease. Five patients had clinical features consistent with both RA and SLE, 11 with only RA, and 5 with only JRA. Based on these observations, the presence of serum anti-DSDNA antibodies should not be used as a sole criterion in the diagnosis of SLE.

Unmet Needs and Current Challenges of Rheumatoid Arthritis: Difficult-to-Treat Rheumatoid Arthritis and Late-Onset Rheumatoid Arthritis.

Despite remarkable advances in the management of RA, there are still unmet needs that rheumatologists need to address. In this review, we focused on difficult-to-treat RA (D2T RA) and late-onset RA (LORA), and summarized their characteristics and management. The prevalence of D2T RA is reported to be 6-28% and many factors have been identified as risk factors for D2T RA, including female sex, long disease duration, seropositivity for rheumatoid factor and anti-cyclic citrullinated peptide antibody and their high titer, baseline high disease activity, and comorbidities. D2T RA is broadly divide

Juvenile Idiopathic Arthritis.

Juvenile idiopathic arthritis (JIA) is the most common rheumatic disease in children. The International League of Associations for Rheumatology (ILAR) has defined JIA as "arthritis of unknown etiology persisting for ≥6 wk with an onset at <16 y of age, after excluding other causes of joint inflammation". Synovial inflammation is the result of a complex interplay of aberrant immune systems (both adaptive and innate) in a genetically susceptible individual, with possible external stimuli/triggers. Diagnosis of JIA essentially remains clinical, and laboratory investigations usually help to assess

Treating rheumatoid arthritis.

Rheumatoid arthritis (RA) affects about 1% of the world's population and can lead to loss of joint function, reduced mobility, and permanent damage to cartilage and bone. Treatment options for RA primarily include disease-modifying antirheumatic drugs (DMARDs) such as methotrexate, but the development of new drugs has complicated treatment decisions. Weighing treatment options for patients with RA largely depends on three major factors: efficacy, adverse reaction profile, and cost. A review of the literature supports methotrexate monotherapy as the current best-practice model for treating RA,

Juvenile Idiopathic Arthritis.

Juvenile idiopathic arthritis (JIA) comprises a group of heterogenous disorders characterized by childhood-onset chronic joint inflammation. It is the most common rheumatologic disease in the pediatric population and an important cause of chronic illness in children. Early recognition and treatment are vital to prevent sequelae of uncontrolled inflammation on the developing skeleton. JIA can have significant complications that general pediatricians should be aware of, especially uveitis, which can be insidious and asymptomatic in very young children, and macrophage activation syndrome, which c

Juvenile idiopathic arthritis.

Juvenile idiopathic arthritis (JIA) is an umbrella term for arthritis of unknown origin, lasting for >6 weeks with onset before 16 years of age. JIA is the most common chronic inflammatory rheumatic condition of childhood. According to the International League Against Rheumatism (ILAR) classification, seven mutually exclusive categories of JIA exist based on disease manifestations during the first 6 months of disease. Although the ILAR classification has been useful to foster research, it has been criticized mainly as it does not distinguish those forms of chronic arthritis observed in adults

Axial Psoriatic Arthritis.

PURPOSE OF REVIEW: To review current understanding of the prevalence, clinical features, outcome measures and recent therapeutic trials in axial psoriatic arthritis (axPsA). RECENT FINDINGS: The prevalence of axPsA is estimated at 40-50%. However, the definition of axPsA remains unclear, therefore these estimates may be inaccurate. Ax PsA appears to be distinct from ankylosing spondylitis in demographic, clinical, genetic and therapeutic features. Because of the lack of widely accepted definition of axPsA it has been difficult to design therapeutic trials for this domain of PsA. Several studi

[Thiamazole-induced arthritis].

We report the case of a 42-year-old male patient with acute onset of asymmetrical polyarthritis of the medium and large joints as well as fever and elevated serological inflammation markers. The symptoms began shortly after initiation of thiamazole treatment for newly diagnosed Graves' disease. Antithyroid arthritis syndrome (AAS) is a rare but serious adverse side effect of antithyroid treatment with thioamides such as thiamazole. Clinically, AAS may present with myalgia, arthralgia, fever, exanthema and polyarthritis. In the case of suspected AAS, when possible the thionamide medication shou

[Juvenile idiopathic arthritis].

Juvenile idiopathic arthritis. Juvenile idiopathic arthritis (JIA) is the most common chronic inflammatory rheumatic disease in childhood. It comprises a heterogeneous group of disorders characterized by joint inflammation, extraarticular features including uveitis and, in some cases laboratory markers. Disease course, therapeutic management and prognosis vary depending on the underlying JIA category. Recent treatment advances, and especially the use of biologic agents, have significantly reduced JIA-associated morbidity. Current treatment approaches emphasize an early aggressive therapy aimin

Rheumatoid arthritis revisited.

The generally accepted theory of the etiopathology of rheumatoid arthritis has not led to a breakthrough so far, and the root cause of RA is still unknown. Surgical experience does not support the idea that the damage in the joint is caused by synovitis. Synovial tissue does not spread on the surface of the cartilage and thus destroy it, and neither is the erosion caused by the synovial tissue. Macroscopically and microscopically synovitis in RA is no different to synovitis in arthrosis. Perhaps the etiopathogenesis should once more be reconsidered.

[Juvenile idiopathic arthritis].

Juvenile idiopathic arthritis (JIA) is the most common cause of chronic inflammation of the joints in childhood. Currently, JIA is divided into 7 subtypes, distinguished on the basis of the symptoms present in the first six months of the illness. Pharmacological treatment is different for every subtype. With all forms of JIA, dental problems can occur. These can include an increasing incidence of dental caries, stomatitis with the use of methotrexate, oral candidiasis with the use of immunosuppressive medication and temporal mandibular joint (TMJ) arthritis. The detection of TMJ arthritis seem

Antithyroid arthritis syndrome.

Antithyroid arthritis syndrome is a constellation of symptoms of myalgia, arthralgia, arthritis, fever and rash associated with the use of antithyroid medications. We report a case of a patient with severe hyperthyroidism likely secondary to Graves' disease who presented with the abovementioned symptoms after being treated with methimazole (antithyroid medication). Our aim is to increase awareness regarding this uncommon but disabilitating and life-threatening adverse effect of antithyroid medications among clinicians. We also discuss the proposed pathophysiology for this immunological reactio

[JUVENILE IDIOPATHIC ARTHRITIS].

Juvenile idiopathic arthritis (JIA) is the most common rheumatic disorder in children and one of the most common causes of part-time or long-term disability. The term juvenile idiopathic arthritis defines the main characteristics of the disease: joint inflammation of unknown origin manifested before the 16th birthday and lasting for more than six weeks. JIA is very rare in infancy, with highest frequency in preschool age. It is not a single disease, but a group of disorders with some common features of different immunopathogenesis and with different clinical manifestations. According to the re

[Childhood septic arthritis].

Childhood acute septic arthritis is most often of hematogenous origin, and usually caused by Staphylococcus aureus. Characteristic symptoms and signs include a swollen, red painful joint, and fever. The diagnosis is confirmed by a joint aspiration. Following a brief 2- to 4-day intravenous phase, the antibiotic course is completed orally to a total of 10 to 14 days. Cephalosporins, clindamycin or staphylococcal penicillins, administered every 6 hours, are recommended as first-line antibiotics because of their appropriate spectrum, excellent penetration, good tolerability in large doses, and mo
