Bipolar disorder--methodological problems and future perspectives.

Since its "rebirth" in 1966, bipolar disorder (BPD) has rapidly come to occupy a central position in the research and treatment of mood disorders. Compared with major depressive disorder (MDD), BPD is a more serious condition, characterized by much more frequent recurrence, more complex comorbidity, and higher mortality. One major problem is the lack of valid definitions in adult and in child psychiatry; the current definitions are unsatisfactory, and heavily favor an overdiagnosis of MDD. Biological research is partially based on those definitions, which have a short half-life. An additional,

Sleep disturbance in bipolar disorder: therapeutic implications.

In this review, the authors detail our current understanding of the crucial role that sleep and its disturbances play in bipolar disorder. Multiple lines of evidence suggest that impaired sleep can induce and predict manic episodes. Similarly, treatment of sleep disturbance may serve as both a target of treatment and a measure of response in mania. The depressive phase of bipolar illness is marked by sleep disturbance that may be amenable to somatic therapies that target sleep and circadian rhythms. Residual insomnia in the euthymic period may represent a vulnerability to affective relapse in

Aripiprazole: pharmacology and evidence in bipolar disorder.

Aripiprazole is a psychotropic agent with a novel pharmacologic profile. The purpose of this article is to review the pharmacology, efficacy and safety of aripiprazole in bipolar disorder. The authors conducted a PubMed search of all English-language articles published between January 1995 and February 2007. The key search term was 'aripiprazole' combined with: 'randomized controlled trial', 'pharmacology', 'pharmacokinetics', 'pharmacodynamics', 'depression', 'mania', 'maintenance' and 'bipolar disorder'. Abstracts and proceedings from national and international psychiatric meetings were also

Diagnosis and classification of pediatric bipolar disorder.

BACKGROUND: Many children and adolescents with apparent bipolar disorder cannot be meaningfully diagnosed using the DSM-IV. The variety of pediatric bipolar phenotypes observed in clinical practice remains unclarified. METHOD: 130 consecutively evaluated bipolar children and adolescents were assessed using semistructured clinical interviews and operational criteria that abandoned adherence to the DSM-IV cardinal symptom, duration of symptom persistence, and episodicity requirements. RESULTS: 97.6% of the total sample manifested either all three, or two of the three symptoms elation, grandios

Immune variations in bipolar disorder: phasic differences.

OBJECTIVES: To characterize the immunological variations of patients with a bipolar disorder (BD) diagnosis in manic (BDm) and depressive (BDd) phases, by the quantification of the serum levels of interleukin (IL)-1beta, -2, -4, -6 and tumor necrosis factor alpha (TNF-alpha). METHODS: Twenty physically healthy patients with a BD type I diagnosis and 33 matched controls were studied, after giving informed consent. The inclusion criteria included at least three weeks without any kind of psychopharmacological treatment, Young Mania Rating Scale score > or =20 for BDm (n = 10) and Hamilton Depres

Bipolar spectrum disorder: differential diagnosis and treatment.

Bipolar disorder commonly presents as a recurrent mood disorder characterized by frequent depressive episodes. Although manic or hypomanic phases are required for the diagnosis to be made based on current diagnostic criteria, a wider expression of mood instability and other historical features or family history may suggest the presence of a bipolar spectrum illness. This article covers the diagnostic issues related to bipolar disorder and the spectrum concept of the illness. A new definition of bipolar spectrum disorder is suggested, and treatment principles and options are discussed. Primary

[Olanzapine in the treatment of bipolar disorder].

Olanzapin is an atypical antipsychotic drug, which is approved for use in acute mania and in the prophylaxis of bipolar disorder. There are eight double-blind, placebo-controlled studies, which show its efficacy in the treatment of acute mania, five, which document its efficacy in the long-term treatment of bipolar disorder and one that shows its use in bipolar depression. The following overview presents and critically evaluates these studies. Quality of life aspects on olanzapine treatment and the safety profile of the drug will also be discussed.

Suicide in bipolar disorder: Risks and management.

Bipolar disorders are prevalent, often severe, and disabling illnesses with elevated lethality largely due to suicide. Suicide rates average approximately 1% annually, or perhaps 60 times higher than the international population rate of 0.015% annually. Suicidal acts typically occur early in bipolar disorders and in association with severe depressive or mixed states. The high lethality of suicidal acts in bipolar disorders is suggested by a much lower ratio of attempts:suicide (approximately 3:1) than in the general population (approximately 30:1). Risk factors can help to identify patients at

Psychopharmacology of pediatric bipolar disorder: a review.

RATIONALE: Pediatric bipolar disorder (PBD) is a chronic and debilitating psychiatric illness. It is associated with many short-term and long-term complications including poor academic and social performance, legal problems and increased risk of suicide. Moreover, it is often complicated by other serious psychiatric disorders including attention deficit hyperactivity disorder, oppositional defiant disorder, conduct disorder and substance use disorders. For these reasons, there is a need for effective treatment for PBD. OBJECTIVES: To review available data from published reports of the treatme

Carbamazepine extended-release capsules in bipolar disorder.

Carbamazepine (CBZ) has long been a therapeutic option for bipolar disorder. Carbamazepine extended-release capsules (CBZ-ERC) are a recent formulation of CBZ approved by the US Food and Drug Administration in 2004 for the treatment of acute manic and mixed episodes associated with bipolar I disorder. This new formulation was developed to improve dosing convenience and decrease daily fluctuations in serum CBZ concentration, thereby lowering the incidence of adverse events. Two randomized, double-blind, placebo-controlled trials and an open-label extension study have demonstrated that CBZ-ERC m

Mood patterns and classification in bipolar disorder.

PURPOSE OF REVIEW: The aim of this review is to highlight recent studies that have questioned the current split of mood disorders into the categories of bipolar and depressive disorders. RECENT FINDINGS: A continuity between bipolar disorders (mainly bipolar II disorder) and major depressive disorder was supported by several lines of evidence: depressive mixed states (mixed depression) and dysphoric (mixed) hypomania (opposite polarity symptoms in the same episode do not support the splitting between mania/hypomania and depression); family history, major depressive disorder is the most common

Recognizing and managing bipolar disorder in children.

Bipolar disorder affects people of all ages, including preschool-aged children. Two major difficulties in diagnosing children with bipolar disorder are its overlap with attention-deficit/hyperactivity disorder (ADHD) and its developmentally distinct presentation from that in adults, with high rates of irritability, chronicity, and mixed states. Comorbid conditions are common in bipolar disorder and, in addition to ADHD, include depression, anxiety disorders, oppositional defiant disorder, and conduct disorder. Family studies have helped to confirm the validity of bipolar disorder in children.

Extended-release carbamazepine (Equetro) for bipolar disorder.

An extended-release formulation of carbamazepine, available since 1997 for treatment of epilepsy, has now been approved under a new name, Equetro, for acute mania and mixed episodes of bipolar disorder. Although the drug was effective in some patients for up to 6 months, it has not been approved for maintenance treatment. Carbamazepine has not been shown to be more effective than lithium or valproate, and it can cause serious adverse effects.

Efficacy of atypical antipsychotics in bipolar disorder.

Bipolar disorder is a severe and recurrent disorder. Atypical antipsychotics have emerged as both an alternative and adjunct to conventional mood stabilisers. The manic phase of the illness is the best studied, and it appears that a class effect with regards to efficacy is present in both monotherapy and augmentation studies. Evidence for efficacy of atypical antipsychotics in depression is emerging. At this stage controlled data are available for both olanzapine and quetiapine. Maintenance data demonstrating efficacy are available for olanzapine. Atypical antipsychotics have utility in treati

Psychiatric and medical comorbidities of bipolar disorder.

OBJECTIVES: This review summarizes the literature on psychiatric and medical comorbidities in bipolar disorder. The coexistence of other Axis I disorders with bipolar disorder complicates psychiatric diagnosis and treatment. Conversely, symptom overlap in DSM-IV diagnoses hinders definition and recognition of true comorbidity. Psychiatric comorbidity is often associated with earlier onset of bipolar symptoms, more severe course, poorer treatment compliance, and worse outcomes related to suicide and other complications. Medical comorbidity may be exacerbated or caused by pharmacotherapy of bipo

[Clinical picture and diagnosis of bipolar disorder].

Since more than one century, the illness considerably evolved. Between the extremes of full-blown manic-depressive disorder and the strictly defined unipolar depression, patients exhibit particular affective conditions characterised by complex or soft symptoms. So the actual classification encompasses 6 clinical forms with sometimes, sub-forms. In this context, an awareness of the different clinical presentations is an important issue in the daily practice, especially hypomania, cyclothymia, mixed states and affective episode with delusion. These conditions are often misdiagnosed and the bipol

Bipolar I disorder, social support and work.

OBJECTIVES: This article reports the estimated lifetime prevalence of bipolar I disorder in the household population and describes characteristics of people of working age (25 to 64) affected by this disorder. The relationship between social support and employment status is examined in people with the disorder. DATA SOURCE: Data are from the 2002 Canadian Community Health Survey: Mental Health and Well-being. ANALYTICAL TECHNIQUES: Weighted frequencies and cross-tabulations were used to estimate the prevalence of bipolar I disorder. Multiple logistic regression modeling was used to examine f

Safety considerations in pharmacotherapy of bipolar disorder.

Bipolar disorder is a chronic, frequently relapsing illness with a prevalence of 1.2% to 3.4% in the general population. It is associated with high disability, higher comorbidity due to medical illnesses, and significant social and economical consequences for patients, their families, and society. The episodic nature of this disease warrants rational use of medications and proper monitoring for adverse events. Various drug classes, such as mood stabilizers, antipsychotics, benzodiazepines, and antidepressants, are used for the acute and maintenance treatment of bipolar disorder. Each group of

Facial affect recognition deficits in bipolar disorder.

Patients diagnosed with bipolar disorder (BPD), by definition, have problems with emotional regulation. However, it remains uncertain whether these patients are also deficient at processing other people's emotions, particularly while manic. The present study examined the ability of 25 manic bipolar patients and 25 healthy participants on tasks of facial recognition and facial affect recognition at three different presentation durations: 500 ms, 750 ms, and 1000 ms. The groups did not differ in terms of age, education, sex, ethnicity, or estimated IQ. The groups did not differ significantly on
