Blood components in neonatal medicine.

During a six-month period, 552 infants were admitted to a level III Neonatal Intensive Care Unit (NICU) and four level II units. Of the 953 cross matches of blood components requested from the blood bank, 83% were administered. Forty-four percent of the infants received multiple blood transfusions from multiple blood donors (mean 3.9 donors, range two to 11). Thirty-one percent received multiple transfusions from only one donor. The mean total volume of blood given was 22.5 ml (range 3 to 120 ml), exclusive of blood used for exchange transfusions. Seventy percent of the blood components were g

Immune complications associated with chronic transfusion.

Blood transfusion and component therapy are an integral part of treatment for patients with certain life-threatening illnesses. Any benefit derived from transfusion must be weighed against the potential risk for complications. Patients who are chronically transfused present a unique challenge to practitioners. The presentation will focus on alloimmunization to red blood cell antigens, platelet refractoriness, transfusion-associated graft-versus-host disease, and cytomegalovirus infection. Prevention or management of these complications also will be discussed.

Transfusion-related acute lung injury in multiple traumatized patients.

BACKGROUND: Many of the multiple traumatized patients who refer to the hospital need transfusion. Transfusion-related acute lung injury (TRALI) is a serious clinical syndrome associated with the transfusion of plasma-containing blood components. In the article, we present a case of TRALI following transfusion of packed red blood cells Case Presentation: A 24 year old male referred to Shahid Beheshti Hospital due to multiple trauma with left femoral and humerus fractures. Due to severe anemia he received 3 units of packed red blood cells. The symptoms of TRALI began 2 hours after transfusion. H

Adjuncts to Blood Component Therapies for the Treatment of Bleeding in the Intensive Care Unit.

Patients who are critically ill following surgical or traumatic injury often present with coagulopathy as a component of the complex multisystem dysfunction that clinicians must rapidly diagnose and treat in the intensive care environment. Failure to recognize coagulopathy while volume resuscitation with crystalloid or colloid takes place, or an unbalanced transfusion strategy focused on packed red blood cell transfusion can all significantly worsen coagulopathy, leading to increased transfusion requirements and poor outcomes. Even an optimized transfusion strategy directed at correcting coagu

Current strategies to prevent transmission of prions by human plasma derivatives.

Protein products prepared from pooled human plasma are an essential class of therapeutics used mostly to control bleeding and/or immunological disorders. Because of the human origin of the starting material, there is a risk that these products may possibly transmit prions causing variant Creutzfeldt-Jakob disease (vCJD). No case of transmission of prions by plasma products has been observed. Case-by-case measures implemented in various countries, and several technical factors may contribute, to various degrees, to the prevention of the risk of transmission of prions by plasma products. Those m

Fresh frozen plasma in patients with disseminated intravascular coagulation or in patients with liver diseases.

Disseminated intravascular coagulation (DIC) and liver diseases are complex clinical conditions. Both disorders frequently disturb the finely tuned coagulation and fibrinolysis equilibrium. In DIC, a wide range of underlying disorders can induce a systemic activation of the coagulation system with generation of soluble fibrin, possible deposition of platelet-rich fibrin clots in the microvasculature and subsequent micro- or macroembolism, impaired organ perfusion and organ failure. Such coagulation activation depletes platelets, coagulation factors, and inhibitors and clinically can result in

Filtration of platelet concentrates.

The transfusion of leukocyte-containing blood products causes a number of side reactions, which among others induces alterations of the immune system of the recipient leading to alloimmunization or immunosuppression. Antibodies against antigenic determinants of the major histocompatibility complex-proteins of class I are the major cause of alloimmunization. Though platelets are easy targets for HLA antibodies, they cannot induce the antibody production themselves. This requires leukocytes, carrying the MHC-proteins of the class II. Standard platelet concentrates for the clinical routine have m

Blood donation and transfusion practices: the 1990 American Association of Blood Banks Institutional Membership Questionnaire.

Responses to the 1990 American Association of Blood Banks (AABB) Institutional Membership Questionnaire were submitted by 2126 regional blood centers, hospital-based blood banks, and transfusion facilities. Data from 2117 of these facilities were considered to be valid. The questionnaire included information on blood donor demographics, number of units collected, and collection procedures; services performed; usage of blood components; and transfusion-transmitted diseases reported during 1989. Institutional members collected 7.4 million whole blood units, of which 90.8 percent were donated for

Improvement in fresh frozen plasma transfusion practice: results of an outcome audit.

Blood components have been in use in clinical practice for many decades now. In spite of fairly clear guidelines regarding their use, inappropriate prescriptions for components are still rampant. We undertook this work to assess the appropriateness of fresh frozen plasma (FFP) transfusions in our hospital. A prospective audit of 504 transfusion orders for 1761 FFP units was conducted over a 6-month period which was followed by a re-audit of 294 FFP prescriptions for 961 units. In the initial audit, we identified 304 (60.3%) prescriptions which were inappropriate according to the British Commit

Safety of a universal, virus-inactivated and prion-depleted, pharmaceutical-quality plasma: a randomized, double-blind, clinical trial in healthy volunteers.

BACKGROUND: Universal plasma is intended to be transfused irrespective of the blood group. We compared the safety and tolerability of a novel, universal blood group-independent plasma with an ABO-matched plasma. STUDY DESIGN AND METHODS: In this randomized, double-blind, active-controlled, crossover, Phase I trial, 30 healthy adult volunteers (blood group A, B, or AB) were randomly assigned to transfusion of 1200 mL of Uniplas LG and 1200 mL of Octaplas LG (both Octapharma AG) or vice versa. In both periods, plasmapheresis (PPh, 600 mL) preceded the infusion. Blood samples were drawn before a

A new apheresis procedure for the preparation of high-quality red cells and plasma.

BACKGROUND: Multicomponent apheresis is an alternative way of preparing blood components that avoids the delay between collection and separation seen with standard whole-blood techniques. STUDY DESIGN AND METHODS: An apheresis device has been modified to facilitate the combined collection of a unit (250 mL) of red cells (RBCs) and a high-volume unit (475 mL) of plasma. The procedure, using 8-percent ACD-A, has been tested in two European blood centers. Each center performed 20 procedures for in vitro evaluation of collected RBCs and plasma and 10 procedures for evaluation of in vivo RBC recov

Universal leukocyte reduction.

Leukocyte reduction of blood components, in the United States, is generally reserved for conditions in which a clinical indication has been documented. There is no evidence that either Creutzfeldt-Jakob disease or variant Creutzfeldt-Jakob disease are transmitted by transfusion in humans or that leukocyte reduction of blood components could reduce their transmission. A number of adverse outcomes following transfusion are alleged to be the result of white blood cells. At this point in time, there are insufficient clinical data to justify the universal leukocyte reduction of blood components.

Pre-hospital blood transfusion in non-traumatic major haemorrhage: a retrospective observational study.

BACKGROUND: Blood component administration for major traumatic haemorrhage is established clinical practice in pre-hospital emergency medicine. However, transfusion practices for non-traumatic haemorrhage lack specific guidance. We analysed historical data on non-traumatic major haemorrhage to describe the incidence and current blood product administration practices. METHOD: This retrospective observational cohort study reviewed Air Ambulance Charity Kent Surrey Sussex patient data from 11 November 2013 to 9 August 2024. The cohort included patients receiving pre-hospital blood components for

[Variations in transfusion practice in Spain].

OBJECTIVES: 1) To study the variations in the use of erythrocyte, platelet and plasma transfusion in Spain between regions, within a single region (Andalusia), and between hospitals; and 2) To determine any correlation between transfusion and blood donor rates within these areas. MATERIAL AND METHODS: The data were gathered from the Administrations of 17 regions, 8 Andalusian provinces, and 30 Andalusian hospitals. The variables selected for the study were the rates of donation, the rates of transfusion of each component/1000 inhabitants, and the rates of transfusion/1000 hospital stays. RES

HIV transmission through transfusion --- Missouri and Colorado, 2008.

Transmission of human immunodeficiency virus (HIV) through transfusion of contaminated blood components was documented in the United States in 1982. Since then, the risk for transfusion-transmitted HIV infection has been almost eliminated by the use of questionnaires to exclude donors at higher risk for HIV infection and the use of highly sensitive laboratory screening tests to identify infected blood donations. The risk for acquiring HIV infection through blood transfusion today is estimated conservatively to be one in 1.5 million, based on 2007-2008 data. This report describes the first U.S.

Transfusion-associated adverse events and implementation of blood safety measures - findings from the 2017 National Blood Collection and Utilization Survey.

BACKGROUND: Serious transfusion-associated adverse events are rare in the United States. To enhance blood safety, various measures have been developed. With use of data from the 2017 National Blood Collection and Utilization Survey (NBCUS), we describe the rate of transfusion-associated adverse events and the implementation of specific blood safety measures. STUDY DESIGN AND METHODS: Data from the 2017 NBCUS were used with comparison to already published estimates from 2015. Survey weighting and imputation were used to obtain national estimates of transfusion-associated adverse events, and th
