Cortical onlay strut allografting is a promising surgical option to reconstruct and reinforce the deficient femur in a hip arthroplasty. However, the union of the allograft to the host bone takes a long time. To accelerate the process of cortical onlay strut allograft healing, we studied the effects of low-intensity pulsed ultrasound (LIPUS) on callus formation. From 2 wk after the operation, LIPUS was given for 20 min/d at each end of the strut allograft. The LIPUS treatment group was assigned 14 allograft transplantations, while 21 control patients were treated without LIPUS. The LIPUS treat

Bilateral symmetric bone nodules were observed in the anterolateral first ribs of an infant with shaking injuries at autopsy. The location prompted diagnostic considerations of healing fractures versus anomalous articulations with pseudarthroses. The forensic pathologist worked with forensic anthropologists and pediatric radiologists to evaluate autopsy findings and compare premortem and postmortem X-rays. Gross examination of the bones by the pathologist and anthropologists confirmed bilateral, callus-like bone nodules in first-rib locations associated with pseudarthroses. Histologic examinat

Tibia fractures are the most common type of long bone fractures in US. This study aimed at comparing the therapeutic results of closed tibial shaft fracture with intramedullary nails inserted with and without reaming. In this randomized clinical trial study, 60 patients with a fracture of the tibia were examined. The patients were randomly divided into two groups. Thirty patients treated through inserting intramedullary nail with reaming technique (group A). The other 30 patients treated through inserting intramedullary nail without reaming technique (group B). After operation physical examina

Femoral head necrosis and non-union are frequent complications after femoral neck fracture. The main reason for failure leading to non-union is an inadequate osteosynthesis and/or poor mechanical conditions, leading to instability. Criteria for optima reduction and fixation techniques, which can prevent non-union in the majority of cases, are described. This knowledge is mandatory for each surgeon as in the non-expert situation up to 30% inadequacy of the "simple" procedure occurs! Although in the elderly endoprosthetic replacement is the treatment of first choice, in the younger and active pa

The process of fracture healing restores the biological and mechanical state of the bone tissue. In contrast to other tissues, bone has the unique capacity to heal through a real repair process resulting not in a scar but in a regular reconstitution of its original tissue structure. During the last two decades, the understanding of bone biology continuously evolved leading to the new concept that preservation of the viability of the bone fragments is the key to unimpaired fracture healing. The biological fracture management provides environmental conditions that allow the natural healing proce

Callotasis is a lengthening technique that involves slow, controlled distraction after subperiosteal-submetaphyseal osteotomy. The technique and its advantages over other methods are described. Results of lengthenings involving 270 operated bone segments (146 femurs and 124 tibias) in 140 patients are reviewed. Ninety-five patients had limb-length inequality and 45 had hypochondroplasia or achondroplasia. On average, 6.6 cm, or 24.6% of initial length, was gained. The mean healing index was 39; the complication rate was 13.3%.

BACKGROUND: There is abundance of literature regarding the treatment of tibial mid-shaft fracture, and intramedullary nailing (IMN) is described as the treatment of choice. However, problems such as malunion and knee pain are known disadvantages of this approach. Minimally invasive plate osteosynthesis (MIPO) technique is another treatment option for tibial mid-shaft fracture. The purpose of this study is to evaluate the clinical, radiological results, and complication rates of tibial mid-shaft fractures treated with MIPO technique.

Hallux valgus is a common forefoot deformity that can be surgically corrected with minimally invasive bunionectomy, which offers advantages such as smaller incisions, reduced soft tissue trauma, and shorter recovery. Despite these benefits, nonunion remains a challenging complication with limited consensus on optimal management. We report the case of a 42-year-old woman with situs inversus totalis who presented with painful left hallux abducto valgus unresponsive to conservative therapy. She underwent a minimally invasive bunionectomy, but at seven months, she continued to have persistent pain

BACKGROUND: A critical component of patient care for osteogenesis imperfecta patients is understanding the development of union following fracture and osteotomy surgery. Studies evaluating the criteria of bony union in this population have varied. An objective standardized method for fracture healing was developed as the radiographic union score for tibial fractures (RUST), and subsequently revised to the modified RUST (mRUST). The RUST score has been shown to have excellent interobserver and intraobserver reliability in a single-center investigation in an OI population. The purpose of the stu

The elderly population suffers from higher rates of complications during fracture healing that result in increased morbidity and mortality. Inflammatory dysregulation is associated with increased age and is a contributing factor to the myriad of age-related diseases. Therefore, we investigated age-related changes to an important cellular regulator of inflammation, the macrophage, and the impact on fracture healing outcomes. We demonstrated that old mice (24 months) have delayed fracture healing with significantly less bone and more cartilage compared to young mice (3 months). The quantity of i

OBJECTIVES: Given the frequency of abusive fractures among infants, and the lack of research and or evidence for the phases of fracture healing seen in this age group, this study aims to describe a timetable of radiological features of fracture healing among infants in the first months of life.
METHODS: We completed a retrospective cross-sectional time-series study of birth-related clavicle fractures from 2006-2013. A total of 108 digital images were available for review from 61 infants. The presence or absence of four features of healing including periosteal reaction, callus formation, bridg

INTRODUCTION: Fibula, ilium and scapula are the preferred donor areas for vascularised bone grafts. For secondary mandibular reconstruction, however, compromises often have to be made as a result of the poor quality of the covering soft tissue and recipient vessels especially in irradiated patients. Callus distraction osteogenesis is a complementary method for optimising the intermaxillary relationship and the facial profile from an aesthetic point of view.
MATERIALS AND METHODS: Thirteen distractions were performed in seven patients following microsurgical mandibular reconstruction over a pe

This paper reviews the technique of callotasis used to correct limb length discrepancy due to congenital causes. Many more lower than upper limb lengthening procedures have been reported. Despite the low complication rate reported by the originators of the technique, patients undergoing limb lengthening because of congenital problems are at significant risk of pin tract sepsis, joint contracture, nerve palsies, angular deformities and fracture through the regenerate bone. A minority of studies focus specifically on limb lengthening for congenital defects. Most series simply include some congen

At the same time as the paper by P. Vives in July 1981, we proposed, in June 1981, a fixation technique with two perpendicular K-wires for correction of rotated malunions of fractures of the neck of the fifth metacarpal. Longitudinal rotation of the distal fragment, either in pronation or in supination, interferes with thumb-little finger opposition and grip. It cannot always be corrected by orthopaedic reduction and osteosynthesis is sometimes necessary: an intramedullary axial K-wire with a curved tip maintains the proximal fragment and reduces its angulation in flexion. A transverse K-wire,

A purine nucleoside triphosphate phosphohydrolase (unspecified diphosphate phosphohydrolase, EC 3.6.1.15) was chromatographically separated from the bulk of alkaline phosphatase activity by gel filtration chromatography of butanol and EDTA extracts of fracture callus and bovine epiphyseal cartilage. The callus enzyme differed from alkaline phosphatase in a variety of characteristics. The purine nucleoside triphosphate phosphatase hydrolyzed a more specific group of substrates, required Ca2+ and Mg2+ for optimal activity, remained unaffected by a potent alkaline phosphatase inhibitor, and demon

GαS, the stimulatory G protein α-subunit that raises intracellular cAMP levels by activating adenylyl cyclase, plays a vital role in bone development, maintenance, and remodeling. Previously, using transgenic mice overexpressing GαS in osteoblasts (GS-Tg), we demonstrated the influence of osteoblast GαS level on osteogenesis, bone turnover, and skeletal responses to hyperparathyroidism. To further investigate whether alterations in GαS levels affect endochondral bone repair, a postnatal bone regenerative process that recapitulates embryonic bone development, we performed stabilized tibial oste

The otic capsule is unique in retaining calcified cartilage, known as globuli interossei, throughout life and shows changes consequently, which are peculiar to it. In Paget's disease of bone, the otic capsule appears resistant to involvement and this occurs with extensive disease at a late stage. In contrast, otosclerosis is a new bone formation of unknown cause that is limited to the otic capsule. In osteogenesis imperfecta, the poor formation of collagen leads to abnormally thin bony trabeculae with a poorly formed otic capsule. In osteopetrosis, the otic capsule is greatly expanded by incre
