Nonmalignant Brain Tumors.

PURPOSE OF REVIEW: This article describes the diagnosis and management of meningioma, pituitary adenoma, craniopharyngioma, and glioneuronal tumors. RECENT FINDINGS: Both meningiomas and pituitary adenomas are common brain tumors. In many cases, these lesions are found incidentally on imaging when patients are being evaluated for a variety of symptoms and signs. While nonmalignant, these tumors are occasionally associated with significant morbidity due to location and resulting secondary symptoms. Rarely, these tumors can also transform into malignant variants. Surgical techniques allow for m

Brain Metastasis Organotropism.

Brain metastases are associated with poor prognosis irrespective of the primary tumor they originate from. Current treatments for brain metastases are palliative, and patients with symptomatic brain metastasis have a one-year survival of <20%. Lung cancer, breast cancer, and melanoma have higher incidences of brain metastases compared with other types of cancers. However, it is not very clear why some cancers metastasize to the brain more frequently than others. Studies thus far suggest that brain-specific tropism of certain types of cancers is defined by a winning combination of the following

Brain iron transport.

Brain iron is a crucial participant and regulator of normal physiological activity. However, excess iron is involved in the formation of free radicals, and has been associated with oxidative damage to neuronal and other brain cells. Abnormally high brain iron levels have been observed in various neurodegenerative diseases, including neurodegeneration with brain iron accumulation, Alzheimer's disease, Parkinson's disease and Huntington's disease. However, the key question of why iron levels increase in the relevant regions of the brain remains to be answered. A full understanding of the homeost

Actinomycotic brain abscess.

Actinomycosis is caused by Gram-positive filamentous anaerobic organisms of genus Actinomyces, which are commensals of mucosal membranes of the oropharyngeal cavity, and gastrointestinal and genitourinary tracts. Central nervous system involvement is rare and may present as cerebral abscess, meningitis, meningoencephalitis, subdural empyema or epidural abscess. The radiological appearances of actinomycotic brain abscesses are not well recognized. Here, we present the characteristic imaging features of an actinomycotic brain abscess.

Brain activation sequences.

OBJECTIVE: Reported brain abnormalities in anatomy and function in psychiatric and neurological patients led to a project based on qualitative electroencephalography examination and analysis in an attempt to find specific brain derived pattern--or sequence of brain locations involved in processing various stimuli--both visual and auditory. METHODS: Specialized software called Brain Activation Sequences was built according to our team member specifications (M.S.). The software utilizes event related potentials recorded during cognitive/emotion processing in participants (healthy controls, neur

Determining Brain Death.

PURPOSE OF REVIEW: Major complexities exist in the assessment of a patient with a catastrophic neurologic disorder that might have progressed to loss of all brain function. The determination of brain death and initial management of the potential organ donor is one of the major key tasks of the neurologist. This article addresses the potential for errors, mimickers, and uncertainties associated with ancillary tests for determining brain death. RECENT FINDINGS: Major professional neurologic organizations including the American Academy of Neurology have published guidelines for the determination

Brain amyloid imaging.

Imaging of brain β-amyloid plaques with (18)F-labeled tracers for PET will likely be available in clinical practice to assist the diagnosis of Alzheimer disease (AD). With the rapidly growing prevalence of AD as the population ages, and the increasing emphasis on early diagnosis and treatment, brain amyloid imaging is set to become a widely performed investigation. All physicians reading PET scans will need to know the complex relationship between amyloid and cognitive decline, how to best acquire and display images for detection of amyloid, and how to recognize the patterns of tracer binding

[Brain abscess - overview].

Brain abscess is a life threatening illness, demanding rapid diagnosis and treatment. Its development requires seeding of an organism into the brain parenchyma, often in an area of damaged brain tissue or in a region with poor microcirculation. The lesion evolves from a cerebritis stage to capsule formation. Brain abscesses can be caused by contiguous or haematogenous spread of an infection, or by head trauma/ neurosurgical procedure. The most common presentation is that of headache and vomiting due to raised intracranial pressure. Seizures have been reported in up to 50% of cases. Focal neuro

[Interaction brain-lungs].

The brain and the lungs interact early and rapidly when hit by a disease process. Often well tolerated by the healthy brain, an impaired respiratory function may deteriorate further a "sick" brain. Hypoxemia is a prognostic factor in the brain-injured patients. At the opposite, an acute brain damage early impacts the lung function. Local brain inflammation spreads rapidly to the lung. It initiates an immunological process weakening the lungs and increasing its susceptibility to infection and mechanical ventilation. Sometimes this process is preceded by a swelling lesion, known as neurogenic pu

Nonenhancing brain metastases.

The hallmark radiological finding in metastatic brain disease is multiple enhancing lesions. We report a case of metastatic lung cancer to the brain with a lack of contrast enhancement. We believe that this unusual finding is due to inadvertent "treatment" of the metastases with the antiangiogenic agent bevacizumab (Avastin).

Brain redox imaging.

Nitroxyl contrast agents (nitroxyl radicals, also known as nitroxide) are paramagnetic species, which can react with reactive oxygen species (ROS) to lose paramagnetism to be diamagnetic species. The paramagnetic nitroxyl radical forms can be detected by using electron paramagnetic resonance imaging (EPRI), Overhauser MRI (OMRI), or MRI. The time course of in vivo image intensity induced by paramagnetic redox-sensitive contrast agent can give tissue redox information, which is the so-called redox imaging technique. The redox imaging technique employing a blood-brain barrier permeable nitroxyl

[Neonatal brain hypothermia].

The neonatal brain hypothermia is one of the effective therapies for neonatal hypoxic-ischemic encephalopathy (HIE). The brain hypothermia is thought to protect the brain from the secondary energy failure after hypoxic-ischemic injury at asphyxia. Some literature wrote the effect for neuronal protection for HIE, but the effect is insufficient for severe one. In severe HIE, the basal ganglia regions are injured with ischemic change in the brain MRI findings and occurred the severe permanent handicaps. I think that the reason for fragile of basal ganglia region are related to the existence of NM

Vascular brain pathologies.

Vascular pathologies of brain comprise a heterogeneous group of abnormalities such as vasculitis, dural arteriovenous malformations, carotid-cavernous fistulas, cerebral venous thrombosis, and intracerebral hemorrhage. Modern imaging techniques are increasingly unraveling these vascular lesions of the brain at a stage when many of them are still asymptomatic. This article focuses on acquired vascular pathologies of brain prevalent in tropical countries that are encountered in clinical practice.

[Brain-gut interactions].

Our digestive tract has an autonomous functioning but also has a bidirectional relation with our brain known as brain-gut interactions. This communication is mediated by the autonomous nervous system, i.e., the sympathetic and parasympathetic nervous systems, with a mixed afferent and efferent component, and the circumventricular organs located outside the blood-brain barrier. The vagus nerve, known as the principal component of the parasympathetic nervous system, is a mixed nerve composed of 90% afferent fibers, which has physiological roles due to its putative vegetative functions. The vagus

Brain cooling therapy.

Therapeutic hypothermia (whole body or selective head cooling) is becoming standard of care for brain injury in infants with perinatal hypoxic ischemic encephalopathy (HIE). Brain cooling reduces the rate of apoptosis and early necrosis, reduces cerebral metabolic rate and the release of nitric oxide and free radicals. Animal models of perinatal brain injury show histological and functional improvement due to of early hypothermia. The brain protection depends on the temperature and time delay between insult and beginning of treatment (more effective with cooling to 33 +/- 0.5 degrees C, and le

Nocardial brain abscess.

Nocardial infections, although rare, are challenging for clinicians to treat. The associated mortality rate remains high; such infections usually occur in immunocompromised patients who have predisposing factors such as malignancy, diabetes mellitus, malnutrition and uremia. However, there have been increasing reports of nocardial infections being observed in immunocompetent patients. Nocardial organisms are mostly isolated from plants and soil, and infection occurs most often as a result of inhalation or direct skin inoculation. Nocardial infections disseminate hematogenously from the primary

[Paradoxical brain embolism].

The presence of right-to-left shunt (RLS), which can result in paradoxical brain embolism, is an important etiology of ischemic stroke especially in young adults. However, the characteristic features of RLS and paradoxical brain embolism have not been recognized in detail, especially in Japan. Patent foramen ovale (PFO) and pulmonary arterio-venous fistula are the most important causes of RLS. According to the registered clinical data of 7,245 acute stroke patients obtained by the Strategies Against Stroke Study for Young Adults in Japan (SASSY-Japan), PFO is more frequent in young patients (n
