Bilateral first branchial cleft anomaly with evidence of a genetic aetiology.

Anomalies of the first branchial cleft (FBC) are uncommon, and recognizing them can be difficult. Although present at birth, many cases do not become evident until later in childhood or adolescence, with an initial clinical presentation in adulthood being encountered only rarely. Typically, FBC anomalies present as a unilateral cyst, sinus, or fistula associated with the external auditory canal, or with swelling or an inflammatory opening in the peri-auricular/parotid area. They are commonly misdiagnosed and are often treated inadequately before being excised completely. A 40-year-old woman pr

The presence of an embryonic opercular flap in amniotes.

The operculum is a large flap consisting of several flat bones found on the side of the head of bony fish. During development, the opercular bones form within the second pharyngeal arch, which expands posteriorly and comes to cover the gill-bearing arches. With the evolution of the tetrapods and the assumption of a terrestrial lifestyle, it was believed that the operculum was lost. Here, we demonstrate that an embryonic operculum persists in amniotes and that its early development is homologous with that of teleosts. As in zebrafish, the second pharyngeal arch of the chick embryo grows disprop

Holoprosencephaly spectrum, ano/microphthalmia, and first branchial arch defects: evidence for a new disorder.

We report on three unrelated Brazilian patients with a holoprosencephaly phenotype, with variable central nervous system involvement, ano/microphthalmia, and first branchial arch anomalies. The features of these patients show a striking similarity to those of the patients reported by Guion-Almeida et al. (1999) and Ribeiro et al. (2005), thus confirming the existence of this rare condition. All cases are isolated and the etiology remains unknown.

Anatomical variations of the facial nerve in first branchial cleft anomalies.

OBJECTIVE: To review our experience with branchial cleft anomalies, with special attention to their subtypes and anatomical relationship to the facial nerve. STUDY DESIGN: Case series. SETTING: Tertiary care center. PATIENTS: Ten patients who underwent resection for anomalies of the first branchial cleft, with at least 1 year of follow-up, were included in the study. The data from all cases were collected in a prospective fashion, including immediate postoperative diagrams. INTERVENTION: Complete resection of the branchial cleft anomaly was performed in all cases. Wide exposure of the faci

[Patient with a parapharyngeal cyst].

In a 34-year-old woman a parapharyngeal mass was found which led to snoring, apnoeas, a feeling of pressure in the pharynx and paraesthesias around the mouth. MRI imaging showed a parapharyngeal mass, most likely a cyst. Patient underwent excision of the mass via an external approach. Histological examination confirmed the parapharyngeal cyst. The literature is inconclusive as to the origin of such cysts. They might be a relic of the 2nd branchial sac, they might be causally related to the cervical sinus of His, they might be a relic of the connection between the thymus and the 3rd branchial p

[Human cervico-facial morphogenesis. Evaluation of acquired data and current outlook. Second part: cervical morphogenesis].

After having recalled the formation of the so-called "branchial" organisation, each component of the segmentary units constituting this organisation is analysed, as well as their particularities. This lead us to recognize the existence of only five branchial arches in the human embryo, without an intermediary arch between the fourth arch and the pulmonary arch. This question is moreover linked to the signification of the so-called "ultimobranchial" body, which must be connected with the fourth pharyngeal pouch. The question of cervical segmentation is inseparable from the question of cephalic

Sphenoidal artery, ramus orbitalis persistens and pterygospinosus muscle--a unique cooccurrence of first branchial arch anomalies in man.

In a 76-year-old Caucasian male, the right middle meningeal was replaced by a branch of the ipsilateral ophthalmic artery (O), i.e. the sphenoidal artery (S); the right foramen spinosum was found to be absent. In the orbit, the right O bifurcated into a medial (merely orbital) and a lateral division, i.e. a meningolacrimal trunk. From that trunk, the S branched off and entered the middle cranial fossa at the superolateral angle of the superior orbital fissure. In addition, an anastomotic channel was formed by a remnant of the ramus orbitalis, connecting the right infraorbital with the deep orb

First branchial cleft anomaly: a report of case.

A rare case of first branchial cleft anomaly in a 21-year-old female is described. A fistulous tract was located near the angle of the mandible and anterior to the sternocleidmastoid muscle, extending upwards under the facial nerve and opening into the external auditory canal. The tract was completely removed and the facial nerve was preserved. Histopathologic examination revealed that the tract was composed of keratinizing squamous cell epithelium with adnexal structures and cartilage.

Congenital midline cervical cleft.

We present a case of congenital midline cervical cleft (CMCC), a sporadic lesion with a predilection for white females. It characteristically occurs in the midline ventral neck and consists of a cephalad skin tag, a mucosal surface, and a caudal sinus. There is an associated submucosal fibrous cord and interwoven bundles of skeletal muscle. This is occasionally tethered to the mandible and/or sternum causing neck contractures. CMCC is usually isolated, but rare cases have been associated with midline clefts of the tongue, lower lip, mandible, and sternum. The embryogenesis is unclear but argum

Congenital cervical salivary fistula.

A study of 35 lesions diagnosed as branchial cleft sinuses or fistulas identified a homogeneous subgroup characterized by ectopic salivary tissue, draining serous or mucoid fluid through a short sinus tract to the base of the neck anteriorly. These lesions should more properly be called congenital cervical salivary fistulas, since they may not arise from the branchial apparatus but rather from the embryonic placodal duct.

Surgical excision of first cleft branchial fistulae.

Three patients were studied who had fistulae in the neck derived from the first branchial cleft. Evidence is presented to show that although these fistulae usually pass superficial to the facial nerve they may also pass deep to one or both main divisions of the nerve. We conclude that a formal superficial conservative parotidectomy with full exposure of the facial nerve is the safest operative course when excising these fistulae.

Aplasia of the first and second branchial arches.

This case report of an otocephalic monster gave the opportunity to study severe orofacial anomalies with a view to clarifying the present position in regard to the so-called first and second arch syndrome. Finding the causal mechanism of this syndrome will eventually lead to prevention and, it is hoped, eradication of this syndrome.

Chromosomal deletion 4p15.32----p14 in a Treacher Collins syndrome patient: exclusion of the disease locus from and mapping of anonymous DNA sequences to this region.

Treacher Collins syndrome is an autosomal dominant condition of bilateral craniofacial abnormalities of structures derived from the first and second branchial arches. A patient with severe manifestations of Treacher Collins syndrome and a de novo chromosomal deletion in region 4p15.32----p14 was identified. Anonymous DNA sequences of loci D4S18, D4S19, D4S20, D4S22, and D4S23 were mapped to the deleted region. DNA probes previously mapped to loci on chromosome 4p (D4S10, D4S15, D4S16, D4S26, D4S35, D4S95, D4S144, RAF1P1, QDPR, and HOX7) were not deleted in this patient. Linkage analysis betwee

Cytodiagnosis of lesions presenting as salivary gland swellings: a report of seven cases.

The cytologic and histologic findings of seven cases presenting clinically as salivary gland swellings are described. These included two cases of pilomatrixoma of skin in the parotid region, two cases of neurilemmoma in the submandibular area, a case of Kimura's disease in the peri-mandibular gland lymphoid tissue, a case of Castleman's disease in intraparotid lymph node, and a case of branchial cleft cyst in the parotid region. It is important to recognize lesions that masquerade as salivary gland tumors so that misdiagnosis and overdiagnosis on smears can be avoided and the patients can be t

Parapharyngeal second branchial cyst: A case report.

Branchial cleft cysts extending to the lateral wall of the pharynx are rare. An eight year old female child presented with a cystic swelling in the left parotid region. A bluish cyst was seen anterior to the anterior pillar of left tonsil. CT scan showed the cyst extending into left parapharyngeal space. External part of the cyst was excised via a transcervical approach and the internal part was marsupalised per orally.
