[Pulmonary complications in burn patients and their functional repercussions].

The authors review the literature about respiratory burns and their initial and late functional repercussions. Upper airway oedema induces initial upper airway obstructive impairment. Pulmonary parenchymal failure is characterised by the functional consequences observed with adult respiratory distress syndrome. Survivors may be free of functional impairment. Smoke inhalation syndrome leads to variable airway obstruction resolving usually within 5 months. Persistence of an obstructive impairment may be due to the development of polyposis and/or stenosis in large airways, bronchiolitis obliterat

Inhalation of liquid nitrogen vapor.

A case of accidental inhalation of liquid nitrogen vapor is reported. The patient, a 29-year-old man, developed acute burns of his lips and oropharynx and presented with signs and symptoms of acute upper airway distress within an hour of the injury. The patient underwent emergency tracheostomy and was placed on intravenous dexamethasone and ampicillin. Large mucosal ulcers developed in the posterior hypopharynx and hard palate but the patient was discharged four days post operatively and had no permanent sequelae.

Balanced bifrontal transcranial direct current stimulation enhances working memory in adults with high-functioning autism: a sham-controlled crossover study.

BACKGROUND: Substance use, alcohol use, and smoking use have all been associated with burn injury. Few studies have investigated associations with substances, alcohol, smoking, inhalational only burns, and patient outcomes. The purpose of the study was to identify risk factors for pulmonary failure in patients suffering inhalation injury, focusing on the impact of substance, alcohol, and cigarette use. METHODS: This is a single-center retrospective analysis of 115 patients admitted to the Johns Hopkins Bayview Burn Center with inhalational injury from January 1, 2010, through September 30, 20

Air Duster Inhalant Abuse Causing Non-ST Elevation Myocardial Infarction.

Inhalant abuse, also known as huffing, is common among teenagers and adolescents in the United States and worldwide. Inhaled aerosols are dangerous due to both the presence of volatile hydrocarbons causing direct organ damage and the risk of the compressed air causing physical trauma (e.g. expansion, barotrauma) or skin trauma from chemical or temperature burn. Here, we present the case of a 35-year-old man who was inhaling multiple canisters of Dust-Off (Falcon Safety Products Inc., Branchburg, NJ) keyboard air duster daily for approximately one month. He presented with intermittent burning c

Risk Factors in Hospitalized Patients With Burn Injuries for Developing Heterotopic Ossification--A Retrospective Analysis.

The aims of this study were to identify the risk factors for developing heterotopic ossification in patients with burns injuries and, second, to review the outcomes associated with the treatment disodium etidronate. Patients with heterotopic ossification were identified using the burns unit computer database. The control group was the patients who were the next admission after admission of the patient who subsequently developed heterotopic ossification. Demographic and clinical data were collected. Univariate and multivariate techniques were used to identify risk factors for heterotopic ossifi

Community-Level Disparities in the Inhalation Injury Patient Population.

Low-income neighborhoods face higher fire risk due to overcrowding, substandard housing, and limited access to safety devices, contributing to greater burn severity among affected residents. Inhalation injury, a frequent consequence, often necessitates early intubation and transfer to burn centers. Given the high degree of coordination required, this study examines how management strategies and outcomes vary across levels of social vulnerability and urbanicity, 2 community-level proxies for access to care. We retrospectively reviewed inhalation injuries diagnosed by bronchoscopy at a single AB

Influence of pre-existing medical condition and predicting value of modified Elixhauser comorbidity index on outcome of burn patients.

OBJECTIVES: To evaluate the impact of comorbidity and predicting value of the modified Elixhauser comorbidity index score on the outcome of burn patients. METHODS: A retrospective study was conducted on 5480 patients admitted during 24 h after burn to the National Burn Hospital from 1/1/2016 to 31/12/2018. Demographic data and pre-existing medical conditions were collected. Modified Elixhauser comorbidity index (ECI) score developed by Walravel et al. was calculated for each patient. The score was the weighted sum of comorbid conditions. Outcomes measured included hospitalization, complicatio

Endothelial glycocalyx shedding in patients with burns.

Shedding of syndecan-1 from the endothelial glycocalyx layer (EGL), referred to as endotheliopathy of trauma (EoT), is associated with poorer outcomes. This study aims to determine if EoT is also present in the burn population. We enrolled 458 burn and non-burn trauma patients at a Level 1 trauma center and defined EoT by a syndecan-1 level of ≥40 ng/mL. Sixty-eight of the enrolled patients had burns with a median TBSA of 19%, with 27.9% also suffering inhalational injury (II). Mortality was similar between the burn and non-burn group, also for patients with EoT. The incidence of II was signif

The use of a noninvasive positive pressure system to facilitate tracheal intubation in a difficult pediatric airway: a case report.

Patients with burns to the head and neck may be difficult to intubate or ventilate via facemask. Furthermore, post-burn scarring and microstomia may reduce the success of rescue supraglottic airway placement. While awake tracheal intubation using a flexible intubation scope is considered the optimal technique for these patients, it may not always be feasible in the pediatric population. We report a case of successful management of a difficult airway in a child with extensive post-burn head and neck deformity using a noninvasive positive pressure system to aid with inhalational induction and de

Forensic aspects of carbon monoxide poisoning by charcoal burning in Denmark, 2008-2012: an autopsy based study.

Carbon monoxide (CO) inhalation is a well-known method of committing suicide. There has been a drastic increase in suicide by inhalation of CO, produced from burning charcoal, in some parts of Asia, and a few studies have reported an increased number of these deaths in Europe. CO-related deaths caused by charcoal burning have, to our knowledge, not been recorded in the Danish population before. In this retrospective study we present all autopsied cases of CO poisoning caused by charcoal burning in the period 2008-2012. 19 autopsied cases were identified, comprising 11 suicides, 4 accidents, an

Endothelial damage occurs early after inhalation injury as measured by increased syndecan-1 levels.

Inhalation injury is a significant cause of morbidity and mortality in the burn patient population. However, the pathogenesis of inhalation injury and its potential involvement in burn shock is not well understood. Preclinical studies have shown endothelial injury, as measured by syndecan-1 (SDC-1) levels, to be involved in the increased vascular permeability seen in shock states. Furthermore, the lung has been identified as a site of significant SDC-1 shedding. Here we aim to characterize the contribution of endotheliopathy caused by inhalation alone in a swine model. When comparing injured a

Prehospital airway and operational management of severe ammonia intoxication: a case report.

BACKGROUND: Ammonia inhalation can cause rapid airway compromise and noncardiogenic pulmonary oedema. Although hospital management has been described, prehospital strategies in severe cases remain largely undocumented. CASE PRESENTATION: A 44-year-old obese male (160 kg) was exposed to an ammonia cloud at a decommissioned power plant. He presented with mucosal burns, severe dyspnoea, and hypoxemia despite high-flow oxygen. Prehospital providers initiated nebulized adrenaline, performed ocular irrigation, and prepared for surgical airway backup. Rapid sequence intubation was conducted under vi

Combined acute inhalation of hydrofluoric acid and nitric acid: a case report and literature review.

We describe a nonlethal, delayed onset case of combined acute inhalation of hydrofluoric acid (HFA) and nitric acid (NA) together with a review of the literature. Our patient was exposed to fumes of a 12% HFA and 22% NA solution in a closed environment and suffered during several months after the incident from exertional dyspnoea but recovered completely. Since HFA and NA are dangerous and widely used substances, preparedness for exposure is mandatory. After inhalational exposure, the principles of decontamination with attention to treatment of the skin for HFA burns together with general medi

Injuries and deaths at a pub fire in Bangkok, Thailand on New Year's Eve 2009.

This investigation describes the 67 people who died and the 153 who were hospitalized from a New Years' Eve fire in a Bangkok pub. We interviewed survivors and reviewed medical charts and forensic reports of decedents. Survivors were young (median age 27 years), single (84.7%) and lived in Thailand (93.6%). Most were on the concert floor when the fire started (74.0%), became aware of danger when they saw flames (61.5%) and escaped through the main entry door (42.9%). Common injuries were burns (75.6%), smoke inhalation (47.4%) and open wounds (32.1%). The decedents' median age was 27 years and

Challenges of PICC placement via the popliteal vein under ultrasound guidance in a patient with severe burns: A case report.

Peripherally inserted central catheters are widely used in patients with extensive burns, with the guidelines recommending insertion through unburned skin. This case report describes a patient who was burned over 88% of their surface area and suffered severe inhalation injury. For him, the popliteal vein was the only vein on unburned skin available for catheter catheterization. Based on evidence, we successfully placed a peripherally inserted central catheter through the popliteal vein under ultrasound while the patient was in the prone position and avoided associated complications.

Inhalation injury: epidemiology, pathology, treatment strategies.

Lung injury resulting from inhalation of smoke or chemical products of combustion continues to be associated with significant morbidity and mortality. Combined with cutaneous burns, inhalation injury increases fluid resuscitation requirements, incidence of pulmonary complications and overall mortality of thermal injury. While many products and techniques have been developed to manage cutaneous thermal trauma, relatively few diagnosis-specific therapeutic options have been identified for patients with inhalation injury. Several factors explain slower progress for improvement in management of pa

Burns sustained in combat explosions in Operations Iraqi and Enduring Freedom (OIF/OEF explosion burns).

BACKGROUND: Burns comprise 5% of casualties evacuated from Operations Iraqi and Enduring Freedom (OIF and OEF). Many OIF/OEF burns result from the enemy's detonation of explosives. We reviewed these to evaluate mission impact and provide recommendations for improved combat burn protection. Data were compared to those from the Vietnam War. METHODS: All OIF/OEF patients with significant burns are treated at the U.S. Army Institute of Surgical Research (ISR). A review from April 2003 to April 2005 was undertaken. Records were obtained and demographics, burn severity and pattern, and early outcom

Cyanide poisoning in patients with inhalation injury - the phantom menace.

Inhalation injury is a serious complication in patients with burns that dramatically increases their morbidity and mortality. These patients are always suspected of having inhalation injury with potential intoxication. We usually encounter carbon monoxide intoxication, but it is necessary to think about the possibility of poisoning by other combustion products. Cyanide intoxications are less common, but their diagnosis and treatment are more complicated. The diagnosis can only be based on the history, clinical findings, and indirect laboratory signs. Direct determination of plasma cyanide leve
