Radical resection for cancer of the hypopharynx and cervical oesophagus with repair by stomach transposition.

The results of 58 consecutive cases of pharyngolaryngo-oesophagectomy and pharyngogastric anastomosis are presented. The Grey Turner technique is used, but modified by adding bilateral chest drainage established by guidance from below the diaphragm and a jejunostomy. The immediate postoperative therapy management of hypocalcaemia with calcium gluconate and long term control with calciferol is described. An overall 3-year survival rate of 29 per cent has been obtained. Seven patients survived longer than 7 years, the longest living over 10 years. The authors conclude that, with careful manageme

Single center experience in localization of insulinoma by selective intraarterial calcium stimulation angiography - a case series of 15 years.

BACKGROUND: Insulinomas are rare insulin-secreting neuroendocrine neoplasms of the pancreas. First-line treatment is the surgical removal of the tumor, however, the localization with standard imaging techniques is often challenging. With the help of selective intraarterial calcium stimulation the insulinoma's localization can be narrowed down to one third of the pancreas which the selected artery supplies. OBJECTIVE: We aimed to prove the usefulness of the calcium stimulation test in case of 9 patients treated between 2006 and 2021 diagnosed with endogenous hyperinsulinemic hypoglycemia confi

Proton pump inhibitor induced hypocalcemia presenting with carpopedal spasm- a rare case report from Nepal.

BACKGROUND: A frequent electrolyte imbalance, hypocalcaemia can cause anything from asymptomatic symptoms to potentially fatal neuromuscular and cardiac symptoms. Tetany, carpopedal spasm, and ECG abnormalities such QT prolongation can result from acute drops in serum calcium. Long-term usage of proton pump inhibitors (PPIs) is frequently disregarded as a reversible etiology, despite the fact that drugs are known causes. To avoid major issues and guarantee timely treatment, early detection is essential. CASE PRESENTATION: Acute painful carpopedal spasms and perioral numbness were reported by

An Unusual Presentation of Multifactorial Hypocalcemia as Myopathy: A Case Report.

Hypocalcemia is a common metabolic disturbance with a broad spectrum of clinical presentations, ranging from asymptomatic laboratory findings to severe, life-threatening complications. While neuromuscular irritability is a well-known feature, myopathy as a primary presentation is rare and often under-recognized. We report the case of a 45-year-old female with type 2 diabetes and hypertension who presented with progressive lower limb weakness and generalized body aches. She had a recent history of an abdominal skin abscess and had received blood transfusions without calcium correction. On exami

Sex-specific Stone-forming Phenotype in Mice During Hypercalciuria/Urine Alkalinization.

Sex differences in kidney stone formation are well known. Females generally have slightly acidic blood and higher urine pH when compared with males, which makes them more vulnerable to calcium stone formation, yet the mechanism is still unclear. We aimed to examine the role of sex in stone formation during hypercalciuria and urine alkalinization through acetazolamide and calcium gluconate supplementation, respectively, for 4 weeks in wild-type (WT) and moderately hypercalciuric [TRPC3 knockout [KO](-/-)] male and female mice. Our goal was to develop calcium phosphate (CaP) and CaP+ calcium oxa

Severe citrate toxicity complicating volunteer apheresis platelet donation.

We report a case of severe citrate toxicity during volunteer donor apheresis platelet collection. The donor was a 40-year-old female, first-time apheresis platelet donor. Past medical history was remarkable for hypertension, hyperlipidemia, and depression. Reported medications included bumetanide, pravastatin, and paroxetine. Thirty minutes from the start of the procedure, the donor noted tingling around the mouth, hands, and feet. She then very rapidly developed acute onset of severe facial and extremity tetany. Empirical treatment with intravenous calcium gluconate was initiated, and muscle

Recombinant human parathyroid hormone therapy (1-34) in an adult patient with a gain-of-function mutation in the calcium-sensing receptor-a case report.

OBJECTIVE: To describe a case of hypocalcemia in a patient with a gain-of-function mutation in the calcium-sensing receptor that was undetected until adulthood and successfully treated with recombinant parathyroid hormone. METHODS: The clinical findings, laboratory data, and a review of the pertinent literature are presented. RESULTS: A 55-year-old woman was hospitalized and seen by the endocrinology consult service for hypocalcemia that was refractory to repeated doses of intravenous calcium gluconate. She expressed concern about chronic leg muscle cramps and paresthesias of the lips and fi

Acceptance of minerals and other compounds by calcium-deprived rats: 24-h tests.

We measured 24-h spontaneous intake of four to eight concentrations of 31 different solutions by groups of rats fed control or low-calcium diets. Relative to controls, those fed low-calcium diet had increased acceptance of one or more concentrations of sodium chloride, sodium acetate, and sodium bicarbonate, but not sodium gluconate. Differences in palatability between these sodium salts were unimportant because the rats fed low-calcium diet consumed more sodium chloride even if this was made less acceptable by adulteration with citric acid. The possibility that calcium-deprived rats have an e

[A 63-year-old patient with worsening general condition, bone demineralization, hypocalcemia and excess parathyroid hormone: late manifestations of pseudohypohyperparathyroidism].

HISTORY AND CLINICAL FINDINGS: A 63-year-old man was hospitalized because of his worsening general condition and weight loss. Physical examination revealed marked thoracic kyphosis with impaired mobility of his back and restricted walking because of pain in the right hip. He also had other bone pains. INVESTIGATIONS: Serum calcium was reduced (1.60 mmol/l) and there was generalized bone demineralization. Subsequently an increased parathormone (PTH) level was noted (499.0 ng/l) as well as markedly increased activity of enzymes involved in bone metabolism, decreased renal excretion of phosphate

A Rare Case of Extensive Hydrofluoric Acid Burn.

Hydrofluoric acid (HF) is a strongly corrosive, highly toxic, and highly dangerous mineral acid. Burns with over 1% TBSA caused by anhydrous HF can lead to deep tissue damage, hypocalcemia, poisoning, and even death. In recent years, HF has become one of the most common substances causing chemical burns and ranks as the leading cause of death from chemical burns. Herein, we report a rare case with 91% TBSA burns caused by 35% HF. The patient developed complications such as shock, severe hypocalcemia, metabolic acidosis, and respiratory failure. Multidisciplinary team consultation (burns, respi

Rickets in Nigerian children: a consequence of calcium malnutrition.

Eleven Nigerian children with clinically and radiologically proven rickets were assessed biochemically. The children had low or low normal concentrations of total and corrected calcium, and elevated plasma alkaline phosphatase (ALP) activity, but normal plasma phosphate concentrations. Their serum 25-hydroxyvitamin D (25-OHD) and 1,25-dihydroxyvitamin D (1,25-(OH)2D) concentrations were not significantly different from those in controls, but the ratio of 1,25-(OH)2D to 25-OHD was significantly greater than that in controls. Parathyroid hormone (PTH) concentrations were greater in rachitic chil

Neonatal Hypocalcemic Seizures in Offspring of a Mother With Familial Hypocalciuric Hypercalcemia Type 1 (FHH1).

CONTEXT: Familial hypocalciuric hypercalcemia type 1 (FHH1) is caused by loss-of-function mutations of the calcium-sensing receptor (CaSR) and is considered a benign condition associated with mild-to-moderate hypercalcemia. However, the children of parents with FHH1 can develop a variety of disorders of calcium homeostasis in infancy. OBJECTIVE: The objective of this work is to characterize the range of calcitropic phenotypes in the children of a mother with FHH1. METHODS: A 3-generation FHH kindred was assessed by clinical, biochemical, and mutational analysis following informed consent. R

Hyperkalemic periodic paralysis episode during halothane anesthesia in a horse.

A 7-month-old Quarter Horse filly was admitted for surgical repair of a right olecranon fracture. Anesthesia was achieved with xylazine hydrochloride, guaifenesin, ketamine hydrochloride, and halothane. Two and a half hours after induction of anesthesia, myotonia, muscle fasciculations, and sweating, concurrent with high serum potassium concentration and associated electrocardiographic changes consistent with hyperkalemic periodic paralysis, were observed. Treatment included intermittent positive-pressure ventilation, changing intravenous administration of fluids from lactated Ringer's solutio

Escalating the dose and downgrading the rhythm: the combination of prescribed and non-prescribed medications resulting in an abnormal heart rhythm.

Older patients often have multiple medical issues which predisposes them to complications of polypharmacy and medication interactions. We present a case of an 89-year-old female who presented to the emergency department after a fall. An electrocardiogram (ECG) showed a junctional bradycardia with a ventricular rate of 50 beats per minute (BPM). Her magnesium was 3.5 mg/dl (1.7-2.3 mg/dl). She had recently increased her milk of magnesia use for constipation. Pertinent other medications included verapamil 280 mg daily. On admission 2 grams of calcium gluconate IV were administered and the verapa

Debridement, antibiotic pearls, and retention of the implant (DAPRI): A modified technique for implant retention in total knee arthroplasty PJI treatment.

We describe a modified surgical technique developed to enhance the classical irrigation and debridement procedure to improve the possibilities of retaining an infected total knee arthroplasty. This technique, debridement antibiotic pearls and retention of the implant (DAPRI), aims to remove the intra-articular biofilm allowing a higher and prolonged local antibiotic concentration using calcium sulfate beads. The combination of three different surgical techniques (methylene blue staining, argon beam electrical stimulation, and chlorhexidine gluconate brushing) might enhance the identification,
