Just flip it!-CRT implantation in a patient with dextrocardia and situs inversus totalis.

Implantation of a cardiac resynchronization therapy (CRT) device in patients with congenital heart disease may be challenging because of the particular underlying anatomy. We present the case of a right-sided transvenous implantation of a dual-chamber CRT-D device in a patient with situs inversus totalis and mirror image dextrocardia. To facilitate our anatomic orientation and to overcome unusual hand-eye coordination problems, we decided to flip the fluoroscopic image projection by 180° (right-left), creating the optical impression of a "normalized" heart position (levocardia). This simple ap

Cardiac Sympathetic Innervation Imaging with PET Radiotracers.

PURPOSE OF REVIEW: The present article reviews the pathophysiology of cardiac sympathetic denervation, the principles of positron emission tomography (PET) imaging of the sympathetic innervation of the heart and its potential clinical role, based on current and expected future evidence. RECENT FINDINGS: Imaging of cardiac sympathetic denervation can be performed with radiolabeled noradrenaline analogues, e.g., 11C-hydroxyephedrine. A greater burden of sympathetic denervation carries prognostic significance, e.g., in patients with ischemic cardiomyopathy and a left ventricular ejection fractio

Device therapies for heart failure with reduced ejection fraction: a new era.

Even with significant advancements in the treatment modalities for patients with heart failure (HF), the rates of morbidity and mortality associated with HF are still high. Various therapeutic interventions, including cardiac resynchronization therapy, Implantable Cardiovascular-Defibrillators, and left ventricular assist devices, are used for HF management. Currently, more research and developments are required to identify different treatment modalities to reduce hospitalization rates and improve the quality of life of patients with HF. In relation to this, various non-valvular catheter-based

A case report on middle cerebral artery aneurysm treated by rapid ventricular pacing: A CARE compliant case report.

RATIONALE: Cerebral aneurysm is a common cause of intracranial hemorrhage, stroke, and death. It is treated with vascular surgeries, such as coil embolism and artery clipping. However, surgery itself is a risk factor that may cause rupture of aneurysm, and leads to irreversible brain damage, and even death. Rapid ventricular pacing (RVP) is a procedure that temporarily lowers blood pressure by increasing heart rate and reducing ventricular filling time. RVP has been widely used to reduce blood vessel tension in many cardiovascular surgeries. PATIENT CONCERNS: A 46-year-old man came to our hos

Left Bundle Branch Area Pacing as a Rescue Strategy After Failed Cardiac Resynchronization Therapy in a Patient With Persistent Left Superior Vena Cava, Absent Coronary Sinus Branches, After Bilateral Maze Procedure: A Case Report.

BACKGROUND Cardiac resynchronization therapy can fail in the presence of anatomical variations, such as persistent left superior vena cava, which can preclude coronary sinus cannulation and left ventricular lead placement. Conduction system pacing, particularly left bundle branch area pacing, has emerged as a physiological alternative for achieving ventricular resynchronization in such anatomically challenging situations and may provide an effective solution when standard cardiac resynchronization therapy (CRT) is not feasible. CASE REPORT We report the case of a 78-year-old man with chronic h

The New Challenge for Heart Endocarditis: From Conventional Prosthesis to New Devices and Platforms for the Treatment of Structural Heart Disease.

Infective endocarditis is a sinister condition with considerable morbidity and mortality. Its relevance in the current era is compounded by the increased use of implanted devices such as replacement valves or cardiac implantable electronic devices. These infections are caused by multiple different bacteria with different virulence, pathogenicity, and antimicrobial resistance. Unlike in native endocarditis, the presence of foreign tissue permits sustenance by inflammatory and thrombotic processes as the artificial surfaces promote inflammatory responses and hypercoagulability. Prevention of the

Atrial pacing improves mitochondrial function in peripheral blood mononuclear cells in patients with cardiac implantable electronic devices.

Mitochondrial dysfunction contributes significantly to the development of atrial fibrillation (AF). Conflicting data regarding the atrial pacing and the risk of AF existed, and the impact of atrial pacing on mitochondrial function remains unknown. Therefore, we sought to examine the association between atrial pacing percentage and mitochondrial function in patients with cardiovascular implantable electronic devices (CIEDs) with atrial pacing capability. This is a cross-sectional study involving 183 patients with CIEDs with atrial pacing capability. The oxidative stress and mitochondrial functi

[Indication, management and problems of cardiac transplantation].

Cardiac transplantation (HTx) remains one of the most important options for the management of end stage heart failure which is treated with optimal medical therapy. Evolving surgical techniques (implantable cardioverter-defibrillator (ICD), cardiac resynchronization therapy (CRT)) and mechanical device-therapie (ventricular assist devices (VAD)) and medical therapies have yielded incremental improvements in outcomes. These alternatives to HTx, however, usually only postpone the occurrence of the final end stage situation. This explains why HTx remains the last option for a substantial number o

Cardiac Resynchronization Therapy: A Review.

IMPORTANCE: Heart failure (HF) affects more than 64 million individuals worldwide, and acute HF is associated with 1-year mortality rates of 23.6% in North America and Europe. Cardiac dyssynchrony from conduction system disease may cause HF progression, particularly in patients with left ventricular (LV) systolic dysfunction. OBSERVATIONS: Electrical dyssynchrony in HF most commonly presents as left bundle-branch block and affects 20% to 30% of patients with reduced LV ejection fraction (LVEF). Cardiac resynchronization therapy, which includes biventricular pacing and conduction system pacing

Isolation of the phrenic nerve to suppress diaphragmatic contraction induced by cardiac resynchronization.

Left phrenic nerve stimulation is a well-known complication of cardiac resynchronization therapy (CRT). We report a case where electronic or anatomical repositioning of the left ventricular (LV) electrode (within the coronary sinus tributaries) was not feasible/effective. Surgical graft interpositioning was performed to successfully isolate the phrenic nerve from the previously implanted coronary sinus epicardial LV lead.

Simplifying Physiological Left Bundle Branch Area Pacing Using a New Nine-Partition Method.

BACKGROUND: Left bundle branch area pacing (LBBaP) is accepted as a physiological form of pacing; however, it is complex and usually requires an expensive electrophysiological recording system. METHODS: A simplified approach ("9-partition method") was explored to perform LBBaP. In this method, a right anterior oblique 30° fluoroscopic image of the ventricle was divided into 9 sections ("3 × 3" partitions). From May 2018 to February 2019, we enrolled 51 consecutive patients who underwent pacemaker implantation. The patients were nonrandomly allocated to either the conventional LBBaP (c-LBBaP)
