The influence of cochlear implantation on vowel articulation.

PURPOSE: Speech of deaf persons differs considerably from that of speakers with normal hearing. The purpose of this study was to investigate the acoustic changes in articulation of corner vowels in deaf children and adults after cochlear implantation. METHODS: Thirteen prelingually deaf children and 12 postlingually deaf adults were included in the study. Voice samples of the isolated corner vowels /a/, /i/ and /u/ were analyzed before and 6-12 months after the implantation. The frequencies of the first (F1) and second (F2) formants, the F1/F2 ratio of all three corner vowels, and the area of

Cochlear implantation in a patient with Perisylvian syndrome.

Perisylvian syndrome is a rare neurological disorder characterised by the partial paralysis of muscles, epilepsy and mild to severe mental retardation. It is associated with hearing loss and delay in language and speech development. This presents additional challenges in the assessment of whether a child is suitable for cochlea implantation. The method to determine whether the hearing loss is of cochlear or central origin and the progress of a child with Perisylvian syndrome who received a cochlear implant is discussed.

Complications of paediatric cochlear implantation: experience in Izmir.

Surgery for cochlear implantation (CI) bears the risks of complication associated with all major surgery, in addition to the particular risks associated with implanting a foreign body into the peripheral auditory system. Here we present a retrospective study involving 227 cochlear implant operations in 205 children to evaluate the rate of intra- and post-operative complications. Complications were defined as major complications, requiring explantation of the device or further operation, causing a significant medical problem, or leading to any degree of facial paralysis or requiring additional

Cochlear implantation concurrent with translabyrinthine acoustic neuroma resection.

OBJECTIVES/HYPOTHESIS: Cochlear implants provide successful auditory rehabilitation for patients with profound sensorineural hearing loss who do not derive at least marginal benefit from conventional hearing aids. Patients with neurofibromatosis type 2 can present with bilateral profound sensorineural hearing loss caused by bilateral vestibular schwannomas. Auditory rehabilitation in these patients can be challenging. We present the case of one such patient who underwent a concurrent translabyrinthine vestibular schwannoma resection and cochlear implantation in the same ear. STUDY DESIGN: A c

Cochlear implantation in a child with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a hereditary disease of connective tissue and affects bone, dentine, sclera, joint, tendon, blood vessels, heart valves, and skin. Approximately 50% of the adult patients with OI have associated hearing impairment. To date, only three cases of cochlear implantation in adults with OI have been reported, but none in children. We present a case of cochlear implantation in a congenitally deaf 6-year-old boy with OI. The Nucleus 24 Contour device was successfully implanted using the suprameatal approach (SMA). At 6 months post-initial stimulation there was no evidenc

Is cochlear implantation possible after acoustic tumor removal?

HYPOTHESIS: This study aimed to assess the possibility of hearing restoration after acoustic tumor removal. BACKGROUND: Hearing restoration surgery may be indicated after surgery of bilateral acoustic tumors or surgery of acoustic tumor in the only hearing ear. The choice is between cochlear implantation and brainstem implantation. METHODS: From the temporal bone collection at the House Ear Institute, Los Angeles, the author histologically examined eight temporal bones from seven patients who had undergone acoustic tumor removal during their lifetime. Special emphasis was put on examining th

Nucleus 22 cochlear implantation results in postmeningitic deafness.

Cochlear implant surgery was performed on 13 patients with postmeningitic deafness (seven adults, six children). Two adults and two children (30.8%) had severe labyrinthitis ossificans requiring radical "drill-out." Five of 13 (38.5%) had some bone growth requiring partial drill-out, and four of 13 (30.8%) had normal insertion with no drill-out. Hearing results for patients with no bone growth were similar to nonmeningitic patients; three of four (75%) had open-set speech recognition. Performance of patients with total drill-out was poor; "auditory only" performance was limited to detection an

Facial nerve stimulation after successful multichannel cochlear implantation.

Postoperative rehabilitation after cochlear implantation may be difficult and frustrating for both patient and audiologist. The occurrence of facial nerve stimulation with associated twitching and discomfort can interfere with the rehabilitative process and be a cause of great concern to the patient. Such stimulation can occur at the time of initial "tune-up" or can be delayed by months or years. The incidence of postoperative facial nerve stimulation 109 consecutive patients operated by the same surgeon over a period of 7 years is reviewed. Possible causes for immediate and delayed stimulatio

Inner ear morphologic changes resulting from cochlear implantation.

Insertion of electrodes into the cochlea can damage the spiral ligament, organ of Corti, osseous spiral lamina, and dendrites. The amount of damage depends on the method of insertion and the amount of surgical trauma. Damage is limited to the area immediately adjacent to the electrode. Damage in the cochlea and prolonged electrical stimulation do not affect ganglion cell population. The ganglion cell population responds directly to the electrical stimulus. Examination of three auditory nerves from three cases and the cochlear nuclei from one case also reveals no effect of prolonged electrical

Promontory stimulation following labyrinthectomy: implications for cochlear implantation.

Transtympanic electric promontory stimulation is a psychoacoustic test used to assess residual acoustic neurons in profound sensorineural hearing loss. The test was performed in six patients who had previously undergone transmastoid (N = 5) or transcanal-oval window (N = 1) labyrinthectomy as a means of determining the feasibility of future cochlear implantation. Four patients had unilateral Meniere's disease, one had labyrinthitis, and one had delayed onset vertigo. All patients perceived a definite auditory sensation in the labyrinthectomized ear during stimulation. The results of threshold,

Multielectrode cochlear implantation in children: the Quebec experience.

The Quebec Cochlear Implant Research Program has implanted three children since August 1987. This paper reports the results obtained with these children. All of them improved their auditory abilities with cochlear implantation and training. However, the abilities which spontaneously recovered following implantation, the speed of improvement, and the overall level of performance are different from one child to the other. Our data suggest that the age at onset of hearing loss, and possibly the number of years of deafness, may be related to the differences in performance.

Outcomes of cochlear implantation in children with isolated auditory neuropathy versus cochlear hearing loss.

OBJECTIVE: Auditory neuropathy (AN) is a heterogeneous clinical entity for which the optimal method of auditory rehabilitation has been a matter of some debate. Such patients often do not receive sufficient benefit from hearing aids. Previous studies have shown that select AN patients may benefit from cochlear implantation (CI), but reported outcomes are variable and likely are a reflection of the heterogeneous patient population included under the umbrella diagnosis of AN. This study compares CI outcomes in a subset of the pediatric AN population who do not have a confounding cognitive disord
