Changes in physical activity after abdominal sacrocolpopexy for advanced pelvic organ prolapse.

OBJECTIVE: The objective of the study was to describe changes in physical activity 1 year after sacrocolpopexy for pelvic organ prolapse. STUDY DESIGN: This was a prospective cohort of 301 randomized trial participants. RESULTS: Compared with baseline, 1 year after surgery, 36% increased, 18% decreased, and 47% did not change preoperative exercise intensity level. In contrast, women were more likely to reduce (24%) than increase (11%) the frequency of major effort activities, like heavy lifting. Of 99 women who reported preoperatively that prolapse interfered substantially with doing exercis

Vaginal Endometriosis Resection Guided by an Intra-Operative Transvaginal Ultrasound.

OBJECTIVE: Vaginal endometriosis occurs in 3.5% to 14.5% of patients with endometriosis [1-3]. Complete resection is crucial, as unnecessary colpotomy may increase the risk of rectovaginal fistula [4], and incomplete resection may leave residual disease, raising the likelihood of recurrence. Ultrasound is commonly used in the diagnosis and preoperative assessment of deep endometriosis [5]. This study demonstrates the use of intraoperative transvaginal ultrasound to guide safe and complete vaginal endometriosis resection. SETTING: An academic-affiliated healthcare center. PARTICIPANTS: Patien

Totally laparoscopic resection of right-sided colon cancer using transvaginal specimen extraction with a 10-mm-long abdominal incision.

BACKGROUND: Natural orifice specimen extraction (NOSE) has been developed as a means of decreasing the incidence of surgical wound complications. We refined the procedure for totally laparoscopic colectomy with transvaginal specimen extraction using the reduced port surgery technique with the ultimate goal of attenuating damage to the abdominal wall. We herein report this innovative technique and its short- and long-term outcomes. METHODS: We prospectively collected data on seven patients who underwent totally laparoscopic colectomy using transvaginal specimen extraction with a 10-mm-long abd

Total pelvic peritonectomy for ovarian cancer with extensive peritoneal carcinomatosis in pelvic cavity.

BACKGROUND: Ovarian cancer is characteristic of superficial implantation and peritoneal carcinomatosis. Hudson introduced a novel technique 50 years ago, which removed the entire Douglas pouch as a false capsule of the tumor (Hudson, 1968 [1]). Angeles et al. standardized the procedure in 10 steps in a previous publication (Angeles et al., 2019 [2]). We made small modifications of the Hudson procedure in this video. Ovarian cancer is characteristic of superficial implantation and peritoneal carcinomatosis. Hudson introduced a novel technique 50 years ago, which removed the entire Douglas pouch

Quality of life following vaginal reconstructive versus obliterative surgery for treating advanced pelvic organ prolapse.

INTRODUCTION AND HYPOTHESIS: Although colpocleisis is effective in selected women, the low-morbidity obliterative procedure for treating pelvic organ prolapse (POP) and its impact on postoperative quality of life (QOL) have rarely been studied. Our aim was to assess QOL in women after colpocleisis and compare it with that of women after reconstructive vaginal surgery. METHODS: This retrospective cohort study included women (aged 35-85 years) with POP who underwent obliterative or reconstructive surgical correction during 2009-2015. Patients who met the inclusion criteria underwent telephone i

Impact of Barbed Suture Closure on Vaginal Cuff Dehiscence Following Robot-Assisted Total Hysterectomy: A Retrospective Cohort Study.

Introduction Vaginal cuff dehiscence (VCD) is a rare but serious complication following hysterectomy, with a higher incidence in minimally invasive surgery (MIS). The choice of suture material and closure technique may influence the risk of VCD. Barbed sutures, such as STRATAFIX™ Spiral PDS Plus, enhance tissue approximation and eliminate the need for knot tying, potentially improving wound healing. This study aimed to evaluate the incidence of VCD following robot-assisted total hysterectomy (RAH) with vaginal cuff closure using barbed sutures. Methods This retrospective cohort study included
