The impact of childhood abuse and combat-related trauma on postdeployment adjustment.

This retrospective study examined the effects of childhood physical abuse (CPA) and combat-related trauma on postdeployment psychiatric symptoms in an outpatient clinical sample of 1,045 U.S. service members. The authors conducted hierarchical multiple regression analyses to examine the impact of CPA and combat-related trauma on alcohol use, anxiety, depression, and posttraumatic stress disorder (PTSD) symptoms. Analyses revealed significant main effects for CPA and combat-related trauma on anxiety, depression, and PTSD. In contrast, no interactive effects were observed. Findings support and e

Positive-expectancy factors on long-term posttraumatic stress disorder symptoms: A prospective 2-year follow-up investigation among military veterans.

Military personnel are trained throughout their career for wartime, yet the expectation and the valence associated with being in combat differs quite extensively. Despite factors that could influence military personnel's perception of being exposed to combat, happenstance in combat theaters frequently results in experiencing combat even for those who would not necessarily expect to. Although the importance of expectations within the context of trauma has been evidenced in multiple contexts, combat expectancy has never been examined as having an influence in the trauma-posttraumatic stress diso

Altered amygdala activation during face processing in Iraqi and Afghanistani war veterans.

BACKGROUND: Exposure to combat can have a significant impact across a wide array of domains, and may manifest as post-traumatic stress disorder (PTSD), a debilitating mental illness that is associated with neural and affective sequelae. This study tested the hypothesis that combat-exposed individuals with and without PTSD, relative to healthy control subjects with no history of PTSD or combat exposure, would show amygdala hyperactivity during performance of a well-validated face processing task. We further hypothesized that differences in the prefrontal cortex would best differentiate the comb

Benefit finding at war: a matter of time.

Benefit finding, described as one's ability to find benefits from stressful situations, has been hypothesized as a buffer against the negative effects of stress on mental health outcomes. Nonetheless, many have questioned the buffering potential of benefit finding in the face of prolonged and excessive stress such as is found in the combat environment. This study suggests that the length of a combat deployment and benefit finding may impact the relationship between combat exposure and posttraumatic stress disorder (PTSD) symptoms. Surveys were distributed to U.S. enlisted soldiers (n = 1,917),

Severity and Symptom Trajectory in Combat-Related PTSD: a Review of the Literature.

PURPOSE OF REVIEW: Combat-related posttraumatic stress disorder is increasingly recognized as having a variable course in returning veterans. Relatively few studies have identified predictors of illness duration or severity in this population. This review sought to synthesize the existing literature. RECENT FINDINGS: The existing literature remains limited and heterogeneous. However, several studies identified hyperarousal and pre-deployment dissociation as predictive of disease severity, and re-experiencing as predictive of suicidality in veterans with combat-related PTSD. No other pre-, per

Canadian Military Nurse Deaths in the First World War.

This paper examines the lives of sixty-one Canadian Nursing Sisters who served during the First World War, and whose deaths were attributed, more or less equally, to three categories: general illness, Spanish Influenza, and killed in action. The response by Canadian Army Medical Corps (CAMC) physicians to the loss of these early female officers who were, in fact, Canada's first female war casualties, suggests a gendered construction of illness at work in the CAMC. While nurses tried to prove themselves good soldiers, military physicians were quick to attribute their illnesses and deaths to hor

Sociodemographic and clinical correlates of cannabis dependence among Israeli combat veterans.

INTRODUCTION: Cannabis is one of the most widely used addictive substances globally. Its use increases the risk for various physical and psychological problems and some cannabis users may develop cannabis dependence. Researchers have explored risk factors for transition to cannabis dependence. Military veterans, and in particular, combat veterans, have an elevated risk for cannabis dependence and several emotional disorders. To date, the field lacks knowledge regarding possible risk factors for the development of cannabis dependence among combat military veterans. METHOD: The current study ex

Anticonvulsant Medication Use in Veterans With Posttraumatic Stress Disorder.

OBJECTIVE: Anticonvulsants have been studied for many indications, including posttraumatic stress disorder (PTSD). The limited efficacy research on anticonvulsants for PTSD is mixed. However, anticonvulsants are prescribed widely to veterans with PTSD. Our objective was to measure trends and factors associated with anticonvulsant prescriptions among veterans with PTSD. METHODS: We obtained administrative and pharmacy data for veterans who initiated PTSD treatment in the US Department of Veterans Affairs (VA) between 2004 and 2013 (N = 731,520). We identified those who received anticonvulsants

Combat-related PTSD in military court: a diagnosis in search of a defense.

As more veterans return from Iraq and Afghanistan, Posttraumatic Stress Disorder (PTSD) often returns with them. As a result, PTSD has quickly become the most prevalent mental disorder diagnosis among active duty United States (U.S.) military. Although numerous studies have not only validated PTSD but have chronicled its negative behavioral impact, it remains a controversial diagnosis. It is widely diagnosed by all types of mental health professionals for even minimal trauma, and DSM-IV PTSD criteria have wide overlap with other mood and anxiety disorders. This, however, has not stopped PTSD f

The Numbing Scale: psychometric properties, a preliminary report.

This study explored the psychometric properties of a new self-report instrument, The Glover Numbing Scale. The scale measures a variety of behaviors reported by individuals experiencing an inability to access feelings other than hostility and rage. The scale was administered to inpatient Vietnam combat veterans with post-traumatic stress disorder (PTSD, n = 323), PTSD diagnosed Veterans Affairs (VA) outpatient and veterans' outreach center veterans (n = 208), two Vietnam veteran noncombat groups (n = 45), two psychiatric noncombat controls (anxiety disorder, n = 40; major depressive disorder,

When a soldier commits suicide in Iraq: impact on unit and caregivers.

There have been studies about the impact of a suicide on family members and friends, but none to date on a military unit in a combat zone. This is the first description, to my knowledge, of the effects of the suicide of a U.S. Army soldier deployed to Iraq. A narrative from the treating psychiatrist's perspective describes both the acute and longer-term (four months) effects of the suicide on members of the soldier's unit, other soldiers at the same base, members of the medical team who attempted to resuscitate him, and mental health care providers. This account describes how the suicide affec

Attachment styles, coping strategies, and posttraumatic psychological distress: the impact of the Gulf War in Israel.

This study examines the association between adult attachment style and the way people reacted to the Iraqi missile attack on Israel during the Gulf War. One hundred forty Israeli students were interviewed 2 weeks after the war and classified according to their attachment style (secure, avoidant, or ambivalent) and residence area (dangerous vs. less dangerous). Ambivalent people reported more distress than secure people. Avoidant persons reported higher levels of somatization, hostility, and trauma-related avoidance than secure persons. These results characterized Ss living in dangerous areas.

Women Vietnam veterans. A comparison of studies.

There is strong evidence that many women exposed to war stress have suffered mental health problems related to their experience, and a substantial number continue to have serious emotional, psychosocial, and other readjustment problems that affect their current level of functioning and life satisfaction. The consistent exposure to severe combat casualties, death and dying, workload extremes, personal deprivation, loss, and danger all take a significant emotional toll. These studies underscore the need for affected women to explore their war experiences and associated feelings with mental healt

Savouring as a moderator of the combat exposure-mental health symptoms relationship.

Engaging in firefights or witnessing death and other types of combat experiences are occupational hazards associated with Post Traumatic Stress Disorder (PTSD) and depression in military personnel returning from combat deployments. The present study examined savouring beliefs as a moderator of the relationship between combat exposure and mental health symptoms among U.S. Army soldiers deployed to Operation Iraqi Freedom and Operation Enduring Freedom. Soldiers (N = 885) completed measures of combat exposure, savouring beliefs, PTSD, and depression. Savouring was negatively related to symptoms

Real-time fMRI amygdala neurofeedback positive emotional training normalized resting-state functional connectivity in combat veterans with and without PTSD: a connectome-wide investigation.

Self-regulation of brain activation using real-time functional magnetic resonance imaging neurofeedback (rtfMRI-nf) is an emerging approach for treating mood and anxiety disorders. The effect of neurofeedback training on resting-state functional connectivity warrants investigation as changes in spontaneous brain activation could reflect the association between sustained symptom relief and brain alteration. We investigated the effect of amygdala-focused rtfMRI-nf training on resting-state functional connectivity in combat veterans with and without posttraumatic stress disorder (PTSD) who were t
