Emergency management of epidural haematoma through burr hole evacuation and drainage. A preliminary report.

BACKGROUND: Blood clot evacuation through an osteoplastic craniotomy, a procedure requiring neurosurgical expertise and modern medical facilities, is the accepted method for treatment of a pure traumatic epidural haematoma following closed head injury. In certain emergency situations and/or in less sophisticated settings, however, use of this procedure may not be feasible. The present study was undertaken to ascertain whether placement of a burr hole and drainage under negative pressure constituted a rapid, effective and safe approach to manage patients with simple epidural haematomas. Tilt testing.

Tilt testing can help to diagnose unexplained syncope, by precipitating an episode during cardiac monitoring. The Italian protocol, now most widely used, involves giving sublingual nitroglycerine after 15 min, while monitoring beat-to-beat blood pressure (BP) and recording on video. Tilt testing is time-consuming but it is clinically useful and can guide therapy. Complications are rare. Syncope types include vasovagal syncope where BP falls after >3 min of tilt-up and later the heart rate falls; classic orthostatic hypotension where there is an immediate, progressive BP fall with minimal heart

Linear thinking does not reflect the newer 21st-century anesthesia concepts. A narrative review.

The brain constitutes a good example of a chaotic, nonlinear biological system where large neuronal networks operate chaotically with random connectivity. This critical state is significantly affected by the anesthetic loss of consciousness induced by drugs whose pharmacological behavior has been classically based on linear kinetics and dynamics. Recent developments in pharmacology and brain monitoring during anesthesia suggest a different view that we tried to explore in this article. The concepts of effect-site for hypnotic drugs modeling a maximum effect, electroencephalographic dynamics du

Polypharmacy-related Shock Symptoms and Complications Associated with Phenothiazine.

This report describes a case of shock symptoms in a 72-year-old woman with epilepsy who had been in a state of polypharmacy, taking multiple antipsychotic drugs. After receiving a normal dose of periciazine, she exhibited impaired consciousness, hypothermia, and hypotension and was admitted to hospital. Despite poor response to vasopressors, conservative treatment led to gradual improvement. Subsequent pharmacokinetic analysis showed non-toxic blood concentrations of periciazine, suggesting that even small doses of phenothiazines could result in toxic symptoms. This case highlights the importa

[Sepsis-associated encephalopathy].

Sepsis-associated encephalopathy is a common but neglected clinical symptom of systemic inflammatory reaction in the early phase. The clinical spectrum of diffuse cerebral dysfunction induced by systemic sepsis--sepsis-associated encephalopathy according to the new terms--varies from transient, reversible encephalopathy, to severe irreversible brain damage. The aim of the present publication is to summarize the pathophysiology, frequent symptoms and possible treatments of the disease based on international and Hungarian articles on this topic. We want to emphasize the importance of monitoring

Continuous amplitude-integrated electroencephalography for prognostication of cardiac arrest patients undergoing extracorporeal cardiopulmonary resuscitation with targeted temperature management.

BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) has been increasingly used for adult cardiac arrest (CA) patients refractory to conventional CPR. However, data on early prognosticators of neurological outcome are lacking. METHODS: CA patients undergoing ECPR were prospectively monitored via amplitude-integrated EEG (aEEG). Targeted temperature management (TTM) was induced using an extracorporeal membrane oxygenation system. aEEG background patterns were classified into continuous normal voltage (CNV), discontinuous normal voltage (DNV), low voltage (LV), flat trace (FT), burst

Open multi-center intracranial electroencephalography dataset with task probing conscious visual perception.

We introduce an intracranial EEG (iEEG) dataset collected as part of an adversarial collaboration between proponents of two theories of consciousness: Global Neuronal Workspace Theory and Integrated Information Theory. The data were recorded from 38 patients undergoing intracranial monitoring of epileptic seizures across three research centers using the same experimental protocol. Participants were presented with suprathreshold visual stimuli belonging to four different categories (faces, objects, letters, false fonts) in three orientations (front, left, right view), and for three durations (0

Anticoagulation Reversal in Intracerebral Hemorrhage: A Case Report on the Efficacy of Andexanet Alfa in an Apixaban-Treated Patient.

Intracerebral hemorrhage (ICH) presents complex clinical challenges, particularly in patients receiving anticoagulation therapy. This case report discusses the management of acute ICH in a 60-year-old male patient on long-term apixaban therapy, who arrived at the emergency department with altered consciousness, right-sided hemiplegia, and mixed aphasia. Computed tomography (CT) imaging revealed a 70 ml left lenticular-capsular hematoma with significant mass effect, necessitating rapid intervention. Due to the patient's deteriorating neurological condition, andexanet alfa was administered as a

Hyperammonemia in a pregnant woman with citrullinemia type I: a case report and literature review.

BACKGROUND: Citrullinemia type I (CTLN1) is a rare urea cycle disorder (UCD) with few adult cases described so far. Diagnosis of late-onset CTLN1 is difficult, and delayed treatment may increase the risk of severe hyperammonemia. Pregnancy is an important risk factor for women with CTLN1. However, the clinical manifestations of CTLN1 in a pregnant woman may be mistaken for pregnancy side effects and ultimately delay a timely diagnosis. CASE PRESENTATION: A 34-year-old woman developed vomiting and disturbance of consciousness after 12 weeks of gestation. A blood test showed hyperammonemia (454

A Simplified Electroencephalogram Monitoring System in the Emergency Room.

OBJECTIVES: The study aims to assess the clinical usefulness and related limitations of our simplified emergency electroencephalography (eEEG) as an assessment tool of lethal abnormal brain waves in the emergency room (ER). METHODS: Electrodes were placed on 4 places: bilateral frontal poles and parietal regions. The derivations to judge consisted of only 2 bipolar leads on the left and right. Abnormal wave was defined as either persistent rhythmic waves or persistent high-amplitude slow waves (<2 Hz). The indications of eEEG were as follows: prolonged impairment of consciousness, suspected s

Impaired Consciousness Following Cerebrospinal Fluid Leakage After Halo Vest Fixation for Odontoid Fracture: A Case Report.

CASE: An 82-year-old woman sustained an Anderson Type II odontoid fracture after a fall and underwent halo vest fixation. Eight weeks later, she developed eyelid edema and altered consciousness. Computed tomography revealed intracranial pin penetration, cerebrospinal fluid (CSF) leakage, and pneumocephalus. The halo vest was removed, and subsequent conservative treatment with bed rest and antibiotics led to full neurological recovery. CONCLUSION: This rare case of intracranial pin penetration and CSF leakage following halo vest fixation emphasizes the importance of close monitoring and early

Syncope: diagnosis of cardiac and noncardiac causes.

Syncope is a sudden and temporary loss of consciousness not caused by trauma or seizures. Patients age 65 and older are at elevated risk of syncope-related falls and sudden cardiac death. Cardiovascular causes are generally electrical (ie, arrhythmias) or mechanical (obstruction of central circulation at a cardiac valve or major vascular structure). Noncardiovascular causes include orthostatic hypotension, vasovagal reaction, micturition, carotid sinus hypersensitivity, and neurologic (eg, TIAs). Many causes of syncope can be diagnosed from a thorough history and physical exam. More extensive

Development of mild encephalitis with a reversible splenial lesion prior to the diagnosis of Kawasaki disease: a pediatric case report.

Kawasaki disease (KD) rarely causes neurological complications. KD is diagnosed based on symptoms alone and can be very difficult to diagnose if other symptoms appear in febrile children before the main symptoms of KD. A 5-year-old boy with fever and consciousness disturbance was hospitalized and diagnosed with mild encephalitis/encephalopathy with reversible splenial lesion (MERS). The fever and consciousness disturbance resolved with intravenous methylprednisolone for 3 days (30 mg/kg/day) and intravenous immunoglobulin (IVIG; 1 g/kg/day) for 2 days, which was initiated as treatment for MERS

Infective Endocarditis-Associated Cerebral Embolism With Successful Early Thrombectomy Followed by Subarachnoid Hemorrhage Six Hours Post-treatment: A Case Report.

Infective endocarditis (IE)-associated cerebral embolism can lead to severe complications, including subarachnoid hemorrhage (SAH). Inflammation spreading from the embolus to the arterial wall may cause vascular fragility, increasing the risk of hemorrhage even after successful recanalization. Furthermore, mechanical stress during thrombectomy may intensify this fragility, potentially precipitating hemorrhage. However, the timing of the onset of these hemorrhagic complications remains unclear. We report a 71-year-old man with IE who presented with sudden consciousness disturbance three months

A 3rd ventricular colloid cyst causing acute hydrocephalus with stunned myocardium: A case report.

BACKGROUND: Third ventricular colloid cysts are benign but may cause acute hydrocephalus, raised intracranial pressure, decreased consciousness level, and sudden death. These ventricular colloid cysts associated with stunned myocardium are rarely reported in the literature. This study reported a case of a third ventricular colloid cyst presented as acute hydrocephalus complicated with severe neurogenic pulmonary edema, stunned myocardium, and heart failure, which survived at the end. CASE PRESENTATION: A 29-year-old woman presented to the emergency department with one day history of headache,

Pharmacokinetics and pharmacodynamics of propofol: changes in patients with frontal brain tumours.

BACKGROUND: Models of propofol pharmacokinetics and pharmacodynamics developed in patients without brain pathology are widely used for target-controlled infusion (TCI) during brain tumour excision operations. The goal of this study was to determine if the presence of a frontal brain tumour influences propofol pharmacokinetics and pharmacodynamics and existing PK-PD model performance. METHODS: Twenty patients with a frontal brain tumour and 20 control patients received a propofol infusion to achieve an induction-emergence-induction anaesthetic sequence. Propofol plasma concentration was measur

Could autonomic system assessment be helpful in disorders of consciousness diagnosis? A neurophysiological study.

Although patients with chronic disorders of consciousness (DOC), including unresponsive wakefulness syndrome (UWS) and minimally conscious state (MCS), show a limited repertoire of awareness signs, owing to a large-scale cortico-thalamo-cortical functional disconnectivity, an activation of some cortical areas in response to relevant stimuli has been described by means of electrophysiological and functional neuroimaging approaches. In addition, cognitive processes associated with autonomic nervous system (ANS) responses elicited by nociceptive stimuli have been identified in some DOC patients.
