Shared Decision Making in Acute Pain Management in Patients with Opioid Use Disorder: A Scoping Review.

The treatment of acute pain over the years has changed with increasing alternative therapies and increased scrutiny of opioid prescriptions. Shared Decision Making (SDM) has become a vital tool in increasing patient engagement and satisfaction in treatment decisions. SDM has been successfully implemented in the management of pain in a variety of settings; however, information regarding the use of SDM for treating acute pain in patients with a history of opioid use disorder (OUD) remains scarce. Following the Preferred Reporting Items for Systematic Reviews and Meta-analysis Extension for Scopi

Shared decision-making for postoperative opioid prescribing and preoperative pain management education decreases excess opioid burden.

INTRODUCTION: Managing postoperative pain requires an individualized approach in order to balance adequate pain control with risk of persistent opioid use and narcotic abuse associated with inappropriately outsized narcotic prescriptions. Shared decision-making has been proposed to address individual pain management needs. We report here the results of a quality improvement initiative instituting prescribing guidelines using shared decision-making and preoperative pain expectation and management education to decrease excess opioid pills after surgery and improve patient satisfaction. METHODS:

Challenges to Employing Shared Decision Making With Adults Under Community Supervision Who Have a Mental Illness.

Adults under community corrections supervision and who have a mental illness (MI) are expected to comply with conditions of release which often include involvement with supportive social services. The rates of technical violation, arrest, and incarceration that result from failure to comply with these mandates are exceedingly high. Shared decision making among officer-supervisors and client-supervisees is a promising approach to promote engagement in community corrections services among supervisees who have MI. This paper reviews recent research on shared decision making and identifies three b

Evaluation of Prostate Cancer Risk Calculators for Shared Decision Making Across Diverse Urology Practices in Michigan.

OBJECTIVE: To compare the predictive performance of a logistic regression model developed with contemporary data from a diverse group of urology practices to that of the Prostate Cancer Prevention Trial (PCPT) Risk Calculator version 2.0. MATERIALS AND METHODS: With data from all first-time prostate biopsies performed between January 2012 and March 2015 across the Michigan Urological Surgery Improvement Collaborative (MUSIC), we developed a multinomial logistic regression model to predict the likelihood of finding high-grade cancer (Gleason score ≥7), low-grade cancer (Gleason score ≤6), or n

A shared decision-making tool for obstructive sleep apnea without tonsillar hypertrophy: A randomized controlled trial.

OBJECTIVES/HYPOTHESIS: Shared decision-making is a process whereby patients and clinicians jointly establish a treatment plan integrating clinical evidence and patient values and preferences. Although this approach has been successfully employed in numerous medical disciplines, often using shared decision-making tools, otolaryngologic research assessing its use is scant. Our primary objective was therefore to determine if the tools we developed reduced decisional conflict for children with obstructive sleep apnea without tonsillar hypertrophy. STUDY DESIGN: Prospective, single-blind, randomiz

Development of the FOCUS tool to guide prioritization of clinical guideline recommendations for shared decision-making.

INTRODUCTION: While clinical practice guidelines (CPGs) provide general recommendations, their application to individual patients requires consideration of differences in preferences, values, and practical circumstances. Shared decision-making (SDM) can help bridge this gap. To better integrate SDM into CPGs, this study aimed to develop a pragmatic tool to support CPG developers in identifying and prioritizing recommendations for which SDM may be particularly relevant. METHODS: The development of our tool, FOCUS (Facilitating priOritization of CPG recommendations to sUpport Shared decision-ma

Navigating the complexities of rectal cancer management: Shared decision-making and patient perspectives in critical choices.

Patients with rectal cancer now may face several potential pathways for treatment, including options to avoid major surgery, omit radiation, or maintain bowel continuity. In preference studies, clinicians often prioritize cancer outcomes, while many patients prefer to avoid an ostomy, surgery, or otherwise maintain quality of life. Treatment morbidity remains high, including major bowel, bladder, and sexual dysfunction, with variable effects on quality of life. Clinicians and patients should work together to incorporate quality of life goals when devising treatment plans. Future research into

Promoting shared decision-making in colorectal cancer screening in primary care: A cluster randomized controlled trial.

INTRODUCTION: In Switzerland, primary care physicians (PCP) prescribe colonoscopy for colorectal cancer (CRC) screening rather than offering a choice between colonoscopy and faecal occult blood test (FOBT). This study evaluated a training program promoting shared decision-making for CRC screening. METHODS: PCP from a research network were randomized 1:1 into intervention or control. The intervention group received study materials, patient decision aids, evidence summary, FOBT sample kit, and personalized feedback on CRC screening practices. PCP documented CRC screening decisions of 40 consecu

Shared decision-making and individual strategies for the patient with chronic pain: How does it develop?

PURPOSE OF REVIEW: Chronic pain management increasingly requires a patient-centered, adaptive approach. This review examines how shared decision-making (SDM) and individualized strategies develop and function in chronic pain care, emphasizing their relevance in practice. RECENT FINDINGS: Recent literature highlights that SDM improves patient satisfaction, engagement, and psychological outcomes across various chronic pain conditions. However, SDM is most effective when viewed as an ongoing, iterative process rather than a one-time decision. Studies show that alignment between patient and clini

"Do Everything" versus realistic surgical interventions: navigating the road to shared decision-making in trauma surgery.

"DO EVERYTHING!!" How many times have you heard that phrase from a distressed family member? We routinely receive this desperate plea, and our knee-jerk reaction is to press on, push harder, be more aggressive. We, as acute care surgeons, have spent our lives learning a craft geared towards "doing everything" to heal patients so this response comes naturally. However, how do we respond to "do everything" when the probability of a meaningful recovery is extremely low or non-existent? These situations create moral tension for surgeons who must reconcile the desire to preserve life with the oblig

Shared decision-making for lung cancer screening in a pre-implementation context: A mixed-methods study.

OBJECTIVES: Lung cancer screening with low-dose computed tomography is increasingly recommended as a strategy to reduce lung cancer mortality. In the context of such screening programmes, shared decision-making (SDM) is essential for informed participation, yet how it should be meaningfully implemented remains unclear. METHODS: We conducted a mixed-methods study combining a scoping review and an exploratory qualitative interview study. The scoping review examined how SDM is described and currently applied in lung cancer screening. Seven databases were searched from inception to February 2025.

Shared Decision-Making with Patients with Multimorbidity and Limited Health Literacy: The Importance of Values Clarification.

Patients with multimorbidity (having two or more chronic conditions) often face complex health decisions involving trade-offs, competing outcomes, and cumulative treatment burden. Shared decision-making (SDM) can support these decisions by applying values clarification: identifying what matters to a patient, a key component of SDM. Values clarification is particularly important for patients with multimorbidity, as separate disease-specific recommendations may conflict and overlook what matters to the patient. Despite its importance, values clarification is underused, particularly when communic

Perceived Shared Decision Making during Maintenance Hemodialysis: Associations with Health Literacy, Social Deprivation, and Care Setting.

KEY POINTS: Perceived shared decision making during routine hemodialysis care was low and heterogeneous across patients. Higher communicative and critical health literacy and lower deprivation were associated with higher perceived shared decision-making. Dialysis autonomy and unit type were associated with perceived shared decision making, suggesting organizational targets for patient-centered care. BACKGROUND: Shared decision-making (SDM) is a core component of patient-centered care. In CKD, SDM research has largely focused on dialysis modality choice, with limited attention to routine clini

Barriers and Facilitators to Shared Decision-Making in EGFRm Non-Small Cell Lung Cancer: Patient Perspectives.

INTRODUCTION: The Global Lung Cancer Coalition Patient Charter highlights that every patient should have involvement in decision-making. In lung cancer, patient involvement in treatment decision-making is often perceived as difficult to achieve. PURPOSE: This study explored the experiences of patients with epidermal growth factor receptor mutated (EGFRm) non-small cell lung cancer (NSCLC) to identify the barriers and facilitators to their involvement in decision-making.
