Reverse Triggered Breath during Pressure Support Ventilation and Neurally Adjusted Ventilatory Assist at Increasing Propofol Infusion.

BACKGROUND: Reverse triggered breath (RTB) has been extensively described during assisted-controlled modes of ventilation. We aimed to assess whether RTB occurs during Pressure Support Ventilation (PSV) and Neurally Adjusted Ventilatory Assist (NAVA) at varying depths of propofol sedation. METHODS: This is a retrospective analysis of a prospective crossover randomized controlled trial conducted in an Intensive Care Unit (ICU) of a university hospital. Fourteen intubated patients for acute respiratory failure received six trials of 25 minutes randomly applying PSV and NAVA at three different p

Impact of sedation levels on outcomes in neurocritical care patients with intracranial hemorrhage: a retrospective cohort study.

Sedation in neurointensive care is essential for managing patients with acute brain injuries. While sedation is commonly employed to alleviate stress responses and enhance patient comfort, the relationship between sedation practices and clinical outcomes remains unclear. This retrospective cohort study analyzed electronic medical records of patients with intracranial hemorrhage admitted to a tertiary care neurosurgical center from January 2020 to December 2023. Patients with a Glasgow Coma Scale (GCS) ≥ 13, early mortality, or prior treatment elsewhere were excluded. Sedation depth was assesse

Melatonin is useful alternative for sedation in children undergoing auditory brainstem responses testing.

Auditory brainstem responses testing (ABRs) is frequently required to assess auditory function in children. It is done usually in outpatient fashion and requires deep sleep to avoid artefacts. Sedation method used for the test should allow a deep sleep while avoiding general anaesthesia that requires special monitoring, dedicated staff and operating room availability. For this purpose, several sedation methods have been used, with the risk of respiratory depression and sides effects. We aim to assess the efficacy and the usefulness of melatonin in sedation for children undergoing auditory brai

Efficacy and safety of remimazolam combined with remifentanil for sedation during awake fiberoptic intubation: a randomized controlled trial.

INTRODUCTION: There has been not study determining if remimazolam combined with remifentanil is the reasonable dosing regimen for sedation during awake fiberoptic intubation (AFOI). This prospective double-blind randomized controlled trial compare efficacy and safety of sedation using different-dose remimazolam combined with remifentanil for AFOI procedure. METHOD: One hundred and fifty patients were randomly assigned to five groups receiving different interventions. The Rf group received only remifentanil infusion of 0.05 mcg/kg/min, the Rm group received only remimazolam 0.6 mg/kg/h, and th

Feasibility of single- vs two-physician procedural sedation in a small community emergency department.

OBJECTIVE: Sedation is commonly required for painful procedures in the emergency department (ED). Some facilities mandate two physicians be present for deep sedation cases. Evidence is lacking, however, that a two-physician approach improves safety outcomes. We report our experience on the feasibility of replacing a two-physician ED procedural sedation policy with a single-physician policy in a small, single-coverage community ED. METHODS: This is a retrospective, before/after, single-center observational study of prospectively collected data from January 2013 through December 2016. In Septem

Use of continuous bispectral EEG monitoring to assess depth of sedation in ICU patients.

Monitoring the depth of sedation in patients under intensive care is difficult. Clinical assessment by the different scoring systems produces insufficient information, especially once deeply sedated patients become unresponsive to any external stimulation. Recently, the bispectral index (BIS), the result of computerized bispectral electroencephalographic monitoring, was found to be the best predictor of depth of anaesthesia during surgical intervention. This report concerns BIS monitoring in 18 randomly selected, deeply sedated, surgical patients in the intensive care unit, who were unresponsi

Evidence That Sedative Effects of Benzodiazepines Involve Unexpected GABAA Receptor Subtypes: Quantitative Observation Studies in Rhesus Monkeys.

In nonhuman primates we tested a new set of behavioral categories for observable sedative effects using pediatric anesthesiology classifications as a basis. Using quantitative behavioral observation techniques in rhesus monkeys, we examined the effects of alprazolam and diazepam (nonselective benzodiazepines), zolpidem (preferential binding to α1 subunit-containing GABAA receptors), HZ-166 (8-ethynyl-6-(2'-pyridine)-4H-2,5,10b-triaza-benzo[e]azulene-3-carboxylic acid ethyl ester; functionally selective with relatively high intrinsic efficacy for α2 and α3 subunit-containing GABAA receptors), M

Efficacy and safety of dexmedetomidine versus midazolam for sedation in mechanically ventilated pediatric patients: a randomized clinical trial.

AIMS: The benefits of Dexmedetomidne in sedation of mechanically ventilated pediatric patients are still challenging. In this study, we compared the efficacy and safety of Dexmedetomidine versus Midazolam for achieving deep sedation in pediatric patients receiving mechanical ventilation. METHODS: A randomized clinical trial was conducted across three affiliated hospitals during February 2023 until February 2024. Eligible mechanically ventilated children aged 1 month to 18 years were randomized 1:1 to receive either Dexmedetomidine (0.25-1 mcg/kg/h) or midazolam (1-4 mcg/kg/min) for sedation,

An initial investigation of bispectral monitoring as an adjunct to nurse-administered propofol sedation for colonoscopy.

BACKGROUND AND AIMS: Bispectral (BIS) monitoring has been used to evaluate depth of sedation in intensive care and surgical patients. We sought to explore its utility as a monitoring device for nurse-administered propofol sedation (NAPS) during colonoscopy. METHODS: Fifty consecutive patients (ASA I or II) receiving NAPS for outpatient colonoscopy were evaluated. BIS scores, sedation scores, and propofol dosing were correlated. The nurses assessed the usefulness of BIS by questionnaire. RESULTS: The mean (SD) dose of propofol required to produce a BIS value </= 60 was 109.4 (43.9) mg and an

Monitoring the depth of palliative sedation by video-pupillometry: A case report.

BACKGROUND: Palliative sedation is sometimes interrupted by undesired arousals. Pupillometry has been used in anesthesiology to monitor pain and sedation but has never been used during palliative sedation. ACTUAL CASE: A 48 years-old patient, with multi-metastatic cancer, underwent palliative sedation to manage global suffering. On the second day, the patient experienced arousal which required medication adjustments to ensure pain relief and increased sedation. POSSIBLE COURSE OF ACTION: Depth of sedation is monitored with clinical scales, such as the Richmond Agitation-Sedation Scale. But t

Technique, sedation, and clinical outcome of endoscopic submucosal dissection for rectal tumor with involvement of dentate line: A retrospective cohort study.

BACKGROUND: Endoscopic submucosal dissection (ESD) of rectal tumors involving the dentate line (RT-DL) is challenging because of the anatomical features of the anal canal. This study aimed to identify optimal techniques and sedation and to determine the clinical outcomes of ESD for RT-DL. METHODS: We retrospectively collected medical records and endoscopic results of patients who underwent ESD for rectal tumors between January 2012 and April 2021. Patients were divided into RT-DL and rectal tumors not involving the dentate line (RT-NDL) groups, as per involvement of the dentate line. The trea

Opioid complications and side effects.

Medications which bind to opioid receptors are increasingly being prescribed for the treatment of multiple and diverse chronic painful conditions. Their use for acute pain or terminal pain is well accepted. Their role in the long-term treatment of chronic noncancer pain is, however, controversial for many reasons. One of the primary reasons is the well-known phenomenon of psychological addiction that can occur with the use of these medications. Abuse and diversion of these medications is a growing problem as the availability of these medications increases and this public health issue confounds

This article outlines essential guidelines and protocols for safe outpatient sedation practice. It emphasizes the clinician's responsibility to prevent and detect life-threatening complications by adhering to recommended monitoring standards. All patients, regardless of sedation depth, must be monitored for oxygenation, ventilation, and hemodynamics. Core requirements include the use of non-invasive devices such as a blood pressure cuff, pulse oximeter, and stethoscope, with capnography mandated for moderate to deep sedation. The article also highlights advanced wireless monitoring tools for e

Sedatives and sedation at the end of life: a nursing home retrospective cohort study.

OBJECTIVES: Sedatives are frequently used at the end of life in specialist palliative care. There is scarce information about their use in nursing homes. Therefore, we aimed to assess the use of (1) sedatives generally and (2) 'sedatives with continuous effect', based on objective operational criteria, within the last week of life in a nursing home. METHODS: This was a retrospective cohort study of residents who died in a German nursing home between 1/2015 and 12/2017, using the nursing home's medical records, which contained drug sheets and nurses' notes. Sedatives analysed were those recomm

Propofol for implantable cardioverter defibrillator implantation in patients with Brugada syndrome.

AIMS: Avoiding propofol in patients with Brugada syndrome has been suggested because of the theoretical risk of provoking ventricular arrhythmias, although propofol may be selected for conscious sedation during electrophysiological procedures in catheterization laboratories. This study aimed to document periprocedural electrocardiographic changes and adverse events in patients with Brugada syndrome undergoing implantable cardioverter defibrillator (ICD) implantation using propofol sedation. METHODS: We reviewed the clinical data of 53 consecutive patients who underwent ICD implantation during

Peripheral Nerve Block as a Supplement to Light or Deep General Anesthesia in Elderly Patients Receiving Total Hip Arthroplasty: A Prospective Randomized Study.

BACKGROUND: Peripheral nerve block combined with general anesthesia is a preferable anesthesia method for elderly patients receiving hip arthroplasty. The depth of sedation may influence patient recovery. Therefore, we investigated the influence of peripheral nerve blockade and different intraoperative sedation levels on the short-term recovery of elderly patients receiving total hip arthroplasty. METHODS: Patients aged 65 years and older undergoing total hip arthroplasty were randomized into 3 groups: a general anesthesia without lumbosacral plexus block group, and 2 general anesthesia plus
