The global impact of life-saving medical devices is directly related to their availability. Access may be limited by cost, availability, or lack of information regarding effectiveness and safety. Addressing the inequity in access requires concerted effort from device developers, the research community, global agencies and professional organisations. We discuss, with examples, three strategies to promote equity: low-cost, purpose-built innovation, improvisation and off-label use. First, developing simple, safe and low-cost innovative devices can be an effective way of increasing global access.

At its advent, carotid artery stenting (CAS) was widely touted by some as a revolution in the revascularization of severe carotid artery disease, destined to replace carotid endarterectomy (CEA). However, 15 years after the early phases of CAS, it still has only a limited role in carotid therapy in the US. This article will explore the reasons why, which are related to expert operator availability, early trials (largely EU based) that did not support the safety of CAS but which were confounded by a variety of factors which severely limited a true assessment of CAS outcomes, mandatory time-line

Hypertension is a major contributor to morbidity and mortality in the United States, and its management remains a significant challenge for clinicians. Transcatheter renal denervation targeting sympathetic nervous system overactivity offers an additive benefit to medications for patients with uncontrolled hypertension as well as for patients who are not able or unwilling to be treated with medications. Two devices were recently approved for commercial use that employ radiofrequency ablative and ultrasound energy for renal denervation. Both devices have shown durable treatment effects up to 3 y

Several implantable devices are now approved in the United States for rehabilitation of conductive, mixed, and sensorineural hearing loss. This paper will review current rehabilitative options including middle ear implants, used for mild to severe sloping sensorineural loss, and bone-anchored hearing aids, for conductive or mixed hearing loss. The indications for these devices differ, as do the bases for exclusion. Audiologic evaluation parameters will be reviewed as a guideline to selection, and as a baseline to determine potential rehabilitative impact. We will also describe emerging applica

INTRODUCTION: Distal locking is a challenging and time-consuming step in interlocked intramedullary nailing of long bone fractures. Current methods have limitations in terms of simplicity, universality, accuracy, speed, and safety. We propose a novel device and software for distal locking using computer vision.
METHODS AND MATERIALS: The device consists of an universal ancillary clamp, a telescopic arm, a viewfinder clamp, and a radio-opaque cross. The software uses a camera photo from the C-arm intensifier and adjusts for geometric projection deformities. The software employs edge detection,

UNLABELLED: Before a high-risk medical device is approved for implantation into patients, there should be evidence not only of its performance and safety with a favourable benefit-risk ratio, but also of its clinical efficacy. Regulatory guidance on study methodologies is lacking, however, so the European Commission funded the CORE-MD project (Coordinating Research and Evidence for Medical Devices) to advise regulators on appropriate designs for clinical trials of high-risk devices. The CORE-MD consortium recommends that evaluation should be planned in four stages. Randomised controlled trials

OBJECTIVES: To determine the safety and efficacy of the conveyor cardiovascular system (CCS) to facilitate the delivery of large profile transcatheter valve devices.
BACKGROUND: Transcatheter valve devices rely on force provided by the operator to be delivered to their intended position. This delivery may be challenging in a variety of anatomic scenarios. The ability to provide steering from the tip of the device by forming an arterial venous loop may help overcome these challenges.
METHODS: Between May, 2019 and October, 2020, five patients were recruited for delivery of transcatheter valve

BACKGROUND: Persistent pain after implantable cardioverter-defibrillator (ICD) implantation is uncommon and often unexplained after ruling out device-related or musculoskeletal causes. Psychological factors can contribute to symptom persistence, complicating treatment. In rare cases, sham surgery has been considered to address psychogenic pain. This report explores its role in managing atypical post-ICD implantation pain.
CASE SUMMARY: A 76-year-old female with hypertrophic cardiomyopathy, atrial fibrillation, and a history of ICD implantation presented with persistent left parasternal pain r

INTRODUCTION: Onychomycosis is a fungal infection of the nail apparatus that affects 10 - 30% of the global population. Current therapeutic options for onychomycosis have a low to moderate efficacy and result in a 20 - 25% rate of relapse and reinfection. New therapeutic options are needed to broaden the spectrum of treatment options and improve the efficacy of treatment.
AREAS COVERED: This review discusses the emerging pharmacotherapeutics; including topical reformulations of terbinafine, new azole molecules for systemic and topical administration, topical benzoxaboroles and topical polymer

The tricuspid valve has become a major focus of novel structural heart interventions, with the Conformité Européenne approval of 5 devices in Europe and the U.S. Food and Drug Administration approval of 2 devices in the United States. Multiple meta-analyses and large population-based registries have shown that although significant tricuspid regurgitation (TR) often accompanies left heart or pulmonary vascular diseases, it is associated with an increased risk of mortality and a reduced quality of life after adjusting for these comorbidities. Echocardiography remains the imaging modality of choi

BACKGROUND: Deep brain stimulation (DBS) is used as a treatment option for patients diagnosed with a form of obsessive-compulsive disorder (OCD) that is highly resistant to conventional treatment methods. In 2009, DBS was granted a humanitarian device exemption-approval by the Food and Drug Administration after promising preliminary data. Monitoring of long-term safety data through post market surveillance of adverse events has not yet been conducted for DBS in OCD patients. This study aims to address this critical knowledge gap.
METHODS: All patient- and device-related (PR; DR) reports from
