Potential for drug-drug interactions in treating cancer-related nausea and distress.

PURPOSE: To determine the extent and severity of drug-drug interactions between anti-emetics and antipsychotics or antidepressants. SUMMARY: Oncology patients are often required to deal with chemotherapy-induced nausea and vomiting at the same time as psychosocial distress. A review of primary literature, as well as several drug interaction databases, was performed with anti-emetics used in The NCCN® 1.2010 Anti-emesis Guidelines (n = 11) and all currently US-marketed antidepressants or antipsychotics (n = 40).(1) The results from these databases were compiled into a single easy-to-use chart

Complex drug-drug-disease interactions between amiodarone, warfarin, and the thyroid gland.

Many patients with cardiac arrhythmias require concomitant therapy with warfarin and amiodarone. Beyond the predictable pharmacokinetic drug-drug interaction requiring a significant warfarin dose reduction, the iodine-rich amiodarone affects the thyroid gland, causing overt hypothyroidism or thyrotoxicosis in 14%-18% of cases. In turn, thyroid disorders may affect warfarin sensitivity, with hypothyroidism and thyrotoxicosis resulting in increased or decreased warfarin requirements, respectively. We describe 3 patients on concomitant amiodarone and warfarin who developed amiodarone-induced thyr

The Impact of Drug Delivery Systems on Pharmacokinetics and Drug-Drug Interactions in Neuropsychiatric Treatment.

Neuropsychiatric drugs are essential for the management of mental health disorders, yet their therapeutic effectiveness is often compromised by complex pharmacokinetics (PK), significant drug-drug interactions (DDIs), limited blood-brain barrier (BBB) penetration, and poor patient adherence. Advanced drug delivery systems (DDSs) offer innovative strategies to overcome these challenges by improving drug solubility, targeting, release kinetics, and systemic exposure profiles. This review critically examines the impact of DDS on the PK and DDI profiles of psychotropic medications, highlighting ke

Drug-drug interactions of a two-drug regimen of dolutegravir and lamivudine for HIV treatment.

The GEMINI trials have recently shown that a two-drug regimen of dolutegravir plus lamivudine was non-inferior to a three-drug regimen in HIV-infected naïve patients. Accordingly, it is important that physicians be aware and confident about the drug-drug interactions (DDIs) involving dolutegravir, lamivudine, and other medications. Areas covered: Here, we firstly update the available information on the pharmacokinetic features of dolutegravir and lamivudine; subsequently, the articles mainly deals with the predictable DDIs for both antiretroviral drugs, attempting to underline their clinical i

Methods of predicting clinically significant DRUG-DRUG interactions beyond validated interaction checkers.

INTRODUCTION: Drug-drug interactions (DDIs) are an important determinant of the safety of pharmacotherapy, yet only a fraction carry clinically significant consequences. Validated, database-driven interaction checkers detect potential interactions quickly and with high sensitivity, but their low specificity generates numerous alerts of doubtful clinical relevance. There is a pressing need for methods that predict clinically significant interactions in a specific patient, ideally before harm occurs. AREAS COVERED: This narrative review, based on a search of PubMed and Google Scholar for papers

CYP3A-Mediated Metabolism of Zastaprazan in Humans and Associated Drug-Drug Interactions.

Background/Objectives: Zastaprazan (JP-1366) is a novel potassium-competitive acid blocker (P-CAB) used for the treatment of gastroesophageal reflux disease (GERD). To date, its metabolic pathways and metabolism-related drug-drug interactions (DDIs) in humans remain incompletely elucidated. This study aimed to determine the relative contributions (fm) of cytochrome P450 isoforms to JP-1366 elimination and assess its DDI potential. Methods/Results: In vitro metabolic studies using human liver microsomes (HLMs) revealed that JP-1366 was first metabolized to M1, which subsequently underwent furth

Assessing Trends in Cytokine-CYP Drug Interactions and Relevance to Drug Dosing.

The regulation of drug-metabolizing enzymes and transporters by cytokines has been extensively studied in vitro and in clinic. Cytokine-mediated suppression of cytochrome P450 (CYP) or drug transporters may increase or decrease the systemic clearance of drug substrates that are primarily cleared via these pathways; neutralization of cytokines by therapeutic proteins may thereby alter systemic exposures of such drug substrates. The Food and Drug Administration recommends evaluating such clinical drug interactions during clinical development and has provided labeling recommendations for therapeu

Interpretable prediction of drug-drug interactions via text embedding in biomedical literature.

Polypharmacy is a promising approach for treating diseases, especially those with complex symptoms. However, it can lead to unexpected drug-drug interactions (DDIs), potentially reducing efficacy and triggering adverse drug reactions (ADRs). Predicting the risk of DDIs is crucial for ensuring safe drug use, particularly by identifying the types of DDIs and the mechanisms involved. Therefore, this study used biomedical literature to proposed hierarchical attention-based deep learning models to predict DDIs and their types. The proposed model consists of two components: drug embedding and DDI pr

Absence of Clinically Meaningful Drug-Drug Interactions with Rezafungin: Outcome of Investigations.

Rezafungin is a novel once-weekly echinocandin for intravenous injection currently in development for the treatment of Candida infections and the prevention of Candida, Aspergillus, and Pneumocystis infections in allogeneic blood and marrow transplant recipients. While in vitro data indicated that rezafungin exposure was unlikely to be affected by commonly prescribed medicines, interactions resulting in the altered systemic exposure of some drugs coadministered with rezafungin could not be excluded. Two phase 1 open label crossover studies, conducted in healthy subjects, examined drug interact

Drug-drug interactions: Implications for anticoagulation, with focus in patients with cancer.

Patients with cancer have increased risks of venous and arterial thromboembolism and/or atrial fibrillation, for which anticoagulation is commonly used. For these indications, three main types of anticoagulants are recommended or used: low-molecular-weight heparin (LMWH), direct oral anticoagulants (DOACs), and vitamin K antagonists (VKAs), all have different advantages and disadvantages. Drug-drug interactions (DDIs) with anticoagulation are often cautioned against by major guidelines, but evidence remains scarce regarding the best management approach for specific drug combinations, particula
