Idiopathic generalized epilepsy with generalized tonic-clonic seizures (IGE-GTC): a population-based cohort with >20 year follow up for medical and social outcome.

Children with idiopathic generalized epilepsy with only generalized tonic-clonic seizures (IGE-GTC) were selected from the Nova Scotia Childhood Epilepsy population-based cohort. All were neurologically and intellectually normal with generalized spike-wave on EEG. Of the epilepsy cohort of 692, 40(5.8%) had IGE-GTC. Follow-up averaged 22.2+/-7.6 years. Twenty-seven (75%) had a complete terminal remission (seizure-free, off medication) for 16.1+/-8.6 years. There were no deaths or intractable seizures. For those >21 years of age at follow up (N=30), major adverse social outcomes were psychiatri

Association of HLA class II alleles in patients with juvenile myoclonic epilepsy compared with patients with other forms of adolescent-onset generalized epilepsy.

Reports have suggested an association of juvenile myoclonic epilepsy (JME) with an HLA-DR allele. We examined the HLA-DR and DQ frequencies in two populations of epilepsy patients: (1) JME patients and (2) patients with other forms of adolescent-onset, idiopathic generalized epilepsy (IGE). We did DNA-based HLA typing on 24 JME patients and 24 patients with non-JME forms of adolescent-onset IGE, forms that are clinically similar to JME. In typing the HLA region, we paid particular attention to the alleles contributing to the HLA-DR13 type and also to the DQB1 locus alleles that are in linkage

Peri-ictal magnetic resonance imaging abnormalities during status epilepticus in adults with idiopathic generalized epilepsy.

OBJECTIVE: Peri-ictal abnormalities on diffusion-weighted magnetic resonance imaging (DWI-PMA) are well established in patients with status epilepticus (SE). SE occurs in approximately 4-9% of patients with idiopathic generalized epilepsy (IGE), and outcomes are generally more favorable than in SE due to other causes. We investigated whether the rate of DWI-PMA differs between SE associated with IGE and SE due to other etiologies. METHODS: We performed a systematic review and individual patient meta-analysis of all studies published up to January 27th, 2025, reporting adult patients with non-

Using Cenobamate for managing idiopathic generalized epilepsy: A single-center experience and systematic literature review.

OBJECTIVE: To evaluate the single-center experience of Cenobamate's (CNB) off-label use in adults with idiopathic generalized epilepsy (IGE). METHODS: We conducted a retrospective review of electronic health records within a large healthcare system to identify IGE patients prescribed CNB. Extensive clinical and medication-related data were extracted, including monthly seizure frequency at baseline, 3, 6, and 12 months, and at last follow-up. We used CNB retention, a marker of its potential effectiveness and tolerability, for 12 months as the primary outcome of interest. Statistical analysis,

Gene-gene interaction network analysis indicates CNTN2 is a candidate gene for idiopathic generalized epilepsy.

Twin and family studies have established the genetic contribution to idiopathic generalized epilepsy (IGE). The genetic architecture of IGE is generally complex and heterogeneous, and the majority of the genetic burden in IGE remains unsolved. We hypothesize that gene-gene interactions contribute to the complex inheritance of IGE. CNTN2 (OMIM* 615,400) variants have been identified in cases with familial adult myoclonic epilepsy and other epilepsies. To explore the gene-gene interaction network in IGE, we took the CNTN2 gene as an example and investigated its co-occurrent genetic variants in I

Levetiracetam vs Lamotrigine as First-Line Antiseizure Medication in Female Patients With Idiopathic Generalized Epilepsy.

IMPORTANCE: After the recent limitations to prescribing valproate, many studies have highlighted the challenging management of female patients of reproductive age with idiopathic generalized epilepsy (IGE). However, no study, to the authors' knowledge, has addressed the comparative effectiveness of alternative antiseizure medications (ASMs) in these patients. OBJECTIVE: To compare the effectiveness and safety of levetiracetam and lamotrigine as initial monotherapy in female patients of childbearing age with IGE. DESIGN, SETTING, AND PARTICIPANTS: This was a multicenter, retrospective, compar

A generalized deep learning network for fractional anisotropy reconstruction: Application to epilepsy and multiple sclerosis.

Fractional anisotropy (FA) is a quantitative map sensitive to microstructural properties of tissues in vivo and it is extensively used to study the healthy and pathological brain. This map is classically calculated by model fitting (standard method) and requires many diffusion weighted (DW) images for data quality and unbiased readings, hence needing the acquisition time of several minutes. Here, we adapted the U-net architecture to be generalized and to obtain good quality FA from DW volumes acquired in 1 minute. Our network requires 10 input DW volumes (hence fast acquisition), is robust to

Determinants of postictal agitation and recovery after tonic-clonic seizures in generalized and focal epilepsy.

OBJECTIVE: The postictal state after bilateral tonic-clonic seizures is often prolonged and can have significant impact on a patient's quality of life. Considerable variability exists in the magnitude of postictal agitation and in the speed of recovery, the determinants of which are not well understood. We studied postictal behavior after tonic-clonic seizures in various epilepsy localizations, focusing on postictal agitation and time to responsiveness. METHODS: We retrospectively identified 15 adult patients each with idiopathic generalized, left temporal lobe, right temporal lobe and fronta

Reduced resilience of brain gray matter networks in idiopathic generalized epilepsy: A graph-theoretical analysis.

PURPOSE: The pathophysiology of idiopathic generalized epilepsy (IGE) is still unclear, but graph theory may help to understand it. Here, we examined the graph-theoretical findings of the gray matter network in IGE using anatomical covariance methods. MATERIALS AND METHODS: We recruited 33 patients with IGE and 35 age- and sex-matched healthy controls. Gray matter images were obtained by 3.0-T 3D T1-weighted MRI and were normalized using the voxel-based morphometry tools of Statistical Parametric Mapping 12. The normalized images were subjected to graph-theoretical group comparison using the

Exceptional response to brivaracetam in a patient with refractory idiopathic generalized epilepsy and absence seizures.

Brivaracetam is currently indicated as adjunctive therapy for patients with focal-onset seizures with or without secondary generalization. However, it has been suggested that it could provide broad-spectrum efficacy given its similarity to levetiracetam and based on the results from preclinical studies and others of patients with generalized epilepsy. We present the case of a woman with refractory idiopathic generalized epilepsy and absence seizures with dramatic response to brivaracetam. Our report supports a consideration of treatment with this new antiepileptic drug on a case-by-case basis

Altered brain rhythms and functional network disruptions involved in patients with generalized fixation-off epilepsy.

Generalized Fixation-off Sensitivity (CGE-FoS) patients present abnormal EEG patterns when losing fixation. In the present work, we studied two CGE-FoS epileptic patients with simultaneous EEG-fMRI. We aim to identify brain areas that are specifically related to the pathology by identifying the brain networks that are related to the EEG brain altered rhythms. Three main analyses were performed: EEG standalone, where the voltage fluctuations in delta, alpha, and beta EEG bands were obtained; fMRI standalone, where resting-state fMRI ICA analyses for opened and closed eyes conditions were comput

EEG features of absence seizures in idiopathic generalized epilepsy: impact of syndrome, age, and state.

PURPOSE: Factors influencing the electroencephalography (EEG) features of absence seizures in newly presenting children with idiopathic generalized epilepsy (IGE) have not been rigorously studied. We examined how specific factors such as state, provocation, age, and epilepsy syndrome affect the EEG features of absence seizures. METHODS: Children with untreated absence seizures were studied using video-EEG recording. The influence of state of arousal, provocation (hyperventilation, photic stimulation), age, and epilepsy syndrome on specific EEG features was analyzed. RESULTS: Five hundred nin

Video-EEG evidence of lateralized clinical features in primary generalized epilepsy with tonic-clonic seizures.

BACKGROUND: Whether cortical or subcortical structures, specifically the thalamus, play the dominant role in generating primary generalized seizures has been the subject of long debate. Most experimental data implicate a hyperexcitable cortical generator of spike-and-wave activity, with the thalamus quickly recruited to sustain the generalized oscillations through a reverberating thalamocortical network. However, there is little clinical evidence to support the cortical generator hypothesis. We present video-EEG recordings of generalized tonic-clonic seizures in three patients with proven prim

[Morphological and functional modifications of the acoustic system in golden hamsters with acoustic generalized epilepsy].

We studied the role of the neuronal circuit consisting of the superior olive-cochlear hair cells-auditory nerve complex in the pathophysiology of reflex audiogenic generalized epilepsy in the epileptic golden hamster. A direct role of the auditory function can be assumed from the genetic lesions found in these animals' acoustic system. These animals are a model of the anatomopathologic substrate of a type of hereditary neurosensorial hypoacusia.

The axial spasm--the predominant type of drop seizure in patients with secondary generalized epilepsy.

In 45 patients suffering from secondary generalized epilepsy, we have recorded 239 drop seizures both by radio-telemetered EEG/split-screen video taping and polygraphically. Twenty of these patients had pure tonic drop seizures. Nine patients each had myoclonic-atonic or pure atonic seizures, whereas seven had slow falls that occurred during gradually developing tonic or akinetic seizures. We term the pure tonic drop seizure "axial spasm." It consists of a uniform pattern of movement lasting from 0.5 to 0.8 s and leading to a specific bearing characterized by a moderate flexion of the hips, th

Common epilepsy variants from the general population are not associated with epilepsy among individuals with tuberous sclerosis complex.

Common genetic variants identified in the general population have been found to increase phenotypic risks among individuals with certain genetic conditions. Up to 90% of individuals with tuberous sclerosis complex (TSC) are affected by some type of epilepsy, yet the common variants contributing to epilepsy risk in the general population have not been evaluated in the context of TSC-associated epilepsy. Such knowledge is important to help uncover the underlying pathogenesis of epilepsy in TSC which is not fully understood, and critical as uncontrolled epilepsy is a major problem in this populat

Social cognition dysfunctions in patients with epilepsy: Evidence from patients with temporal lobe and idiopathic generalized epilepsies.

BACKGROUND AND AIM: Despite an extensive literature on cognitive impairments in focal and generalized epilepsy, only a few number of studies specifically explored social cognition disorders in epilepsy syndromes. The aim of our study was to investigate social cognition abilities in patients with temporal lobe epilepsy (TLE) and in patients with idiopathic generalized epilepsy (IGE). MATERIALS AND METHODS: Thirty-nine patients (21 patients with TLE and 18 patients with IGE) and 21 matched healthy controls (HCs) were recruited. Genome-wide copy number variation in epilepsy: novel susceptibility loci in idiopathic generalized and focal epilepsies.

Epilepsy is one of the most common neurological disorders in humans with a prevalence of 1% and a lifetime incidence of 3%. Several genes have been identified in rare autosomal dominant and severe sporadic forms of epilepsy, but the genetic cause is unknown in the vast majority of cases. Copy number variants (CNVs) are known to play an important role in the genetic etiology of many neurodevelopmental disorders, including intellectual disability (ID), autism, and schizophrenia. Genome-wide studies of copy number variation in epilepsy have not been performed. We have applied whole-genome oligonu

Autosomal dominant cortical myoclonus and epilepsy (ADCME) with complex partial and generalized seizures: A newly recognized epilepsy syndrome with linkage to chromosome 2p11.1-q12.2.

We describe a pedigree in which eight individuals presented with a non-progressive disorder with onset between the ages of 12 and 50 years. It was characterized by predominantly distal, semi-continuous rhythmic myoclonus (all patients), generalized tonic-clonic seizures (all patients) and complex partial seizures (three patients). Most individuals had rarely suffered seizures and had a normal cognitive level, but three individuals with intractable seizures had mild mental retardation. The pattern of inheritance was autosomal dominant with high penetrance. We defined this disorder as autosomal
