Tourniquet use in foot and ankle surgery is common practice; however, the technique varies among foot and ankle surgeons and there are no standard guidelines. To analyze trends in foot and ankle tourniquet use, the authors conducted an e-mail survey. One thousand six hundred sixty-five foot and ankle surgeons were sent a tourniquet-use survey via e-mail, across Canada and the United States. Nineteen percent of the recipients completed and returned the surveys. Eleven (3.4%) rarely or never use a tourniquet and 8 (2.5%) use an Esmarch bandage tourniquet at the ankle. Most use pneumatic ankle cu

Large-volume liposuction can be associated rarely with major medical complications and death. The case of exsanguinating retroperitoneal hemorrhage that led to cardiopulmonary arrest in an obese 47-year-old woman who underwent large-volume liposuction is described. Extensive liposuction is not a minor procedure. Performance in an ambulatory setting should be monitored carefully, if it is performed at all. Reporting of adverse events associated with outpatient procedures performed by plastic surgeons should be mandated. Hemodynamic instability in the early postoperative period in an otherwise h

A fistula developed between the left common iliac artery and the distal left ureter of an 83-year-old woman who had undergone aortofemoral bypass grafting many years previously and in whom a Double-J stent was in place for 3 weeks while she was awaiting extracorporeal shock wave lithotripsy. Exsanguinating hemorrhage into the urinary tract necessitated emergency nephrectomy and vascular repair. Although these measures were successful temporarily, the patient died 6 weeks later of repeat myocardial infarction and acute renal failure. The increasing frequency of ureteral stent use and of an oper

Hepatic rupture as a late complication of toxemic pregnancy is a rare yet lethal condition requiring rapid recognition and surgical management. The clinical triad of toxemia, right upper quadrant pain, and sudden hypotension is the diagnostic hallmark of presentation. Most patients present near the time of delivery and are found to have subcapsular hematomas of the right hepatic lobe with free rupture into the peritoneal cavity and resultant exsanguinating hemorrhage. The association of toxemia and disseminated intravascular coagulation with secondary microembolic damage to the liver and other

OBJECTIVE: We present a review of our experience in the management of thoracic vascular injuries during the past 10 years in one of Sweden's busiest emergency departments, with morbidity and mortality as the main outcome measures.
DESIGN: Of eight patients who sustained chest trauma with thoracic vascular injuries, six presented with shock. Angiography was the gold standard in diagnosing aortic and subclavian injuries.
RESULTS: Lesions of the inferior vena cava, left internal mammary artery, and intercostal vessels were detected only at surgery. Two patients died of exsanguination. Managing

Among 28 autopsied patients with aortoenteric fistulas, the mean age was 67 years (range, 19 to 91 years) and 23 (82%) were men. The most common causes were aortobifemoral bypass grafts (16 cases [57%]) and gastrointestinal carcinomas (6 cases [21%]). Fistulas developed at proximal anastomosis sites in 15 of 16 patients with grafts and were associated with radiation injury in 3 of 6 patients with carcinomas. Aortoenteric fistulas involved the duodenum in 16 (57%), esophagus in 9 (32%), and other gastrointestinal sites in 3 patients (11%). They involved the abdominal aorta in 20 cases (71%) and

Role 2 Afloat provides a damage control resuscitation and surgery facility in support of maritime, littoral and aviation operations. Resuscitative endovascular balloon occlusion of the aorta (REBOA) offers a rapid, effective solution to exsanguinating haemorrhage from pelvic and non-compressible torso haemorrhage. It should be considered when the patient presents in a peri-arrest state, if surgery is likely to be delayed, or where the single operating table is occupied by another case. This paper will outline the data in support of endovascular haemorrhage control, describe the technique and e

Arteriovenous malformations (AVMs) of the jaws are relatively rare, with fewer than 200 cases reported in the literature. Their real importance lies in their potential to result in exsanguination, which usually follows an unrelated treatment, such as tooth extraction, surgical intervention, puncture wound or blunt injury in involved areas, with the dentist unaware of the existence of the AVM. The present case illustrates an AVM in an 18-year-old female with swelling on the right side of the face. This case report is unique because although there was no history of bleeding episodes, thorough ex

Bleeding is a common sequela of oral and periodontal surgery. Generally, bleeding is self-limiting. Following traumatic injury or surgical procedures, hemorrhage can range from a minor leakage or oozing at the site, to extensive bleeding leading to complete exsanguinations. Significant postsurgical hemorrhage following periodontal surgery is uncommon due to the primary closure of the soft tissues. This case report describes the unique formation of a "liver clot" or "currant jelly clot" following periodontal flap surgery. The likelihood of this may be attributed to many factors, like infection,

A simple rapid technique for cannulating the right atrium during a resuscitative Emergency Department thoracotomy (EDT) for exsanguinating trauma is described. Following the thoracotomy and pericardiotomy, an ordinary Foley urinary bladder balloon catheter is inserted into the right atrial appendage for rapid, large volume normothermic blood and fluid infusion. A method for simplifying this maneuver, as well as the potential complications of over-resuscitation with myocardial distention and fluid overload, myocardial cooling, air embolism, and tricuspid valve occlusion are discussed. Eight cases of life-threatening sequelae of relatively minor penetrating injuries of the gluteal region, involving the gluteal or internal pudendal arteries, are reported. The modes of presentation included exsanguinating external hemorrhage (two), acute false aneurysm (one), and chronic false aneurysm (five). Failure to appreciate the nature of the lesion led to inappropriate initial management in seven instances. This series emphasizes the danger of direct incision of lesions in this region which are suspected of being abscesses that are in fact false aneurysms ("pulsatile abscesses") and de

PURPOSE OF REVIEW: This review article discusses the most common umbilical cord vessel anomalies and umbilical cord emergencies, as well as their implications on anesthetic management.
RECENT FINDINGS: Umbilical cord anomalies and emergencies pose significant risks to both the fetus and the mother. Fetal complications can include fetal heart tone issues, hypoxia, preterm delivery, unexpected neonatal ICU admissions, exsanguination, and fetal demise. Maternal complications can include emergency cesarean delivery and postpartum hemorrhage. Recognition of these anomalies and their potential comp

This report describes the first use of Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) in the pre-hospital setting to control catastrophic haemorrhage. The patient, who had fallen 15 meters, suffered catastrophic internal haemorrhage associated with a pelvic fracture. He was treated by London's Air Ambulance's Physician-Paramedic team. This included insertion of a REBOA balloon catheter at the scene to control likely fatal exsanguination. The patient survived transfer to hospital, emergency angio-embolization and subsequent surgery. He was discharged neurologically normal aft

Vasa previa are umbilical vessels which run along the fetal membranes over or near the internal cervical os. Unprotected by the umbilical cord or placenta these vessels most often originate from velamentous insertion, low-lying and/or bilobated placenta. Ante- or intrapartum rupture of fetal membranes can provoke laceration of vasa previa causing painless vaginal bleeding with rapid fetal exsanguination and demise. Reduction of associated perinatal morbidity and mortality depends entirely on accurate antenatal diagnosis. In this review we present two cases of prenatally diagnosed vasa previa w

Vasa previa, defined as fetal vessels coursing within the membranes between the presenting part and the cervix, occurs in approximately 1:2500-5000 pregnancies. Type II vasa previa consists of fetal vessels crossing over the internal os connecting a bilobed placenta or a succenturiate lobe with the main placental mass. These vessels are prone to compression during labor or may tear when membranes rupture potentially resulting in fetal exsanguination and neonatal death. This complication could be avoided altogether if the vessels could be obliterated in utero. The purpose of this communication

PURPOSE: To report the successful endovascular repair of an acute axillary artery hemorrhage.
CASE REPORT: An 87-year-old woman with Charcot-Marie-Tooth ataxia presented with an enormous shoulder hematoma and clinical signs of exsanguination after a fall. Angiography demonstrated complete avulsion of the right subscapular artery from the axillary artery, and active bleeding into a hematoma of at least 1500 mL. Endovascular repair with a balloon-mounted covered stent-graft was performed percutaneously, which controlled the bleeding and averted surgery. The patient recovered uneventfully and wa

Two patients who underwent surgical exclusion of the thoracic aorta for chronic dissecting aneurysms of the descending aorta died 4 and 8 years after surgery due to complications in part related to the operative technique. One patient developed pseudoaneurysm of the proximal aortic stump with fatal exsanguination through an aortapulmonary fistula. The other patient developed progressive dilatation of the remaining abdominal aorta with subsequent rupture. The surgical approach in this case was extremely difficult because of the previous exclusion procedure which determined a highly demanding re

It is estimated that between one and four million persons per year are bitten by dogs in the United States. While most injuries associated with the bites are minor, serious sequelae, and even death, may occur. Most victims of fatal dog attacks are children < 1 year of age or elderly women. The most frequent cause of death is hemorrhage and shock from major vessel damage. A case is reported in which an elderly woman was attacked by her pet Chow dog. The victim received multiple superficial abrasions, contusions, and lacerations from the dog attack. A large perforation of the right external pude
