Adipocytes are larger in lymphedematous extremities than in controls.

Lymphedema is caused by dysfunctional lymph vessels or as a complication of cancer treatment leading to edema and adipose tissue deposition. One hypothesis is that adipocyte hypertrophy contributes to the volume increase in lymphedema. The aim of the study was to compare adipocyte size in arm and leg lymphedema and controls. The adipocyte size difference was also compared between the arms and legs. Furthermore, any link between adipocyte size difference and gender, lymphedema onset, duration, previous radio- and chemotherapy was studied, as well as any relationship to total excess volume incre

Umbilicated papules on the bilateral upper and lower extremities.

Cryptococcosis is a rare opportunistic infection with morphologically diverse cutaneous presentations. Primary infection typically occurs in the lungs with subsequent hematogenous dissemination to other organ systems, especially in immunocompromised patients. Herein, we report a woman in her 70's who presented with pruritic, umbilicated papulonodules of the bilateral upper and lower extremities present for many weeks. She was diagnosed with disseminated Cryptococcus and subsequently evaluated for potential pulmonary and meningeal disease involvement. She died as a result of multiple medical co

[Differentiated treatment strategies for bone metastases of the extremities].

BACKGROUND: Bone metastases are the most common malignant bone tumours and most commonly occur in the spine, pelvis, proximal femur and proximal humerus. Prostate and breast cancer most often metastasize to the bone. DIAGNOSTICS: In patients with a history of tumours and local unspecific pain in the back and extremities, further diagnostic examinations should be performed. For the initial diagnosis a conventional radiograph of the whole bone in two planes is sufficient. For further diagnostics, the imaging may be supplemented with CT, MRI, scintigraphy and a PET-CT (PET-MRI) if the findings a

Acute Compartment Syndrome of Upper Extremities with Tendon Ruptures.

BACKGROUND: It is well known that acute compartment syndrome is associated with fracture of the forearm, while involvement of soft tissue injury including musculotendinous injury remains unclear. The purpose of this study was to evaluate the soft tissue involvement, including musculotendinous ruptures, in acute compartment syndrome of the upper limb. METHODS: We retrospectively enrolled 16 patients who underwent surgical treatment for acute compartment syndrome of the upper extremity. The average age of the patients was 47 years (range, 14 to 79) and the mean follow-up period after the surger

Compound or Specially Designed Flaps in the Lower Extremities.

Novel and combined tissue transfers from the lower extremity provide new tools to combat soft tissue defects of the hand, foot, and ankle, or fracture nonunion. Flaps can be designed for special purposes, such as providing a gliding bed for a grafted or repaired tendon or for thumb or finger reconstruction. Propeller flaps can cover soft tissue defects of the leg and foot. In repairing severe bone and soft tissue defects of the lower extremity, combined approaches, including external fixators, one-stage vascularized bone grafting, and skin or muscle flap coverage of the traumatized leg and foo

The results of application of pathogenetically substantiated diagnostic algorithm for determination of the treatment method in patients, suffering postthrombotic disease of the lower extremities, are adduced. Using algorithm proposed a clinical state was estimated, subfascial pressure on the shin was determined, ultrasound duplex scanning (USDS) of venous system was conducted, the stimulation electroneuromyography of the shins done, and a level of D-dimer (DD) with activity of antithrombin-III (AT--III) in general and regional blood flow with calculation of its ratio were established. Clinical applications of SPECT/CT in imaging the extremities.

Today, SPECT/CT is increasingly used and available in the majority of larger nuclear medicine departments. Several applications of SPECT/CT as a supplement to or replacement for traditional conventional bone scintigraphy have been established in recent years. SPECT/CT of the upper and lower extremities is valuable in many conditions with abnormal bone turnover due to trauma, inflammation, infection, degeneration or tumour. SPECT/CT is often used in patients if conventional radiographs are insufficient, if MR image quality is impaired due to metal implants or in patients with contraindications

[Endovascular treatment of multilevel arteriosclerosis obliterans of lower extremities].

OBJECTIVE: To evaluate the clinical effect of percutaneous transluminal angioplasty (PTA) and PTA and stenting (PTAS) in treating multilevel arteriosclerosis obliterans (ASO) of lower extremities. METHODS: Between January 2007 and October 2009, 29 cases of multilevel ASO of lower extremities were treated with PTA or PTAS. There were 17 males and 12 females with an average age of 71 years (range, 56-83 years). The clinical manifestations were limbs numb with cold sensation, intermittent claudication (< 100 m), and 19 patients had rest pain. According to Fontaine stages classification, there we

Osteoid osteoma: a diagnosis for radicular pain of extremities.

Osteoid osteoma is a benign tumor commonly found in young patients. The most interesting feature of the tumor is its peculiar obvious pain. The intensity of pain produced by osteoid osteoma is unproportionate to its nature and size. Much has been written about the mechanism of this symptom of the tumor and the specific characteristics of the pain that can be misleading when appearing as radicular pain especially when some subtle neurologic signs and symptoms accompany the pain. In this article, 12 patients who were referred to our clinic with radicular pain and neurological signs and symptoms

Management of peripheral arterial disease of the lower extremities.

Smoking should be stopped and hypertension, diabetes mellitus, dyslipidemia, and hypothyroidism treated in patients with peripheral arterial disease (PAD) of the lower extremities. Statins decrease the incidence of intermittent claudication and improve exercise duration until the onset of intermittent claudication in persons with PAD and hypercholesterolemia. Antiplatelet drugs such as aspirin or clopidogrel, especially clopidogrel, angiotensin-converting enzyme inhibitors, and statins should be given to all persons with PAD. Beta blockers should be given if coronary artery disease is present.

Macro- and microcirculation in the lower extremities--possible relationship.

AIMS: Impaired blood supply is a significant risk factor for diabetic foot ulceration and gangrene. A possible relationship between peripheral macroangiopathy and the spectral components of microvascular skin blood flow in the lower extremities was tested in diabetic patients (DP) and non-diabetic subjects (C). PATIENTS AND METHODS: Basal skin blood flow (BSBF) was recorded for 30min at the right and left medial malleolus (predominantly nutritive capillary circulation) by laser Doppler flowmetry in 64 DP and 31 C. Its oscillatory components were analyzed using wavelet transform. Peripheral ar

Atherosclerosis obliterans of the lower extremities in Thai patients.

BACKGROUND: Major limbs loss and high mortality rate were observed in the management of Thai patients with atherosclerosis obliterans (ASO) of the lower extremities. These were the results of delayed diagnosis and treatment together with the associated morbidities. There is a lack of information of this disease resulting in a lack of knowledge and awareness of this problem among general practitioners in Thailand. OBJECTIVES: The purposes of this study were (1) to identify the prevalence of this disease in a tertiary care hospital, (2) to enumerate the risk factors and comorbidities, (3) to id

[Specific surgical strategy for gangrene of both lower extremities].

At admission of patients with gangrenes of both lower extremities the surgical strategy was not determined. The author has also failed to find corresponding recommendations in the available literature. On the basis of his clinical experiences with 12 patients who underwent big amputation of lower extremities for bilateral gangrene, the author proposes the surgical methods. The optimal variant is considered to be a by turns amputation with the interamputation period being determined by a relative stabilization of the life-support systems.

[Treatment of penetrating injuries of neck, chest and extremities].

Penetrating injuries such as gunshot wounds have traditionally been rare in Europe, with the result that European surgeons have only limited experience in their management. Socio-economic changes in the last decade have led to an increase in the frequency of injuries of this type, partly due to the expansion of organized crime and partly because of involvement of the European military in peace-keeping missions in Europe and elsewhere in the world. The purpose of this article is to offer some guidelines in the management of extremely serious or rarely encountered injuries to the neck, chest and

[The interdisciplinary approach in reconstructive surgery of the extremities].

In cases of extended post-traumatic soft-tissue and bone loss as well as with mutilating infection or radical tumor resection, multidisciplinary options are required to salvage extremities and functional rehabilitation. A surgical team approach allows for reduction of amputation rates, wound healing complications, and secondary procedures in limb oncology and trauma. The goals and limitations of cooperative surgical concepts are described. In the future, continuing medical education will focus not only on indications and techniques but also on complication management, medicolegal problems, and
