Anticoagulant activity was determined in salivary gland extracts from four species of black flies, i.e., Simulium vittatum Zetterstedt, Simulium argus Williston, Simulium metallicum Bellardi, and Simulium ochraceum Walker. Inhibition of coagulation factor Xa occurred among all four, whereas thrombin inhibition was detected in S. argus and S. vittatum only. Both bovine and human alpha-thrombins were inhibited with the highest activity occurring with S. argus salivary gland extracts. Factor Xa inhibition was highest in S. ochraceum, an anthropophilic species and vector of Onchocerca volvulus, an

CONTEXT: Fondaparinux, a factor Xa inhibitor, is approved for thromboprophylaxis after orthopedic surgery and for treatment of venous thromboembolism. It may also be efficacious, safe, and cost-effective for other patients; thus, more widespread use of fondaparinux is likely. The effect of fondaparinux on coagulation testing needs to be thoroughly examined.
OBJECTIVE: To report the effects of fondaparinux on coagulation tests (prothrombin time, activated partial thromboplastin time, fibrinogen, antithrombin, factor VIII, thrombin time, anti-factor Xa) across diverse methodologies.
DESIGN: Sa

Coronavirus disease 2019 (COVID-19) appears to be associated with increased arterial and venous thromboembolic disease. These presumed abnormalities in hemostasis have been associated with filter clotting during continuous renal replacement therapy (CRRT). We aimed to characterize the burden of CRRT filter clotting in COVID-19 infection and to describe a CRRT anticoagulation protocol that used anti-factor Xa levels for systemic heparin dosing. Multi-center study of consecutive patients with COVID-19 receiving CRRT. Primary outcome was CRRT filter loss. Sixty-five patients were analyzed, includ

The regulation of human Factor IXa was studied in vitro in human and mouse plasma and in vivo in the mouse. In human plasma, approximately 60% of the 125I-Factor IXa was bound to antithrombin III (ATIII) by 2 h, with no binding to alpha 2-macroglobulin or alpha 1-proteinase inhibitor, as assessed by gel electrophoresis and IgG- antiproteinase inhibitor-Sepharose beads. In the presence of heparin, virtually 100% of the 125I-Factor IXa was bound to ATIII by 1 min. The distribution of 125I-Factor IXa in mouse plasma was similar. The clearance of 125I-Factor IXa was rapid (50% clearance in 2 min)

BACKGROUND: Monitoring of unfractionated heparin (UFH) in critically ill patients is challenging due to disease-related alterations of the coagulation system, directly affecting conventional laboratory tests, including activated partial thromboplastin time (aPTT). Measurement of anti-factor Xa activity (Anti-Xa) as an alternative to aPTT often lacks 24/7 availability. Whole blood viscoelastic testing (VET) may provide a feasible point-of-care alternative to guide UFH dosing in this setting. Additionally, VET is readily available in the environment found in our clinic, The aim of this study was

Direct oral anticoagulants (DOACs) have largely replaced vitamin K antagonists owing to their predictable pharmacokinetics and improved safety. In emergency situations such as urgent surgery, major bleeding, or thrombolysis for acute ischemic stroke, rapid assessment of clinically relevant DOAC activity is essential. Conventional viscoelastic testing (VET) using devices such as ROTEM, TEG 5000, TEG6s, or ClotPro is insufficiently sensitive to detect DOACs, prompting the development of modified assays with specific activators. This systematic review summarizes recent advances in VET-based DOAC

There is increasing recognition that thrombotic complications may occur in patients with cirrhosis, and literature on antithrombotic treatment in these patients is rapidly emerging. Due to extensive haemostatic changes in patients with cirrhosis, careful monitoring of anticoagulant therapy may be required. Recent data suggest that plasma levels of low molecular weight heparin (LMWH) are substantially underestimated by the anti-activated factor X (anti-Xa) assay in patients with cirrhosis. We studied the in vitro recovery of antithrombin (AT)-dependent and -independent anticoagulant drugs in pl

BACKGROUND: Low-molecular-weight heparin enoxaparin is widely used in pharmacological thromboprophylaxis after coronary artery bypass grafting (CABG). The aim of this study was to compare anti-factor X activity (anti-Xa) levels when the thromboprophylactic dose of enoxaparin was provided after CABG, with two different administration routes: continuous intravenous infusion (CIV) and subcutaneous bolus (SCB) injection. We hypothesized that the current standard method of SCB administration might lead to lower anti-Xa levels than recommended in other patient groups, due to reduced bioavailability.

BACKGROUND AND PURPOSE: Patients undergoing surgery for traumatic spine injuries have a high risk of venous thromboembolism (VTE). Standard fixed or weight-based enoxaparin regimens may leave some patients below prophylactic anti-factor Xa (anti-Xa) target ranges. Whether anti-Xa-guided enoxaparin dose adjustment can be implemented while maintaining acceptable observed bleeding outcomes has not been studied specifically in postoperative spine trauma patients. This study evaluates anti-Xa target attainment, symptomatic VTE, and safety outcomes after anti-Xa-guided enoxaparin titration.

BACKGROUND: Venous thromboembolism (VTE), encompassing deep vein thrombosis and pulmonary embolism, constitutes a significant cause of postoperative morbidity and mortality among surgical patients. Despite the widespread use of low-molecular-weight heparin (LMWH) for thromboprophylaxis, conventional fixed-dose regimens frequently yield subprophylactic anti-factor Xa (anti-Xa) levels, particularly in individuals with obesity, renal impairment, or altered drug pharmacokinetics. These interindividual variations in LMWH pharmacodynamics underscore the need for personalized anticoagulant strategies

BACKGROUND: Low-molecular-weight heparin (LMWH) is recommended in the treatment of unstable angina (UA)/non-ST-segment-elevation myocardial infarction (NSTEMI), but no relationship has ever been shown between anticoagulation levels obtained with LMWH treatment and clinical outcomes.
METHODS AND RESULTS: In all, 803 consecutive patients with UA/NSTEMI were treated with subcutaneous enoxaparin and were followed up for 30 days. The recommended dose of enoxaparin of 1 mg/kg BID was used throughout the population except when physicians decided on dose reduction because of a history of a recent ble

Direct oral anticoagulants (DOACs), including direct thrombin inhibitors (dabigatran) and direct factor Xa inhibitors (apixaban, rivaroxaban, edoxaban), have transformed anticoagulant management in recent years due to their predictable pharmacodynamics, rapid onset of action, and fixed dosing without the need for routine laboratory monitoring. Unfractionated heparin (UFH) remains the anticoagulant of choice for patients who are acutely unwell and treated in intensive care units due to its short half-life, reversibility, ease of dose titration, and nonrenal dependent excretion. It is therefore

The optimum heparin monitoring method during extracorporeal membrane oxygenation (ECMO) is unknown. We report a protocol utilizing only anti-factor Xa (anti-Xa) to manage anticoagulation in 22 consecutive ECMO patients. Anti-Xa was monitored with heparin titration every hour until goal 0.4-0.8 IU/ml. Once therapeutic, monitoring was progressively spaced up to every 6 hours. Patients received frequent antithrombin III (ATIII). Extracorporeal membrane oxygenation indications were as follows: 13 cardiorespiratory failures, eight extracorporeal cardiopulmonary resuscitations (ECPRs), and one pulmo

This article about unfractionated heparin (UFH) and low-molecular-weight heparin (LMWH) is part of the Seventh American College of Chest Physicians Conference on Antithrombotic and Thrombolytic Therapy: Evidence-Based Guidelines. UFH is a heterogeneous mixture of glycosaminoglycans that bind to antithrombin via a pentasaccharide, catalyzing the inactivation of thrombin and other clotting factors. UFH also binds endothelial cells, platelet factor 4, and platelets, leading to rather unpredictable pharmacokinetic and pharmacodynamic properties. Variability in activated partial thromboplastin time

BACKGROUND: Enoxaparin is an anticoagulant used for pharmacologic thromboprophylaxis in pediatrics. Enoxaparin pharmacokinetics can be altered in the setting of obesity. Optimal enoxaparin dosing for thromboprophylaxis in children with obesity remains unclear.
PROCEDURE: A retrospective review was conducted of pediatric patients who weighed ≥60 kg with BMI ≥ 95th percentile, received enoxaparin for thromboprophylaxis, and had at least one appropriately drawn anti-factor Xa (anti-Xa) from 2013 to 2022. Anti-Xa levels were reviewed for patients initially treated with enoxaparin 30 mg every 12 h

BACKGROUND: Intracranial hemorrhage (ICH) is a complication of oral anticoagulation and is associated with significant morbidity and mortality. Clinical need exists for biomarkers to measure anticoagulation in patients with factor Xa inhibitor-associated ICH to assess the hemostatic effect of reversal agents. This study explored the utility of thromboelastography (TEG) to assess anticoagulation in emergency department (ED) patients who received activated prothrombin complex concentrate (aPCC) reversal for factor Xa-inhibitor-associated ICH.
METHODS: This was a prospective, single-center, coho

There is a real and viable need in the anticoagulant arena for a safe, robust, effective oral anticoagulant drug that measures up to warfarin in providing protection against thrombosis and compares favorably (if not better) with regard to safety profile, without the need for routine and often cumbersome monitoring. Many potential antithrombotic agents are in development, and the oral factor Xa inhibitors are one such class of drug. By inhibiting both free and clot-bound factor Xa complexes, the oral factor Xa inhibitors prolong activated partial thromboplastin time and prothrombin times in a d

Reviparin sodium (Clivarine), Knoll AG) is a low molecular weight heparin (LMWH) with a mean peak molecular weight of 3900 Da. It is characterised by a narrow molecular weight distribution profile, with an anti-factor Xa (anti-Xa):anti-factor IIa (anti-IIa) ratio of >or=3.6. In healthy human volunteers, plasma anti-Xa activity was up to five times higher and lasted three times longer with reviparin compared with unfractionated heparin (UFH). Unlike UFH, reviparin has negligible effects on global clotting tests. Reviparin has been shown to be as effective as UFH in different prophylactic indica
