Primary bone tumors are infrequently encountered in a sports medicine practice. We describe a case in which a young athletic woman with a medical history significant for ulcerative colitis initially presented to our clinic with chronic hip pain. Her initial roentograms were negative for boney pathology and her history and examination were consistent with trochanteric bursitis. A 13-year-old boy with the diagnosis of osteochondritis dissecans underwent arthroscopic examination of his knee joint. At surgery a cartilaginous mass in the medial femoral condyle was identified. Biopsy samples confirmed the diagnosis of chondroblastoma, which was removed completely through the arthroscope. A second arthroscopic examination of the knee was conducted at 6 months, and showed no residual disease. We describe the case history of a 13 years old boy who developed osteogenic sarcoma of the left thigh, six years after diagnosis of osteogenesis imperfecta tarda with a positive family history. Only four other patients with this disease combination are reported in the literature. Preoperative treatment with high-dose Methotrexate caused marked tumor regression, as shown at examination of amputation material. The inter-relationships between the two disorders are discussed and the literature is briefly reviewed.

BACKGROUND: Radiation therapy plays an essential part in modern treatment regimes of musculoskeletal tumors. Nevertheless damage to the surrounding tissue does occur inevitably. Postradiogenic changes of bone are associated with decreased stability and an increased fracture rate. Twenty-one patients were treated with Zickel nail for neoplastic pathological fracture or impending pathological fracture: in the femoral neck in three, in the intertrochanteric region in three, and in the subtrochanteric region in fifteen. Nineteen patients were restored to bed-chain status by the third postoperative day, and twenty were able to walk. Failure of fixation did not occur and there were no infections.

PURPOSE: In case of soft tissue sarcomas (STS), an en-bloc resection with safe margins is recommended. To ensure safe removal without tumor rupture, STS of the groin area, retroperitoneal or pelvic mesenchymal tumors may require incision or resection of the inguinal ligament. CASE: A 78-year-old man was followed for an incidentally found, asymptomatic lesion in his right proximal femur that was unchanged radiographically for 11 years. He developed pain and was believed to have experienced a stress fracture through the lesion. The lesion was biopsied, showing a high-grade pleomorphic sarcoma with an underlying senescent intraosseous lipoma. He was ultimately treated with wide excision and reconstruction of the proximal femur.
CONCLUSION: This case highlights the importance of obtaining a tissue diagnosis for lesions that become symptomatic.

Three femoral head chondroblastomas (one associated with aneurysmal bone cyst) were excised, using a technique of percutaneous extraarticular curettage. Whereas a standard approach to the femoral head lesion requires hip arthrotomy plus a cortical window in the femoral head or neck, the extraarticular technique enters the tumor cavity via a tunnel drilled from the lateral femoral cortex through the medullary canal of the femoral neck. Cartilaginous lesions of the skeleton are very unusual. It is extremely important to correlate the roentgenographic features, the clinical features, and the histological features to arrive at a definite diagnosis. Most cartilaginous lesions are benign or of low-grade malignancy. However, there are some subtypes of chondrosarcoma that behave in a highly aggressive fashion.

A case of a giant cell tumor of the right proximal femur that developed malignant change following surgical treatment ("secondary" malignant giant cell tumor) is presented. The patient had a cystic lesion that was initially interpreted as an aneurysmal bone cyst. A local recurrence six months later demonstrated the histological features of typical giant cell tumor. A further recurrence 22 months after the original treatment showed histological malignancy. Death was due to massive local recurrence and to extension of the malignancy to the pleura.
