[Combination of 5-fluorocytosine and amphotericin B in the treatment of chromomycosis].

The authors discuss the problem of resistance of chromomycosis to 5-fluorocytosine (5-FC), observed in a previous study in 7 patients initially treated with the drug and subsequently treated with 5-FC associated with other medicaments. All the associations, including that with amphotericin B, failed to overcome resistance. Experiments in mice and "in vitro" show a synergism between 5-FC and amphotericin B against "C. albicans", "C. neoformans" and "Fonsecaeae pedrosoi" already confirmed in human subjects infected with the above mentioned fungi. Resistance to 5-FC seems to be irreversible, as s

Dermatologic Clues to Emerging Tropical Infections: A Narrative Review for Early Recognition and Differential Diagnosis.

Tropical infections often present with characteristic dermatologic manifestations, which can serve as early indicators for diagnosis. However, these signs are frequently misidentified or underreported in emerging and outbreak settings. This narrative review critically evaluates the diagnostic utility of dermatologic findings across over 25 tropical infections, spanning viral (e.g., dengue, Zika, chikungunya, Mayaro virus, O'nyong-nyong virus), bacterial (e.g., Bacillus anthracis, Mycobacterium ulcerans, Rickettsia spp.), parasitic/helminthic (e.g., Loa loa, Ancylostoma braziliense, Tunga penet

Cutaneous manifestations of subcutaneous and systemic fungal infections in tropical regions: a retrospective study from a referral center in southern Taiwan.

BACKGROUND: Deep cutaneous fungal infections, including subcutaneous mycoses and systemic fungal infection with cutaneous involvement, cause significant morbidity and mortality in light of increasing immunocompromised patients and global warming. Although a few studies reviewed deep fungal infections in temperate regions, a relevant study in tropical regions is lacking. We evaluated features of deep cutaneous fungal infections in southern Taiwan among the tropical regions. METHODS: We retrospectively reviewed all histopathological specimens with deep cutaneous fungal infections in a single re

Case Report: Chronic Fungal Meningitis Masquerading as Tubercular Meningitis.

Phaeohyphomycosis causes a wide spectrum of systemic manifestations and can affect even the immunocompetent hosts. Involvement of the central nervous system is rare. A 48-year-old farmer presented with chronic headache, fever, and impaired vision and hearing. Serial MRIs of the brain showed enhancing exudates in the basal cisterns, and lesions in the sella and perichiasmatic and cerebellopontine angle regions along with enhancement of the cranial nerves and leptomeninges. Cerebrospinal fluid (CSF) showed lymphocytic pleocytosis with elevated protein and decreased glucose on multiple occasions.

A comparative study of extracellular enzymes from chromoblastomycosis agents reveals the potential association of phospholipase with the severity of the lesions.

Chromoblastomycosis is a chronic, progressive subcutaneous mycosis that is endemic in tropical and subtropical countries. Cladophialophora carrionii and Fonsecaea pedrosoi are prevalent etiological agents. The potential role of the proteolytic activity of extracellular enzymes in these fungi and its relationship with the pathogenesis of the disease has not been proven. Some phenotypic traits have been associated with the virulence of other fungi; i.e., their different rate of protease, phospholipase, and esterase excretion, melanin, and thermotolerance. The aim of this study was the identifica

Mycetoma in China: A Case Report and Review of the Literature.

Mycetoma is a chronic granulomatous infectious disease that can affect the skin, subcutaneous tissue, fascia and bone. It can be caused by filamentous bacteria or fungi and usually involves the legs and feet. Mycetoma is endemic in tropical and subtropical regions and is easily misdiagnosed in clinical practice because of its nonspecific clinical features and lack of awareness of the disease. Although mycetoma is very rare in mainland China, an increasing number of cases have been reported in recent years. Here, we report a case of mycetoma in a patient who was misdiagnosed many years before r

Lipolytic activity of chromoblastomycosis agents measured by infrared spectroscopy and chemometric methods.

The lipase activity of nine strains of six chromoblastomycosis agents (Fonsecaea pedrosoi, Phialophora verrucosa, Cladophialophora bantianum, Cladophialophora carrionii, Rhinocladiela aquaspersa and Exophiala jeanselmei) grown on solid medium was investigated using Fourier transform infrared spectroscopy and hierarchical clustering analysis. The data was quantified by p-nitrophenyl palmitate assay using partial least squares (PLS) regression. These methods allowed the correlation of six genera and species within the 1230-1650 and 2800+3000 cm(-1) spectral ranges among strains grown for 14 days

Treatment-Resistant Chromoblastomycosis Successfully Managed With Surgical Excision.

Chromoblastomycosis is an uncommon, chronic granulomatous fungal infection of the skin and subcutaneous tissue. Chromoblastomycosis is most commonly caused by the traumatic inoculation of dematiaceous (pigmented) fungi, most commonly Fonsecaea species, Phialophora species, and Cladophialophora species. Chromoblastomycosis usually affects agricultural workers in tropical and subtropical climates. The World Health Organization classifies chromoblastomycosis as a neglected tropical and occupational disease that commonly affects middle-aged men in poor to middle-income countries. The cutaneous les

Chromoblastomycosis in Australia: an historical perspective.

AIMS: To study the histological features of a large series of patients with chromoblastomycosis to ascertain whether the earlier presentation of patients with this disease has resulted in any changes in the histological features that were recorded over 60 years ago. METHODS: A key word search of the database of our pathology laboratory over the period 1 January 2004 to 30 June 2012 was carried out for cases reported as chromoblastomycosis. RESULTS: Seventy cases of chromoblastomycosis were reported over this period. A further four cases of subcutaneous chromomycosis were found in this search

Partial chemical characterization of antigenic preparations of chromoblastomycosis agents.

Antigenic preparations (saline, methylic, metabolic and exoantigens) of four agents of chromoblastomycosis, Fonsecaea pedrosoi, Phialophora verrucosa, Cladophialophora (Cladosporium) carrionii and Rhinocladiella aquaspersa were obtained. Partial chemical characterization of these antigenic preparations was obtained by determination of the levels of total lipids, protein, and carbohydrates, and identification of the main sterols and carbohydrates. Methylic antigens presented the highest lipid contents, whereas metabolic antigens showed the highest carbohydrate content. Total lipid, protein, and

[A case of chromomycosis arising in a Japanese-Brazilian patient].

A 41-year-old Japanese-Brazilian man, living in Japan since 1991, visited our hospital in August 1998 complaining of a scaly annular erythema which had been present on his right forearm since 1996. Granulomatous inflammation was revealed in the dermis upon skin biopsy. Sclerotic cells were present within the granulomatous lesions. Fonsecaea pedrosoi was isolated from tissue cultures. The restriction fragment length polymorphism pattern of mitochondrial DNA of the causative fungus was compatible with F. pedrosoi type 1, the commonest type in Japan. Since type 4 is usually found in South America

Deep fungal co-infection with Trichosporon asahii and Candida parapsilosis in an immunocompetent individual: a case report and review of literature.

Deep fungal infections are endemic to tropical and subtropical regions and can occur in individuals who are both immunocompetent and immunocompromised. Clinically, these infections may present as verrucous plaques, subcutaneous nodules, abscesses or ulcerations. Common causative organisms in immunocompetent hosts include Sporothrix schenckii, Madurella mycetomatis and Fonsecaea pedrosoi. However, emerging evidence suggests that Trichosporon asahii and Candida parapsilosis are increasingly implicated in these infections. A 64-year-old Cambodian man with a history of recently treated pulmonary t
