Refractory symptomatic schizophrenia resulting from frontal lobe lesion: response to clozapine.

A 34-year-old man with a 10-year history of persistent auditory hallucinations and passivity delusions had failed to respond to a variety of conventional antipsychotic medications. He had a history of head trauma 8 years before the onset of psychiatric symptoms. Recent investigations revealed a post-traumatic infarct, situated in the left frontal lobe, on a magnetic resonance imaging scan. Treatment with clozapine for more than 2 years resulted in a marked improvement in his psychotic symptoms. The localization of the brain lesion may be related to the etiology of his symptoms and to the clini

Upper motor neuron predominant degeneration with frontal and temporal lobe atrophy.

The autopsy findings of a 78-year-old man mimicking primary lateral sclerosis (PLS) are reported. He showed slowly progressive spasticity, pseudobulbar palsy and character change, and died 32 months after the onset of symptoms. Autopsy revealed severe atrophy of the frontal and temporal lobes, remarkable neuronal loss and gliosis in the precentral gyrus, left temporal lobe pole and amygdala, mild degeneration of the Ammon's horn, degeneration of the corticospinal tract, and very mild involvement of the lower motor neurons. The anterior horn cells only occasionally demonstrated Bunina body by c

Understanding symptoms of medical frontal lobe disorder: A clinical case study.

A case of anterior communicating artery aneurysm with damage to inferior medial frontal areas (Brodmann areas 25, 32, 24-inferior) is presented. Four prominent deficits are discussed: (1) anterograde amnesia, (2) inert perseverative card sorting, (3) motor stereotypies, and (4) reduplicative paramnesia. These four deficits are discussed as negative or positive symptoms, related either to damage in inferior medial frontal (Brodmann areas 25, 32, 24-inferior) regions or release phenomena of superior medial frontal (Brodmann areas 6-medial and 24-superior) regions. It is concluded that the inferi

Phosphorus magnetic resonance spectroscopic imaging in patients with frontal lobe epilepsy.

Phosphorus magnetic resonance spectroscopic imaging has previously demonstrated localized metabolic abnormalities within the epileptogenic region in patients with temporal lobe epilepsy, including alkalosis, increased inorganic phosphate level, and decreased phosphomonoester levels. We studied 8 patients with frontal lobe epilepsy, finding interictal alkalosis in the epileptogenic region compared to the contralateral frontal lobe in all patients (7.10 +/- 0.05 vs 7.00 +/- 0.06, p < 0.001). Seven patients exhibited decreased phosphomonoester levels in the epileptogenic frontal lobe compared to

Further characterization of the executive memory impairment following frontal lobe lesions.

In this article we describe a 41-year-old man who, following an operation to repair a ruptured anterior communicating artery aneurysm, manifested the "hallmark" features of a dysexecutive memory impairment. Of particular note was the patient's apparently normal level of recognition memory but impaired recall on tasks matched for difficulty in control subjects. However, further testing revealed that the patient's recognition memory was not normal under all circumstances. Implications of these data for the interpretation and further investigation of the dysexecutive deficit are discussed.

Frontal lobe degeneration of non-Alzheimer type. IV. White matter changes.

The cerebral white matter in 16 cases of frontal lobe non-Alzheimer degeneration with dementia (FLD), four cases of Pick's disease and five age-matched controls was studied microscopically. All cases of dementia had white matter alterations, consisting of gliosis and loss of myelin, with a regional spread of changes that roughly corresponded with that of the cortical degeneration. The white matter changes were less severe than the cortical alterations, although the relative degree of severity between grey and white matter pathology varied from case to case. The white matter changes were in som

Mass effect of a large frontal pneumosinus dilatans on the left frontal lobe: a case report and literature review.

Pneumosinus dilatans (PD) is a rare condition characterized by abnormal expansion of the paranasal sinus without bone destruction or significant mucosal disease. Its association with Dyke-Davidoff Masson syndrome (DDMS) is particularly uncommon. We report the case of a 54-year-old female with learning disabilities, pseudoseizures, and asthma who was referred to our clinic after an MRI performed for decreased visual acuity revealed marked dilatation of the large left frontal sinus. She denied nasal symptoms but reported frontal headaches. Endoscopic examination was unremarkable. Imaging demonst

Frontal Disconnection for Treating Mild Malformation of Cortical Development with Oligodendroglial Hyperplasia in Epilepsy (MOGHE) in the Frontal Lobe.

Malformation of cortical development is an important cause of drug-resistant epilepsy in young children. Mild malformation of cortical development with oligodendroglial hyperplasia in epilepsy (MOGHE) has been added to the last focal cortical dysplasia (FCD) classification and commonly involves the frontal lobe. The semiology at the onset of epilepsy is dominated by non-lateralizing infantile spasm; the boundaries of the malformation are usually difficult to determine by magnetic resonance imaging (MRI) and positron emission tomography (PET), and electroencephalography (EEG) findings are often

Relationship Between Brain Activity and Impaired Consciousness in Frontal Lobe Seizures.

BACKGROUND AND OBJECTIVES: Impaired consciousness in epilepsy negatively affects quality of life. Previous work has focused on temporal lobe seizures, where cortical slow waves are associated with depressed subcortical arousal and impaired consciousness. However, it is unknown whether frontal lobe seizures also show cortical slow waves or a different activity pattern with impaired consciousness. METHODS: Intracranial EEG (icEEG) recordings from patients at 3 centers were retrospectively assessed to identify seizures originating in the frontal lobe. Seizures were classified as focal preserved

Predicting Disability in Progressive Supranuclear Palsy Using Bedside Frontal-Lobe Signs.

BACKGROUND: Frontal lobe signs in progressive supranuclear palsy (PSP) are prevalent and occur early in the disease. Although they are recognized in clinical practice, studies are needed to systematically investigate them for an in-depth understanding of the neurological substrate and their potential prognostic implications in the disease. OBJECTIVES: To study the predictive role of frontal lobe signs in PSP, as well as to describe their neuropsychological and anatomical correlations. METHODS: Nine recognized signs of frontal lobe dysfunction were assessed in 61 patients with PSP. Those sign

Electroencephalography Study of Frontal Lobe Evoked by Dynamic Random-Dot Stereogram.

PURPOSE: The purpose of this study was to investigate the cortical electrical activity and electroencephalography (EEG) features of the frontal lobe evoked by dynamic random dot stereogram (DRDS) and to probe the functional connectivity (FC) between the frontal lobe and occipital lobe when processing 3D perception based on the binocular disparity. METHODS: The EEG experiment involved 14 healthy adults with normal stereopsis (<60″) and normal corrected visual acuity (20/20). The Neuroscan system and 32-channel EEG cap were used to record EEG signals based on the DRDS stimuli. The maximum energ

Cognitive dissonance resolution depends on executive functions and frontal lobe integrity.

The free choice paradigm constitutes one of the most explored paradigms of cognitive dissonance research. Typically, once asked to choose between two similarly rated items, subjects subsequently exhibit an increased preference for chosen items and a decreased preference for rejected ones. Recent studies have demonstrated that such choice-induced preference change (CIPC) occur exclusively for remembered choices, suggesting a mechanism that ensures subjective coherence across time. In the present work we predicted that in order for CIPC to occur, not only must past choices be remembered, but exe

Rhythmic rocking stereotypies in frontal lobe seizures: A quantified video study.

OBJECTIVES: Rhythmic, stereotyped movements occur in some epileptic seizures. We aimed to document time-evolving frequencies of antero-posterior rocking occurring during prefrontal seizures, using a quantitative video analysis. METHODS: Six seizures from 3 patients with prefrontal epilepsy yet different sublobar localizations were analyzed using a deep learning-based head-tracking method. RESULTS: Mean rocking frequency varied between patients and seizures (0.37-1.0Hz). Coefficient of variation of frequency was low (≤12%). DISCUSSION: Regularity of body rocking movements suggests a mechanis

Heart rate variability in frontal lobe epilepsy: Association with SUDEP risk.

OBJECTIVES: Frontal lobe epilepsy (FLE) may impair autonomic heart rate modulation. Decreased heart rate variability (HRV) may enhance risk of sudden death. Our objective was to describe whole day and wakefulness/sleep HRV parameters from FLE patients in comparison with those of healthy controls and correlate HRV parameters to SUDEP-7 scores. METHODS: Ten patients with FLE and 15 healthy controls underwent a 24-hour electrocardiogram holter. The SUDEP-7 score was calculated for patients. Subgroups were identified according to active epilepsy, number of generalized seizures, cognitive deficit,

Parkinsonian Syndrome with Frontal Lobe Involvement and Anti-Glycine Receptor Antibodies.

Background: Atypical Parkinsonian syndromes with prominent frontal lobe involvement can occur in the 4R-taupathies progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD). Secondary forms of movement disorders may occur in the context of autoimmune encephalitis with antineuronal antibodies, such as anti-glycine receptor (anti-GlyR) antibodies, which are typically associated with Stiff-Person spectrum syndrome, or progressive encephalomyelitis with rigidity and myoclonus. Overlaps between neurodegenerative and immunological mechanisms have been recently suggested in anti-IgLON5

Altered intrinsic brain activity associated with outcome in frontal lobe epilepsy.

Frontal lobe epilepsy (FLE) is the second most common type of the focal epilepsies. Our understanding of this disease has been revolutionized over the past decade, but variable treatment outcomes persist and the underlying functional mechanisms responsible for this have yet to be deciphered. This study was designed to determine how intrinsic brain connectivity related to treatment response in patients with FLE. 50 patients with FLE and 28 healthy controls were enrolled in this study and underwent functional MRI at baseline. At the end of 12-month follow up period, all patients with FLE were cl
