Breakthrough invasive fungal disease in patients receiving posaconazole primary prophylaxis: a 4-year study.

Posaconazole (PSC) is currently recommended as primary prophylaxis in neutropenic patients with acute myeloid leukaemia (AML) and in allogenic haematopoietic stem cell transplantation (AHSCT) recipients with graft-versus-host disease (GVHD). Studies focusing on breakthrough invasive fungal disease (IFD) upon PSC prophylaxis show disparate results. In order to evaluate the incidence of IFD in patients on PSC prophylaxis and identify IFD risk factors, we carried out a retrospective study of all consecutive patients on PP from January 2007 to December 2010 in our hospital. Breakthrough IFDs were

Efficacy and safety of amphotericin B lipid complex injection (ABLC) in solid-organ transplant recipients with invasive fungal infections.

BACKGROUND: Fungal infections following solid-organ transplantation are a major source of morbidity and mortality. This report describes the efficacy and safety of Amphotericin B Lipid Complex Injection (ABLC) in solid-organ transplant recipients. METHODS: Three open-label, second-line treatment studies evaluated ABLC as a treatment for severe, life-threatening mycoses in patients who were refractory to or intolerant to conventional antifungal (mostly amphotericin B [AmB]) therapy or had pre-existing renal disease. RESULTS: The 79 solid-organ transplant recipients (25 heart, 20 liver, 17 kid

Z-Nucleic Acids: A Regulator of Macrophage PANoptosis and Fungal Biofilm in Fungal Keratitis.

PURPOSE: This study aimed to investigate the role of Z‑nucleic acids (Z‑NAs) in the pathogenesis of fungal keratitis (FK), focusing on its regulatory effects on fungal immune escape, inflammation, and biofilm activity. METHODS: A mouse model of FK was established by the combination of stromal scrapes and corneal contact lens with Fusarium solani. Disease severity was assessed using clinical scoring and histopathological examination. In vitro, bone marrow-derived macrophages (BMDMs) were infected with F. solani, and the efficiency of fungal phagocytosis and escape by BMDMs was determined throu

Antifungal susceptibility, clinical findings, and biofilm resistance of Fusarium species causing keratitis: a challenge for disease control.

Fusarium keratitis (FK) is an important clinical condition that can lead to blindness and eye loss, and is most commonly caused by the Fusarium solani species complex (FSSC). This study evaluated the susceptibility of planktonic cells and biofilms of FSSC (n = 7) and non-FSSC (n = 7) isolates obtained from patients with keratitis from a semi-arid tropical region to amphotericin B (AMB), natamycin (NAT), voriconazole (VRZ), efinaconazole (EFZ), and luliconazole (LCZ). Analysis of clinical data showed that trauma was the most common risk factor for FK patients. Disease onset was longer in non-FS

Detection of Fusarium solani using cutinase antibody and its application in diagnosing fungal keratitis in an animal model.

Fungal keratitis (FK) is a sight-threatening infectious disease that can result in blindness if not appropriately treated. Although keratitis associated with filamentous fungi was rarely reported in the past, the incidence of fungal keratitis due to contact lens usage has increased. The clinical manifestations of fungal keratitis often resemble those of other corneal infections, potentially delaying accurate diagnosis and treatment. In this study, we developed a Fusarium-specific polyclonal peptide antibody targeting the cutinase of F. solani and evaluated its diagnostic potential for FK in an

Urinary Tract Infection (UTI) Caused by Fusarium proliferatum in an Agranulocytosis Patient and a Review of Published Reports.

Infections caused by Fusarium species are increasing in frequency among immunocompromised hosts, but urinary tract infection (UTI) due to Fusarium proliferatum has not been reported in the literature so far. We describe a case of UTI caused by F. proliferatum in a 47-year-old man who was diagnosed with rectal cancer and metastasis. He underwent radical resection of rectal carcinoma and local resection of hepatic metastases. After the first adjuvant chemotherapy, the patient presented the obvious high fever, severely diarrhea and progressive decline of the white blood cell count. The direct mic

Fungal aerosols in rabbit breeding environment: Metagenetic insight into PM2.5 based on third-generation sequencing technology.

Fungal aerosols are a vital environmental hazard factor impeding the development of the rabbit breeding industry and threatening public health. This study aimed to determine fungal abundance, diversity, composition, diffusion, and variability in aerosols in rabbit breeding environments. Twenty PM2.5 filter samples were collected from five sampling sites (i.e. En5, In, Ex5, Ex15, and Ex45) in a modern rabbit farm in Linyi City, China. Fungal component diversity at the species level was analyzed in all samples using third-generation sequencing technology. Results revealed that fungal diversity a

Addition of aerosolized deoxycholate amphotericin B to systemic prophylaxis to prevent airways invasive fungal infections in allogeneic hematopoietic SCT: a single-center retrospective study.

Invasive fungal infections (IFIs) still pose major challenges in allogeneic hematopoietic SCT (HSCT), and effective antifungal prophylaxis remains a matter of debate. The aim of this retrospective study was to evaluate the toxicity and the impact of aerosolized deoxycholate amphotericin B (aero-d-AmB) on respiratory tract IFIs (airways IFIs) in a homogeneous cohort of allogeneic HSCT patients, transplanted at one institution. Since 1999, 102 consecutive patients were transplanted from matched related (N = 71) or unrelated donor (MUD). Aero-d-AmB was administered for a median time of 16 days (r

Amphotericin B Lipid Complex (Abelcet) in the treatment of invasive mycoses: the North American experience.

Abelcet, or Amphotericin B lipid Complex, is unique formulation, comprising an equimolar mixture of amphotericin B complexed with two lipids. In preclinical studies, Abelcet was clearly demonstrated to be less toxic than amphotericin B desoxycholate and to be effective in models where amphotericin B was ineffective at its maximum tolerated dose. Pharmacokinetic studies in animals also showed that the concentration of Abelcet in blood is similar or reduced compared to levels seen with conventional amphotericin B, with accumulation in the liver, lungs and spleen. Phase I clinical trials determin

The changing epidemiology of fungal infections: are the lipid-forms of amphotericin B an advance?

The incidence of invasive fungal infections is increasing and new fungal species are emerging as important pathogens. In cancer patients, the main risk factor for the development of systemic fungal disease is severe, prolonged neutropenia. Other factors, such as mucosal damage, presence of a central venous line, immunosuppressive therapy and treatment with broad spectrum antibiotics are contributory. Candida spp. are the fungi most commonly isolated in neutropenic patients. There has been a dramatic increase in non-C. albicans species, such as C. glabrata and C. krusei, largely as a result of

Atypical Clinical Presentation of a Newer Generation Anti-Fungal Drug-Resistant Fusarium Infection After a Modified Multi-Visceral Transplant.

BACKGROUND Fusarium spp. infections have become an emerging and lethal threat to the immunocompromised patient population, especially those with neutropenia. Recently there have been increased reports in solid organ transplant recipients. Presentation is commonly as soft tissue infections several months post-transplant. With high morbidity and mortality, efficacious antifungal therapy is essential. This remains challenging with limited data and no established clinical breakpoints defined. CASE REPORT We report on a modified multi-visceral transplant patient that developed a Fusarium infection

Epidemiology of filamentous fungal infections in burned patients: A French retrospective study.

INTRODUCTION: Filamentous fungal infections (FFI) seem to become more frequent in burn patients, in whom they are usually accepted to cause severe. However published data regarding their incidence and consequences in that context remain scarce. The aim of this study was to evaluate the incidence of mould infections in our burn centre, and to review characteristics and outcomes of patients with such infections. METHODS: This retrospective single-centre study reviews all patients admitted in our centre with acute burns (2000-2011) and positive culture for moulds. Wound infections were defined a

Shielding the Next Generation: Symbiotic Bacteria from a Reproductive Organ Protect Bobtail Squid Eggs from Fungal Fouling.

The importance of defensive symbioses, whereby microbes protect hosts through the production of specific compounds, is becoming increasingly evident. Although defining the partners in these associations has become easier, assigning function to these relationships often presents a significant challenge. Here, we describe a functional role for a bacterial consortium in a female reproductive organ in the Hawaiian bobtail squid, Euprymna scolopes Bacteria from the accessory nidamental gland (ANG) are deposited into the egg jelly coat (JC), where they are hypothesized to play a defensive role durin
