Gait analysis in myelomeningocele: possibilities and applications.

Gait analysis with a fully integrated laboratory is a relatively new instrument in the armamentarium of the pediatric orthopedic surgeon. The introduction of it has been especially successful in neuromuscular pathology and, particularly, in cerebral palsy. In spina bifida, however, it also enhances substantially the possibilities of detailed analysis of the locomotion problem. It is, furthermore, a very useful instrument in evaluating treatment and in follow-up. With a few examples, this paper tries to show the possibilities and advantages of a gait laboratory in the evaluation of patients wit

The effect of limb-length discrepancy on gait.

We evaluated the gait of thirty-five neurologically normal children who had a limb-length discrepancy of the lower extremities that ranged from 0.8 to 15.8 per cent of the length of the long extremity (0.6 to 11.1 centimeters). The twenty-two boys and thirteen girls had an average age of thirteen years (range, eight to seventeen years). No patient had a substantial angular or rotational deformity of the lower extremities. We found no correlation between the actual discrepancy or the per cent discrepancy and any of the dependent kinematic or kinetic variables, including pelvic obliquity. Discre

Work and power in gait of stroke patients.

Biomechanical analysis of the work and power patterns involved in gait provides insight into the nature of gait deficits and suggests methods for improvement. The purpose of this study was to describe the work and power characteristics during gait for both limbs of hemiplegic patients, and to determine the work and power variables related to self-selected speeds of walking. The gait of 30 ambulatory adults between the ages of 47 and 79 years was studied using two-dimensional cinematography and force-plate data in a link-segment model. About 40% of the positive work required for walking was per

Gait problems in children. A matter of rotation.

The gait disturbances commonly seen in children are usually physiologic and correct spontaneously. Conditions that fall in this category include internal femoral torsion, internal tibial torsion, metatarsus adductus, bowlegs, and knock-knees. However, metatarsus adductus may require casting, and bowlegs, if persistent, may be Blount's disease. Rickets may cause either bowlegs or knock-knees. Clubfeet are resistant to correction and warrant early orthopedic consultation.

Hydrocephalus as a cause of disturbances of gait in the elderly.

To investigate the possible role of symptomatic normal-pressure hydrocephalus (NPH) in unexplained walking difficulties of the elderly, we used CT to measure lateral ventricular size. We compared data in 50 patients over the age of 60 with a disturbance of gait and 80 control subjects of similar age who walked well and were mentally sound. Forty-six of the 50 gait patients (92%) had a ventricular span of more than 12 mm, whereas only 5 of the 80 controls (6.3%) had ventricles of that size. There was a narrow zone of overlap. Seventeen of the gait patients later improved after surgical shunting

Long-term robot-assisted gait training in patients with cerebral palsy: meaningful changes in gait parameters require time.

INTRODUCTION: Robot-assisted gait training (RAGT) is increasingly used in children with cerebral palsy (CP), but evidence regarding the timing and persistence of meaningful changes in gait remains limited. This study aimed to describe long-term changes in spatiotemporal gait parameters, gait kinematics, and walking endurance in ambulatory patients with CP undergoing systematic, repetitive RAGT, and to determine whether the timing of clinically meaningful change differs between ambulatory levels. METHODS: A study group consisting of 211 patients with hypertonic CP (93 females, 118 males; mean

Interpersonal gait synchronization reduces gait asymmetry and induces a carry-over effect during overground side-by-side walking.

INTRODUCTION: Interpersonal gait synchronization is widely observed during side-by-side walking and has been hypothesized to contribute to gait rehabilitation. Nonetheless, the specific effects of gait synchronization on gait asymmetry, as distinguished from mere side-by-side walking, have not been experimentally investigated under overground walking conditions. This study aimed to examine the effects of interpersonal gait synchronization on gait asymmetry during overground side-by-side walking. METHODS: Ten pairs of participants performed overground side-by-side walking tasks under synchroni

Variable gait training to improve gait stability in ankle-foot orthosis users with neuromuscular disorders: A pilot study.

BACKGROUND: Ankle-foot orthoses aim to improve walking in people with neuromuscular disorders with lower leg muscle weakness. However, clinical experience indicates that some ankle-foot orthosis users report limited gait stability improvements, suggesting suboptimal adaptation. Variable gait training may improve adaptation, and thereby gait stability. We aimed to investigate whether variable gait training improves gait stability in individuals with neuromuscular disorders who initially experienced stability problems when using ankle-foot orthoses. METHODS: Three stiffness-optimized dorsal lea

Gait-to-Contact (G2C): A Novel Deep Learning Framework to Predict Total Knee Replacement Wear from Gait Patterns.

PURPOSE: Total knee replacement (TKR) is the most common inpatient surgery in the US. Studies leveraging finite element analysis (FEA) models have shown that variability of gait patterns can lead to significant variability of wear rates in TKR settings. However, FEA models can be resource-intensive and time-consuming to execute, hindering further research in this area. This study introduces a novel deep learning-based surrogate modeling approach aimed at significantly reducing computational costs and processing time compared to traditional FEA models. METHODS: A published method was used to g

Analyzing Gait Dynamics and Recovery Trajectory in Lower Extremity Fractures Using Linear Mixed Models and Gait Analysis Variables.

In a prospective study, we examined the recovery trajectory of patients with lower extremity fractures to better understand the healing process in the absence of complications. Using a chest-mounted inertial measurement unit (IMU) device for gait analysis and collecting patient-reported outcome measures, we focused on 12 key gait variables, including Mean Leg Lift Acceleration, Stance Time, and Body Orientation. We employed a linear mixed model (LMM) to analyze these variables over time, incorporating both fixed and random effects to account for individual differences and the time since injury

Robot-Assisted Gait Training to Improve Gait Patterns in Two Adolescents with Childhood-Onset Stroke: A Case Report.

AIM: Although the effectiveness of robot-assisted gait training (RAGT) in stroke has been reported, evidence in adolescents with childhood-onset stroke remains limited. This study reports the safety and clinical efficacy of RAGT using a robotized knee-ankle-foot orthosis in two adolescents with different gait patterns. METHODS: Case 1 (female, 15 years; Fugl-Meyer Assessment of Lower Extremity (FMA L/E) score 24) had moderate motor paralysis and walked with a short leg brace and cane, showing an extension-thrust pattern. Case 2 (male, 18 years; FMA L/E 26) used a short leg brace and demonstra

In-Clinic and Natural Gait Observations master protocol (I-CAN-GO) to validate gait using a lumbar accelerometer.

Traditional measurements of gait are typically performed in clinical or laboratory settings where functional assessments are used to collect episodic data, which may not reflect naturalistic gait and activity patterns. The emergence of digital health technologies has enabled reliable and continuous representation of gait and activity in free-living environments. To provide further evidence for naturalistic gait characterization, we designed a master protocol to validate and evaluate the performance of a method for measuring gait derived from a single lumbar-worn accelerometer with respect to r

Self-assessed Dynamic Gait Index correlates with physical therapist assessed Dynamic Gait Index in people with multiple sclerosis.

BACKGROUND: The Dynamic Gait Index (DGI) is a clinical measure of walking ability comprised of eight walking tasks. In people with multiple sclerosis (PwMS) the DGI has demonstrated validity, reliability, and ability to identify fallers. A self-assessed version of the DGI that demonstrates concurrent validity with the original DGI in people with MS would be valuable for remote assessment of walking ability. We therefore developed a questionnaire-based self-assessed version of the DGI (sDGI) that asks participants to self-rate their predicted ability to perform the eight DGI walking tasks. The

Gait-combined closed-loop brain stimulation can improve walking dynamics in Parkinsonian gait disturbances: a randomised-control trial.

OBJECTIVE: Gait disturbance lowers activities of daily living in patients with Parkinson's disease (PD) and related disorders. However, the effectiveness of pharmacological, surgical and rehabilitative treatments is limited. We recently developed a novel neuromodulation approach using gait-combined closed-loop transcranial electrical stimulation (tES) for healthy volunteers and patients who are post-stroke, and achieved significant entrainment of gait rhythm and an increase in gait speed. Here, we tested the efficacy of this intervention in patients with Parkinsonian gait disturbances. METHOD

Prediction of Parkinsonian Gait in Older Adults with Dementia using Joint Trajectories and Gait Features from 2D Video.

Older adults with dementia have a high risk of developing drug-induced parkinsonism; however, formal clinical gait assessments are too infrequent to capture fluctuations in their gait. Camera-based human pose estimation and tracking provides a means to frequently monitor gait in nonclinical settings. In this study, 2160 walking bouts from 49 participants were recorded using a ceiling-mounted camera. Recorded color videos were processed using AlphaPose to obtain 2D joint trajectories of the participant as they were walking down a hallway of the unit. A subset of 324 walking bouts from 14 partic

Evaluation of Patients' Posture and Gait Profile After Lumbar Fusion Surgery by Video Rasterstereography and Treadmill Gait Analysis.

This protocol provides guidance on how to perform high resolution video rasterstereography and treadmill gait analysis on patients after lumbar fusion surgery to obtain results about altered variables of gait and posture. These observed changes can then be correlated with the patient-reported outcome measure of pain relief. The rasterstereographic device projects lines of parallel light onto the surface of the tested subject's back. The deformation of these lines is recognized by the device. From these data, a special software then generates a 3-D profile based on the principle of triangulatio

The Immediate Impact of Hemodialysis on Gait Metrics.

BACKGROUND: Gait disturbances are common in patients undergoing hemodialysis and are associated with increased fall risk, mobility decline, and adverse health outcomes. Prior research suggests that hemodialysis may impact gait parameters such as speed, stride length, and variability; however, findings are inconsistent. OBJECTIVES: To evaluate acute changes in gait metrics before and after hemodialysis using an artificial intelligence (AI) based video gait analysis system. METHODS: We initially enrolled 38 hemodialysis patients, two were excluded due to clothing interference with video analys
