Same-stage total knee arthroplasty and osteotomy for osteoarthritis with extra-articular deformity. Part I: Tibial osteotomy, prospective study of 26 cases.

BACKGROUND: In patients with advanced knee osteoarthritis and extra-articular knee deformity (EKD), ligament balance may be difficult to achieve during total knee arthroplasty (TKA). Treatment options include two-stage surgery with the first stage involving correction of the EKD and same-stage TKA and tibial osteotomy (1S-TKA-TO). The objective of this study was to assess outcomes in 26 patients managed with 1S-TKA-TO. HYPOTHESIS: 1S-TKA-TO produces satisfactory clinical and anatomical outcomes and is not associated with higher morbidity rates compared to TKA alone or two-stage TKA-TO. MATER

Association between changes in knee load and effusion-synovitis: evidence of mechano-inflammation in knee osteoarthritis using high tibial osteotomy as a model.

OBJECTIVE: Although mechanically-induced inflammation is an appealing explanation linking different etiologic factors in osteoarthritis (OA), clinical research investigating changes in both biomechanics and joint inflammation is limited. The purpose of this study was to evaluate the association between change in surrogate measures of knee load and knee effusion-synovitis in patients with medial compartment knee OA undergoing high tibial osteotomy (HTO). METHODS: Thirty-six patients with medial compartment knee OA and varus alignment underwent 3D gait analysis and 3T magnetic resonance imaging

Improving the accuracy of tibial component placement during total knee replacement in varus knees with tibial bowing: A prospective randomised controlled study.

BACKGROUND: Lateral tibial bowing leads to varus placement of the tibial component during total knee replacement in varus knees. Lateralised tibial jig placement can improve the accuracy of the tibial cut. METHODS: A total of 227 patients (300 knees) undergoing total knee replacements were randomised into two groups. In the study group, the point of intersection of the distal tibial diaphyseal line at the tibial plateau drawn on long films was represented by zones. Knees with femoral bowing >5° (28%) were excluded. Tibial jig placement on the proximal tibia was lateralised according to the zo

Rebound of Frontal Plane Malalignment After Tension Band Plating.

BACKGROUND: Despite the popularity of tension band plating (TBP) current literature lacks clinical data concerning recurrence ("rebound") of frontal plane malalignment. This study investigated the rebound phenomenon after TBP in idiopathic genu varum/valgum deformities. We analyzed factors that may contribute to the development of rebound after removal of TBP. METHODS: Patients who had correction of idiopathic valgus or varus deformities by TBP at the distal femoral and/or proximal tibial growth plate were selected from a prospective consecutive database. Only patients who had plates removed

Can osteoarthritic patients with mild varus deformity be indicated for high tibial osteotomy?

BACKGROUND: Whether osteoarthritic patients with mild varus deformity can be indicated for high tibial osteotomy (HTO) is not established. We examined the preoperative characteristics and postoperative outcomes of HTO in patients with mild genu varum compared to patients with greater varus deformity. METHODS: Seventy-one patients who underwent HTO were included in this retrospective study. Patients were divided into either mild varus (MV, mechanical femorotibial angle (mFTA) ≤4°, n = 31 (44%)) and greater varus (GV, mFTA >4°, n = 40 (56%)) groups. Preoperative characteristics on single photon

Optimizing ergonomic risk assessment using fuzzy irregular cellular automata: a novel approach to modeling musculoskeletal disorders in industrial workstations.

Musculoskeletal disorders (MSDs) and structural musculoskeletal abnormalities cause substantial work-related pain, disability, and productivity loss in industrial workforces; traditional screening tools and single-variable analyses can miss complex, interdependent risk patterns that arise from combined biomechanical exposures, workstation mismatches, and worker characteristics. We aimed to develop and validate a data-driven framework - Fuzzy Irregular Cellular Automata (FICA) - to integrate multi-modal ergonomic measurements, detect high-risk abnormality clusters, quantify their associations w

Novel subtype of coxitis knee associated with acetabular dysplasia of the hip: a case series.

BACKGROUND: Multiple joint arthritis patterns require a comprehensive understanding to optimize patient management. This study aimed to present a patient cohort that deviated from known definitions of coxitis knee (CK), identifying and characterizing this atypical group. METHODS: Patients undergoing both total hip arthroplasty and total knee arthroplasty between January 2008 and December 2018 were retrospectively reviewed. The patients were classified into a typical coxitis knee group (classic, long leg arthropathy, and windswept deformity) and an atypical coxitis knee group. Leg-length discr

Late-onset tibia vara: a comparative analysis.

We reviewed the clinical and radiographic characteristics and response to treatment of 11 children (16 knees) with idiopathic late-onset tibia vara (adolescent Blount's disease) followed for an average of 5.7 years (range, 3-10 years). The clinical characteristics were very consistent: black race; 2:1 male predominance; normal height; marked obesity; knee pain as the primary presenting complaint; and slowly progressive genu varum deformity that averaged 19 degrees (range, 10-45 degrees). Radiographically, the epiphyses were wedge shaped owing to medial flattening, the physes were irregular in

Rotational positioning of the tibial tray in total knee arthroplasty: a CT evaluation.

INTRODUCTION: Various surgical techniques have been described to set the rotational alignment of the tibial baseplate during total knee arthroplasty. The self-positioning method ("self-adjustment") aligns the tibial implant according to the rotational alignment of the femoral component which is used as a reference after performing repeated knee flexion/extension cycles. Postoperative computed tomography scanning produces accurate measurements of the tibial baseplate rotational alignment with respect to the femoral component. HYPOTHESIS: The rotational positioning of the tibial baseplate match

Influence of the post-operative axis on the clinical results of total knee replacement for severe varus deformities: does a slight residual varus improve the results?

PURPOSE: Some recent articles have suggested that in the case of large varus deformity, it may be advantageous to leave some residual post-operative varus as a means of improving functional outcome. The objective of this study is to compare the results of total knee replacement (TKR) performed for significant varus deformity (HKA < 170°) where there is a residual post-operative varus (HKA < 180°) to the results of TKR for significant varus deformity with either neutral post-operative (HKA = 180°) or mild valgus post-operative alignment (HKA > 180°). METHODS: This series was made up of 208 kne

Total knee arthroplasty with use of computer-assisted navigation compared with conventional guiding systems in the same patient: radiographic results in Asian patients.

BACKGROUND: The value of computer-assisted surgery in total knee arthroplasty for arthritic knees continues to be debated. We hypothesized that the usefulness of computer assistance is related to the magnitude of the deviation from the preoperative mechanical axis and that computer-assisted surgery may be beneficial under certain circumstances. METHODS: Patients with bilateral knee osteoarthritis and genu varus deformity who were to have staged bilateral total knee arthroplasty were enrolled. The patients randomly underwent computer-assisted total knee arthroplasty in one knee and conventiona

Sagittal biplanar 'L'-shaped tibial osteotomy associated with osteochondral allograft for symptomatic varus knee and low patella.

INTRODUCTION: The "L"-shaped tibial osteotomy is a relatively recent surgical technique aimed at correcting the mechanical axis without altering patellar height or posterior tibial slope (common complications associated with conventional medial opening wedge osteotomies). CASE PRESENTATION: We report the case of a young patient with symptomatic genu varum, osteochondral lesion in the medial femoral condyle, and low patellar height. A sagittal biplanar "L"-shaped proximal tibial osteotomy was performed, fixed with a Puddu plate and two anteroposterior screws. The osteotomy gap was filled with

Approach to the Patient with Achondroplasia-New Considerations for Diagnosis, Management, and Treatment.

Achondroplasia is the most common disproportionate short-stature skeletal dysplasia. Features associated with achondroplasia are rhizomelia, macrocephaly, midface hypoplasia, and typical cognition. Potential medical complications include foramen magnum stenosis, hydrocephalus, middle ear dysfunction, obstructive and central sleep apnea, spinal stenosis, and genu varum. Recently, vosoritide, a C-type natriuretic peptide analogue, was approved by the Food and Drug Administration with the primary indication of increasing linear growth in all children with achondroplasia and open growth plates. Improved distribution of leg forces after fibular resection and synostosis.

PURPOSE: Genu varum- a common symptom in achondroplasia- may lead to ankle valgus in children. Ankle valgus may be mitigated by tibiofibular synostosis, but this is not always the case. The mechanical environment around the growth plates plays an important role in ankle valgus. The purpose of this project is 1) to determine the strain through the distal fibular growth plate before and after tibiofibular synostosis, and 2) postulate whether the change in strain affects ankle valgus. This project measured the distal fibular strain in a porcine hind leg model. METHODS: The lower legs of seven pi
