Preoperative Frailty Predicts Postoperative Neurocognitive Disorders After Total Hip Joint Replacement Surgery.

BACKGROUND: Frailty is a reduced capacity to recover from a physiologically stressful event. It is well established that preoperative frailty is associated with poor postoperative outcomes, but it is unclear if this includes cognitive decline following anesthesia and surgery. This retrospective observational study was a secondary analysis of data from a previous study (the Anaesthesia, Cognition, Evaluation [ACE] study). We aimed to identify if preoperative frailty or prefrailty is associated with preoperative and postoperative neurocognitive disorders or postoperative cognitive dysfunction.

Development of a machine learning-based prediction model for postoperative delirium in frail elderly patients undergoing noncardiac surgery under general anesthesia.

BACKGROUND: In frail older adults, the incidence of postoperative delirium (POD) is markedly increased, leading to greater morbidity, prolonged length of stay, and higher healthcare costs. An accurate POD prediction model can direct preventive strategies and improve patient outcomes. Employing advanced machine-learning techniques, this study develops a POD prediction model using comprehensive preoperative and intraoperative data. METHODS: We enrolled 2,089 frail patients aged ≥ 65 years undergoing general anesthesia for noncardiac surgery at Fuyang People's Hospital between February 2023 and

Safety of early surgery for geriatric hip fracture patients taking clopidogrel: a retrospective case-control study of 120 patients in China.

BACKGROUND: Geriatric hip fracture patients receiving clopidogrel are a surgical challenge. In China, most of these patients undergo delayed surgical treatment after clopidogrel withdrawal for at least 5 to 7 days. However, delayed surgery is associated with increased complications and mortality in the older adults. This retrospective paralleled comparison study investigated the safety of early surgery for geriatric hip fracture patients within 5 days of clopidogrel withdrawal. METHODS: Acute hip fracture patients (≥65 years) who were hospitalized in the orthogeriatric co-management ward of B

Effect of desflurane versus propofol on perioperative neurocognitive disorders in older adults undergoing major urological surgery: a randomized trial.

BACKGROUND: Perioperative neurocognitive disorders (PND), encompassing postoperative delirium (POD), delayed neurocognitive recovery (dNCR), and postoperative neurocognitive disorder (postoperative NCD), are commom complications of surgery in older adults. Urological surgery carries particularly high risk, with reported incidences reaching 30%. Elevated serum biomarkers in affected patients suggest diagnostic and prognostic utility. Although propofol may reduce postoperative neurocognitive disorder risk compared with other inhalational agents, data on desflurane remain limited despite its rapi

Association of postoperative delirium with serum and cerebrospinal fluid proteomic profiles: a prospective cohort study in older hip fracture patients.

Postoperative delirium (POD) is a common neuropsychiatric complication in geriatric inpatients after hip fracture surgery and its occurrence is associated with poor outcomes. The purpose of this study was to investigate the relationship between preoperative biomarkers in serum and cerebrospinal fluid (CSF) and the development of POD in older hip fracture patients, exploring the possibility of integrating objective methods into future predictive models of delirium. Sixty hip fracture patients were recruited. Blood and CSF samples were collected at the time of spinal anesthesia when none of the

Subclinical Hepatic Injury and Dysfunction Following Surgery: Evidence From a Young and Aged Laparotomy Murine Model.

Elderly patients represent a growing proportion of surgical cases and have elevated postoperative risks, partly due to age-related hepatic vulnerability. Conventional liver enzymes (ALT/AST) lack sensitivity for detecting early dysfunction, limiting perioperative assessment. We investigated whether sensitive biomarkers could reveal subclinical hepatic injury and metabolic dysfunction after surgical stress in aged mice. Young (3 months) and aged (24 months) C57BL/6J mice underwent standardized laparotomy with mesenteric vascular manipulation or sham anesthesia. At 48 h, liver histology, ALT, AS

Effects of Bacillus coagulans supplementation on the growth performance and gut health of broiler chickens with Clostridium perfringens-induced necrotic enteritis.

Postoperative delirium (POD) is a common postoperative complication, characterized by acute, transient and fluctuating declines in consciousness and attention, with an incidence that increases with age. POD is associated with various adverse postoperative outcomes, including prolonged hospital stays, higher medical costs and increased morbidity and mortality rates. Moreover, it has been suggested that POD, as an early manifestation of postoperative cognitive impairment, may serve as a precursor to long‑term cognitive dysfunction. Given its considerable clinical impact, the prevention and manag

Perioperative Myocardial Injury/Infarction After Non-cardiac Surgery in Elderly Patients.

At present, we have entered an aging society. Many diseases suffered by the elderly, such as malignant tumors, cardiovascular diseases, fractures, surgical emergencies and so on, need surgical intervention. With the improvement of Geriatrics, surgical minimally invasive technology and anesthesia level, more and more elderly patients can safely undergo surgery. Elderly surgical patients are often complicated with a variety of chronic diseases, and the risk of postoperative myocardial injury/infarction (PMI) is high. PMI is considered to be the increase of cardiac troponin caused by perioperativ
