Redefining the Economics of Geriatric Orthopedics.

INTRODUCTION: The heath care system in the United States is in the midst of a transition, in large part to help accommodate an older and more medically complex population. Central to the current evolution is the reassessment of value based on the cost utility of a particular procedure compared to alternatives. The existing contribution of geriatric orthopedics to the societal burden of disease is substantial, and literature focusing on the economic value of treating elderly populations with musculoskeletal injuries is growing. MATERIALS AND METHODS: A literature review of peer-reviewed public

Ethical issues in geriatric dermatology.

Physicians should be cognizant of the multitude of unique issues that their geriatric patients present and aware of the overall elder patient's general health status both physically and cognitively, potential drug interactions, and their short-term as well as long-term goals. While respecting patients' autonomy, we must evaluate their ability to make their own decisions regarding their health care. This will require time and patience on our part. We also must overcome our own potential prejudices about what we view as important for the older patient. Finally, we may need to appropriate involve

["Atypical" anemia in a geriatric patient].

A gastroenterologic examination was performed on an 83-year-old woman due to unexplained anemia; however, no bleeding site could be identified. Because of the advanced age of the patient, no further diagnostic efforts were made and iron-deficiency anemia was suspected. After 3 weeks, an emergency laparotomy was performed for acute diffuse peritonitis, which was diagnosed as perforation of the jejunum by a wooden foreign body. After extraction of the foreign body and closure of the perforation in the small intestine, healing proceeded without complications.

[Family work in geriatric psychiatry].

Dementia and other late-life conditions in neuropsychiatry impose a heavy burden on society as a whole and in families in particular. A number of clinical presentations involving family aspects may be seen in clinical practice in a range of diagnoses (Alzheimer's disease, other dementias, depression or any other psychiatric disorder in the elderly). As a consequence, there is a growing trend towards working with families in geriatric psychiatry. Marital issues are not a frequent target for specific clinical intervention, but couple therapy may be useful regardless of age. The effectiveness of

[Abdominal pain in the geriatric patient].

Abdominal pain is a frequent symptom among elderly hospitalized patients. Taking the patient's history is often difficult because of a cognitive impairment. The objective of the present article is to provide a short summery of important diseases leading to abdominal pain and their special features in the elderly patient population. Especially atypical and less symptomatic courses render establishing the correct diagnosis more difficult. The higher comorbidity, the often more advanced disease stage at the time of seeking medical attention and an delayed accurate diagnosis all contribute to the

Gynecologic issues in geriatric women.

The number of women over the age of 65 is projected to almost double in the next 20 years, and clinicians need to be comfortable treating conditions common to this cohort. This review covers several common gynecologic conditions seen in older women, including atrophic vaginitis, lichen sclerosis, pelvic floor disorders, and postmenopausal bleeding. We conclude with evidence-based screening recommendations for gynecologic cancers in older women and tips on doing a pelvic examination.

[What's new in geriatric medicine].

Several studies clarified the role of different interventions such as vitamine D replacement, denosumab treatment, and vertebroplasty in the prevention and management of falls and fractures. A trial tested the effectiveness of pharmaceutical assistance at the time of discharge, emphasizing the potential benefits for the patients and the health care system. Syncopal episodes frequently lead to hospital admission. A retrospective study evaluated the diagnostic yield of different tests and emphasized the importance to actively seek orthostatic hypotension in older patients. Finally, advances rema

Wound care in the geriatric client.

With our aging population, chronic diseases that compromise skin integrity such as diabetes, peripheral vascular disease (venous hypertension, arterial insufficiency) are becoming increasingly common. Skin breakdown with ulcer and chronic wound formation is a frequent consequence of these diseases. Types of ulcers include pressure ulcers, vascular ulcers (arterial and venous hypertension), and neuropathic ulcers. Treatment of these ulcers involves recognizing the four stages of healing: coagulation, inflammation, proliferation, and maturation. Chronic wounds are frequently stalled in the infla

[Guideline for integrated geriatric assessment].

A clinical practice guideline was developed as a response to the increasing of elderly in Mexican population due to the epidemiological transition; this instrument allows the assessment of health conditions for people from 60 years of age and older, and it can be a tool for helping family physicians and nurses in providing care for the main health problems of this group of age. The guideline for gerontologic assessment includes six principal health priorities in older people (loss of vision, difficulty to hear, falls or problems walking, nutritional disorders, memory difficulties, and sleep di

The many faces of geriatric depression.

PURPOSE OF REVIEW: Recent studies on five aspects of geriatric depression, namely subsyndromal depression, risk factors and association with chronic pain, cardiovascular disease and cognitive impairment, are reviewed. RECENT FINDINGS: Subsyndromal depression in the elderly population is not uncommon in east Asia; the prevalence is about 8-9%. Risk factors of geriatric depression include poor health, brain injury, low folate and vitamin B12 and raised plasma homocysteine levels. Depressed elderly with chronic pain are prone to suicidal ideation. Depression is a risk factor for cardiovascular d

[Cardiac failure in the geriatric patient].

Cardiac insufficiency is a disease of old age. Analyses of subgroups have showed that old patients benefit to a particular extent from therapeutic measures the effectiveness of which has been confirmed in numerous studies. In the light of this knowledge, it is all the more difficult to understand why this group of patients are still not receiving effective treatment. In this area, there is an urgent need for improvement. Also difficult to understand is the fact that the guidelines for the treatment of chronic cardiac failure issued by the German Cardiology Society pay so little attention to ca

Surgical considerations in the geriatric patient.

The geriatric patient has a high incidence of foot deformities and may be considered a good surgical candidate if healthy and psychologically ready for the surgical procedure. Foot problems in the geriatric population that may require surgical intervention can be grouped into approximately five categories, including (1) localized orthopedic disorders (ie, bunions, hammer toes, and so on); (2) skin and nail problems (ie, onychomycosis); (3) degenerative and inflammatory arthritis; (4) diabetic foot disorders; and (5) neuromuscular disease. However, age-related changes in cardiovascular, pulmona

"Limited" parathyroidectomy in geriatric patients.

OBJECTIVE: Elderly patients with primary hyperparathyroidism accompanied by other diseases are often denied referral for parathyroidectomy because of the associated risks of general anesthesia and bilateral neck exploration. However, marked symptomatic improvement is recognized after successful parathyroidectomy. The purpose of this report is to examine the postoperative outcome of geriatric patients undergoing "limited" parathyroidectomy. METHODS: Since 1993, 291 consecutive patients with primary hyperparathyroidism were treated with "limited" parathyroidectomy guided by preoperative localiz

[State of bullying in a geriatric hospital].

Recently, reports of bullying and beatings among school children has brought these problems to the public attention. Unfortunately similar problems exist among the aged and institutionalized patients in geriatric hospitals, e.g., covert teasing and bullying of the weak. Therefore, the present status of bullying or being bullied in our geriatric hospital was analyzed and their backgrounds studied to develop clinical countermeasures against bullying. Results of this study were as follows; 1. Nineteen percent of patients were victims of bullying, and patients who had performed the bullying were 3
