A case report of colonic ileus due to eosinophilic nodular lesions caused by Gnathostoma doloresi infection.

Gnathostomiasis is primarily a disease of the skin characterized as creeping eruption or mobile erythema. However, larval Gnathostoma sometimes migrate into an unexpected site to elicit serious illness. Here we describe a case of colonic ileus caused by Gnathostoma doloresi. The patient was a 57-year-old man living in Miyazaki Prefecture, Japan, which is known as an area endemic for this parasite. One week after having eaten a few slices of the flesh of a snake (Agkistrodon halys), he developed severe abdominal pain. An abdominal radiograph revealed multiple gas-fluid levels with a distended b

Food-borne parasitic zoonoses in the Philippines.

A number of food-borne parasitic zoonoses have been recorded in the Philippines and include echinostomiasis, artyfechinostomosis, fascioliasis, heterophydiasis, carneophallosis, clonorchiasis, paragonimiasis, taeniasis, echinococcosis/hydatidosis, diphyllobothriosis/spirometrosis and sparganosis, intestinal capillariasis, gnathostomiasis, angiostrongylosis, toxoplasmosis and sarcosporidiosis. Some are now rarely observed while others continue to be public health problems. Many are endemic in certain areas of the Philippines because of the habit of consuming raw or partly cooked fish, snails, c

Neuroparasitic infections: imaging features and diagnostic algorithms.

Neuroparasitic infections are caused by a variety of organisms that affect the central nervous system (CNS). Given the increasing rates of international travel, climate change, and immunosuppression, neuroparasitic diseases represent a major global health concern. Accurate diagnosis relies on non-invasive medical imaging combined with exposure history and laboratory testing. Timely institution of appropriate therapy can drastically improve patient outcomes. This article represents an international collaboration of radiologists on multiple continents to illustrate exemplary radiologic cases and

Gnathostoma binucleatum: pathological and parasitological aspects in experimentally infected dogs.

Lesions and antibody kinetics produced by inoculation of Gnathostoma binucleatum larvae into dogs are described, as well as the morphology of the recovered parasites. In four out of five infected bitches parasite phases were found in the stomach. Only one bitch eliminated eggs and adult parasite phases in feces. In this bitch, the prepatency period lasted 22 weeks and the patency period 14 weeks. Necropsy results showed a copiously vascularized 8-cm diameter fibrous nodule lodged in the greater curvature of the stomach. Two bitches that eliminated no eggs showed 1- to 2-cm diameter nodules on

Mucocutaneous manifestations of helminth infections: Trematodes and cestodes.

In the 21st century, despite increased international travel for vacation, work, and medical missions and immigration into the United States, there is little published in the dermatology literature regarding the cutaneous manifestations of helminth infections. It has been estimated that 20% to 70% of international travelers suffer from some travel-related health problem. Approximately 17% of travelers seek medical care because of cutaneous disorders, many related to infectious etiologies. This review will focus on cutaneous diseases caused by helminth infections. Part I of the review focused on

Evaluation of human IgG subclass antibodies in the serodiagnosis of angiostrongyliasis.

Immunoglobulin G subclass antibody (IgG1, IgG2, IgG3, and IgG4) responses to the rat lungworm, Angiostrongylus cantonensis, were analyzed using the immunoblotting technique in an attempt to further improve the sensitivity and specificity for the serodiagnosis of human angiostrongyliasis. Serum samples from patients with proven angiostrongyliasis and from clinically suspected cases of angiostrongyliasis with eosinophilic meningitis were tested. Sera from patients with other parasitic illnesses and from healthy volunteers were also analyzed. The results indicate that the immunoblotting used to d

Purification of a specific immunodiagnostic Parastrongylus cantonensis antigen by electroelution from SDS-polyacrylamide gels.

A 31-kDa glycoprotein antigen was purified by electrophoresing the crude extract of Parastrongylus cantonensis adult worms in a 12% SDS-polyacrylamide gel, identifying the 31-kDa component with prestained molecular weight standards, cutting the desired gel strip, and then isolating it by electroelution. Antigen fraction of 31 kDa was re-electrophoresed, transferred to a nitrocellulose membrane and found to be reactive with only the sera from patients with parastrongyliasis. No reactive band was observed with the sera from other related parasitic infections, eg, gnathostomiasis, toxocariasis, f

Gnathostoma binucleatum: excretion-secretion antigen analysis obtained from advanced third-stage larvae in in vitro culture.

The paper describes an introductory characterization of antigenic stimulation of excretion-secretion products (ESP) of Gnathostoma binucleatum advanced third-stage larvae cultured in vitro and proteinases present in this products. Excretory and secretory proteins were obtained after 10 larvae were maintained in 5% CO(2) RPMI medium. The supernatant was collected each week for two months. The proteins were dialyzed, concentrated, and separated in 10% SDS-PAGE gels under reducing conditions and transferred to nitrocellulose paper for immunoblot analyses. G. binucleatum immunized mice serum was u

Enzyme-linked immunosorbent assay for detection of antibody to Gnathostoma antigen in patients with intermittent cutaneous migratory swelling.

A sensitive enzyme-linked immunosorbent assay is described for the detection of immunoglobulin G antibody to Gnathostoma antigen in the sera of patients with intermittent cutaneous migratory swelling who were suspected of being infected by the tissue nematode Gnathostoma spinigerum. The antigen used was a crude somatic aqueous extract of the third larval stage obtained from naturally infected eels. The 1:320 dilution of all sera from 46 patients gave enzyme-linked immunosorbent assay values above the mean (+2 standard deviations) of normal healthy controls, which was arbitrarily used as a cuto

Effect of some physical factors on the viability of third-stage Gnathostoma binucleatum larvae.

To diminish the risk of transmission to humans of advanced third-stage larvae (A3L) of Gnathostoma binucleatum in fish foods, we evaluated the effects of some physical factors on larval viability. A3L protected within fish meatballs were subjected to freezing, refrigeration, boiling, dry heat, and immersion in lemon juice. By freezing, larvae were killed in 48 h, by refrigeration after 30 days, by boiling in 4 min, and by broiling for 60 min. By lemon juice immersion (pH 2.5), encysted larvae were killed after 5 days and nonencysted larvae in 7 h. Results show that freezing fish at -10 to -20°

Gnathostoma spinigerum: immunodepression in experimental infected mice.

Mice were infected with 8- or 25-infective worms of advanced third stage Gnathostoma spinigerum larvae (L3) which were obtained from natural infected eels. On day 14, 60 and 200 post infections (PI), spleen cells of infected mice were tested for lymphoproliferative responses in vitro against the mitogen and specific L3 somatic antigen in order to clarify the cellular immune status of the host upon this nematode infection. Reduced responsiveness to Con A was observed in infected mice. These depressed responses were more pronounced in chronically infected mice (day 200, PI) than in day 14 and da

A dot-blot enzyme-linked immunosorbent assay (dot-blot ELISA) using an electroeluted 31-kDa glycoprotein from adult worms of Parastrongylus cantonensis as the specific antigen was evaluated for the immunological diagnosis of patients infected with P. cantonensis. The sensitivity and specificity for the detection of serum antibody to P. cantonensis in dot-blot ELISA were both 100%, as determined with serum samples of ten P. cantonensis-infected patients, 60 patients with other related parasitic infections, and 20 uninfected controls. The test was as sensitive and specific as the immunoblot test

Fish as the natural second intermediate host of Gnathostoma spinigerum.

Gnathostomiasis is a helminthic disease most frequently occurring in Thailand. Human infections are usually found to be caused by Gnathostoma spinigerum, although five species of the genus Gnathostoma exist in Thailand, and three of these are capable of infecting man. In Thailand, 47 species of vertebrates--fish (19), frogs (2), reptiles (11), birds (11) and mammals (4)--have been reported to serve naturally as the second intermediate (and/or paratenic) hosts of G. spinigerum. Of these, fish, especially swamp eels (Monopterus albus), were found to be the best second intermediate/paratenic host
