Scrotal reconstruction following fournier gangrene using the medial circumflex femoral artery perforator flap.

Fournier gangrene can have devastating consequences. Often in the male, the testicles become exposed and demand both a functional and esthetic coverage. Local medial thigh cutaneous flaps usually still are available and have evolved to become a preferred solution. The medial circumflex femoral artery perforator (MCFAP) flap based on musculocutaneous perforators of the gracilis muscle represents yet another one of these options. As with all muscle perforator flaps, no muscle need be expended. A case report illustrates the role of the MCFAP flap for creation of a neoscrotum.

Hypoglossal-Facial Side-to-End Neurorrhaphy With Concomitant Masseteric-Zygomatic Nerve Branch Coaptation and Muscle Transfer for Facial Reanimation: Technique and Case Report.

BACKGROUND: Hypoglossal-facial direct side-to-end neurorrhaphy has become widely used for facial reanimation in patients with irreversible facial nerve damage. Although this procedure achieves good restoration of facial function, it has disadvantages such as mass movement and lack of spontaneity. OBJECTIVE: To present a new facial reanimation technique using hypoglossal-facial direct side-to-end neurorrhaphy with concomitant masseteric-zygomatic nerve branch coaptation and secondary muscle transfer to reduce mass movement and achieve a spontaneous smile in patients with facial paralysis. MET

Response of muscular and cutaneous vessels to physiologic stimulation of chemoreceptors (38505).

Experiments were performed to determine whether physiologic stimulation of carotid chemoreceptors produces different responses in blood vessels of skin and muscle. Carotid chemoreceptors of anesthetized dogs were stimulated with hypoxic and hypercapnic blood and responses were observed in the isolated, perfused gracilis muscle and hindpaw. Chemoreceptor stimulation produced vasoconstriction in the muscle and vadodilatation in the paw (a predominantly cutaneous vascular bed). Responses to hypoxia without hypercapnic acidosis tended to be less pronounced. The study indicates that physiologic sti

Muscle transposition and free muscle transfer in facial nerve reanimation.

PURPOSE OF REVIEW: This review highlights recent advances in facial reanimation surgery particularly related to muscle transposition and free functional muscle transfer (FFMT). RECENT FINDINGS: FFMT and muscle transposition are mainstays in the treatment of chronic facial paralysis. Recent literature evaluates single versus dual innervated FFMT, reanimation of the periocular region and lower lip depressors, and indications for such methods over gold standard FFMT techniques. New donor sites for muscle transposition and FFMT are also described. SUMMARY: Gracilis FFMT (GFMT) continues to be th

[McArdle disease revealed by exercise intolerance associated with severe rhabdomyolysis].

McArdle's disease (MAD) is a rare hereditary myopathy secondary to a deficit in myophosphorylase, an essential enzyme for the use of muscular glycogen reserves. Exercise intolerance to a variable degree is the fundamental manifestation. Muscular enzymes are usually normal or slightly elevated, except during episodes of rhabdomyolysis. Generally, the electromyogram has poor sensitivity for the diagnosis of exercise myopathies. The muscular biopsy can be misleadingly normal. The role of MRI in the diagnosis of MAD is not well clarified in the literature. We report the case of a 16-year-old patie

[Facial asymmetry after vascular pedicled muscle transfer. Additional criterion for evaluating outcome in managing chronic facial paralysis].

Today neurotization and muscle transplantation are considered an ideal method of reconstructing the function of the facial nerve and the mimetic muscles in long-established facial palsy. Transplantation of the gracilis muscle as a segmental muscle provides an appropriate size for the face; however, determining the amount of muscle is difficult in order to obtain the desired dynamic and symmetric results. Prompted by the question of an "ideal" transplant size, a retrospective study was undertaken. Since 1991, 11 microneurovascular gracilis muscle transfers have been performed in 10 patients wit

The prevalence of the larval stage of Taenia ovis at the Port Elizabeth Abattoir.

The prevalence of the larval stage of Taenia ovis and the origin of the animals were recorded for a period of 39 days at the Port Elizabeth abattoir. Cysticercosis was more frequently found in the M. triceps brachii of sheep, but the actual cysticerci were fewer than those found in the semimembranosus, semitendinosus, adductor, biceps femoris, vastus, rectus femoris and gracilis group of muscles of the hind limb (leg muscles). Most animals originated from farms in the Oudtshoorn district.

Entrapment of below-the-knee bypass grafts.

A case is presented in which a below-the-knee saphenous vein bypass graft was compressed between the sartorius and the semimembranosis, semitendinosis, and gracilis muscles. Angiography and noninvasive Doppler showed occlusion with leg extension but no obstruction to flow with flexion. Recognition of this entrapment syndrome and its prompt correction can preserve patency of the graft.

Free-flap surgery with double innervation for smiling restoration in long standing facial paralysis: a literature review.

PURPOSE: Currently, free-flap surgery with double innervation is considered a reliable and effective treatment in case of long standing facial paralysis. This study aimed to provide a current review of the literature (the "current state of the art") on lower third of the facial reanimation in restoring the voluntary and spontaneous smile in adult population using dual innervation free flap surgery. METHODS: We conducted a comprehensive literature review of all relevant papers by searching the electronic databases MEDLINE, Embase, Scopus, and the Cochrane Library. RESULTS: We included 12 retr

A case report of upper limb loss of substance: Use of functional gracilis free flap, brachioradialis transposition and bioglass for bone regeneration.

Complex trauma of the upper limb is a common consequence of involvement in serious accidents. Loss of substance including nerve, bone, tendons and vascular defects are challenging surgical issues. A 27- year-old male presented with complex upper limb trauma and loss of a proximal third of the posterior forearm structure as well as loss of active finger extension, ulnar and radial nerve territory anesthesia and ulnar fracture. A composite nerve-tendon-muscle-skin gracilis free flap was retrieved from the contralateral leg, related to tendon transfer of BR to ELP, to supply active hand extension

Minimal changes in excitation-contraction coupling in spastic human muscle.

Spasticity results from upper motor neuron lesions and can create a deforming force and pain, and is often accompanied by contracture. Although the origin of spasticity is neural, there is ample evidence of secondary muscle changes. Here, we use direct measurement of the force-frequency relationship (FFR) to characterize human muscle's physiological properties. This study directly quantified the FFR of both healthy and spastic human skeletal muscles. Muscle force was measured intraoperatively in healthy gracilis (n = 13; aged 39.4 ± 10.6 yr; surgery due to brachial plexus injury) and spastic b

Prelaminated Gracilis Flap with Buccal Mucosal Graft for Salvage of Devastated Urethra.

In patients with devastated bulbous urethra, that is, bulbar necrosis, failed fasciocutaneous repairs and "watering can perineum" repair options are limited by paucity of reliable local tissue suitable for reconstruction. In this case report we demonstrate a novel variation of a two-stage technique for reconstruction of a devastated bulbous urethra in a 57-year-old male who suffered penetrating trauma to his previously reconstructed urethra. Because of extensive loss of local tissue from the prior reconstruction and subsequent trauma and infection a 2-stage technique with use of gracilis was e

[FACIAL NERVE PARALYSIS - THERAPEUTIC APPROACH, FACIAL REANIMATION AND ADJUNCTIVE TREATMENT].

Paralysis of the facial mimetic muscles causes loss of voluntary and non-voluntary muscle function, as well as facial tone. This is a devastating condition with profound functional, aesthetic and psychological consequences. Etiologies include congenital paralysis and acquired paralysis following viral infection, trauma, head and neck tumors, iatrogenic damage and more. Clinical presentation includes ocular symptoms (dry eye, epiphora, corneal irritation), nasal symptoms (nasal obstruction) and oral symptoms (drooling and speech disturbances). Reconstruction of facial nerve function is based on

Anterior cruciate ligament replacement: comparison of bone-patellar tendon-bone grafts with two-strand hamstring grafts. A prospective, randomized study.

BACKGROUND: The purpose of this investigation was to evaluate replacement of a torn anterior cruciate ligament with either a bone-patellar tendon-bone autograft or a two-strand semitendinosus-gracilis autograft to compare the results of clinical testing, patient satisfaction, activity level, functional status, and muscle strength. METHODS: Fifty-six patients with a torn anterior cruciate ligament were enrolled in a prospective, randomized, controlled study. Twenty-eight underwent reconstruction with a bone-patellar tendon-bone autograft, and twenty-eight were treated with a two-strand semiten

Adductor magnus muscle transfer to restore knee extension: Anatomical studies and clinical applications.

BACKGROUND: Loss of knee extension causes significant impairment. Though nerve-based reconstruction is preferable in cases of femoral nerve palsy or injury, these surgeries are not always appropriate if the pathology involves the quadriceps muscles or presentation too late for muscle reinnervation. Muscle transfers are another option that has been underutilized in the lower extremity. We describe the successful restoration of knee extension by adductor magnus muscle transfer without functional donor morbidity, along with anatomical considerations. METHODS: Ten fresh frozen cadaveric lower lim

Pressing the nerve alters muscle fiber types of the peroneus longus in rats: preliminary evidence for external anal sphincteroplasty.

BACKGROUND: Studies have demonstrated that anal reconstruction with a gracilis graft pressing the dominant nerve could be used to treat fecal incontinence. However, the detailed mechanism by this remains unknown. Herein, we evaluated the alteration in muscle fiber types and contractility of the peroneus longus muscle in rats after pressing its dominant nerves. MATERIAL AND METHODS: The rat soleus and peroneus longus were exposed during surgery. The superficial peroneal nerve was pressed so that the peroneus longus temporarily lost its innervation. The epimysium between the soleus and the pero
