Improvement in tuberculosis infection control practice via technical support in two regions of Ethiopia.

BACKGROUND: Globally recommended measures for comprehensive tuberculosis (TB) infection control (IC) are inadequately practiced in most health care facilities in Ethiopia. The aim of this study was to assess the extent of implementation of TB IC measures before and after introducing a comprehensive technical support package in two regions of Ethiopia. METHODS: We used a quasi-experimental design, whereby a baseline assessment of TB IC practices in 719 health care facilities was conducted between August and October 2013. Based on the assessment findings, we supported implementation of a compre

Contemporary issue: Starting nurse suicide prevention during pre-licensure education.

BACKGROUND: Undergraduate nursing students are at a high risk for mental health challenges, suicidal behaviors, and risk factors. High stress levels and personal and academic pressure may lead to mental health challenges such as depression, anxiety, and post-traumatic stress disorder in undergraduate nursing students. Nursing students experience barriers to mental health such as lack of support, academic burnout, and stigma. AIM: This paper presents the components for a comprehensive suicide prevention plan that nursing faculty can implement into pre-licensure nurse education. METHOD: The ac

Distributing medical expertise: the evolution and impact of telemedicine in arkansas.

Arkansas's telemedicine system has evolved since 2003 from a support mechanism for high-risk pregnancy consultations to an initiative that spans medical specialties, including asthma care, pediatric cardiology, gynecology, and mental health. The system has also expanded care to diverse populations, including incarcerated women and people with HIV/AIDS. This article describes the system's evolution, organization, and diverse activities. It also shows how telemedicine can have a positive impact on a rural state and how such a state can become an engine for change regionally. The Arkansas telemed

Applying the EPIS framework to policy-level considerations: Tobacco cessation policy implementation among California Medicaid managed care plans.

BACKGROUND: In 2016, the California Department of Healthcare Services (DHCS) released an "All Plan Letter" (APL 16-014) to its Medicaid managed care plans (MCPs) providing guidance on implementing tobacco-cessation coverage among Medicaid beneficiaries. However, implementation remains poor. We apply the Exploration, Preparation, Implementation, Sustainment (EPIS) framework to identify barriers and facilitators to fidelity to APL 16-014 across California Medicaid MCPs. METHODS: We assessed fidelity through semi-structured interviews with MCP health educators (N = 24). Interviews were recorded,

An urban green space intervention with benefits for mental health: A health impact assessment of the Barcelona "Eixos Verds" Plan.

BACKGROUND: Mental health disorders account for over 30% of the global burden of disease. There is a positive association between green space exposure and better mental health, and therefore urban greening can be an effective public health tool. Barcelona is a compact city with one of the highest population and traffic densities in Europe, with limited green spaces. Under the umbrella of the Superblock model, the Barcelona City council is implementing the Eixos Verds Plan for extensive street greening. We estimated the potential mental health benefits of this plan. METHODS: We performed a qua

Statistical Regression Model of Water, Sanitation, and Hygiene; Treatment Coverage; and Environmental Influences on School-Level Soil-Transmitted Helminths and Schistosome Prevalence in Kenya: Secondary Analysis of the National Deworming Program Data.

According to the Kenya National School-Based Deworming program launched in 2012 and implemented for the first 5 years (2012-2017), the prevalence of soil-transmitted helminths (STH) and schistosomiasis substantially reduced over the mentioned period among the surveyed schools. However, this reduction is heterogeneous. In this study, we aimed to determine the factors associated with the 5-year school-level infection prevalence and relative reduction (RR) in prevalence in Kenya following the implementation of the program. Multiple variables related to treatment, water, sanitation, and hygiene (W

Prioritizing congenital syphilis control in south China: a decision analytic model to inform policy implementation.

BACKGROUND: Syphilis is a major public health problem in many regions of China, with increases in congenital syphilis (CS) cases causing concern. The Chinese Ministry of Health recently announced a comprehensive 10-y national syphilis control plan focusing on averting CS. The decision analytic model presented here quantifies the impact of the planned strategies to determine whether they are likely to meet the goals laid out in the control plan. METHODS AND FINDINGS: Our model incorporated data on age-stratified fertility, female adult syphilis cases, and empirical syphilis transmission rates

A Multi-level Model to Understand Cervical Cancer Disparities in Appalachia.

The Appalachian region experiences higher incidence and mortality due to cervical cancer compared with other regions of the United States. The goal of the Ohio State University Center for Population Health and Health Disparities (CPHHD), called the Community Awareness Resources and Education (CARE) project, was to understand reasons for this disparity. The first wave (2003-2008) of funding included three projects focusing on the known risk factors for cervical cancer, lack of screening, smoking, and infection with human papillomavirus (HPV). On the basis of the results of these projects, the s

The role of the traditional leader in implementing maternal, newborn and child health policy in Malawi.

Traditional leaders play a prominent role at the community level in Malawi, yet limited research has been undertaken on their role in relation to policy implementation. This article seeks to analyse the role of traditional leaders in implementing national maternal, newborn and child health (MNCH) policy and programmes at the community level. We consider whether the role of the chief embodies a top-down (utilitarian) or bottom-up (empowerment) approach to MNCH policy implementation. Primary data were collected in 2014/15, through 85 in-depth interviews and 20 focus group discussions in two dist

Factors associated with coverage in community-directed treatment with ivermectin for onchocerciasis control in Oyo State, Nigeria.

Community-directed distribution with ivermectin (CDTI) has been adopted by the African Programme for Onchocerciasis Control (APOC) as its main strategy for achieving sustained high coverage in endemic communities. This article describes the coverage results achieved when CDTI was introduced in four local government areas of Oyo State, Nigeria. Using a household survey after the second distribution, researchers documented that 68.6% of the community overall received the drug, and as did 85.0% of those who were eligible (not pregnant, not sick and at least 5 years of age). Six factors were assoc

Healthcare worker perceived barriers to early initiation of antiretroviral and tuberculosis therapy among Tanzanian inpatients.

SETTING: Clinical trials have shown that early initiation of antiretroviral therapy in HIV-infected patients with tuberculosis saves lives, but models for implementation of this new strategy have been under-studied in real-world settings. OBJECTIVE: To identify the barriers and possible solutions for implementing concurrent early treatment with antiretroviral and anti-tuberculosis therapy in a large East African referral hospital where the prevalence of both infections is high. DESIGN: In-depth interviews among hospital administrators, laboratory technicians, nurses, pharmacists, and physici

Antimicrobial Resistance in Somalia: Policy Without Implementation in a Fragile Health System.

Antimicrobial resistance (AMR) is quietly becoming Somalia's next health catastrophe. While the world focuses on immediate crises, an estimated 8,400 deaths annually are directly attributed to AMR, with another 32,700 linked to AMR complications. These numbers place Somalia among the top 10 countries globally for AMR-related mortality, surpassing deaths from maternal disease, cardiovascular conditions, and infectious disease combined. Yet Somalia has a national action plan. The problem is not policy; it is implementation. Despite developing an AMR National Action Plan aligned with global strat

Dynamic effects of outpatient pooling scheme on socioeconomic inequality in healthcare utilization in China from 2011 to 2020.

Despite achieving universal health coverage (UHC), socioeconomic inequalities in healthcare utilization persist in China, particularly in outpatient care, due to limited financial protection. To address this gap, the Chinese government introduced an outpatient pooling scheme under the Urban Employee Basic Medical Insurance (UEBMI), aimed at reducing cost-sharing burdens. This study evaluates the policy's impact on socioeconomic disparities in outpatient care utilization from 2011 to 2020, providing critical insights into the role of insurance reforms in promoting health equity.
