A late Miocene hominid partial pelvis from Hungary.

Substantial differences among the pelves of anthropoids have been central to interpretations of the selection pressures that shaped extant hominoids, yet the evolution of the hominoid pelvis has been poorly understood due to the scarcity of fossil material. A recently discovered partial hipbone attributed to the 10 million-year-old fossil ape Rudapithecus hungaricus from Rudabánya, Hungary, differs from the hipbones of cercopithecids and earlier apes in functionally significant ways. Comparisons were made to extant and other fossil anthropoids using combination of non-landmark-based and linear

Normal tissue complication probability modeling of acute hematologic toxicity in patients treated with intensity-modulated radiation therapy for squamous cell carcinoma of the anal canal.

PURPOSE: To identify dosimetric parameters that correlate with acute hematologic toxicity (HT) in patients with squamous cell carcinoma of the anal canal treated with definitive chemoradiotherapy (CRT). METHODS AND MATERIALS: We analyzed 33 patients receiving CRT. Pelvic bone (PBM) was contoured for each patient and divided into subsites: ilium, lower pelvis (LP), and lumbosacral spine (LSS). The volume of each region receiving at least 5, 10, 15, 20, 30, and 40 Gy was calculated. Endpoints included grade ≥3 HT (HT3+) and hematologic event (HE), defined as any grade ≥2 HT with a modification

The impact of preoperative cervical degeneration on proximal junctional kyphosis in adult spinal deformity surgery with an upper-thoracic upper instrumented vertebra.

OBJECTIVE: Preoperative cervical malalignment, including an increased cervical sagittal vertical axis and proximal junctional angle (PJA), has been associated with proximal junctional kyphosis (PJK) following thoracolumbar deformity correction. However, the influence of cervical disc degeneration on PJK risk remains unclear. This study evaluated whether preoperative MRI-based cervical degeneration predicts PJK following thoracolumbar deformity correction in patients with adult spinal deformity (ASD). METHODS: One hundred twenty-six patients with ASD and preoperative cervical MRI who underwent

Tricortical bone graft for primary reconstruction of comminuted distal humerus fractures.

A tricortical bone graft harvested from the ilium was used to reconstruct a severely comminuted open intra-articular distal humerus fracture in an adult patient. The patient demonstrated a satisfactory functional and radiologic outcome despite loss of the lateral trochlear lip. When the lateral portion of the fractured trochlea cannot be repaired, excision of the fragments and insertion of an autogenous corticocancellous bone graft from the iliac crest can restore satisfactory function, even in the setting of an open fracture provided there is limited contamination and tissue devitalization. A reappraisal of the anatomy of the human lumbar erector spinae.

In the lumbar region the longissimus thoracis and iliocostalis lumborum are separated by the erector spinae aponeurosis and its ventral reflection--the lumbar intermuscular aponeurosis. Lumbar fibres of the longissimus arise from the ilium and the lumbar intermuscular aponeurosis and insert into the accessory processes and proximal ends of the transverse processes of the lumbar vertebrae. Lumbar fibres of iliocostalis insert into the costal elements of the first four lumbar vertebrae. The lumbar insertions of these muscles are homologous to their thoracic insertions. The lumbar intermuscular a

Symptoms of sacral fractures after posterior spinal instrumented fusion: A case series.

BACKGROUND: Improvements in spinal fusion devices and techniques have enabled stronger spinal fusion, resulting in excellent clinical outcomes. Nevertheless, complications associated with implants, such as screw misalignment, screw lubrication, cage dislocation, and skin issues, might occur. This study aimed to investigate the characteristics and symptoms of sacral fractures after spinal instrumented fusion. METHODS: This case series retrospectively examined the medical records of eight patients (one man and seven women; mean age: 74 years) diagnosed with sacral fractures after undergoing pos

What happens at the L5/S1 facet joint when implants are placed across the sacroiliac joint?

BACKGROUND: Injuries to the posterior pelvic ring are often stabilized with fixation across the sacroiliac joint (SIJ). However, the compensatory changes at the neighboring L5/S1 facet joint are unknown. The objective of this study was to determine the compensatory change in pelvic kinematics and contact forces at the L5/S1 facet joint after fixation across the sacroiliac joint (SIJ) using a cadaveric model. METHODS: Five fresh-frozen cadaveric pelvis specimens were dissected to remove non-structural soft tissue. Retroreflective markers were fixed to the L5 body, S1 body and bilateral anterio

A long term follow-up study of the development of hip disease in Mucopolysaccharidosis type VI.

Hip problems in Mucopolysaccharidosis type VI (MPS VI) lead to severe disability. Lack of data on the course of hip disease in MPS VI make decisions regarding necessity, timing and type of surgical intervention difficult. We therefore studied the development of hip pathology in MPS VI patients over time. Data were collected as part of a prospective follow-up study. Standardized supine AP pelvis and frog leg lateral radiographs of both hips were performed yearly or every 2years. Image assessment was performed quantitatively (angle measurements) and qualitatively (hip morphology). Clinical burde

Application of a 3-dimensional printed navigation template in Bernese periacetabular osteotomies: A cadaveric study.

The aim of the present study was to describe the application of 3D printed templates for intraoperative navigation and simulation of periacetabular osteotomies (PAOs) in a cadaveric model.Five cadaveric specimens (10 sides) underwent thin-slice computed tomographic scans of the ala of ilium downwards to the proximal end of femoral shaft. Bernese PAO was performed. Using Mimics v10.1 software (Materialise, Leuven, Belgium), 3D computed tomographic reconstructions were created and the 4 standard PAO bone cuts-ischial, pubic, anterior, and posterior aspects of the ilium-as well as rotation of the

Comparison of conventional surgery with motorized trephine in bone harvest from the anterior iliac crest.

OBJECTIVE: A prospective study was performed to compare morbidity associated with the harvest of corticocancellous block grafts (CCBG) by conventional surgery and procurement of cancellous cores (CC) by means of a motorized trephine from the anterior ilium. STUDY DESIGN: Seventy-six patients requiring 30 mL or less of cancellous bone for maxillofacial reconstruction were placed into 2 treatment groups. One group underwent harvest of CCBGs in the traditional open medial approach to the anterior ilium. The second group had CCs harvested through a 0.5- to 1.0-cm incision with a motor-driven trep

Scoliosis Surgery in a Patient With Advanced Friedreich's Ataxia-It Is Not Too Late.

Friedreich's ataxia is a multisystem disorder with scoliosis being the most common non-neurological manifestation. While scoliosis surgery is typically performed in adolescent, ambulatory patients, few data exist on surgical outcomes in patients with advanced disease. We present a 38-year-old woman with late-stage Friedreich's ataxia and pronounced thoracolumbar scoliosis (Cobb angle 48°) causing severe pain and limited sitting tolerance. After posterior corrective spondylodesis (T4-ilium), she reported marked improvements in pain, sitting tolerance, function, and quality of life in the SF-36

Traumatic L5-S1 spondylolisthesis: report of three cases and a review of the literature.

The literature reports that traumatic spondylolisthesis of L5 is an uncommon lesion. The authors report their experience of three cases of this particular fracture-dislocation of the lumbosacral spine. They stress the importance of certain radiographic signs in the diagnosis: namely, the presence of unilateral multiple fracture of the transverse lumbar apophysis. As far as the treatment is concerned, they state the need for an open reduction and an internal segmental fixation by posterior approach. A preoperative MRI study appears mandatory in order to evaluate the integrity of the L5-S1 disc.
