High rate of potentially inappropriate medication use in older people: a case-control study.

Annually, 172 million fall events cause temporary or permanent impairment in older adults, and this number is increasing. Contributing factors that increase the risk for falls include age, polypharmacy, and malnutrition. This study evaluated medications mainly included in the EU(7)-PIM (potentially inappropriate medication) list. From March 21, 2022, to July 6, 2022, 945 patients who experienced a fall and visited the Department of Emergency Medicine at the Albert Szent-Györgyi Health Centre of the University of Szeged in Hungary. Data from 886 patients were collected (study group). The contro

Self-reported medication in community-dwelling older adults in Germany: results from the Berlin Initiative Study.

BACKGROUND: Older adults have the highest drug utilization due to multimorbidity. Although the number of people over age 70 is expected to double within the next decades, population-based data on their medication patterns are scarce especially in combination with polypharmacy and potentially inappropriate medication (PIM). Our objective was to analyse the frequency of polypharmacy, pattern of prescription (PD) and over-the-counter (OTC) drug usage, and PIMs according to age and gender in a population-based cohort of very old adults in Germany. METHODS: Cross-sectional baseline data of the Ber

Concurrent use of medicines with similar adverse drug reaction profiles in older adults: identifying potential deprescribing opportunities using population-level dispensing data.

INTRODUCTION: Older adults are more vulnerable to medicine-related harm. Identifying concurrent use of medicines with similar adverse drug reaction (ADR) profiles may support opportunities for safer prescribing and deprescribing. AIM: To examine the frequency of concurrent use of medicines with overlapping ADR profiles in older adults. A secondary aim was to examine the contribution of potentially inappropriate medicine (PIMs) to overlapping ADR profiles. METHOD: We used data from the 10% sample of dispensing claims from the Australian Pharmaceutical Benefits Scheme between April and June 20

A Prospective Study on Potentially Inappropriate Drug Use and All-Cause Mortality in Community-Dwelling Older Adults.

BACKGROUND: Potentially inappropriate prescribing (PIP), encompassing potentially inappropriate medications (PIMs) and prescribing omissions (PPOs), is prevalent among older adults and consistently associated with adverse health outcomes. However, evidence on its impact on mortality remains limited. We examined the associations of PIMs and PPOs with long-term mortality in a cohort of community-dwelling older adults. METHODS: One thousand two hundred and ten participants from the third follow-up (1999-2007) of a longitudinal prospective cohort study were followed for mortality until 03/2022. P

Potentially Inappropriate Psychotropic Drugs in Nursing Homes: An Italian Observational Study.

BACKGROUND AND OBJECTIVES: Physicians often face difficulties in selecting appropriate medications for older adults with multiple comorbidities. As people age, they are more likely to be living with a number of chronic conditions (multimorbidity) and be prescribed a high number of medications (polypharmacy). Multimorbidity is frequent in nursing home (NH) residents and the use of potentially inappropriate medications, especially psychotropic drugs, is widespread. This retrospective cross-sectional cohort study examined the frequency of potentially inappropriate psychotropic drugs using the Bee

[Assessment of drug therapy in psychiatric elderly patients based on the AGS Beers criteria.].

The study objective was to analyze the frequency of prescription of potentially inappropriate medications (PIMs) to elderly patients treated in a psychiatric in-patient setting. This study was carried out using AGS Beers criteria, an anti-polypharmacy tool developed under the auspices of the American Geriatric Association. The study indicates a high frequency of use of PIMs from the following pharmacological groups: benzodiazepine derivatives, non-steroidal anti-inflammatory drugs (long-term use), as well as drugs with significant anticholinergic potential. The AGS Beers criteria can help opti

Polypharmacy in Chronic Neurological Diseases: Multiple Sclerosis, Dementia and Parkinson's Disease.

Polypharmacy is an important aspect of medication management and particularly affects elderly and chronically ill people. Patients with dementia, Parkinson's disease (PD), or multiple sclerosis (MS) are at high risk of multi medication due to their complex symptomatology. Our aim was to provide an overview of different definitions of polypharmacy and to present the current state of research on polypharmacy in patients with dementia, PD, or MS. The most common definition of polypharmacy in the literature is the concomitant use of ≥5 medications (quantitative definition approach). Polypharmacy r

Preventive Antibiotic Use in Nursing Homes: A Not Uncommon Reason for Antibiotic Overprescribing.

A substantial portion of "potentially inappropriate" systemic antibiotics in nursing homes are prescribed with the intention of preventing the development of bacterial infections. In the past, such practices were generally considered acceptable; however, they now are being increasingly scrutinized due to concerns about limited benefits and the potential for adverse effects, including contributing to antimicrobial resistance. As a result of these issues and because of the frequency of these practices, unnecessary prophylactic antibiotic use is an appropriate target for antibiotic stewardship pr

AI Workshop for Primary Care Providers: Applying Large Language Models to Reduce Inappropriate Polypharmacy in Older Adults.

This descriptive study was based on an AI workshop for local community primary care providers. Polypharmacy with inappropriate polypharmacy is a very common geriatric syndrome in older adults. It is still hard to resolve. As the application of Large Language Models (LLMs) to clinical practice is growing, LLMs could offer a potential solution to reduce inappropriate polypharmacy in older adults. Our 1.5-hour AI workshop demonstrated how to use LLMs to reduce inappropriate polypharmacy in older adults. We found that ChatGPT identified all potentially inappropriate medications (PIM) for one geria

Clinical Decision Support System for PIM in Elderly Patients: Implementation and Initial Evaluation in Ambulatory Care.

The high prevalence of PIMs in elderly is a major healthcare concern and indicates the need for medication monitoring systems. Most PIM CDSS have shown positive effects respecting PIM prescription but these results were more consistently in hospital settings compared with ambulatory care. We describe the post-implementation evaluation of a PIM CDSS for general practitioners (GP) in the ambulatory setting and explore GP interactions with the PIM alerts. The CDSS generated 3218 unique alerts and involved 2863 elderly patients. Benzodiazepines was the drug with the most alerts triggered. Only 129
