Clinical value of vaginal cones for the management of female stress incontinence.

In women with mild stress incontinence (mean urine loss of 3.7 g/h), pelvic floor training using vaginal cones resulted in a subjective success rate of 70% (19/27), that is complete cure or reduction by >50% of the original severity. However, in women with severe stress incontinence (mean urine loss of 20.7 g/h) cone therapy was successful in only 14% (7/50). The predictive value of six parameters (age, parity, urine loss by pad test, frequency of incontinence, contractile strength of the vagina, and perceived problems) was analyzed and two showed significant prognostic values: a smaller amoun

Rectal hypersensitivity worsens stool frequency, urgency, and lifestyle in patients with urge fecal incontinence.

PURPOSE: Rectal sensory mechanisms are important in the maintenance of fecal continence. Approximately 50 percent of patients with urge incontinence have lowered rectal sensory threshold volumes (rectal hypersensitivity) on balloon distention. Rectal hypersensitivity may underlie the heightened perception of rectal filling; however, its impact on fecal urgency and incontinence is unknown. This study was designed to investigate the impact of rectal hypersensitivity in patients with urge fecal incontinence. METHODS: Prospective and retrospective audit review of all patients (n = 258) with an in

Evaluating the Efficacy of High-Intensity Focused Electromagnetic (HIFEM) Therapy for Postprostatectomy Incontinence in Men.

PURPOSE: Urinary incontinence (UI) is a common complication after radical prostatectomy (RP), adversely affecting patients' quality of life. This study aimed to evaluate the efficacy and safety of high-intensity focused electromagnetic (HIFEM) therapy as a non-invasive treatment for post-prostatectomy UI. PATIENTS AND METHODS: Twenty-seven men (mean age ± SD: 67.9 ± 3.4 years) with persistent UI after RP underwent six HIFEM sessions (28 min, twice weekly) using the BTL EMSELLA® chair. Outcomes were assessed after the sixth session and at one-month follow-up. Primary endpoints were changes in

Compression or obstruction: Prospective analysis of the function of the Adjustable Transobturator Male System (ATOMS) based on preand postoperative urodynamic data.

INTRODUCTION: This analysis, based on pre- and postoperative urodynamic data, is the first to elucidate the influence of the Adjustable Transobturator Male System (ATOMS, A.M.I. GmbH, Feldkirch, Austria) on the lower urinary tract and disclose possible obstructive properties. METHODS: A prospective study was performed in patients who had stress urinary incontinence and were scheduled for ATOMS implantation after radical prostatectomy. Apart from continence assessment (24-hour pad test, International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form [ICIQ-SF]), urodyna

Does electroacupuncture benefit mixed urinary incontinence? A systematic review and meta-analysis with trial sequential analysis.

INTRODUCTIN AND HYPOTHESIS: Mixed urinary incontinence (MUI) comprises a combination of urgency and stress. The efficacy and safety of electroacupuncture (EA) for the treatment of MUI remain unclear. OBJECTIVE: To assess the efficacy and safety of EA in treating MUI. METHODS: We searched PubMed, CENTRAL, Embase, Web of Science, four Chinese databases, clinical research registration platforms, grey literature, and the reference lists of the selected studies. Risk of bias and quality were evaluated using the Revman 5.4 and Jadad scores. Meta-analysis was performed using Stata 15.1 software. Tr

Tibial nerve stimulation compared with sham to reduce incontinence in care home residents: ELECTRIC RCT.

BACKGROUND: Urinary incontinence is prevalent in nursing and residential care homes, and has a profound impact on residents' dignity and quality of life. Treatment options are limited in these care contexts and care homes predominantly use absorbent pads to contain incontinence, rather than actively treat it. Transcutaneous posterior tibial nerve stimulation is a non-invasive, safe, low-cost intervention that is effective in reducing urinary incontinence in adults. OBJECTIVE: To determine the clinical effectiveness of transcutaneous posterior tibial nerve stimulation to treat urinary incontin

Rumba Dance Combined with Breathing Training as an Exercise Intervention in the Management of Stress Urinary Incontinence in Postmenopausal Women: A Randomized Controlled Trial.

Purpose: Stress urinary incontinence (SUI) refers to involuntary leakage from the urethra, synchronous with exertion/effort, sneezing or coughing, which has a negative effect on quality of life. Studies have shown that mild-to-moderate physical activities reduce the risk of SUI by multiple mechanisms. The objective of this study was to determine whether the Rumba dance combined with breathing training (RDBT) can reduce the severity of incontinence and improve the quality of life of patients with SUI. Methods: A randomized clinical trial was conducted with women who were sedentary, were postmen

Phase 1 clinical trial of Vesicare™ (solifenacin) in the treatment of urinary incontinence after radical prostatectomy.

PURPOSE: Permanent and prolonged incontinence following robot assisted radical prostatectomy (RARP) is known to result from injury to the external sphincter. We hypothesize that pre-existing detrusor overactivity may also contribute to delayed return of continence (>3 months). This pilot study examines the safety profile and efficacy of muscarinic receptor antagonist, solifenacin, in incontinent men after prostatectomy. MATERIALS AND METHODS: Men using three or more pads 7 days post catheter removal were invited to enroll into the study (5 mg daily of solifenacin for 3 months). All subjects r

Assessing the Value of PureWick™ for Female Urinary Incontinence in Non-Acute Settings: Multi-Country Cost-Effectiveness and Budget Impact Analyses.

INTRODUCTION: Urinary incontinence (UI) imposes a substantial clinical and economic burden. The PureWick™ device is a non-invasive alternative for women to pads/diapers and indwelling catheters, but its value has never been assessed internationally. We aimed at estimating the cost-utility and budget impact of introducing PureWick in nursing-home and home-care settings in Belgium, France, Germany, Italy, The Netherlands, Spain and the UK. METHODS: A 1-year decision model compared PureWick + pad with pad alone from the payer perspective (National Health Service-NHS/Personal Social Services). Mo

Overlapping sphincteroplasty and modified lotus petal flap for delayed repair of traumatic cloaca.

Traumatic cloaca is a disabling condition characterized by disruption of the perineal body, anterior sphincter tears and loss of the distal rectovaginal septum. Anterior overlapping sphincteroplasty is the method of choice to treat faecal incontinence caused by obstetric injury. However, reconstruction of large perineal body defects may be a challenging task for surgeons. Herein we describe the successful use of a modified lotus petal flap following overlapping sphincteroplasty to repair a traumatic cloaca that had occurred during vaginal delivery 20 years earlier. After 3 months of follow-up

[Urinary incontinence physopathology].

The lower urinary tract is responsible for storing and expelling urine in a micturition cycle that occurs several times daily. The regulation of this function is influenced by numerous endogenous and exogenous parameters. Anatomical integrity of the bladder-sphincter complex and sensory/motor nerve control is essential. The involuntary loss of urine that characterizes urinary incontinence (UI) reflects its dysfunction. There are several types of urinary incontinence: stress urinary incontinence (SUI), urge urinary incontinence (UUI), and mixed urinary incontinence. The mechanisms are numerous

[Recommendations for endoscopic, laboratory and electrophysiological examinations in the investigation of non-neurological female urinary incontinence].

CYSTOURETHROSCOPY: Cystourethroscopy is not recommended in the initial work-up of urinary incontinence except in the following circumstances: Microscopic or macroscopic haematuria requiring screening for an associated tumour especially in the presence of risk factors for urothelial tumour (smoking, occupational exposure). Signs of bladder irritation in the absence of urinary tract infection. Unexplained bladder or pelvic pain. Recurrent urinary tract infections. Discordance between clinical features and urodynamic assessment, which fails to reproduce the symptoms described by the patient. Work

Factors predictive of outcome in tension-free vaginal tape procedure for urinary stress incontinence in a teaching hospital.

INTRODUCTION: A study was performed to determine which patients' characteristics before tension-free vaginal tape (TVT) for stress incontinence are predictive of a failed outcome. METHODS: A prospective cohort of 305 women with urinary stress incontinence underwent a TVT procedure in a teaching hospital. TVT was considered successful when the patient was fully satisfied and no leakage was seen at the standardized stress test. Logistic regression analysis examined the relationship between outcome and 32 pre-, intra-, and postoperative patient characteristics. All operations were done by traine

Intermittent catheterisation for neuropathic urinary incontinence.

Eighty six children with neuropathic urinary incontinence who had been treated by clean intermittent catheterisation for more than five years were reviewed. Eighty five had congenital lesions, and one traumatic paraplegia. During the day, 72 (84%) patients were dry or had minor stress incontinence only, 11 were damp but controlled with pads, and only three were continuously wet. Eleven abandoned clean intermittent catheterisation, five because of poor control, four by choice despite good control, and only two because of deterioration of upper urinary tract disease. Most of them usually had bac

Skin interaction with absorbent hygiene products.

Skin problems due to the use of absorbent hygiene products, such as diapers, incontinence pads, and feminine sanitary articles, are mostly due to climate or chafing discomfort. If these conditions are allowed to prevail, these may develop into an irritant contact dermatitis and eventually superficial skin infections. Skin humidity and aging skin are among the most significant predisposing and aggravating factors for dermatitis development. Improved product design features are believed to explain the decline in observed diaper dermatitis among infants. Where adult incontinence-related skin prob

The effect of preoperative membranous urethral length on likelihood of postoperative urinary incontinence after robot-assisted radical prostatectomy.

BACKGROUND: Urinary incontinence after radical prostatectomy affects many men. In addition to surgical and patient factors, longer preoperative membranous urethral length (MUL) has been suggested to be associated with improved postoperative urinary continence outcomes. Here, we assess the association of preoperative MUL and the risk of persistent postoperative urinary incontinence after robot-assisted radical prostatectomy (RARP) for prostate cancer on extended follow-up. METHODS: All participants underwent RARP at the University of California, San Francisco between 2000-2018. Patients were e
