Correlation of endolymphatic hydrops with clinical characteristics in patients with unilateral Ménière's disease.

PURPOSE: The development of delayed magnetic resonance imaging (MRI) of the inner ear after intratympanic gadolinium (Gd) injection has allowed the identification of endolymphatic hydrops (EH). We aimed to investigate the correlations between EH and auditory-vestibular clinical symptoms in patients with unilateral Ménière's disease (MD). METHODS: In this retrospective study, 91 patients with definite MD (DMD) and 20 patients with probable MD (PMD) underwent intratympanic injection Gd-enhanced MRI of the inner ear. Pure tone audiometry (PTA), speech discrimination score (SRS), electrocochleogr

Endolymphatic Hydrops in Patients With Intralabyrinthine Schwannomas.

Purpose: The presence of endolymphatic hydrops (EH) in patients with intralabyrinthine schwannomas (ILSs) is poorly understood. This study aims to determine whether there is a correlation between endolymphatic hydrops and clinical presentations of ILS. Methods: Data from nine patients with ILSs were retrospectively reviewed between 2007 and 2020. Temporal bone MRI with intratympanic or intravenous injection of gadolinium was applied to detect ILSs and EH. Results: 3D real inversion recovery (IR) sequence MRI of the temporal bone confirmed ipsilateral EH in four patients (4/6). All four patient

Vestibular Endolymphatic Hydrops Visualized by Magnetic Resonance Imaging and Its Correlation With Vestibular Functional Test in Patients With Unilateral Meniere's Disease.

Background: Currently, 3 Tesla-MRI following intratympanic gadolinium injection has made it possible to assess the existence and the severity of hydrops in each compartment of the endolymphatic spaces in vivo. However, the relationship between vestibular endolymphatic hydrops (EH) visualized by MRI and vestibular functional tests, especially the correlation between caloric test, video-head impulse test, and semicircular canal hydrops, has not been well-investigated. Objective: The purpose of this study is to investigate the relationship between the severity of EH in each compartment of otolith

Changes in endolymphatic hydrops in a patient with Meniere's disease observed using magnetic resonance imaging.

We describe a case report of a patient with Meniere's disease whose changes in endolymphatic hydrops were observed using magnetic resonance imaging (MRI). Gadolinium was injected intratympanically through the tympanic membrane, and MRI scans performed with a 3-T MRI unit revealed endolymphatic hydrops inside the perilymphatic space filled with gadolinium. We evaluated the relationship between the image findings and hearing level. The correlation between the degree of endolymphatic hydrops observed by MRI and hearing level in patients with Meniere's disease offers a promising new method to stud

Histamine-induced mucociliary dysfunction and otitis media with effusion.

Histamine, which has been found in middle ear effusions, is a potent pharmacological mediator released at an early stage of allergic reactions or general inflammatory process, increasing permeability of small blood vessels. Histamine might be involved in the origin and chronicity of middle ear effusions. In this study we studied the effect of histamine on the mucociliary function. First we examined the effect of histamine and its H1 and H2 blockers on the ciliary activity in the middle ear. 10(-2) M of histamine deteriorated ciliary activity; however, at lower concentrations ciliostimulatory e

Possible involvement of free radicals in lipopolysaccharide-induced labyrinthitis in the guinea pig: a morphological and functional investigation.

Intratympanic injection of bacterial lipopolysaccharide impaired caloric responses and caused severe and widespread morphological damage to vestibular end organs and the endolymphatic sac in the guinea pig. These effects could be blocked with N-nitro-L-arginine methylester, a competitive inhibitor of nitric oxide synthase, with superoxide dismutase, an O2 scavenger, with dexamethasone, and with ebselen, a scavenger of peroxynitrite. These observations indicate that enhanced nitric oxide and superoxide production, resulting in formation of peroxynitrite, is probably an important factor responsi

Visualization of endolymphatic hydrops and correlation with audio-vestibular functional testing in patients with definite Meniere's disease.

OBJECTIVES: Visulization of endolymphatic hydrops has been reported using magnetic resonance imaging (MRI) in patients with Meniere's disease. However, the relationship between the endolymphatic hydrops visualized by MRI and audio-vestibular functional tests have not been sufficiently investigated, such as pure tone audiometry (PTA), electrocochleography (EcoG), and vestibular evoked myogenic potential (VEMP). METHODS: Here we attempted to visualize endolymphatic hydrops in patients with Meniere's disease using 3Tesla (3T)-MRI following intratympanic gadolinium-diethylene-triamnie petaaetic a

Monocyte-dependent innate immunity prevents severe complications of acute bacterial infection in the middle ear in a murine model.

INTRODUCTION: Acute otitis media (AOM) is a common middle ear infection. The role of immune cells, particularly monocytes, in preventing complications of AOM remains unclear. Therefore, this study aimed to investigate the role of monocytes in immune response to bacterial AOM using a murine model. METHODS: Wild-type C57BL/6 mice were infected with GFP-tagged Pseudomonas aeruginosa (PA01) via intratympanic (IT) middle ear injection to mimic bacterial AOM. Monocyte depletion (MD) was performed using clodronate liposomes. RESULTS: The survival rate after PA01 IT injection was significantly lower

Relationship between endolymphatic hydrops and vestibular-evoked myogenic potential.

CONCLUSION: Vestibular-evoked myogenic potential (VEMP) can be used to examine endolymphatic hydrops, especially in the vestibule. OBJECTIVE: To investigate the relationship between the degree of endolymphatic hydrops revealed by magnetic resonance imaging (MRI) and VEMP. METHODS: Gadolinium diluted with saline was injected intratympanically in 49 ears (40 patients). One day after the injection, the endolymphatic space in the vestibule and the cochlea was visualized by 3 Tesla MRI. A VEMP test was done, and VEMP was judged as absent when the VEMP was within the noise level. RESULTS: VEMP wa

Emerging options in immune-mediated hearing loss.

OBJECTIVE: AIED (autoimmune inner ear disease) is an autoimmune process that leads to the dysfunction of the inner ear, resulting in fluctuating, audiovestibular symptoms. Although the pathogenesis is likely heterogeneous, immune processes within the inner ear ultimately lead to histopathologic changes and sensorineural hearing loss. This review will discuss the latest evidence on treatment options. METHODS: A literature search on articles pertaining to the treatment of autoimmune inner ear disease was performed on PubMed. RESULTS: Corticosteroid treatment continues to remain as first line t

Intratympanic administration of dexamethasone attenuates radiation induced damage to middle ear mucosa.

Radiotherapy (RTx) is a highly effective treatment for head and neck cancer that can cause concurrent damage to surrounding healthy tissues. In cases of nasopharyngeal carcinoma (NPC), the auditory apparatus is inevitably exposed to radiation fields and sustains considerable damage, resulting in dysfunction. To date, little research has been conducted on the changes induced by RTx in the middle ear and the underlying mechanisms involved. Dexamethasone (DEX) is widely used in clinical practice because of its immunosuppressive and anti-inflammatory properties. The present study investigated the

Eosinophilic otitis media and comorbid asthma.

PURPOSE OF REVIEW: Eosinophilic otitis media (EOM) is an intractable otitis media characterized by numerous eosinophils infiltrating the middle ear cavity, which is part of the upper airway. EOM shows a high rate of comorbidity with asthma. They are considered to have a 'one airway, one disease' relationship. Here, we summarize our current knowledge regarding the characteristics of EOM, EOM's relationship with asthma and the efficacy of optimal treatments for EOM. RECENT FINDINGS: The greater the severity of asthma, the more pronounced the development of EOM. Asthma control is usually inadequ

Intratracheal Instillation of Stem Cells in Term Neonatal Rats.

Prolonged exposure to high concentrations of oxygen leads to inflammation and acute lung injury, which is similar to human bronchopulmonary dysplasia (BPD). In premature infants, BPD is a major complication despite early use of surfactant therapy, optimal ventilation strategies, and noninvasive positive pressure ventilation. Because pulmonary inflammation plays a crucial role in the pathogenesis of BPD, corticosteroid use is one potential treatment to prevent it. Nevertheless, systemic corticosteroid treatment is not usually recommended for preterm infants due to long-term adverse effects. Pre

Are intratympanic corticosteroids effective for Ménière’s disease?

Ménière’s disease affects the inner ear and its main symptoms are vertigo, hearing loss and fluctuating aural symptoms. Nowadays, there are many therapeutic alternatives, being the use of intratympanic corticosteroids one that has become popular. To answer this question, we searched in Epistemonikos database, which is maintained by screening multiple databases. We identified four systematic reviews including 15 studies overall, of which seven were randomized trials. We extracted data and generated a summary of findings table using the GRADE approach. We concluded intratympanic corticosteroids
