Treatment options for insulin-dependent diabetes mellitus: a comparison of the artificial endocrine pancreas, continuous subcutaneous insulin infusion, and multiple daily insulin injections.

The essential components of an intensive insulin therapy program are maintenance of stable basal concentrations of insulin between meals and appropriate, timely increases in the concentration of circulating insulin after ingestion of meals. Closed-loop systems that both continuously monitor the plasma glucose concentration and continuously infuse insulin intravenously can reproducibly achieve near-normal glycemia in patients with insulin-dependent diabetes mellitus. Near-normal glycemia can also be achieved with use of a continuous subcutaneous insulin infusion (CSII) or multiple daily injecti

Does pain score in response to a standardized subcutaneous local anesthetic injection predict epidural steroid injection outcomes in patients with lumbosacral radiculopathy? A prospective correlational study.

OBJECTIVE: Epidural steroid injections (ESI) are the most commonly performed pain procedures. Despite numerous studies, controversy continues to surround their effectiveness. The purpose of this study is to determine whether a standard, clinical local anesthetic injection can predict outcomes for ESI. STUDY DESIGN: In this multicenter study, 103 patients received two ESI 2 weeks apart. Prior to their first injection, subjects rated the pain intensity of a standardized subcutaneous (SQ) injection of lidocaine prior to the full dose. Numerical rating scale pain scores were correlated with leg a

Subcutaneous vitamin B12 administration using a portable infusion pump in cobalamin-related remethylation disorders: a gentle and easy to use alternative to intramuscular injections.

BACKGROUND: Cobalamin (cbl)-related remethylation disorders are a heterogeneous group of inherited disorders comprising the remethylation of homocysteine to methionine and affecting multiple organ systems, most prominently the nervous system and the bone marrow. To date, the parenteral, generally intramuscular, lifelong administration of hydroxycobalamin (OHCbl) is the mainstay of therapy in these disorders. The dosage and frequency of OHCbl is titrated in each patient to the minimum effective dose in order to account for the painful injections. This may result in undertreatment, a possible ri

Progressive granulomatous pneumonitis in response to cosmetic subcutaneous silicone injections in a patient with HIV-1 infection: case report and review of the literature.

Silicone, commonly used in cosmetic procedures owing to its presumed inertness, can yield serious sequelae including acute embolization and pneumonitis. Chronic pulmonary sequelae in response to silicone injection have not been previously described. We report a case of chronic progressive granulomatous pneumonitis in response to subcutaneous silicone injections in a transgender male-to-female patient infected with human immunodeficiency virus 1 (HIV-1). After receiving silicone injections to the buttock, the patient developed rapid onset dyspnea, pleuritic chest pain, fever, and chills. Chest

Methotrexate subcutaneous injection for the initial treatment of rheumatoid arthritis can achieve earlier remission compared with oral administration.

In Japan, subcutaneous methotrexate (SC-MTX) administration was introduced in 2022; however, available data regarding its use as an initial treatment or its effectiveness when switching from oral MTX remain limited. The present study aimed to examine the introduction timing and effects of a SC-MTX injection in patients with rheumatoid arthritis (RA). The oral MTX administration group (n=202, O-MTX), initial SC-MTX group (n=59, SC-MTX) and SC-MTX-switched group (n=25, SWITCH) were compared for the disease activity score in the disease activity score in 28 joints with C-reactive protein (DAS28-C

Oral capsule administration of biomacromolecules that achieves bioavailability and pharmacokinetics comparable to subcutaneous injection in dogs - the BIONDD® technology.

Biomacromolecules including peptides, proteins, antibodies, and gene medicines, comprise important classes of therapeutics in treating disease. A drawback of biologics is their inconvenient route of administration, as they are mostly injected. Most patients prefer the oral administration route over injections. To accommodate this need, we developed the BIONDD® technology, which is an oral administration device for biomacromolecules contained in a standard 00 or 0-sized capsule. Upon ingestion and actuation in the stomach, the macromolecule is delivered from the cavity of the device's spike int

The Pharmacokinetics and Pilot Efficacy of Levamisole Administered by Subcutaneous Injection of a Combination Product in Domestic Goats (Capra aegagrus hircus).

Anthelmintic resistance is a major welfare issue in goats, and the efficacy of anthelmintic drugs lies in judicious use. Recently, an injectable combination (levamisole-doramectin) product labeled for cattle became available. This study's goal was to compare the levamisole pharmacokinetic parameters of this new combination drug to a commercially available oral levamisole formulation in goats. Six adult goats received a 9 mg/kg dose subcutaneously of the combination product. Blood samples were collected at 14 time points over 48 h. After a 14-day washout period, the same goats were given 12 mg/

Early Remission of Opioid Use Disorder in an Adolescent Using Buprenorphine Extended - Release Subcutaneous Injection: A Case Report.

Opioid use disorder (OUD) continues to be a major public health crisis, with the current epidemic being driven by synthetic opioids such as illicitly manufactured fentanyl. While medications exist to treat OUD, only sublingual and subdermal buprenorphine formulations are approved for patients aged 16-17 years. Furthermore, almost all pediatric patients who are diagnosed with OUD do not receive medication as treatment. This case describes the innovative use of buprenorphine extended-release subcutaneous injection in a 17-year-old with OUD who has achieved early remission after four months of tr

Immunomodulation by subcutaneously injected methacrylic acid-based hydrogels and tolerogenic dendritic cells in a mouse model of autoimmune diabetes.

Type 1 diabetes is an autoimmune disease associated with the destruction of insulin-producing β cells. Immunotherapies are being developed to mitigate autoimmune diabetes. One promising option is the delivery of tolerogenic dendritic cells (DCs) primed with specific β-cell-associated autoantigens. These DCs can combat autoreactive cells and promote expansion of β-cell-specific regulatory immune cells, including Tregs. Tolerogenic DCs are typically injected systemically (or near target lymph nodes) in suspension, precluding control over the microenvironment surrounding tolerogenic DC interactio

Single Subcutaneous Injection of Lysophosphatidyl-Choline Evokes ASIC3-Dependent Increases of Spinal Dorsal Horn Neuron Activity.

Lysophosphatidyl-choline (LPC), a member of the phospholipid family, is an emerging player in pain. It is known to modulate different pain-related ion channels, including Acid-Sensing Ion Channel 3 (ASIC3), a cationic channel mainly expressed in peripheral sensory neurons. LPC potentiates ASIC3 current evoked by mild acidifications, but can also activate the channel at physiological pH. Very recently, LPC has been associated to chronic pain in patients suffering from fibromyalgia or osteoarthritis. Accordingly, repetitive injections of LPC within mouse muscle or joint generate both persistent
