Intra-Articular Pressure in the Distal Radioulnar Joint: A Biomechanical Study.

PURPOSE: The aim of the study was to use cadaveric models to assess the effect of loading the forearm muscles in different forearm rotations, with or without disruption to the stabilizing components, on the intra-articular pressure of the distal radioulnar joint (DRUJ). METHODS: Ten forearms with no severe osteoarthritis or injury to the DRUJ stabilizers were used. They were placed in a vertical support, and pressure sensors measured pressure within the DRUJ in 5 forearm rotations (neutral, pronation, supination, extension, and flexion) under the following 6 conditions: 1) no loading; 2) load

Prediction of three-dimensional contact stress and ligament tension in the ankle during stance determined from computational modeling.

BACKGROUND: Our goal was to quantify and visualize the three-dimensional loading relationship between the ligaments and articular surfaces of the ankle to identify and determine the stabilizing roles of these anatomical structures during the stance phase of gait. MATERIALS AND METHODS: We applied discrete element analysis to computationally model the three-dimensional contact characteristics and ligament loading of the ankle joint. Physiologic loads approximating those at five positions in the stance phase of a normal walk cycle were applied. We analyzed joint contact pressures and periankle

Partial scapholunate ligament injuries treated with arthroscopic debridement and thermal shrinkage.

PURPOSE: To present the early results of arthroscopic debridement and thermal shrinkage using radiofrequency probes for partial (Geissler grades I and II) scapholunate (SL) interosseous ligament injuries of the wrist. METHODS: Sixteen patients with a mean age of 34 years (range, 18-54 y) presenting with chronic dorsoradial wrist pain unresponsive to initial conservative treatment for a mean period of 12 weeks were included in this study. No patient showed radiologic signs of static dissociation (SL interval, <3.5 mm; mean SL angle, 49 degrees ) before surgery. Diagnostic arthroscopy showed a

A Novel Anatomical Description of the Normal Terminal Fibular Artery and the Terminal Fibular Window: Radiological Findings and Surgical Implications.

A novel anatomical description and radiological finding, related to the anatomical course of the terminal fibular artery (FibA) was recently reported. In the article, the authors found the terminal FibA to change its course, from being posterior and medial to the fibula to surface anteriorly and laterally before residing below the tibiofibular interosseous membrane, in what the authors named the Terminal Fibular Window (TFW). These findings and descriptions were crucial in developing a new technique to access the terminal FibA for limb salvage bypass operations. The aim of this study is to ass

Musculoskeletal Comorbidities and Quality of Life in ENPP1-Deficient Adults and the Response of Enthesopathy to Enzyme Replacement Therapy in Murine Models.

Ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1) deficiency leads to cardiovascular calcification in infancy, fibroblast growth factor 23 (FGF23)-mediated hypophosphatemic rickets in childhood, and osteomalacia in adulthood. Excessive enthesis mineralization and cervical spine fusion have been previously reported in patients with biallelic ENPP1 deficiency, but their effect on quality of life is unknown. We describe additional musculoskeletal complications in patients with ENPP1 deficiency, namely osteoarthritis and interosseous membrane ossification, and for the first time evaluate

The impact of coronal alignment on distal radioulnar joint stability following distal radius fracture.

PURPOSE: Shift of the distal fragment of a distal radius fracture (DRF) in the coronal plane (coronal shift) may compromise the contributions of the distal oblique bundle (DOB) of the interosseous membrane to distal radioulnar joint (DRUJ) stability. The purpose of the study was to test our hypothesis that coronal shift of the distal fragment would increase dorsal-volar DRUJ displacement in response to applied load. METHODS: A distal radius osteotomy was performed proximal to the sigmoid notch base and the ulnar styloid was cut (to simulate triangular fibrocartilage complex detachment) in 10

Genotype-phenotype study in type V osteogenesis imperfecta.

Type V osteogenesis imperfecta (OI) presents with moderate-to-severe skeletal deformity and is characterized by hyperplastic callus formation at fracture sites and calcification of the interosseous membranes of the forearm and lower leg. The facial dysmorphism is not well characterized and has not been described in previous reports. Inheritance is autosomal dominant, although the genetic aetiology remained unknown until very recently. The aims of this study were to establish the genetic aetiology in patients with type V OI and further characterize patients with this condition, and to ascertain

Osteosarcoma in the Setting of Genetically Confirmed IFITM5-Related Osteogenesis Imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a group of skeletal dysplasias characterized by recurrent fractures and fragile bones. Two pathogenic variants in the IFITM5 gene cause distinct forms of OI: The recurrent c.-14C>T variant causes OI type V (MIM #610967), characterized by hyperplastic callus formation and interosseous membrane ossification, while the rare c.119C>T (p.Ser40Leu) variant causes a phenotypically distinct, severe form of IFITM5-related OI without these hallmark features. Radiographically, OI-related skeletal lesions may resemble malignant bone tumors such as osteosarcoma,

Bifocal Radial Fracture/Dislocation and Distal Ulnar Fracture-A Rare Case of Proximal Forearm Instability Not Yet Classified and Literature Review.

INTRODUCTION: Monteggia fractures, first described by Giovanni Battista Monteggia, involve a fracture of the proximal ulna with anterior dislocation of the radial head. Bado's 1967 classification divides these injuries into four types. Rare mixed patterns exist, overlapping with other forearm injuries such as Galeazzi and Essex-Lopresti lesions. These complex fractures/dislocations pose significant diagnostic and therapeutic challenges and are not adequately represented in current classification systems. Methods and Case Presentation: We report the case of a 56-year-old woman with a complex fo

The Effect of Pre-Bending Flexible Nails on Outcomes Following Stabilization of Paediatric Forearm Fractures.

BACKGROUND: Potential complications after elastically stable intramedullary nailing (ESIN) of pediatric forearm fractures include nonunion and malunion. Malunion, especially affecting the radial bow, has been linked to poor postoperative forearm rotation. Pre-bending of nails has been cited as a key step to achieve optimal construct biomechanics and maintain reduction. We hypothesise that unlike in larger medullary diameter bones such as the femur, pre-bending of nails for the treatment of paediatric forearm fractures has no effect on postoperative outcomes. METHODS: Forearm fractures treated

Risk Factors for Chronic Syndesmotic Instability After the First Episode of Acute Ankle Sprain: A Cross-Sectional Study of 356 Cases.

BACKGROUND: Chronic syndesmotic instability (CSI) can be associated with severe long-term pain and disability. This study aimed to investigate the risk factors associated with the development of CSI in patients who underwent conservative treatment after the first episode of acute ankle sprain (AAS) that did not result in a satisfactory outcome. METHODS: We retrospectively analyzed the clinical data of patients who underwent ankle arthroscopy for suspected CSI between January 2020 and September 2023. The patients were divided into the instability and stability groups based on the syndesmotic s

Does the Skeletal Phenotype of Osteogenesis Imperfecta Differ for Patients With Non-COL1A1/2 Mutations? A Retrospective Study in 113 Patients.

BACKGROUND: Osteogenesis imperfecta (OI) is a heritable disease characterized by bone fragility and other extra skeletal manifestations. Most patients with OI have mutations in the COL1A1 or COL1A2 genes. However, a significant minority of patients with clinical OI have non-COL1A1/2 mutations, which have become easier to detect with the use of genetic panels. Traditional understanding of OI pathogenesis was expanded because of these new mutations, and their phenotypic-genotypic relationship is largely unknown. We hypothesized that patients with non-COL1A1/2 mutations have different skeletal cl

Comparison of External Torque to Axial Loading in Detecting 3-Dimensional Displacement of Syndesmotic Ankle Injuries.

BACKGROUND: Current imaging techniques try to quantify 3-dimensional displacement of syndesmotic ankle injuries using 2-dimensional measurements, which may obscure an exact diagnosis. Therefore, our aim was to determine 3-dimensional displacement of syndesmotic ankle injuries under load and torque using a weightbearing computed tomography (WBCT) and to assess the relation with previously established 2-dimensional measurements. METHODS: Seven paired cadaver specimens were mounted into a radiolucent frame. WBCT scans were obtained to generate 3-dimensional models after different patterns of axi

Hypermineralization and High Osteocyte Lacunar Density in Osteogenesis Imperfecta Type V Bone Indicate Exuberant Primary Bone Formation.

In contrast to "classical" forms of osteogenesis imperfecta (OI) types I to IV, caused by a mutation in COL1A1/A2, OI type V is due to a gain-of-function mutation in the IFITM5 gene, encoding the interferon-induced transmembrane protein 5, or bone-restricted interferon-inducible transmembrane (IFITM)-like protein (BRIL). Its phenotype distinctly differs from OI types I to IV by absence of blue sclerae and dentinogenesis imperfecta, by the occurrence of ossification disorders such as hyperplastic callus and forearm interosseous membrane ossification. Little is known about the impact of the muta

Bridle transfer for paresis of the anterior and lateral compartment musculature.

In the Riordan (bridle) transfer, the posterior tibialis muscle as motor is routed through the interosseous membrane and anastomosed into a "bridle" formed by the distal tibialis anterior and peroneus longus muscles. In theory, the bridle provides inversion/eversion balance even if the transfer effects only tenodesis. However, the procedure has been criticized because its insertion is not into bone. This review analyzes the use of bridle transfer in flaccid paresis involving musculature innervated by the peroneal nerve. Surgery was performed 1 to 3 years after injury for patients with traumati

Do we need to suture the pronator quadratus muscle when we do open reduction and internal fixation for fracture of the distal radius.

BACKGROUND: Open reduction and internal fixation is often used for the treatment of distal radius fracture. Opening the pronator quadratus muscle during the process of open reduction and internal fixation is necessary to achieve sufficient exposure. Therefore, knowledge on how to suture the pronator quadratus muscle will be of essence. AIM: The aim of the present study was to determine if suturing the pronator quadratus during the treatment of the distal radius fracture can enhance limb function . METHODS: A total of 126 patients were enrolled for the study. The patients underwent open reduc

Altered fibular growth patterns after tibiofibular synostosis in children.

BACKGROUND: Iatrogenic synostosis of the tibia and fibula following an operation on the leg in a child has been reported rarely in the literature, and the effects of this complication on future growth, alignment, and function are not known. This is a retrospective case series, from one institution, of crossunions of the distal parts of the tibia and fibula complicating operations on the leg in children. The purpose is to alert surgeons to this possible complication. METHODS: The senior author identified eight cases of iatrogenic tibiofibular synostosis seen in children since 1985. The patient

Sequelae of an acute Essex Lopresti lesion: a case report.

INTRODUCTION: the forearm is considered a joint in itself where the ulna and radius interact. It is composed of the proximal radioulnar joint (PRUJ), the interosseous membrane (IOM), with the central band as the main component, and the distal radioulnar joint (DRUJ), which includes the triangular fibrocartilage complex (TFCC). Essex-Lopresti lesion (ELL) is a complex injury caused by axial forearm loading, leading to longitudinal radioulnar dissociation and stability loss due to IOM rupture, PRUJ injury with radial head involvement, and DRUJ injury with TFCC involvement. Early diagnosis is cru
