[The problem of the prognostic significance of perforations of the kidney hollow system and its vascular system; involvement of the lymphatic system and of surrounding organs in patients with hypernephroid kidney cancers].

On the basis of 51 patients with a hypernephroid renal carcinoma the prognostic importance of the size of the tumour, of the infraction into the renal hollow system, the invasion of lymphatic and vascular system as well as of the border of the organ after tumour nephrectomy and a 5-year observation time was discussed. While the isolated infraction into the hollow system was not relevant in smaller tumours, it implies an additional prognostically negative factor in advanced tumours. In the same measure the invasion of renal capsule or perihilary tissue resulted in a prognostically negative fact

Navigating Entire Collecting System During Supine Percutaneous Nephrolithotomy: Is Rigid Nephroscopy Enough? A Prospective Study by International Alliance of Urolithiasis Supine Percutaneous Nephrolithotomy Working Group.

Background: During percutaneous nephrolithotomy (PCNL), accessibility to the entire collecting system is crucial to check the presence of any residual stone fragments. In this study, we aimed to identify the rate of accessibility of all caliceal cavities using lower-, middle- and, upper-pole punctures and the eventual benefit of simultaneous utilization of retrograde/antegrade flexible nephroscopy. Materials and Methods: Data of patients undergone supine PCNL in five different institutions were collected prospectively. Access status to other poles of the kidney with a rigid nephroscope, antegr

Expression of subunits of the metalloendopeptidase meprin in renal cortex in experimental hydronephrosis.

Meprin A is a metalloendopeptidase in the proximal tubular epithelium of rodents that is capable of hydrolyzing a great variety of peptides and proteins. The aim of the present investigation was to investigate effects of ureteral ligation on the expression of meprin subunits. Ureteral ligation resulted in marked decreases in the expression of both alpha- and beta-meprin subunits within 12 h of ureteral obstruction. Even greater downregulation of expression of meprin alpha- and beta-mRNA was noted at 24, 48, and 96 h after ureteral ligation. The greatest decrease in meprin mRNA expression in ob

Robotic-assisted Laparoscopic Ureterocalicostomy (RALUC): How we do it.

PURPOSE: Ureterocalicostomy refers to the anastomosis of the lower pole calyces with the ureter after excision of the hydronephrotic lower renal pole (1, 2). Indications for ureterocalicostomy include previous failed pyeloplasty, ureteropelvic junction obstruction (UPJO) with anatomical abnormalities, such as intrarenal pelvis or short ureter (3) and proximal ureteral strictures (4). The purpose of this video is to demonstrate the technique of Robotic-Assisted Laparoscopic Ureterocalicostomy (RALUC) in a patient with UPJO and intrarenal pelvis. MATERIALS AND METHODS: Preoperatively, a retrogr

The Impact of Pelvicalyceal System Anatomy on Stone Formation in Patients with Single Lower Pole Calyceal Stone on Computed Tomography Urography.

BACKGROUND: To compare various lower pole pelvicalyceal anatomical factors of stone bearing kidney with contralateral normal kidneys and determine whether these factors predispose to stone formation in one kidney. METHODS: A descriptive study was done with Computed Tomography of 54 patients with solitary lower pole calculus in one kidney and normal contralateral kidney were included. Various lower pole pelvicalyceal anatomical factors like infundibulopelvic angle, infundibular width, infundibular length and calyceopelvic height of both stone bearing and contralateral kidneys were measured and

A comparison among PCNL, Miniperc and Ultraminiperc for lower calyceal stones between 1 and 2 cm: a prospective, comparative, multicenter and randomised study.

BACKGROUND: Conventional Percutaneous Lithotripsy (PCNL) has been an effective, successful and easy approach for especially > 1 cm sized calyceal stones however risks of complications and nephron loss are inevitable. Our aim is to compare the efficacy and safety of PCNL, MiniPerc (MP) and UltraMiniPerc (UMP) for lower calyceal stones between 1 and 2 cm with a multicenter prospective randomized study. METHODS: Between January 2015 and June 2018, 132 consecutive patients with single lower calyceal stone were enrolled. Patients were randomized in three groups; A: PCNL; B: MP; C: UMP. 44 patients

The role of percutaneous nephrolithotomy in the management of medium-sized (1-2 cm) lower-pole renal calculi.

OBJECTIVES: The objective of this study was to determine the efficacy (defined by stone-free rates) and safety of percutaneous nephrolithotomy (PNL) in the treatment of medium sized (1-2 cm) symptomatic lower pole renal calculi, and establishment of the short-term morbidity. METHODS: We performed a retrospective analysis of 60 evaluable patients who had undergone PNL for 1 to 2 cm diameter lower-pole (LP) stones between November 2006 to March 2009 and compared these results with other treatment modalities in published literature. RESULTS: In all cases, stones were located in the lower calix.

Can infundibular height predict the clearance of lower pole calyceal stone after extracorporeal shockwave lithotripsy?

PURPOSE: To evaluate the efficacy of extracorporeal shock wave lithotripsy (SWL) on lower calyceal calculi in relation to the renal anatomical factors and determine which of these factors can be used to select patients who will benefit from SWL. MATERIALS AND METHODS: We analyzed retrospectively 78 patients with single radiopaque lower calyceal stones treated with SWL. The patients were evaluated 3 months after lithotripsy with a simple abdominal X-ray and a kidney ultrasound scan. The success of the treatment, removal of all fragments, was correlated with renal anatomical factors measured in

Pinball calculus in a ureterosigmoidostomy.

We report a complication during the treatment of lithiasis with extracorporeal shock wave lithotripsy in a patient with a ureterosigmoidostomy. This woman presented with renal colic bilaterally and renal insufficiency and was found to have an extremely mobile calculus. A significant gaseous reflux from the sigmoid colon was found to propel the solitary calculus in a retrograde fashion across the ureteroileal anastomosis up the ureter into one kidney, and then later, after re-descent to the level of the anastomosis, up into the opposite kidney. After several days of playing hide and seek with t

Normal and near normal caliceal patterns in reflux nephropathy.

Distorted, clubbed calices associated with focal renal scars or areas of more diffuse parenchymal loss make up the accepted urographic diagnostic criteria of chronic reflux nephropathy. It is widely believed that this diagnosis should not be suggested in the absence of the above findings and that scarring with normal calices indicates a vascular aetiology. This study describes five children with marked renal parenchymal loss all of whom have normal or near normal calices underlying the areas of parenchymal thinning and all of whom show severe vesico-ureteric reflux on micturating cystography.

Pacemaker process of ureteral peristalsis in multicalyceal kidneys.

The mechanical activity of isolated strips from different areas of the pyeloureteral system was investigated in 10 pigs: calyx, renal pelvis, pyeloureteral junction and ureter. Additionally, electrical activity was measured in some pyeloureteral preparations using the sucrose-gap technique. Regular spontaneous activity with an average frequency of 9.5/min was recorded in calyceal strips, decreasing to 5.4/min in renal pelvis, 5.7/min in pyeloureteral preparations and to 1.2/min in ureteral preparations. The activity of renal pelvis, pyeloureteral and ureteral preparations was less regular, and

[Optimization of diagnostic and therapeutic tactics for primary megaureter in children].

Obstructive megaureter (MU) and refluxing MU were treated in 2000-2004 in 580 and 711 children, respectively. This number was by 18.9% higher than in 1990-1994. Out of 1291 children with MU, two groups of patients were singled out: group 1 with primary obstructive MU (n = 158) and group 2 with primary refluxing MU (n = 185). In patients of groups 1 and 2 the following operations were made: ureteral reimplantation (n = 126), Koen's operation (n = 104), Politano-Leadbetter operation (n = 12), Lich-Greguaru operation (n = 8), nephrureterectomy (n = 32), heminephrureterectomy (n = 27), transurethr

Renal pelvicalyceal dilation in antepartum pyelonephritis: ultrasonographic findings.

The purpose of the present study was to determine whether pregnant women with pyelonephritis have differences of pelvicalyceal systems, compared with normal pregnant control subjects, that might predispose to upper urinary tract infection. Ultrasonographic examination of both kidneys in coronal and axial planes of 24 women with clinical pyelonephritis and positive urine cultures was compared with results in control subjects matched for gestational age, parity, and race. Women with right or bilateral pyelonephritis had increased dilation of the right calyceal system, compared with controls (1.7
