Axillary lymphadenectomy prepared by fat and lymph node suction in breast cancer.

AIMS: To describe a new technique of axillary dissection and evaluate the results. METHODS: Axillary lymphadenectomy prepared by fat and lymph node suction was performed on 43 patients with breast cancer and uninvolved axilla on physical examination. The mean follow-up was 18 months (range 12-27). After lipolysis and liposuction of the axilla, lymph node dissection was performed by axilloscopy in patients treated with conservative surgery. For patients undergoing modified radical mastectomy, a mastectomy with open axillary dissection was carried out. The axillary space was drained until less

Comparison of Ultrasound-guided Erector Spinae Plane Block Versus Rhomboid Intercostal Block for Perioperative Analgesia in Breast Cancer Surgery.

OBJECTIVE: Over 75% of women who have post-mastectomy reconstruction feel significant pain right away, and about 50% endure chronic pain. Thus, increasing the efficiency of postoperative pain management is crucial. Our study investigated the effects of ultrasound-guided erector spinae plane blocks (ESPB) and rhomboid intercostal blocks (RIB) on perioperative fentanyl use and pain scores in patients undergoing radical mastectomy surgery. METHODS: This was a double-blind, randomised controlled trial conducted at a tertiary care hospital. Patients with breast cancer aged 18-70 years and American

Evaluation of axillary lymph nodes in breast cancer patients with clinically negative axilla using contrast enhanced ultrasonography.

Contrast enhanced ultrasonography enables dynamic evaluation of the microvasculature down to the capillaries when using high resolution ultrasound probes. It's application in the evaluation of axillary lymph nodes in breast cancer patients with clinically negative axilla has been studied in 42 patients. The results of pre operative CEUS evaluation was correlated with histopathology status of axillary nodes after the harvesting of nodes during modified radical mastectomy or sentinel node biopsy. Heterogeneous enhancement with micro bubbles of the axillary nodes was found to be the most distingu

[Diagnostic management of nipple discharge].

Nipple discharge may be a symptom of a variety of diseases. The importance of nipple discharge is its possible association with breast carcinoma. Not only the precise clinical evaluation but also particularly imaging evaluation by mammography, ultrasound, galactography or MR-galactography is necessary to establish the exact diagnosis and plan proper treatment. Depending on the underlying pathology--most commonly intraductal papilloma, fibrocystic changes or breast carcinoma--the optimal therapeutic regimen, such as single lactiferous duct excision on the one end and modified radical mastectomy

Axillary lymphadenectomy for intraductal carcinoma of the breast.

During a ten year period, 175 axillary lymph node dissections were done as part of the treatment for intraductal carcinoma of the breast; 98 patients were treated with modified radical mastectomy and 77 were treated by mammary preservation, consisting of excision of the lesion, axillary dissection and radiation therapy. One of 175 axillary node dissections yielded positive nodes. Axillary dissection for intraductal carcinoma of the breast is unlikely to yield involved nodes and is not indicated for use in most instances. It should be reserved for lesions demonstrating microinvasion.

Breast cancer: the military's experience at Wilford Hall USAF Medical Center.

We reviewed the experience with breast cancer at Wilford Hall USAF Medical Center for the years 1978 through 1988. A total of 868 cases were identified in the Wilford Hall Tumor Registry; overall 5-year and 10-year survivals were 63% and 39%, respectively. Infiltrating ductal carcinoma represented the principal histologic category. The other predominant variants included invasive lobular carcinoma, lobular carcinoma in situ, and ductal carcinoma in situ. Until recently, most of these patients (90%) had modified radical mastectomy as their definitive surgical therapy, with chemotherapy reserved

Metastatic breast cancer to the adductor magnus: A case report and literature overview.

Metastatic breast cancer is commonly found in the pectoralis major and minor muscles, ribs, spine, pelvis, long bones of the extremities, liver, lung, and the brain. It is unusual to see breast cancer metastases to distal skeletal muscle. We report a case of a patient with a history of stage IIA infiltrating lobular carcinoma of the right breast presenting with a metastatic lesion in the right adductor magnus. She was treated four years prior to the onset of the metastases with a modified radical mastectomy but refused postoperative chemotherapy or adjunctive radiation.

Metastatic carcinoma in axillary lymph nodes without pathologic evidence of a primary mammary lesion: a case report.

A 56-year-old nullipara presented with a mass in the right axilla. Pathologically, two enlarged lymph nodes were found to contain metastatic adenocarcinoma consistent with a primary from the breast. Clinically, no breast mass was palpable, and xeromammography showed diffuse ductal hyperplasia and a small focus suggestive of malignancy. Modified radical mastectomy was done, and no tumor was found pathologically in the excised breast. The patient received local radiotherapy and is alive and well one year postoperatively. Attention is drawn to this rare occurrence, and patient management is brief

Positron emission tomography in diagnosis of occult adenocarcinoma of the breast.

Occult adenocarcinoma with clinically apparent axillary lymphadenopathy represents a challenging surgical problem. Mammography is frequently unable to identify a primary breast carcinoma, and extramammary sources are common and equally difficult to identify. This may leave the clinician and patient with a conundrum of whether to proceed with "blind" mastectomy. A 35-year-old white female presented with axillary adenopathy and a normal breast physical exam. Mammography was unable to demonstrate a specific tumor. Excisional biopsy of the axillary lymph node demonstrated metastatic adenocarcinoma

Thoracic manifestations of breast cancer and its therapy.

Breast cancer is the second most common cause of cancer-related death in women. In most patients, imaging demonstrates thoracic changes resulting from either treatment, complications of treatment, or tumor recurrence or metastasis. The postsurgical imaging appearance of the chest wall depends on the surgical method used (radical mastectomy, modified radical mastectomy, breast-conserving surgery, breast reconstruction). The most common surgery-related complication is seroma. Radiation therapy frequently causes radiation pneumonitis, which occurs approximately 4-12 weeks after the completion of

Axillary clearance in operable breast cancer: still a necessity?

Complete axillary dissection, as part of radical mastectomy, was the standard of care for the first three-quarters of this century. Long-term follow-up of these patients showed substantial cure rates for positive-node patients before systemic therapy was available, indicating a therapeutic value to nodal dissection. There was also good control of the axilla; axillary recurrence after removal of positive nodes was quite low. Even today, in patients with positive nodes, complete axillary clearance as part of a modified radical mastectomy or a breast conservation approach with lumpectomy leads to

Outcome of pectoralis major myomammary flap for post-mastectomy breast reconstruction: extended experience.

BACKGROUND: In the past decade there has been an enormously expanding interest among rural Egyptian patients, relatives, and treating oncologists in post-mastectomy reconstruction as an integral part of patient care. We introduced our technique of pectoralis major myomammary cutaneous flap for single-stage reconstruction of large breasts, nipple, and areola. METHODS: The new technique is based on performing modified radical mastectomy in the classic manner, and then using a pectoralis major myocutaneous pedicled flap from the other side in breast reconstruction. The flap is based on the pecto

Immediate, bilateral transverse rectus abdominis musculocutaneous (TRAM) flap reconstruction after mastectomy.

Although breast reconstruction has been shown to provide psychological benefits in mastectomy patients, there is reluctance to perform immediate, bilateral TRAM flap reconstruction because of concerns regarding magnitude of the procedure, length of hospitalization, potential complications, and long-term recovery. Between June, 1990 and March 1992, 15 patients underwent immediate, bilateral TRAM flap reconstruction following bilateral mastectomy at the University of Michigan Hospitals. Diagnoses included lobular carcinoma in situ (nine patients), strong family history of breast cancer (five), o

Mastopathy of the accessory breast in the bilateral axillary regions occurring concurrently with advanced breast cancer.

We herein report a 41-year-old Japanese woman who demonstrated advanced cancer in the left breast occurring concurrently with mastopathy of the accessory breast tissue in the bilateral axillary regions, which appeared to be metastatic lymphadenopathy. A preoperative examination, including a mammogram, US, and CT, did not provide us with a definite diagnosis of the axillary masses: it was essential to diagnose the masses preoperatively since a bilateral mastectomy with nodal dissection is called for if the right axillary masses are metastatic from a cancer in the right breast. An intraoperative

Ductal carcinoma in situ with microinvasion. A curable entity using surgery alone without need for adjuvant therapy.

Of 408 negative axillary node (NAN) patients surgically treated at Roswell Park Cancer Institute (Buffalo, NY, 1976 through 1987), 36 (8.8%) presented with ductal carcinoma in situ with microinvasion (DCISM). In more than 50% of the patients (20/36) the disease was detected solely by mammography (microcalcifications and/or radiological density less than 1.5 cm). Thirty-three patients underwent modified radical mastectomy; three had wide excision +/- axillary dissection. Residual disease after excisional biopsy was found in 22 of 33 mastectomy specimens (67%): 11 (33%) pure ductal carcinoma in

Adjuvant chemotherapy with cyclophosphamide, methotrexate, and 5-fluorouracil, vincristine, and prednisone compared with single-agent L-phenylalanine mustard for patients with operable breast carcinoma and positive axillary lymph nodes: 20-year results of a Southwest Oncology Group study.

BACKGROUND: Adjuvant combination chemotherapy with cyclophosphamide, methotrexate, and 5-fluorouracil plus vincristine and prednisone (CMFVP) was compared with single-agent L-phenylalanine mustard (L-PAM) for the treatment of patients with axillary lymph node positive primary breast carcinoma over 20-years of follow-up. METHODS: Four hundred forty-one women with axillary lymph node positive breast carcinoma were randomized to receive either combination chemotherapy with cyclophosphamide (60 mg/m(2) orally every day for 1 year), fluorouracil (300 mg/m(2) intravenously [IV] weekly for 1 year),

Male breast cancer presenting as an axillary mass: a case report and literature review.

The presentation of breast cancer in a male as a solitary axillary mass is very unusual. In general, though the disease presents later in men than in women, the presentation is essentially the same in either gender group. Late and/or atypical presentation, however, delays diagnosis and treatment and carries a much worse prognosis.

Considering wider myofascial involvement as a possible contributor to upper extremity dysfunction following treatment for primary breast cancer.

BACKGROUND: Breast cancer is the most common malignancy in women. Although scarring is recognized as contributing to limited shoulder movements, compromised tissue gliding in a wider range of fascial and connective tissue structures is under-recognized. AIM: To report on soft tissue patterns in patients with upper limb dysfunction after modified radical mastectomy. METHODS: Tissue gliding was assessed in the neck, chest wall, abdomen, axilla and upper arm. Scarring, areas and directions of tightness were mapped on upper body charts. RESULTS: Eighteen shoulders were evaluated. All patients h
