Intraoperative continuous monitoring of evoked facial nerve electromyograms in acoustic neuroma surgery.

BACKGROUND: Preservation of facial nerve function is one of the most important goals in acoustic neuroma surgery. We have been using intraoperative continuous monitoring of evoked facial nerve electromyograms (EMGs) since 1997 in acoustic neuroma surgery. We therefore investigated surgically treated patients to clarify the usefulness of this monitoring, and to determine safety criteria for preserving facial nerve function. METHODS: This intraoperative continuous monitoring of evoked facial nerve EMG is a method for checking the EMG evoked by continuous direct electrical stimulation of the fac

Time-frequency analysis of somatosensory evoked potentials for intraoperative spinal cord monitoring.

PURPOSE: To evaluate the potential use of time-frequency analysis and its reliability in intraoperative somatosensory evoked potential (SEP) monitoring. METHODS: One hundred ninety-one patients undergoing thoracic and/or lumbar spinal surgery were studied retrospectively. The SEP signals were recorded during different stages of surgery. Averaged SEP was analyzed by short-time Fourier transform. The main peak in the time-frequency interpretation of SEP was measured in peak power, peak time, and peak frequency. The variability of these parameters was compared with that of amplitude and latency

Does electrode placement influence quality of intraoperative monitoring in vestibular schwannoma surgery?

OBJECT: Continuous recording of electromyographic signals (EMG) is a standard method for intraoperative monitoring of facial nerve function in cerebello-pontine angle surgery. Subcutaneous needle electrodes in the facial muscles are used in different setups. The goal of this study was to compare two commonly used electrode setups concering sensitivity for pathological EMG activity. PATIENTS AND METHODS: A group of 10 patients undergoing vestibular schwannoma surgery were examined. Continuous EMG from facial muscles was recorded using needle electrodes in setups according to Kartush or Møller,

[The usefulness of electromyographical monitoring with intraoperative brain mapping during motor lesionectomy].

INTRODUCTION: Surgery for resection of supratentorial lesions near the motor areas and/or internal capsule carries an associated risk of damage to cortical or subcortical motor pathways. Intraoperative brain mapping, using direct cortical electric stimulation, is a surgical adjunct used during lesionectomy in functional cortex. The technique of observing movements elicited by electrical stimulation has proved useful for intraoperative localization of motor pathways but it is difficult observe the entire body at once. Thus, a small movement may be missed while attention is focused on another si

Intraoperative monitoring of facial and cochlear nerves during acoustic neuroma surgery. 1992.

Preservation of facial nerve function during acoustic neuroma surgery can be improved significantly by monitoring of facial electromyography (EMG) during surgery. Mechanical trauma during dissection causes EMG activity that can be played over a loudspeaker for direct feedback to the surgeon. Electrical stimulation can be used to locate the nerve even when it is out of direct view, and the threshold for stimulation provides a measure of facial (or other motor nerve) integrity. Cochlear nerve function also can be monitored by the recording of auditory brain stem responses or compound action pote

Central venous sampling for intraoperative parathyroid hormone monitoring: are peripheral guidelines applicable?

Intraoperative parathyroid hormone (PTH) monitoring has become an integral adjunct to minimally invasive parathyroidectomy. Guidelines for predicting therapeutic excision of all hyperactive parathyroid tissue have been routinely based on peripheral blood samples drawn at various time intervals. Whether these same guidelines can be used to predict success based on central blood draws has not been established. The authors wanted to evaluate whether peripheral criteria were applicable when PTH levels were drawn from a central location. Simultaneous peripheral venous (PV) and central venous (CV) P

Impact of brain shift on intraoperative neurophysiological monitoring with cortical strip electrodes.

BACKGROUND: Intraoperative neurophysiological monitoring has become the standard procedure for locating eloquent regions of the brain. Such continuous electrical stimulation of motor pathways is usually applied by means of flat silicon-embedded electrodes placed directly on the motor cortex. However, shifting of the silicon strip on the cortical surface as well as electrode displacement due to brain shift underneath the electrode can lead to inaccurate recordings not directly caused by intraoperative impairment of the motor cortex or the motor pathways. METHOD: This prospective study was cond

Image-guided placement of eye muscle electrodes for intraoperative cranial nerve monitoring.

OBJECTIVE: Monitoring of the oculomotor system during cranial base or brainstem surgery requires extraocular intraorbital insertion of electromyography electrodes. We investigated the use of image-guidance technology for anatomically correct intraorbital electrode placement. METHODS: For neuronavigation, an optical tracking system was used in a standard fashion. Needle electrodes were inserted percutaneously into the lateral rectus, inferior rectus, and superior oblique muscle along the axis of a hand-held pointer or by means of an electrode applicator to allow direct tracking with the naviga

Intraoperative monitoring to preserve central visual fields during occipital corticectomy for epilepsy.

Photic driving using a flashing strobe light was recorded via intracranial electrodes in two patients with occipital epilepsy being evaluated for surgery. The same technique was used to monitor the visual cortex intraoperatively. Visual evoked potentials (VEPs) were also obtained using the intracranial electrodes in one patient. Preoperative photic driving occurred in a separate location from the cortical areas producing ictal epileptiform activity. VEPs were located in the same site as photic driving. Photic driving was monitored throughout the resection and remained unaffected at the end of

Intraoperative monitoring of the inferior alveolar nerve during mandibular sagittal-split osteotomy.

In order to evaluate the risk of nerve injury and to prevent iatrogenic damage at different stages of bilateral sagittal-split osteotomy (BSSO) of the mandible, we monitored the function of the inferior alveolar nerve (IAN) continuously on both sides in 13 orthognathic patients undergoing BSSO. The IAN was stimulated at the mental foramen with two monopolar needle electrodes fixed to the dental splint, and the orthodromic sensory nerve action potentials (SNAP) of the IAN were recorded with a silver-wire electrode inserted near the oval foramen on each side. The latencies, amplitudes, and senso

Investigation of a coaxial bipolar nerve stimulator for intraoperative motor nerve monitoring.

Intraoperative cranial motor nerve monitoring has improved the preservation of cranial nerve function during cerebellopontine angle and skull base surgery. Further improvements are needed to provide greater selectivity and sensitivity during intraoperative motor nerve stimulation. Use of bipolar stimulator probes has been recommended by previous investigators. The authors hypothesized that a coaxial bipolar stimulator design might provide important advantages over existing designs. This study compared the results of bipolar and monopolar stimulation using a coaxial stimulator probe in a rat mo

[Intraoperative monitoring in patients undergoing surgery of lesions involving the cavernous sinus].

We reviewed the intraoperative monitoring and anesthesia in 45 patients, who had undergone operation of lesions involving the cavernous sinus between September, 1990, and May, 1993. The patient was placed in a 30 degree of head-up position to reduce the bleeding from the cavernous venous plexus. However, air embolism during the operation has not been noted in any patients. In 13 of 45 patients, a transient or permanent internal carotid artery (ICA) occlusion was performed intraoperatively under the brain protection by thiopental and the monitoring of electroencephalograph, somatosensory evoked

Monitoring of emission as direct intraoperative control for nerve sparing retroperitoneal lymphadenectomy.

An intraoperative test to identify emission relevant lumbar postganglionic nerves during nerve sparing retroperitoneal lymph node dissection is presented. The neurophysiological course of the ejaculation into the posterior urethra, the emission, is described. A retroperitoneal nerve sparing procedure was done for nonseminomatous testicular tumors bilaterally in 7 patients with pathological stage I disease and unilaterally in 4 with pathological stage IIa disease. While the isolated lumbar nerves were electrostimulated, the seminal vesicles and bladder neck were monitored by suprapubic transves

Intraoperative somatosensory evoked potential monitoring of the sciatic nerve: an animal model.

Sciatic, peroneal, and tibial nerves were isolated in 18 hind limbs in 10 adult mongrel cats. A pair of needle electrodes was used to stimulate both divisions of the sciatic nerve individually at the level of the popliteal fossa. The sciatic nerve was injured by complete or partial transection, crush, and controlled compression. Motor function was correlated with intraoperative cortical somatosensory evoked potential and spinal somatosensory evoked potential tracings. We observed that significant changes in the waveforms of cortical somatosensory evoked potential and spinal somatosensory evoke
