OBJECT: The JC virus is a human neurotropic polyomavirus that causes progressive multifocal leukoencephalopathy and is closely related to simian virus 40. Several recent reports have indicated a possible association between the JC virus and the development of various human brain tumors. The authors examined the presence of JC virus DNA sequences in primary brain tumors in pediatric patients to evaluate the hypothesis that particular brain tumors can arise in the pediatric population as a consequence of infection with the JC virus.
METHODS: Genomic DNA sequences were isolated from 62 brain tum

BACKGROUND: White dot syndromes (WDS) are a group of idiopathic multifocal inflammatory conditions that can be recognized and distinguished by lesion morphology, other specific clinical features, imaging findings, and disease course. Our purpose is to describe an atypical case of WDS with choriocapillaris ischemia shown by multimodal imaging including swept-source OCT angiography (OCTA) in a 30-year-old woman with latent tuberculosis.
FINDINGS: At presentation, visual acuity in the left eye was 20/500. Clinical findings included macular granularity, diffuse outer retinal discoloration with sa

AIM: To investigate short-term changes in choroidal thickness in response to peripheral myopic defocus induced by two designs of multifocal corneal gas permeable contact lenses (MFGPCL) in young adults.
METHODS: Seventeen participants, with a mean age of 24.5±4y, underwent choroidal thickness and vascularity index measurements using enhanced depth imaging optical coherence tomography (EDI OCT) at baseline, one day, and one week following MFGPCL wear. Two center-distance MFGPCL designs with similar center zone diameters of 3.0 mm but different peripheral add powers (low add: +1.5 D and high ad

A 43-year-old black woman with acquired immunodeficiency syndrome developed bilateral multifocal choroidopathy characterized by slowly enlarging round to oval lesions in the posterior pole and midperiphery. Systemic evaluation revealed no evidence of mycobacterial, fungal, or spirochetal disease. Fluorescein angiography of the lesions showed early hypofluorescence with late staining of the lesions, which appeared deep to the retinal circulation. There was no evidence of retinal involvement. Over a 4-month period of observation, the lesions appeared to enlarge slowly, with no evidence of vitreo

Choroidal lymphoma is a rare intraocular malignancy that often presents with subtle and nonspecific clinical signs, frequently masquerading as inflammatory or metastatic choroidal disease and leading to delayed diagnosis. Because histopathologic confirmation is not always immediately feasible, multimodal imaging plays a pivotal role in raising clinical suspicion, guiding differential diagnosis, and monitoring disease activity. This narrative review summarizes current evidence regarding characteristic imaging findings of choroidal lymphoma across available modalities, including color fundus pho

We report a rare case of acute posterior multifocal placoid pigment epitheliopathy (APMPPE) in an elderly patient and a review of the recent scientific literature. A 65-year-old Caucasian man, forty-five days after return from tropical travel he developed right-sided facial palsy with right hemiplegia, dysarthria and a positive Babinski sign. He started prednisone 90 mg/day. Clinical, laboratory and imaging examinations were reported. Five months later there was a complete neurological and ophthalmological recovery. Although APMPPE was described primarily in young adults, we report the first c

PURPOSE: The authors document an animal model of retinal pigment epithelial (RPE) detachments.
METHODS: N-ethylmaleimide (NEM) and sodium iodate (sulfhydryl-alkylating agents) were injected into the vitreous of Dutch pigmented rabbits. Subretinal fluid samples were withdrawn 0.5 and 3 hours after NEM injections and analyzed for albumin content by gel electrophoresis. Selected eyes were examined histologically.
RESULTS: Injections of 9.2 micrograms NEM caused wrinkling of the retina within 15 minutes, at which time fluorescein angiography revealed multifocal RPE detachments. At this stage, hi

The human polyomavirus, JCPyV, is the causative agent of progressive multifocal leukoencephalopathy (PML) in immunosuppressed and immunomodulated patients. Initial infection with JCPyV is common and the virus establishes a long-term persistent infection in the urogenital system of 50-70% of the human population worldwide. A major gap in the field is that we do not know how the virus traffics from the periphery to the brain to cause disease. Our recent discovery that human choroid plexus epithelial cells are fully susceptible to virus infection together with reports of JCPyV infection of choroi

PURPOSE: Birt-Hogg-Dubé Syndrome (BHDS) is a rare autosomal dominant condition that can predispose patients to numerous cutaneous fibrofolliculomas and other cutaneous lesions, pulmonary cysts with spontaneous pneumothorax, and multifocal renal tumors and cancer. The genetic mutations responsible for BHDS are related to tumor suppression and the mammalian target of rapamycin (mTOR) pathway. Previous reports of the ocular findings in BHDS include eyelid fibrofolliculomas, "flecked chorioretinopathy," and one report of choroidal melanoma. We report a patient with BHDS who presented with choroida

PURPOSE: Acute posterior multifocal placoid pigment epitheliopathy (APMPPE) is an acquired inflammatory disorder affecting the retina, retinal pigment epithelium, and choroid of healthy adults. APMPPE has been reported to occur after diverse infectious diseases, suggesting a possible immune disorder. The primary site of inflammation remains hypothetical.
METHODS/RESULTS: A previously healthy 37-year-old patient developed APMPPE soon after the onset of mumps. Indocyanine green angiography (ICGA) revealed numerous hypofluorescent lesions throughout the posterior pole, outnumbering the lesions d

Multiple evanescent white dot syndrome (MEWDS) is a rare fundus disease, characterized by acute vision loss and visual field defects. Many previous studies have explained the possible pathogenesis and clinical features of primary MEWDS. However, as the number of reported cases increases, secondary MEWDS occurs in other related retinal diseases and injuries, exhibiting some special characteristics. The associated retinal diseases include multifocal choroiditis/punctate inner choroidopathy (MFC/PIC), acute zonal occult outer retinopathy, best vitelliform macular dystrophy, pseudoxanthoma elastic

We report a rare case of bilateral tuberculous uveitis associated with tuberculin skin testing (TST) in an otherwise healthy junior high school student. A 12-year-old boy presented with severely decreased vision in both eyes and a history of an intradermal PPD (purified protein derivative) skin test 12 days earlier. The best-corrected visual acuity (BCVA) was 20/1000 in both eyes (OU). Ocular examination showed multifocal grayish-yellow choroidal tubercles. Laboratory examinations, including T-SPOT.TB, revealed no abnormal findings. He was diagnosed with tuberculous uveitis. Combined corticost

Vascular occlusion in the eyes from cardiac myxomas was diagnosed in two cases, belatedly in one and not until after death in the other. Suspicion of a myxomatous origin should be aroused by the combination of unexplained retinal (or choroidal) vascular disease occurring with multifocal neurological symptoms and with systemic symptoms suggesting atypical subacute bacterial endocarditis. The first patient had unilateral retinal artery occlusion by embolic material believed to have been myxomatous. Removal of the cardiac myxoma resulted in disappearance of this material (although the eye remaine
