Peer support in adolescents and young adults with chronic or rare conditions in northern America and Europe: Targeted literature review.

PROBLEM: Adolescents and young adults with chronic or rare conditions face unique risks to their physical, social and emotional development. Research suggests that peer support improves their quality of life and reduces social isolation. However, there is a paucity of current information considering multiple intervention formats. ELIGIBILITY CRITERIA: A targeted literature review was conducted to identify peer support interventions and assess their feasibility, acceptability and efficacy for this population. Searches were conducted in MEDLINE, Embase and American Psychological Association Psy

A Retrospective Medical Record Review of Adults with Non-Cancer Diagnoses Prescribed Medicinal Cannabis.

Research describing patients using medicinal cannabis and its effectiveness is lacking. We aimed to describe adults with non-cancer diagnoses who are prescribed medicinal cannabis via a retrospective medical record review and assess its effectiveness and safety. From 157 Australian records, most were female (63.7%; mean age 63.0 years). Most patients had neurological (58.0%) or musculoskeletal (24.8%) conditions. Medicinal cannabis was perceived beneficial by 53.5% of patients. Mixed-effects modelling and post hoc multiple comparisons analysis showed significant changes overtime for pain, bowe

Disease Burden Affects Aging Brain Function.

BACKGROUND: Most older adults live with multiple chronic disease conditions, yet the effect of multiple diseases on brain function remains unclear. METHODS: We examine the relationship between disease multimorbidity and brain activity using regional cerebral blood flow (rCBF) 15O-water PET scans from 97 cognitively normal participants (mean baseline age 76.5) in the Baltimore Longitudinal Study of Aging (BLSA). Multimorbidity index scores, generated from the presence of 13 health conditions, were correlated with PET data at baseline and in longitudinal change (n = 74) over 5.05 (2.74 SD) year

Association between chronic conditions and urinary incontinence in females: a cross-sectional study using national survey data.

BACKGROUND: Urinary incontinence affects up to half of women, yet few speak to their health care provider about or receive treatment for the condition. To aid with identifying subpopulations at risk for urinary incontinence, we examined the associations between 10 chronic health conditions and urinary incontinence among Canadian adult females. METHODS: We conducted a cross-sectional analysis of survey data from the Canadian Community Health Survey (2013-2014) involving female respondents aged 25 years or older living in a private dwelling. Presence of chronic conditions and urinary incontinen

Thrombophilic conditions in non-cirrhotic portal vein thrombosis.

OBJECTIVE: To study the prevalence of thrombophilic conditions in patients with acute and chronic portal vein thrombosis (PVT) and to compare it with those in patients suffering from deep vein thrombosis (DVT) after lower limb arthroplasty and in healthy subjects. METHODS: Twenty-six patients with spontaneous PVT (20 chronic, 6 acute) with normal liver function and not receiving anticoagulants were evaluated for thrombophilic conditions. Levels of protein C, protein S and antithrombin were compared with those in 50 healthy controls. Factor V gene 'Leiden' mutation (FVL) and high homocysteine

Chronic heart failure part 1: pathophysiology, signs and symptoms.

Chronic heart failure is a common and complex clinical syndrome that results from impaired cardiac relaxation or contraction. Patients with chronic heart failure may experience multiple debilitating symptoms, such as fatigue, pain, and peripheral oedema. However, breathlessness may be considered the most debilitating symptom. This is the first of two articles on chronic heart failure, and outlines the pathophysiology of the condition, its causes, assessment, and signs and symptoms. Part 2 will discuss the treatment and management of the condition, including pharmacological strategies, device i

Frailty before disease onset: risk of mortality and chronic conditions in disease-free middle-aged adults.

Frailty, defined by multisystem physiological decline and heightened vulnerability to stressors, is typically associated with aging and chronic disease. However, its relevance in apparently healthy individuals remains poorly understood. Identifying frailty in this population could inform primary prevention, as these individuals often remain outside healthcare systems. We investigated the association between frailty and incident chronic conditions and mortality in middle-aged adults free of chronic disease at baseline. Using UK Biobank data, we included 183,783 participants (53.6% female; media

Iron deficiency anaemia revisited.

Iron deficiency anaemia is a global health concern affecting children, women and the elderly, whilst also being a common comorbidity in multiple medical conditions. The aetiology is variable and attributed to several risk factors decreasing iron intake and absorption or increasing demand and loss, with multiple aetiologies often coexisting in an individual patient. Although presenting symptoms may be nonspecific, there is emerging evidence on the detrimental effects of iron deficiency anaemia on clinical outcomes across several medical conditions. Increased awareness about the consequences and

Geo-clustered chronic affinity: pathways from socio-economic disadvantages to health disparities.

OBJECTIVE: Our objective was to develop and test a new concept (affinity) analogous to multimorbidity of chronic conditions for individuals at census tract level in Memphis, TN. The use of affinity will improve the surveillance of multiple chronic conditions and facilitate the design of effective interventions. METHODS: We used publicly available chronic condition data (Center for Disease Control and Prevention 500 Cities project), socio-demographic data (US Census Bureau), and demographics data (Environmental Systems Research Institute). We examined the geographic pattern of the affinity of

Physiology of Pain.

Acutely ill patients are challenging to frontline nurses because they frequently also have multiple chronic conditions. This article empowers all nurses to develop a foundational understanding of the physiology of acute and chronic pain. The skills, knowledge, and attitude to care for patients experiencing pain are a legal and ethical responsibility of all nurses. This article discusses the physiology of pain to include the neuronal receptors that respond to various painful stimuli, substances that stimulate nociceptors, the nerve pathways, modulation of the perception of pain, and acute verse

Comorbidities and Inflammation: How Chronic Diseases Prime the Host Response in Sepsis.

Considered a global public health priority, sepsis is characterized by life-threatening organ dysfunction caused by a dysregulated host response to infection. Its heterogeneous clinical presentation arises, in part, from pre-existing chronic conditions such as hypertension, metabolic syndrome, diabetes, alcohol exposure, psychosocial stress, and periodontitis, which induce persistent systemic changes even before the infectious event. This narrative review synthesizes evidence from experimental models and clinical studies to clarify the molecular and immunological mechanisms by which chronic co

Biologics for chronic inflammatory skin diseases: an update for the clinician.

The introduction and continuous development in biological drugs has greatly improved the therapeutic quality for patients with chronic inflammatory skin conditions. Current approaches to the biologic treatment of psoriasis, atopic dermatitis, chronic spontaneous urticaria, and hidradenitis suppurativa include licensed use of traditional antitumor necrosis factor agents, selective interleukin antagonists (IL-4, IL-12/23, IL-17), and the IgE inhibitor omalizumab, and as the knowledge on the pathogenesis of these diseases expands, off-label uses of the currently available biologics are becoming i

Role of Tai Chi in the treatment of rheumatologic diseases.

Rheumatologic diseases (e.g., fibromyalgia, osteoarthritis, and rheumatoid arthritis) consist of a complex interplay between biologic and psychological aspects, resulting in therapeutically challenging chronic conditions to control. Encouraging evidence suggests that Tai Chi, a multi-component Chinese mind-body exercise, has multiple benefits for patients with a variety of chronic disorders, particularly those with musculoskeletal conditions. Thus, Tai Chi may modulate complex factors and improve health outcomes in patients with chronic rheumatologic conditions. As a form of physical exercise,

Further evidence that painful and painless restless legs syndrome are contrasting phenotypes: a study of adult twins and their families.

OBJECTIVE: We aimed to test in adults that painful and painless phenotypes of the restless legs syndrome (RLS) differed in genetic influence and associations with comorbidities. METHODS: Twins Research Australia administered questionnaires to adult twins and their families. Responders self-reported RLS (Cambridge-Hopkins diagnostic questionnaire) and painful and painless subsets, their history of doctor-confirmed iron deficiency, selected primary and chronic pain conditions, and medical conditions known to be associated with RLS. Association analyses were conducted using logistic regression,

Fatal Epstein-Barr Virus (EBV) Myopericarditis in a Young Adult Male: A Case Report.

Epstein-Barr virus (EBV), typically known for a self-limited infection, the "infectious mononucleosis" (IM), can rarely provoke cardiac complications. We present here a case of fatal EBV myopericarditis in a young adult recently diagnosed with IM. A 16-year-old patient, whose medical history was characterized by multiple chronic pathologic conditions, was diagnosed with IM during a hospitalization. A month after discharge, he was admitted to the hospital, where an exacerbation of chronic pericarditis was diagnosed. Then, a few hours later, the patient died. Autopsy, histologic, and microbiolog

Mercy killing of a 72-year-old woman through heroin intoxication.

Active euthanasia is legally permissible in only eight jurisdictions worldwide and may only be administered by qualified personnel following specific selection criteria. Mercy killing refers to the deliberate termination of the life of an individual suffering from a terminal chronic medical condition. Detecting both illegally performed active euthanasia and instances of mercy killing presents challenges for forensic pathologists. The presented case describes a mercy killing involving a 72-year-old woman with multiple chronic conditions who was killed by her grandson via heroin intoxication aft

Screaming Body and Silent Healthcare Providers: A Case Study with a Childhood Sexual Abuse Survivor.

Stressful early life experiences cause immune dysregulation across the lifespan. Despite the fact that studies have identified childhood sexual abuse (CSA) survivors as a particularly vulnerable group, only a few attempts have been made to study their lived-experience of the physical health consequences of CSA. The aim of this study was to explore a female CSA survivor's lived-experience of the physical health consequences of CSA and how she experienced the reactions of healthcare providers. Seven interviews were conducted with this 40-year-old woman, Anne, using a phenomenological research ap
