Peroral endoscopic myotomy using FlushKnife BT: a single-center series.

BACKGROUND AND STUDY AIMS: Peroral endoscopic myotomy (POEM) is an evolving new treatment strategy for achalasia. Although several kinds of electrosurgical knives have been used in performing POEM, the best device has yet to be determined. The FlushKnife BT is a waterjet-emitting short needle-knife with a small ball tip (BT) that offers the potential to perform all aspects of POEM with a single device. In this study, we evaluated the safety and efficiency of the FlushKnife BT for POEM. PATIENTS AND METHODS: A total of 54 consecutive patients with achalasia and other spastic esophageal motilit

[Laparoscopic myotomy in achalasia cardia treatment: experience after 36 operations].

INTRODUCTION: Laparoscopic Heller-Dor operation, a standard method in the treatment of achalasia, has been performed at the Center for Esophageal Surgery of the First Surgical Clinic since April 2006. OBJECTIVE: The aim of this study was to present this surgical procedure and initial experiences after 36 consecutive laparoscopic Heller-Dor operations. METHODS: This partly retrospective, partly prospective study presented our results after laparoscopic Heller-Dor operation (presentation of the treatment method). We performed a standard anterior esophagocardioymiotomy, without releasing the po

Age affects presenting symptoms of achalasia and outcomes after myotomy.

Older patients with achalasia presumably have had a longer, more indolent course than younger patients. This study was undertaken to determine the impact of patient age and duration of symptoms on symptom severity and outcome after Heller myotomy. Two hundred sixty-two patients (142 men and 120 women), of average age 49 years +/- 17.7 (SD), have undergone laparoscopic Heller myotomy. Patients scored their symptoms using a Likert scale and subjectively rated their overall outcome. Data are presented as median, mean +/- SD, when appropriate. Follow-up is 25 months, 32 months +/- 28.7. Symptom se

Posterior myotomy/myectomy for persistent stooling problems in Hirschsprung's disease.

BACKGROUND: Hirschsprung's disease (HD) patients after pull-through (PT) may have recalcitrant constipation or recurrent enterocolitis (EC). Posterior myotomy/myectomy (POMM) are possible options for these problems. This study analyzed the outcome of POMM in HD patients post-PT. METHODS: Records of 348 HD patients were reviewed, and 32 were found to have undergone a POMM post-PT (1981 to 2002). Outcomes after this procedure were assessed. Statistics used linear and logistic regression. RESULTS: Of the 32 patients, 29 had complete records for analysis. Of those with pure constipation (12), 6

Treatment of Zenker's diverticula by cricopharyngeus myotomy under local anesthesia.

Optimum surgical management of the hypopharyngeal diverticulum is controversial. The authors discuss 48 consecutive patients (average age 72.1 years) with documented hypopharyngeal diverticula who were treated by cricopharyngeus myotomy, leaving the diverticula in situ. All came to the hospital with dysphagia; other symptoms included postdeglutitive cough, regurgitation, aspiration, and weight loss. Seven patients had had previous surgery for a Zenker's diverticulum with recurrence. Aspiration pneumonia was treated in 9 patients; 28 patients had concurrent chronic obstructive pulmonary disease

Manometric variations following spiral myotomy for long-gap esophageal atresia.

We have previously demonstrated that a spiral myotomy and delayed definitive procedure is a viable alternative for esophageal reconstruction in long-gap esophageal atresia. In this study we sought to determine whether this procedure leads to esophageal motility disturbances and to compare the manometric findings with controls as well as with those seen in children with esophageal atresia and primary anastomosis. Six beagles had esophageal transection and a spiral myotomy, one had esophageal transection without a myotomy, and two served as normal controls. Following esophageal reconstruction, e

Left ventricular myotomy. Physiologic approach to surgical therapy for IHSS.

Twenty patients were operated on for idiopathic hypertrophic subaortic stenosis (IHSS) between 1966 and 1980. All were in New York Heart Association functional class 3 or 4, and 17 had overt congestive failure. The mean resting gradient across the left ventricular (LV) outflow tract preoperatively was 78 mm Hg. Seventeen patients underwent transaortic LV myotomy, one had mitral valve replacement (MVR), and two patients with rheumatic mitral insufficiency (MI) and IHSS underwent myotomy and MVR. There was one operative death (5 percent). Mean follow-up was 5.8 years. Eighteen of 19 survivors we

Comparative study between classical myotomy, subsynovial myotomy, or posterior ligament electrocoagulation in the arthroscopic management of anterior disc displacement without reduction of the temporomandibular joint.

Anterior disc displacement without reduction of the temporomandibular joint is a prevalent condition associated with pain and functional limitation, and minimally invasive arthroscopic approaches are increasingly favored over open surgery due to lower morbidity. This study compared three arthroscopic interventions: classical myotomy (CM), minimally invasive anterior arthroscopic myotomy (MIAAM), and posterior ligament electrocoagulation (PLE). A total of 86 patients were included: 24 (27.9%) underwent CM, 30 (34.8%) underwent MIAAM, and 32 (37.2%) underwent PLE. Outcomes were assessed using vi

Clinical outcomes and safety of high-resolution manometry guided superficial partial circular muscle myotomy in per-oral endoscopic myotomy for Jackhammer esophagus: Two cases report.

BACKGROUND: Jack hammer esophagus is a relatively rare disease and to date, there is no dramatic treatment option. Recently, conventional per-oral endoscopic myotomy (POEM) have expanded their area into Jackhammer esophagus. However, several complications such as post procedure motility disorders (e.g., passage disturbance) are issues after POEM. To overcome these issues, we here introduced high-resolution manometry (HRM)-guided superficial partial circular muscle myotomy, which involves cutting only the superficial layer of the esophageal circular muscle. CASE SUMMARY: We report two cases of

Objective GERD following POEM significantly increased by length of myotomy.

INTRODUCTION: Gastroesophageal reflux disease (GERD) is a known potential sequelae of per oral endoscopic myotomy (POEM) for achalasia. Though the incidence is well documented, the risk factors are far from settled. We aimed to identify predictors of objective post-POEM GERD using endoscopic incidence of esophagitis and wireless pH testing with attention to myotomy length and intraoperative endoluminal functional lumen imaging probe (EndoFLIP) use. METHODS: We evaluated 193 patients undergoing POEM at our institution between March 2017 and December 2024. Patients who underwent upper endoscopy

Laparoscopic Heller Myotomy for Symptomatic Epiphrenic Diverticula: A Modern Technique.

Epiphrenic diverticula are one of the three types of esophageal diverticula, along with Zenker's and midesophageal, and are extremely rare. They are almost always associated with underlying esophageal motility disorders such as achalasia, diffuse esophageal spasm, and nutcracker esophagus. They are false diverticula and are found slightly above the cardio-esophageal junction on the right side. Symptoms usually appear in the digestive system and include dysphagia, regurgitation, and chest pain, as well as in the respiratory system with the occurrence of chronic cough and aspiration pneumonia. Paraoesophageal hernia causing recurrent dysphagia after Heller myotomy in achalasia.

A woman in her 60s with a medical history of achalasia treated by laparoscopic Heller myotomy presented with recurrent dysphagia 10 years postsurgery. This persistent symptom was initially suspected to be due to a rare recurrence of achalasia. However, a large paraoesophageal hernia was unexpectedly found during the upper gastrointestinal series. The patient underwent laparoscopic exploration, during which the integrity of the prior Heller myotomy was confirmed, and the paraoesophageal hernia was successfully reduced. Following the intervention, the patient's symptoms completely resolved, high

Per-oral endoscopic myotomy: a multi-centre New Zealand experience.

BACKGROUND AND AIMS: Per-oral endoscopic myotomy (POEM) is an established treatment for achalasia. We aim to review outcomes of all POEM cases performed in New Zealand for achalasia. METHODS: A retrospective review of all POEM procedures performed in the five hospitals offering POEM between November 2015 and December 2022 was undertaken. The primary outcome was clinical success, defined as Eckardt score ≤3. Secondary outcomes included procedural complications. RESULTS: One hundred and sixty-six index and four redo POEM procedures were performed by seven clinicians. Ninety-six (58%) were male

Revisional Therapy for Recurrent Symptoms After Heller Myotomy for Achalasia.

PURPOSE: Symptom recurrence after initial surgical management of esophageal achalasia occurs in 10-25% of patients. The aim of this study was to analyze safety and efficacy of revisional therapy after failed Heller myotomy (HM). METHODS: A retrospective review of a prospective database was performed searching for patients with recurrent symptoms after primary surgical therapy for achalasia. Patients with previously failed HM were considered for the final analysis. The Foregut questionnaire, and the Atkinson and Eckardt scales were used to assess severity of symptoms. Objective investigations

Anesthesia for Per-oral endoscopic myotomy (POEM) - not so poetic!

Peroral endoscopic myotomy (POEM) is a promising natural orifice transluminal endoscopic procedure for the treatment of esophageal motility disorders, with similar effectiveness as of Heller myotomy. It is performed under general anesthesia in endoscopy suite. Creation of submucosal tunnel in the esophageal wall is a key component. The continuous insufflation of CO2 inadvertently tracks into surrounding tissues and leads to capno mediastinum, capno thorax, capno peritoneum, and subcutaneous emphysema. Thus, the challenges, for an anesthesiologist are not only providing remote location anesthes
