Percutaneous nephrolithotomy of patients with staghorn stone and incidental purulent fluid suggestive of infection.

BACKGROUND AND PURPOSE: Patients undergoing percutaneous nephrolithotomy (PCNL) sometimes have purulent fluid in the puncture site in spite of having no signs or symptoms of infection preoperatively. We report the safety and effectiveness of PCNL in 45 patients with staghorn renal stones and incidental purulent fluid in the pelvicaliceal system. PATIENTS AND METHODS: Of 1264 patients who underwent PCNL at our medical center from February 2002 to May 2006, 45 patients had purulent fluid in the initial puncture. These patients were asymptomatic, and the preoperative work-up did not suggest infe

Simultaneous bilateral hemorrhagic pleural effusion and hemorrhagic ascites during percutaneous nephrolithotomy: a case report.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) may cause severe thoracic and abdominal complications. Because their onset can be insidious and their intraoperative manifestations atypical, recognition may be delayed. We report a case of sudden bilateral hemorrhagic pleural effusion with hemorrhagic ascites during PCNL and discuss strategies for intraoperative identification and management. CASE PRESENTATION: A 63-year-old woman underwent PCNL for multiple right renal calculi. During the procedure, renal pelvic bleeding occurred, followed by a sudden increase in airway pressure to 40 cmH2O, a

Standard versus miniaturized percutaneous nephrolithotomy in pediatric patients: a systematic review and meta-analysis.

BACKGROUND: Although miniaturized percutaneous nephrolithotomy (mini-PCNL) is a less invasive alternative to standard percutaneous nephrolithotomy (PCNL), its comparative outcomes in pediatric patients remain unclear. OBJECTIVES: To compare the efficacy and safety of mini-PCNL versus standard PCNL in children with nephrolithiasis. DESIGN AND SETTING: Systematic review and meta-analysis conducted according to PRISMA guidelines, including randomized controlled trials (RCTs) and retrospective cohort studies of pediatric patients undergoing mini-PCNL or standard PCNL. METHODS: Major databases (

Peripheral nerve block analgesia after percutaneous nephrolithotomy: a systematic review and network meta-analysis.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) is a common surgical procedure for the removal of kidney stones. Adequate postoperative analgesia is crucial for enhancing recovery and minimizing complications. Peripheral nerve blocks (PNBs) have emerged as a promising method for managing postoperative pain in PCNL patients. This systematic review and network meta-analysis (NMA) aim to evaluate the efficacy of various PNBs for analgesia after PCNL. EVIDENCE ACQUISITION: A comprehensive literature search was conducted in electronic databases including PubMed, Embase, Web of Science, and the C

Impact of Residual Stone Fragments on Risk of Unplanned Stone Events Following Percutaneous Nephrolithotomy.

Purpose: Residual stone fragments are common after percutaneous nephrolithotomy (PCNL), however, there is an unclear relationship between the presence of these residual stone fragments and the risk of unplanned stone events (USE). We investigated how the size of the largest residual stone fragment (LRSF) impacts the risk of USE post-PCNL. Materials and Methods: We conducted a retrospective cohort study of PCNL between 2018 and 2022. Preoperative computed tomography of the abdomen and pelvis (CTAP) imaging and postoperative CTAP imaging were reviewed. The primary outcome was the incidence of po

Prospective Randomized Trial of Antibiotic Prophylaxis Duration for Percutaneous Nephrolithotomy in Low-Risk Patients.

Introduction and Objective: Postoperative infection and sepsis account for the most common complications following percutaneous nephrolithotomy (PCNL), as high as 14% in low-risk patients. Although the American Urological Association (AUA) recommends perioperative antibiotics for 24 hours or less for PCNL, practice patterns vary regarding duration of antibiotic therapy. We aimed to compare the efficacy of 24-hour antibiotic coverage vs short-course protocol of antibiotic prophylaxis for PCNL. Materials and Methods: Low-risk patients with a sterile preoperative urine culture undergoing PCNL wer

Vascular air embolism during percutaneous nephrolithotomy without pneumopyelogram: a case report of successful rescue.

Vascular air embolism (VAE) is a rare complication of percutaneous nephrolithotomy. Paradoxical air embolization resulting from VAE may be more likely to occur in patients with an atrial-septal defect, such as patent foramen ovale. Here, the case of a female patient with VAE that occurred during percutaneous nephrolithotomy is presented. Although the patient was diagnosed with patent foramen ovale, she recovered well without any severe paradoxical air embolization symptoms. To our knowledge, this is the first report of VAE with paradoxical air embolization that occurred in a patient with paten

Propensity score-matched analysis comparing retrograde intrarenal surgery with percutaneous nephrolithotomy in anomalous kidneys.

BACKGROUND: Endourologic interventions for urolithiasis in patients with anomalous kidneys can be challenging, and comparisons between these interventions are not well studied. We aim to compare the safety, outcomes and complications of retrograde intrarenal surgery (RIRS) versus percutaneous nephrolithotomy (PCNL) in patients with urolithiasis in anomalous kidneys. METHODS: A propensity score-matched pair analysis (PSM) was performed on pooled patient data from 20 centers. 569 patients with anomalous kidneys (horseshoe kidney [HSK], ectopic kidney, malrotated kidney) and urolithiasis who rec

Renal Collecting System Injury During Percutaneous Nephrolithotomy, More Likely When Using Continuous Flow Sheaths?

Background: Renal collecting system injuries are a rare occurrence during percutaneous nephrolithotomy (PCNL). However, when they do happen, the potential for complications rises exponentially the longer the injury goes undetected. This case highlights a possible higher rate of injury when using continuous flow sheaths. Case Presentation: A 65-year-old woman with history of large volume obstructing left-sided renal pelvis stone had an emergent ureteral stent placed for infection and was referred for interval management. Patient was taken for PCNL in the prone split-leg position where a continu

Significance of preoperative neutrophil-lymphocyte count ratio on predicting postoperative sepsis after percutaneous nephrolithotomy.

We evaluated the usefulness of preoperative neutrophil-lymphocyte count ratio (NLCR) in predicting postoperative sepsis after percutaneous nephrolithotomy (PCNL). In total, 487 patients who underwent PCNL for renal stones were included in the present retrospective study. The stone burden, number of tracts and location, operation time, fluoroscopy time, presence of residual stones, and blood transfusion rates were postoperatively recorded in all patients. All patients were followed up for signs of systemic inflammatory response syndrome (SIRS) and sepsis. The association of sepsis/SIRS with the

Factors affecting complications according to the modified Clavien classification in complete supine percutaneous nephrolithotomy.

INTRODUCTION: An increase in percutaneous nephrolithotomy (PCNL) has been accompanied by an increase in complications. We identified the parameters affecting the severity of complications using the modified Clavien classification (MCC). METHODS: From 2008 to 2013, 330 patients underwent complete supine PCNL using subcostal access, one-shot dilation, rigid nephroscopy, and pneumatic lithotripsy. We assessed the impact of the following factors on complication severity based on the MCC: age, gender, body mass index, hypertension, diabetes, previous stone surgery and extracorporeal shock wave lit

Skin treatment and tract closure for tubeless percutaneous nephrolithotomy: University of California, Irvine, technique.

BACKGROUND AND PURPOSE: After percutaneous nephrolithotomy (PCNL), a nephrostomy tube has been routinely placed to ensure hemostasis, provide drainage, and maintain access to the collecting system should a second-look procedure be necessary. Recently, efforts have been expended to either reduce the size of the nephrostomy tube or eliminate it altogether. We describe the tubeless technique of closure and skin treatment after PCNL using FloSeal as a sealant for tubeless PCNL. TECHNIQUE: A 7F 11.5-mm occlusion balloon catheter is passed retrograde over the through-and-through guidewire. Next, un

Access related complications during percutaneous nephrolithotomy: urology versus radiology at a single academic institution.

PURPOSE: A recent survey revealed that only 11% of urologists performing percutaneous nephrolithotomy routinely obtained percutaneous access themselves. Reasons for this trend may include lack of training, comfort level and perceived need for radiological involvement. In this study we evaluated percutaneous access for percutaneous nephrolithotomy obtained by interventional radiologists or a urologist at a single academic institution, and compared access trends and complications. MATERIALS AND METHODS: Two cohorts of patients who had undergone percutaneous nephrolithotomy between 1999 and 2003
