Neuroradiological Correlation of the Lateral Third Periventricular, Pituitary Gland and Stalk, Pineal Gland, Cerebral Aqueduct, and Foramen Magendie and Luschka With Intraventricular Neuroendoscopic Images: A Case Series.

Neuroendoscopy procedures in pediatrics have expanded beyond the endoscopic third ventriculostomy. As such, a direct and angled endoscope allows further visualization around the corner, capturing the surrounding anatomy. Intraoperative live images look different than radiological images. Hence, in this single institutional experience, we correlate neuroradiology images with intraoperative intraventricular endoscopic views of the third-fourth ventricle, pituitary, pineal gland, cerebral aqueduct, and foramen magendie and luschka. Our collective case series reveals a few interesting case scenari

Low-dose Intraventricular Recombinant Tissue Plasminogen Activator (rtPA) Priming Combined with Neuroendoscopic Lavage and Continuous Ventricular Irrigation for Severe Preterm Intraventricular Hemorrhage : A Technical Note.

Severe preterm intraventricular hemorrhage (IVH) with post-hemorrhagic ventricular dilatation (PHVD) may be difficult to manage when the clot burden is extensive and complete endoscopic evacuation is limited by clinical instability and rigid-scope constraints. We describe a staged clearance strategy in a preterm infant with bilateral grade IV IVH and progressive PHVD. Bilateral external ventricular drains (EVDs) were inserted for cerebrospinal fluid diversion. Low-dose intraventricular recombinant tissue plasminogen activator (0.1-0.3 mg per ventricle) was started 48 hours later and repeated e

Systematic Review: Success Rate of Endoscopic Endonasal versus Combined Endonasal and Transorbital Neuroendoscopic Approach for Nontraumatic Cerebrospinal Fluid Leak Repairs in the Lateral Recess of Sphenoid Sinus.

Background  Cerebrospinal fluid (CSF) leaks from the lateral recess of the sphenoid sinus (LRS) occur due to a skull base defect and are important to treat due to the associated morbidity, e.g., life-threatening meningitis. Nontraumatic CSF leaks have a predilection toward obesity which is a rising phenomenon. LRS is notoriously difficult to access because of its lateral location and its associated neurovascular complications. An alternative surgical corridor has been explored which is the transorbital neuroendoscopic (TONES) approach. Objective  To compare the success rate of the endoscopic e

A Review of Physical Simulators for Neuroendoscopy Skills Training.

BACKGROUND: Minimally invasive neurosurgical approaches reduce patient morbidity by providing the surgeon with better visualization and access to complex lesions, with minimal disruption to normal anatomy. The use of rigid or flexible neuroendoscopes, supplemented with a conventional stereoscopic operating microscope, has been integral to the adoption of these techniques. Neurosurgeons commonly use neuroendoscopes to perform the ventricular and endonasal approaches. It is challenging to learn neuroendoscopy skills from the existing apprenticeship model of surgical education. The training metho

[A comparison of neuro-endoscopic techniques with other surgical procedures in the treatment of arachnoid cysts in children].

The aim of the study was to evaluate the use of neuroendoscopic techniques (in comparison with other surgical procedures) in the treatment for arachnoid cysts in children. The analysis was performed on results of treatment 22 children with arachnoid cysts submitted to neuroendoscopic procedures. The group consisted of 6 girls and 16 boys, aged from 1 day to 18 years (mean age 5.3 years, SD +/- 5.6). The control group treated with other, non-endoscopic surgical procedures consisted of 61 patients (20 girls and 41 boys aged from 10 days to 17 years, mean age 7 years, SD +/- 6). Criteria of succe

Neuroendoscopic Surgery with a Combination of Image Detectable Sheath, Intraoperative Computed Tomography Scan, and Navigation System Improves Accuracy and Safety in Minimally Invasive Evacuation of Intracerebral Hematoma: Technical Note.

BACKGROUND: Recent studies have confirmed the effectiveness of minimally invasive endoscopic surgery for intracerebral hematoma (ICH). However, improvements are needed because incomplete hematoma removal may offset the surgical benefits of the technique. We describe a technique of neuroendoscopic surgery using an image detectable sheath, intraoperative computed tomography (iCT) scan, and a navigation system. METHODS: This is a retrospective study of 15 consecutive patients with spontaneous ICH who received neuroendoscopic surgery. During the surgery, a transparent sheath was fastened tightly

[Surgery for giant intracranial aneurysms using advanced technology].

PURPOSE: Outcome of surgery for giant intracranial aneurysms is still unsatisfactory. The reason for complications is occlusion of perforators or parent arteries by the aneurysmal clipping itself or temporary occlusion of the main arteries. We report the surgical outcome of treatment of giant aneurysms using several advanced techniques which we devised to prevent these complications. MATERIALS AND METHODS: The subjects were eight patients with giant intracranial aneurysms who underwent surgery during the recent five years. Six patients had ruptured and two had unruptured aneurysms. Aneurysms

Neuroendoscopy Followed by Radiotherapy in Cystic Craniopharyngiomas--a Long-Term Follow-Up.

BACKGROUND: Treatment of cystic craniopharyngiomas is often complicated because of adherence of craniopharyngiomas to adjacent brain structures. A strategy involving neuroendoscopic procedures followed by stereotactic radiotherapy is less invasive, avoiding brain damage. However, long-term follow-up of this strategy has not been investigated in depth. METHODS: Cystic craniopharyngiomas in 9 patients were treated with neuroendoscopic cyst aspiration and fenestration, followed by fractionated stereotactic radiotherapy (FSRT). The neuroendoscopic procedure focused on widening of cyst fenestratio

Endoscopic transaqueductal removal of fourth ventricular neurocysticercosis: report of three cases.

In this study, we try to present our experience of transaqueductal removal of the fourth ventricular neurocysticercosis (FVNCC) with a flexible neuroendoscope. Three cases of the fourth ventricular cysticercosis were transaqueductally removed with FUJINON EB-270P flexible neuroendoscope through a frontal precoronal burr hole. The diagnosis was established on imaging and confirmed on histology in all of the cases. Complete excision of cysts in the fourth ventricle was performed in all cases with no significant intraoperative or postoperative complications. The shunt and the string-of-beads mult

Magnetic resonance cisternography for preoperative evaluation of arachnoid cysts.

INTRODUCTION: With a high likelihood of clinical improvement and low rates of complications, minimally invasive neuroendoscopic surgery is becoming the treatment of choice for symptomatic or growing arachnoid cysts. In neuroendoscopic surgery, visualization of anatomical landmarks is essential in achieving successful fenestration without complications. Because of the restricted visual field in neuroendoscopic surgery, preoperative anatomical assessment is very helpful. Magnetic resonance cisternography (MRC) with high spatial resolution and contrast, using for example 3-D Fourier transformatio

Clinical Efficacy of Endoscopic Surgery for Intracranial Abscesses.

Intracranial abscesses, including brain abscesses, ventriculitis, and subdural empyema, are critical conditions with high mortality rates. Conventional treatments involve antibiotics and surgical drainage or curettage; however, neuroendoscopic surgery has shown promising outcomes and requires further study. Herein, we present three cases that were successfully managed using neuroendoscopic surgery. Two patients had brain abscesses with ventricular rupture secondary to dural defects and were treated with transventricular endoscopic removal and dural repair. The third patient, a male in his 90s,

Neurocognitive Complications after Ventricular Neuroendoscopy: A Systematic Review.

In recent years, neuroendoscopic treatment of hydrocephalus and various ventricular pathologies has become increasingly popular. It is considered by many as the first-choice treatment for the majority of these cases. However, neurocognitive complications following ventricular neuroendoscopic procedures may occur leading mostly to amnesia, which might have a grave effect on the patient's quality of life. Studies assessing neurocognitive complications after ventricular neuroendoscopic procedures are sparse. Therefore, we conducted a systematic review assessing the available literature of neuroco

A review of neuroendoscopy and potential applications in veterinary medicine.

The endoscope was first developed over 200 yr ago. Endoscopy has since been applied to many disciplines of medicine. Its application to the nervous system was initially slow and not widely accepted and mainly involved the biopsy of tumors and the treatment of hydrocephalus. Several reasons for neuroendoscopy's limited use include inadequate endoscope technology, high skill level required, the advent of the surgical microscope, and the development of other treatments such as ventricular shunting. Over the past 50 yr, improvements in optical glass lenses, fiber optics, and electrical circuitry h

[Optimization of the Surgical Environment and Perioperative Management in Endoscopic Intraventricular Surgery].

Neuroendoscopic intraventricular surgery provides a minimally invasive approach but is inherently challenging, as it is performed within a fragile, three-dimensional ventricular space while relying on a limited two-dimensional endoscopic view. Loss of orientation and subtle technical errors can result in hemorrhage or neural injury. This chapter describes practical strategies for achieving stable and reproducible neuroendoscopic intraventricular surgery, with emphasis on operating room setup, instrumentation, teamwork, and perioperative management. Key principles include aligning the surgeon,

Endoscopic assisted microneurosurgery for cerebral aneurysms.

While there have been many advances in the field of microneurosurgery, the clipping of aneurysms remains an intricate procedure. Technical complications include residual aneurysm, perforator injuries, parent artery occlusion and cranial nerve injuries. The neuroendoscope is a useful tool and adjunct to the microsurgical clipping of these aneurysms. We study the usefulness of the neuroendoscope in enhancing visualisation during surgery. Twenty-four cases of ruptured cerebral aneurysms were operated on over a duration of 6 months in which a 1mm diameter rigid endoscope was used. We discuss our p

Terson's syndrome after endoscopic colloid cyst removal: case report and a review of reported complications.

The authors report on a patient with Terson's syndrome after endoscopic colloid cyst resection of the third ventricle. This never reported complication in neuroendoscopy is explained by an increased intracranial pressure due to rinsing. Increased rinsing was needed to prevent the ventricles from collapsing. A free outflow channel and rinsing pace should always be matters of concern during neuroendoscopic procedures. Complications in neuroendoscopic colloid cyst removal are reviewed from the literature.
