Optical coherence tomography angiography (OCTA) assessing microvascular and macrovascular health in healthy octogenarians.

Subclinical cardiovascular abnormalities are recognized as primary risk factors for cardiovascular disease (CVD). Most studies explore microcirculation, while the associations of peripheral microvascular structures and abnormalities have been understudied. This study aimed to use OCTA to gauge the microvascular structure in the proximal nailfold and estimate cardiovascular health via echocardiography, arterial tonometry, and blood tests in samples of healthy octogenarians. We employ unsupervised machine learning methodologies, and vascular patterns are clustered to identify possible microcircu

Frailty and functional decline after elective endovascular abdominal aortic aneurysm repair in octogenarians.

OBJECTIVE: Frailty can be used to estimate perioperative and long-term mortality risk in octogenarians undergoing elective endovascular abdominal aortic aneurysm repair (EVAR). Its association with functional decline, an important surrogate of quality of life in older adults, has not been well-studied in this population. We aimed to evaluate whether preoperative factors such as frailty are associated with 1-year functional decline after elective EVAR in octogenarians and to compare the performance of three different frailty assessments. METHODS: The Vascular Quality Initiative (VQI) database

Paddle Lead Spinal Cord Stimulation for Chronic Pain in Octogenarians: A Case Series.

BACKGROUND: Spinal cord stimulation (SCS) is a surgical technique for chronic pain involving the placement of percutaneous, or paddle leads into the spinal epidural for pain treatment. Paddle lead SCS is a more invasive procedure due to the requirement of bone removal (laminotomy/laminectomy) and may therefore be less commonly offered to older patients. METHODS: A retrospective review was conducted of a database of all paddle lead spinal cord stimulators implanted from January 1st, 2019, to July 1st, 2024, by a single surgeon. Octogenarian patients were identified within this cohort and their

Limb salvage in octogenarians with critical limb ischemia after lower extremity bypass surgery.

OBJECTIVES: Peripheral artery disease is a worldwide epidemic that affects millions of patients, especially the elderly. It has a prevalence of 20% in individuals >80 years old. Although peripheral artery disease affects >20% of octogenarians, information about limb salvage rates in this patient population is limited. Therefore, this study aims to understand the impact of bypass surgery on limb salvage in patients aged >80 years with critical limb ischemia. METHODS: We conducted a retrospective analysis by querying the electronic medical records at a single institution from 2016 through 2022

Mitral valve surgery in octogenarians: Identifying factors that can guide clinical decision-making.

BACKGROUND: Mitral valve surgery in octogenarians is a clinical challenge due to the impact of inherent, age-related comorbidities. Within the context of an aging population, the number of mitral surgery candidates over 80 has been gradually increasing. We sought to evaluate our institutional experience with mitral valve surgery in octogenarians to identify factors that may prove useful during the process of clinical decision-making. METHODS: We retrospectively reviewed our institutional database for all patients over 80 years of age who underwent mitral valve surgery in our department from O

Percutaneous coronary intervention for chronic total occlusion in octogenarians: a propensity score study.

Feasibility and efficacy of complex percutaneous coronary intervention (PCI) in the elderly, a more frail population due to more comorbidities is incompletely understood. We therefore set out to compare success and complication rate of PCI for chronic total occlusion (CTO) in octogenarians, in comparison to non-octogenarians. Data from 267 patients (58 patients over 80 years of age and 209 under 80 years of age) who had undergone CTO PCI were analyzed. To compare the results we calculated the propensity score and used inverse probability of treatment weighting. We evaluated demographic, clinic

Safety and efficacy of transvenous lead extraction in octogenarians using powered extraction sheaths.

BACKGROUND: In the aging population equipped with cardiac implantable electronic devices, an increasing number of octogenarians require lead extractions. This patient population is often considered as a high-risk group for surgical procedures. We, therefore, investigated the safety and efficacy of transvenous lead extraction in octogenarians using powered extraction sheaths. METHODS: Between January 2013 and March 2017, 403 patients underwent lead extraction at two high-volume lead extraction centers. A total of 71 octogenarians were treated with laser lead extraction and were included in thi

Octogenarians develop infrapopliteal arterial occlusive disease in the absence of traditional risk factors.

BACKGROUND: With increasing longevity, a growing proportion of patients who present with lower extremity peripheral arterial disease (LE-PAD) are ≥80 years old. While smoking and diabetes mellitus (DM) have traditionally been the main risk factors associated with PAD, we noted a pattern of severe infrapopliteal PAD in patients ≥80 years old in the absence of these traditional risk factors. As recognition of patterns of disease affects decisions regarding diagnostic and therapeutic approach, we sought to confirm this observation. METHODS: A single-center retrospective review was performed on a

How safe is open abdominal aortic aneurysm surgery for octogenarians in New Zealand?

BACKGROUND: Abdominal aortic aneurysm (AAA) is an important cause of mortality for the aged, a group that has been denied surgery in the past for fear of peri-operative mortality. Is this attitude still justified? METHODS: Analysis of prospectively gathered data from a vascular database. RESULTS: 10.9% of all open AAA operations were in patients older than 79 years with an 8% mortality rate compared to 3% for younger patients. For fit elderly patients with ASA scores less than 3, mortality was just under 4%. Renal failure and wound dehiscence were more common in the elderly. CONCLUSION: Whe

Long-term outcomes after isolated aortic valve replacement in octogenarians: a modern perspective.

BACKGROUND: With the recent advent of percutaneous valve therapy, an increased need for the evaluation of outcomes after open aortic valve replacement (AVR) in elderly patients is warranted. This study compares the short- and long-term survival outcomes of octogenarians after AVR with younger age groups in the modern surgical era. METHODS: A retrospective review was performed on patients who underwent isolated, primary AVR from 1996 to 2006 at the Emory Healthcare Hospitals. Five-hundred fifteen patients were divided into three age groups: 60 to 69 (n = 206), 70 to 79 (n = 221), and 80 to 89
