Clinicopathological features and management of IgM multiple myeloma and Waldenstrom macroglobulinemia.

Waldenström macroglobulinemia (WM) is the most common malignancy associated with an IgM paraprotein, but in rare cases, a clonal IgM may be the result of IgM multiple myeloma (IgM-MM). Although there are some overlapping features associated with these two entities, there are specific characteristics that can help differentiate IgM-MM from WM. In each patient a thorough clinical, pathologic, and genomic evaluation is required to distinguish these conditions and allow for accurate diagnosis and appropriate treatment.

Non-invasive markers of bone turnover and plasma cytokines differ in osteoporotic patients with multiple myeloma and monoclonal gammopathies of undetermined significance.

AIMS: To determine whether various markers of bone turnover and/or plasma cytokines differ in patients with multiple myeloma (MM) compared with patients with monoclonal gammopathies of undetermined significance (MGUS). METHODS: We studied 22 MM patients and 18 MGUS patients presenting over an 18-month period and compared their data with those from 20 age- and sex-matched patients presenting with primary osteoporosis. According to the Salmon and Durie classification, there were eight patients with stage I, nine with stage II and five with stage III disease. All patients had densitometric evide

A case of myasthenia gravis associated with thymoma, multiple schwannomas and monoclonal IgA gammopathy.

Myasthenia gravis, malignant thymoma, multiple schwannomas and monoclonal IgA gammopathy coexisted in a 54-year-old Japanese man. Serum acetylcholine receptor antibody activity was located in the IgG fraction, but not in the paraprotein peak. It was speculated that all these disorders might fall under the spectrum of the diseases associated with thymic abnormality or that some growth factors might induce neoplasms and activation of B cell clones of autoantibodies (including acetylcholine receptor antibody) in this case.

Solitary plasmacytoma of the submandibular lymph node with stromal amyloid deposits.

This report concerns a case of solitary extramedullary plasmacytoma of the left submandibular lymph node in a 56-year-old man. The tumor showed monoclonal proliferation of abnormal plasma cells which revealed highly positive stainings of both methylgreen pyronin and kappa light chain using the immunoperoxidase technique in the cytoplasms, and further revealed massive amyloid deposits in the stroma, which suggested the possibility of sequential amyloid formation upon the secretion of paraprotein by tumor cells.

[Lymphocyte membrane markers in monoclonal gammopathies].

The distribution of peripheral blood lymphocytes with immunoglobulins (Ig) in 14 patients with monclonal gammapathies has been studied. The patients have been divided in two groups, one including patients never treated, the other those studied during or after treatment. The results, compared with 11 control subjects, show that lymphocytes with surface Ig from the same class of the serum paraprotein increased in every case of the first group and in some cases of the second.

Chemotherapy in the management of extramedullary plasmacytoma.

The results of chemotherapy in 24 patients with extramedullary plasmacytoma are reported. Complete regressions, including disappearance of monoclonal paraprotein and healing of bone lesions, were seen in 12 of 20 (60%) patients with disseminated disease. Extramedullary plasmacytoma responds better to chemotherapy than myeloma, and treatment should be pursued with vigour until all signs of disease have disappeared. Sensitivity to single-agent chemotherapy may vary, and if treatment fails with one agent, others should be tried.

Where are the immunoglobulins? A review of non-secretory multiple myeloma.

Multiple myeloma (MM) is a malignant neoplasm of clonal plasma cells, typically associated with the production of a monoclonal protein. In 1-3% of cases, MM presents without measurable monoclonal protein (M protein) in the serum or urine and normal serum-free light chains; these cases are referred to as non-secretory MM (NSMM). This definition has changed over time according to the sensitivity of laboratory methods for detecting paraproteins. NSMM has been previously reported to have a less aggressive presentation and clinical course compared to secretory MM; however, the literature is conflic

[Acute renal failure].

The acute renal failure is characterized by a rapid deterioration of the renal function. In addition to the usual prerenal, intrinsic and postrenal causes of an acute renal failure distinct causes have to be considered for oncological patients. Factors imminent to the malignant disease, e. g. paraneoplastic syndromes or retroperitoneal bulks can account for an acute renal failure. Paraproteins as produced by a multiple myeloma are other possible causes for renal dysfunction. For some anticancer drugs nephrotoxicity is a potential side effect, in particular for cisplatin, methotrexate, ifosfami

Myelin-associated glycoprotein in demyelinating disorders.

The myelin-associated glycoprotein (MAG) is a 100-kD, integral membrane glycoprotein that is a member of the immunoglobulin superfamily and appears to function in interactions between myelin-forming cells (both oligodendrocytes and Schwann cells) and the axolemma. Its early loss in the development of multiple sclerosis plaques in comparison to other myelin proteins suggests that it could play a key role in the pathogenesis of this disease. This selective loss of MAG may relate to the high susceptibility of human MAG to cleavage by a Ca2+-activated neutral protease. Human MAG also contains a hi

[Recurrent polyneuropathy with a 19-year course, associated with a benign IgG monoclonal gammapathy].

A case of recurrent polyradiculoneuritis in a 54-year-old man is described. It is exceptional because of its lengthy development over a period of 19 years and by its association with a paraprotein of the IgE type without anyother anomal. Corticotherapy had a favourable effect on the clinical symptoms but did not affect the amount of monoclonal immunoglobulin in the serum. The nosological position of this neuropathy and the type of gammapathy are discussed. An immune mechanism seems likely, but no proof of a link between the neuropathy and the gammapathy could be found.

Immunotyping as an Alternative to Reducing Agents for Resolving Monoclonal Immunoglobulin Aggregates Observed on Immunofixation.

BACKGROUND: Polymerized immunoglobulins can be difficult to interpret when observed on serum immunofixation electrophoresis (IFE). The use of reducing agents, such as beta-mercaptoethanol (BME), has been instrumental in resolving cases in which the monoclonal protein is unidentifiable due to polymerization. However, this procedure can be inconvenient to laboratories. We therefore investigated the use of immunotyping (IT) to resolve immunoglobulin aggregates as a potential replacement for BME pretreatment. METHODS: Patient samples with paraprotein polymerization were selected from our routine

A simple method to overcome paraproteinemic interferences in chemistry and immunoassays.

BACKGROUND: Interferences on chemistry and immunoassay results due to paraproteinemia may lead to erroneous diagnoses and treatment. Such interferences are difficult to recognize and even more difficult to deal with. This report describes 1 such case where multiple measurands were affected and how the interferant was overcome. CASE REPORT: Paraproteins present in an immunoglobulin (Ig)G-lambda multiple myeloma specimen interfered with results of total bilirubin, direct bilirubin, inorganic phosphate, iron, ferritin, and total thyroxine measured on 3 platforms: AU5800, Alinity ci, and cobas pu

Long-term survival of a patient with multiple myeloma--a cure? A case report.

Multiple myeloma is a disease characterized by the proliferation of malignant plasma cells, the over-production of a monoclonal immunoglobulin (paraprotein) and the subsequent depression of the other normal immunoglobulins. Whereas many patients respond to treatment and enter a plateau phase of variable duration, these patients eventually die of their disease. This report describes a patient with Stage III multiple myeloma diagnosed in 1966 and now with documentation of prolonged disease-free survival and perhaps cure, with the patient presently alive and well without evidence of disease.

Investigation of unusual high serum indices for lipemia in clear serum samples on siemens analysers dimension.

INTRODUCTION: We present our work of monitoring 202 different patients with markedly elevated serum index for lipemia whereby serum samples were clear. We tried to clarify the cause of occurrence of these indices which were detected in the years 2006-2010 on Siemens Dimension analyzers. MATERIALS AND METHODS: In samples with unusual lipemia index we measured the concentration of lipids (total cholesterol, triglycerides, HDL and LDL cholesterol, Lp(a), ApoA1, ApoB), total proteins and checked for possible interferents (rheumatoid factor, immunoglobulins). We performed serum protein and immuno-

Multiple myeloma presenting with an acute bacterial infection.

Increased susceptibility to bacterial infections is a common manifestation of multiple myeloma (MM), arising mainly from a defect in humoral immunity and is associated with major morbidity and mortality. The propensity to infection is increased in the first months after the initial diagnosis and in patients with renal dysfunction. Gram-positive infections, mainly pneumonia from Streptococcus pneumoniae, occur more frequently in patients with untreated disease while Gram-negative infections, mainly of the urinary tract, are more common after chemotherapy. However, an acute bacterial infection i

Low immunoglobulin A levels detected via the tissue transglutaminase assay can reveal previously undetected monoclonal proteins.

Increased awareness of coeliac disease and the 2009 NICE guidance has led to an increase in patients being screened for Immunoglobulin A deficiency. We have shown previously that this provides an opportunity for the early identification of other underlying primary immunodeficiency, e.g. common variable immunodeficiency. In this context, the underlying gastrointestinal problem appears to be related to bacterial overgrowth. Here, we demonstrate that in addition this also provides an opportunity to reveal underlying secondary immunodeficiency due to other causes in patients with gastrointestinal

Monoclonal gammopathy of uncertain significance.

Monoclonal gammopathy of uncertain significance is a premalignant plasma cell neoplasm with a high prevalence in the population over 50 years of age and an annual risk of progression of 1%. Multiple recent studies have led to advances in understanding both the pathogenesis of these disorders and their risk of progression to other diseases. Patients require lifelong follow-up, and a multidisciplinary and risk-adapted approach is essential. In recent years, an increasing number of entities associated with a paraprotein, known as clinically significant monoclonal gammopathies, have been recognize

AL amyloidosis in a young adult: remission with autologous stem cell transplantation.

Autologous stem cell transplant is one of the therapies employed in the treatment of primary amyloidosis or AL. The authors report on a 46-year-old patient with bilateral periorbital hematomas, macroglossia who presented, during the investigation, IgG-Kappa paraprotein in serum. The diagnosis of primary amyloidosis or AL was confirmed and the treatment proposed consisted of high-dose melphalan as conditioning regimen before autologous stem cell transplant, which determined complete remission of the disease, along with the disappearance of clinical signs and absence of the monoclonal component.

Treatment of recalcitrant scleromyxedema with thalidomide in 3 patients.

Scleromyxedema is a generalized, papular, and sclerodermoid form of lichen myxedematosus associated with monoclonal gammopathy and systemic changes. Despite anecdotal reports of success with various agents, no satisfactory treatments are currently available. We report 3 adult patients with recalcitrant scleromyxedema associated with paraproteinemia who were treated with thalidomide. All 3 patients had marked improvement of the skin lesions and joint mobility after the first 2 months of therapy, with further amelioration after 4 months, and reduction in paraprotein levels.

Nephrotic syndrome caused by protein thrombi in glomerulocapillary lumen in Waldenström's macroglobulinaemia.

Waldenström's macroglobulinaemia (WM) is described as a disorder of plasmacytoid lymphocytes. The renal complications of WM are less common and severe than those of multiple myeloma. We present a case of WM complicated by nephrotic syndrome. A biopsy specimen of the kidney revealed the intraglomerular thrombi of immunoglobulin M paraprotein. Corticosteroid pulse therapy and plasmapheresis were effective in improving proteinuria and reducing protein thrombi. The nephrotic syndrome caused by protein thrombi in WM may be reversible, at least in its early stage.
